Evaluation of Reasonable $^{18}F$-FDG Injected Dose for Maintaining the Image Quality in 3D WB PET/CT (PET/CT 검사에서 영상의 질을 유지하기 위한 적정한 $^{18}F$-FDG 투여량의 평가)

  • Purpose: $^{18}F$-FDG injected dose to the patient is quite different between the recommended dose from manufacturer and the actual dose applied to each of hospitals. injection of inappropriate $^{18}F$-FDG dose may not only increase the exposed dose to patients but also reduce the image quality. we thus evaluated the proper $^{18}F$-FDG injected dose to decrease the exposed dose to patients considering the image quality. Materials And Methods: NEMA Nu2-1994 phantom was filled with $^{18}F$-FDG increasing hot cylinder radioactivity concentration to 1, 3, 5, 7, 9 MBq/kg based on the ratio of 4:1 between the hot cylinder and background activity. after completing the transmission scan using ct, emission scan was acquired in 3D mode for 2 minutes 30 seconds/bed. ROI was set up on hot cylinder and background radioactivity region. after measuring $SUV_{max}$ those regions, then analyzed SNR at the points. clinical experiment has been conducted the object of patients who have came to smc from november 2009 to august 2010, 97 patients without having a hepatic lesions were selected. ROI was set up in the liver and thigh area. after measuring $SUV_{max}$, the image quality was compared following the injected dose. Results: in phantom study, as the injected radioactivity concentration per unit mass was 1, 3, 5, 7, 9 MBq/kg, $SUV_{max}$ was 23.1, 24.1, 24.3, 22.8, 23.6 and SNR was shown 0.48, 0.54, 0.56, 0.55, 0.55. according to increment of the injected dose, $SUV_{max}$ and SNR was increased under 5 MBq/kg but they were decreased over 7 MBq/kg. in case of clinical experiment, as increased the injected radioactivity concentration per unit mass was 4.72, 5.34, 6.16, 7.41, 8.68 MBq/kg, $SUV_{max}$ was 2.68, 2.67, 2.26, 1.88, 1.95 and SNR was shown 0.52, 0.53, 0.46, 0.46, 0.44. if the injected dose exceeds 5 MBq/kg, showed a decrease pattern as phantom study. Conclusion: increasing $^{18}F$-FDG injected dose considered patient's body weight improve image quality within a certain range. if it exceeds the range, it can be reduced image quality due to random and scatter coincidences. this study indicates that the optimal injected dose was 5 MBq/kg per unit mass the injected radioactivity concentration in 3d wb pet/ct.

  • The possibility of inadvertent introduction of therapeutic gene expressing viral vectors has raised safety concerns about germ-line infection. Particularly, for indications such as prostate cancer and ovarian cancer, the proximity of the point of viral administration to organs of the reproductive system raises concerns regarding inadvertent germ-line transmission of genes carried by the virus vector. To evaluate the safety of in vivo adenovirus mediated gene transfer, we explored the biodistribution, persistance and potential germ-line transmission of p53-expressing adenovirus (Ad-CMV-p53). Both male and female Balb/c mice were injected with $1{\times}10^9$ PFU of Ad-CMV-p53. The PCR analysis showed that there were detectable vector sequences in liver, kidney, spleen, seminal vesicle, epididymis, prostate, ovary, and uterus. The RT-PCR analysis for detecting inserted gene, p53 showed that Ad-CMV-p53 viral RNA were present in spleen, prostate and ovary. Direct injected male and female mice of adenovirus vector into testis and ovary were mated and their of offspring were evaluated for germ-line transmission of the adenoviral vector. The PCR and RT-PCR analysis showed no evidence of germline transmission, although vector sequences were detected in DNA extracted from gonadal tissues. Real-time PCR result confirmed a significant decrease of adenovirus in gonad tissues 1 week after injection. We have also analysed the cell specific localization of viral DNA in gonad tissues by using in-situ PCR. Positive signals were detected in interstitial tissue but not in seminiferous tubule in sperm. In the case of ovary, adenovirus signal were localized to the stromal tissue, but no follicular signals were observed. Together, these data provide strong evidence that the risk of the Inadvertent germ-line transmission of vector sequences following intraperitoneal or direct injection into genito-urinary system of adenovirus is extremely low.

