Factors Influencing Clinical Nurses' Intention to Report Medication Administration Errors (임상간호사의 투약오류보고 의도에 영향을 미치는 요인)

  • Purpose : This study aimed to identify factors influencing clinical nurses' intention to report medication administration errors. Methods : This cross-sectional study collected data from 121 nurses in charge of administering medication at a university hospital in Korea using structured questionnaires. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and multiple linear regression. Results : Participants' mean age was 26.90±3.99 years, and 89.3% were women. Their mean clinical career duration was 3.88±4.26 years. The average levels of patient safety culture, attitude toward reporting medication administration errors, and intention to report medication administration errors were 7.51 out of 10, 3.36 out of 5, and 4.85 out of 6, respectively. The multiple regression analysis results indicated that the statistically significant influencing factors were patient safety culture (𝛽=.21, p =.018) and attitude toward reporting medication administration errors (𝛽=.22, p =.015). Conclusion : To improve the intention to report medication administration errors among clinical nurses, a patient safety culture must be established, along with an education provision for improving their attitudes toward reporting such administration errors.

Analysis of Factors Affecting the Inpatient Satisfaction in Integrated Nursing Care Service Wards using a Healthcare Service Survey Database (간호⋅간병통합서비스 병동 입원환자 만족도 영향요인: 의료서비스 경험 조사 자료 이용 )

  • Purpose : In this study, we aimed to identify factors influencing the inpatient satisfaction in integrated nursing care service wards. Methods : Using data from the 2020 and 2021 healthcare service experience surveys conducted by the Ministry of Health and Welfare, this study analyzed a weighted sample of 416,020 participants using descriptive statistics, pairedt-test, Pearson's correlation, and multiple linear regression analyses. Results : Inpatient satisfaction in integrated nursing care service wards was most significantly affected by the experiences with nursing services and perceived health status, followed by experiences with doctor services, length of hospitalization, age, experiences with medical institution services, education level, gender, presence of chronic diseases, type of hospital, and occupation. Most significant positive impacts were associated with experiences with nursing and doctor services, especially in general hospitals and high-level facilities, among males and elderly patients > 60 years old. In contrast, high education level, presence of chronic diseases, and unemployment were associated with low inpatient satisfaction. Conclusion : Effective programs and guidelines must be developed and implemented to expand and promote the integrated nursing care and medical services and overall utilization of healthcare institutions, with a special focus on person-centered care.

Effects of Grit, Patient Safety Competence, and Patient Safety Culture on the Patient Safety Nursing Activities of Nurses in Comprehensive Nursing Service Wards (간호⋅간병통합서비스병동 간호사의 그릿, 환자안전역량, 환자안전문화가 환자안전간호활동에 미치는 영향 )

  • Purpose : In this study, we aimed to assess the impacts of grit, patient safety competence, and patient safety culture on the patient safety nursing activities of nurses in comprehensive nursing service wards. Methods : Here, a self-reported survey of 179 nurses from three tertiary hospitals was performed. Data from 171 valid responses were analyzed using descriptive statistics, independent t -test, one-way analysis of variance, Pearson's correlation, and stepwise multiple regression analyses. Results : The key sub-factors influencing the patient safety nursing activities were attitude (𝛽=.30, p <.001) and skill (𝛽=.26, p <.001) in the patient safety competence factor and safety environment (𝛽=.26, p <.001) in the patient safety culture factor. The regression model explained 42.7% of the variance in patient safety nursing activities (F=43.29, p <.001). Conclusion : Overall, these findings highlight the importance of improving the nurse attitude and skills related to patient safety as well as the safety environment in hospitals to enhance the patient safety nursing activities in comprehensive nursing service wards. Targeted educational and training programs should be provided along with organizational support to establish a safe and supportive nursing environment in comprehensive nursing service wards.

The Association between Safety Care Activity and Documentation of Nursing Records among Nurses in General Hospitals (병원간호사의 안전간호활동과 간호기록 수행간의 관계)

  • Purpose : The purpose of this study was to identify the association between safety care activity and documentation of nursing records among nurses working in hospitals and to provide basic data for developing hospital policy for the documentation of nursing records. Methods : By using a self-reported questionnaire, data were collected from 212 nurses working in six general hospitals in Jeju province from November 2015. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and stepwise multiple regression with the SAS WIN 9.2 program. Results : Safety care activity was positively correlated with the documentation of nursing records (r=.83, p<.001). The documentation of nursing records was significantly predicted by safety care activity, working department, and nursing delivery system, and 70.9% of the variance in the documentation of nursing records was explained (F=172.31, p<.001). Conclusions : In this study, safety care activity was the most influencing factor for the documentation of nursing records. Improving work circumstances and building a system are required for nurses' safety care activity to lead to good documentation of nursing records.

Effect of Role Conflict on Job Satisfaction by Comprehensive Nursing Care Service Ward Nurse: Moderating and Mediating Effect of Resilience (간호·간병통합서비스 병동간호사의 역할갈등이 직무만족에 미치는 영향: 회복탄력성의 조절 및 매개효과)

  • Purpose : This study aimed to identify the moderating and mediating effects of resilience on the relationship between role conflict and job satisfaction among nurses working in a comprehensive nursing care service ward. Methods : Participants were 130 comprehensive nursing care service ward nurses from four hospitals. Data were collected using structured questionnaires and analyzed by t-test, ANOVA, Scheffe test, Pearson correlation coefficients, and hierarchical multiple regression analysis, using SPSS 25.0. The mediating effect of resilience on the relationship between role conflict and job satisfaction was analyzed using multiple regression analysis according to Baron and Kenny's procedure. Results : Statistically significant negative correlations were found between role conflict and resilience (r=-.25, p<.007) and between role conflict and job satisfaction (r=-.38, p<.001). A significant positive correlation was found between resilience and job satisfaction (r=.54, p<.001). A partial mediating effect of resilience was found between role conflict and job satisfaction (Z=-2.53, p=.012) but no moderating effect was found. Conclusion : To improve the job satisfaction of comprehensive nursing care service ward nurses, interventions for alleviating role conflict and improving resilience as well as evaluations of the two variables are necessary.

Transition of Simulation-Based Learning in Nursing Schools: A Concept Analysis (간호대학 시뮬레이션 교육의 이행(transition)에 대한 개념 분석)

  • Purpose : The purpose of this study was to identify the attributes, antecedents, and consequences of the transition of simulation-based learning (SBL) in nursing schools. Methods : The study was conducted in accordance with Walker and Avant's conceptual analysis process. We searched MEDLINE, CINAHL, EMBASE, Google Scholar, and RISS (Korean Education and Research Information Service) databases, resulting in nine studies for an in-depth review. Results : The attributes of transition of SBL include (1) preparing for a professional role, (2) practicing in a real clinical setting, and (3) progressing toward expected competency. Antecedents of the concept include novice status, changing roles, clinical experience in controlled settings, and expected competency in the clinical setting. Conclusion : The transition of SBL includes the important feature of progression toward expected competency. Further research is needed to identify graduate nurses' experiences during this transition to establish a strategy for improving it and developing a measurement tool that reflects attributes of the concept.

A Concept Analysis on Patient-Centered Care in Hospitalized Older Adults with Multimorbidity (복합질환을 가진 입원노인 대상 환자중심간호 개념분석)

  • Purpose : The aim of this study was to explore the attributes, antecedents, and consequences of patient-centered care (PCC) for older adults with multimorbidity in acute care hospitals. Methods : The concept analysis performed by Walker and Avant was used to analyze PCC. Fifteen studies from the literature related to PCC appear in systematic literature reviews in the fields of theology, medicine, psychology, and nursing. Results : PCC in acute care hospitals was defined according to the five attributes of 'maintaining patient autonomy', 'empowering self-care', 'individualized and relationship-based care', 'shared decision-making', and 'creating a homelike environment'. Antecedents of PCC were found to be a respect for patients' preferences, qualifications of the nursing staff, care coordination and integration, and organizational support. Consequences of effective PCC were a functional status; health-related quality of life; satisfaction with care, mortality, and medical costs from the perspective of the patient and family; and quality of care and therapeutic relationships from nurses' viewpoints. Conclusion : PCC as defined by the results of this study will contribute to the foundation of institutionalization and the creation of a safe and healthy acute care hospital culture focused on patients' preferences and values.