An Experimental Method of Calcium Absorption in Rat Small Intestine - in situ Closed Gut Segment Technique - (장관내(腸管內) 칼슘흡수 실험방법에 관한 연구)

  • The method of intestinal Ca absorption in rats was studied by injecting the test solution of $^{45}Ca$ into ligated ileal loops (an in situ closed gut sement technique). The amount of Ca absorbed was measured in regard to the amount of soluble $^{40}Ca$ and $^{45}Ca$ which disappeared from the ligated ileal loops for 30min, and appeared portal blood for 15 min after the injection. The 15~20cm ligated segment of mid ileum and the test solution containing $0.2{\sim}0.5{\mu}Ci$ $5{\mu}g$ Ca/0.5ml, pH 7.0, $37^{\circ}C$ are appropriate for this in situ closed gut segment technique which turn out to be highly available for study of Ca absorption. The rats fed casein diet absorbed more Ca by 2~5 times than the rats fed egg albumin diet.

Histopathological Studies of Witches' Broom Infected Jujube Trees Treated by Oxytetracycline (옥시테트라싸이클린을 처리한 대추나무 빗자루병 감염목의 병태해부학적 연구)

  • Histopathological changes in the witches'-broom mycoplasma (MLO) infected jujube trees after trunk injection of oxytetracycline HCl (OTC) solution were investigated by using light microscope, fluorescence microscope and electron microscope. In contrast to the healthy leaves, adaxial and abaxial bundle sheath extensions in the major veins of diseased leaves were highly developed, but mesophyll cells were very small, loose and necrosed. In the diseased leaves, excessive phloem was formed, xylem differentiation was supressed and polyphenols were accumulated. Jujube trees treated with 500ml of 2000ppm solution of OTC showed complete remission of witches'-broom symptoms above the injection site, but cells in the sheath extention were hypertrophied and sieve tube necrosis occurred in the secondary phloem of major veins. MLO-specific fluorescence was not observed in the samples of diseased jujube trees treated with OTC, while those from non-treated infectedstress distinctly showed a moderate amount of MLO-specific fluorescence. In the observation under the electron microscope, sieve element of diseased tissues and its associated companion cells were found to have collapsed. Parenchymas cells contained many large starch filled plastids and most samples of diseased and OTC-treated tissues showed an abnormal accumulation of starch compared to the healthy controls. Mycoplasma-like organisms were observed only within mature sieve elements in diseased tissues, but never in OTC-treated tissues.

  • Engine oil leaks comprise the largest percentage of the major quality problems that have occurred during the engine production process for military land system equipment over the last two years. To minimize this, it is essential to understand the root cause of the engine oil leakage and perceive the more common leakage area, as well as reduce the quality costs of pre-identification for leaks in the engine assembly process and single items. Therefore, in this study, the mass production quality of engines installed and operated in power equipment was improved by visualizing the engine oil leakage injected with the fluorescent dye and increasing the identification of the engine oil leak phenomenon. For this reason, the possibility of substituting the engine oil was considered and the engine oil was injected with the fluorescent dye. The maintenance was improved by reforming the engine oil injection process and leakage inspection process. Finally, these results suggest that the quality costs between mass production processes can be minimized by ensuring the reliability of military engines by reducing the user's complaints and improving the quality control capabilities of engine manufacturers.