Factors Influencing Professional Quality of Life in Intensive Care Unit Nurses of University Hospitals (간호·간병통합서비스병동 간호사의 직무스트레스가 소진에 미치는 영향: 감성지능의 조절 및 매개효과 )

  • Purpose : This study aims to identify job stress, emotional intelligence, and exhaustion of the nurses in comprehensive nursing service units and then verify the moderating and mediating effects of emotional intelligence on the relationship between job stress and exhaustion. Methods : Participants are 118 nurses working in two general hospitals. The collected data are analyzed using the SPSS WIN 27.0 and AMOS 18.0 programs by assessing frequency and percentage, mean and standard deviation, independent t-test and one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression analysis and bootstrapping. Results : Exhaustion has a statistically significant positive correlation with job stress (r=.40, p <.001) and a statistically significant negative correlation with emotional intelligence (r=-.26, p =.004). A partial mediating effect of emotional intelligence is found between job stress and exhaustion, however, there is no moderating effect. Conclusion : It is expected that to reduce the exhaustion of nurses in comprehensive nursing service units, it is necessary to assess job stress and emotional intelligence, and strengthen emotional intelligence along with job stress intervention.

Factors Influencing Nursing Performance of Intensive Care Unit Nurses: Role of Clinical Reasoning Competence, Positive Psychological Capital, and Nursing Work Environment (중환자실 간호사의 임상추론역량, 긍정심리자본, 간호근무환경이 간호업무성과에 미치는 영향)

  • Purpose : This study aimed to investigate the impact of clinical reasoning competence, positive psychological capital, and nursing work environment on nursing performance of intensive care unit (ICU) nurses in tertiary general hospitals. Methods : Data were collected from 169 ICU nurses in tertiary general hospitals in Busan, South Korea, from July 27 to August 15, 2023, using a self-report questionnaire. After excluding 12 questionnaires with insufficient responses, 157 responses were analyzed. Descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and stepwise multiple regression analysis were employed. Results : Factors influencing nursing performance included clinical reasoning competence (β=.51, p <.001), hope as a subfactor of positive psychological capital (β=.31, p <.001), and the nursing foundations for quality of care in the nursing work environment (β=.21, p <.001). The explanatory power of the regression model was 62% (F=87.41, p <.001). Conclusion : The study underscores the importance of improving clinical reasoning competence among ICU nurses to enhance nursing performance and foster a hopeful attitude through reflective practices and personal growth. Organizational interventions, such as educational and psychological support programs, along with creating a supportive nursing environment, are crucial for improving nursing performance among intensive care unit nurses.

Reliability, Validity, and Conversion Index of the Workload Management System for Critical Care Nurses(WMSCN) (중환자 분류도구(WMSCN)의 신뢰도, 타당도 및 환산지수 검증)

  • Purpose: The purpose of this study is to establish reliability and validity and to identify the conversion index. Method: The WMSCN for ICU was revised from Workload Management System for Nurses(WMSN) of Walter Reed Army Hospital. Reliability of the WMSCN was evaluated interrater reliability between head nurses and staff nurses at 124 patients in April 2008. Validity through the correlation between direct nursing care hours and WMSCN score was conducted at 20 ICUs of 10 hospitals. Finally the conversion index was identified by total nursing hours and it divided by WMSCN score. Results: The scores by nurses were highly correlated with head nurses’(p=.967), and also scores of the WMSCN were highly correlated with the direct nursing care hours(p<.001). The distribution of patient classification ranks into class V(61.3%), class IV(24.2%) and class VI(11.3%). The scores of the WMSCN were no differences between MICU and SICU. Finally, the conversion index was 8.2 minutes. Conclusion: WMSCN is available to classify the nursing workload for critical care patients. The repeated evaluation of validity and reliability are requisite to use WMSCN effectively. And the conversion index should be adjusted to estimate the appropriate staffing in Korea.