The Evaluation of the Difference of the SUV Caused by DFOV Change in PET/CT (PET/CT 검사에서 확대된 표시시야가 표준섭취계수에 미치는 영향 평가)

  • Purpose: The limited FOV(Field of View) of CT (Computed Tomography) can cause truncation artifact at external DFOV (Display Field of View) in PET/CT image. In our study, we measured the difference of SUV and compared the influence affecting to the image reconstructed with the extended DFOV. Materials and Methods: NEMA 1994 PET Phantom was filled with $^{18}F$(FDG) of 5.3 kBq/mL and placed at the center of FOV. Phantom images were acquired through emission scan. Shift the phantom's location to the external edge of DFOV and images were acquired with same method. All of acquired data through each experiment were reconstructed with same method, DFOV was applied 50 cm and 70 cm respectively. Then ROI was set up on the emission image, performed the comparative analysis SUV. In the clinical test, patient group shown truncation artifact was selected. ROI was set up at the liver of patient's image and performed the comparative analysis SUV according to the change of DFOV. Results: The pixel size was increase from 3.91 mm to 5.47 mm according to the DFOV increment in the centered location phantom study. When extended DFOV was applied, $_{max}SUV$ of ROI was decreased from 1.49 to 1.35. In case of shifted the center of phantom location study, $_{max}SUV$ was decreased from 1.30 to 1.20. The $_{max}SUV$ was 1.51 at the truncated region in the extended DFOV. The difference of the $_{max}SUV$ was 25.9% higher at the outside of the truncated region than inside. When the extended DFOV was applied, $_{max}SUV$ was decreased from 3.38 to 3.13. Conclusion: When the extended DFOV was applied, $_{max}SUV$ decreasing phenomenon can cause pixel to pixel noise by increasing of pixel size. In this reason, $_{max}SUV$ was underestimated. Therefore, We should consider the underestimation of quantitative result in the whole image plane in case of patient study applied extended DFOV protocol. Consequently, the result of the quantitative analysis may show more higher than inside at the truncated region.

  • Purpose: To evaluate the efficiency of needling and injection technique without steroid for symptomatic calcific tendinitis of the shoulder. Material and Methods: We chose 12 symptomatic calcific tendinitis patients, whose ages ranged from 35 to 64. Procedure ware dry needling and injection of prolotherapic agent near the lesion. Results: All the case revealed prominent improvement without limitation of shoulder function, especially in active painful stage. Conclusion: Sonographic injection technique for calcific tendinitis would be one of the good modality not only for symptomatic relieve but also good functional recovery.

Pain Management in the Patient with Herpes Zoster (대상포진 환자의 통증치료에 관하여)

  • Fifty-one cases of acute herpes zoster infection were analyzed to search for the most effective management strategy of the disease, including pain relief and decreasing the incidence of postherpetic neuralgia. Anti-viral treatment was not included. Analgesics and nerve blocks, such as stellate ganglion block or epidural block, were helpful. Corticosteroid was administered in most of the cases either systemically or epidurally or both. Epidural administration of local anesthetic in combination with corticosteroid seemed to have certain advantages of excellent pain relief as a result of sympathetic blockade and regional plus systemic anti-inflammatory effects of the steroid, although this was not proved by definite statistical evidence.

  • 재료의 표면개질은 표면층의 조직변화에 대한 개질법과 표면피복에 의한 개질 법으로 나눌 수 있다. 조직변화에 의한 개질법으로는 침탄, 질화, 이온주입 및 금속 확산 등이 있고, 표면피복에 의한 개질법으로는 도장, 도금, 육성용접, 물리증착(PVD) 및 화학증착(CVD) 등이 있는데, 용사법은 표면피복에 의한 개질법에 속한다. 용사기술 은 비교적 최근에 발달된 표면피복 기술로서 그림1과 같이 플라즈마, 가스화염 또는 아크열원을 이용하여 금속 또는 비금속 재료를 용융 혹은 반용융 상태로 모재에 고속 도로 분사하여 충돌 적층시켜 피복하는 공정으로 다른 표면개질기술에 비해서 여러 가지 잇점을 가지고 있다. 이것은 거의 모든 재질의 모재(금속, 세라믹, 유기재료 등) 에 대해 피막의 형성이 가능하고, 용사재료의 종류도 다양하다(금속, 합금, 각종 세라 믹, 플라스틱, 각종 복합재료 등). 또한 노재크기의 제한이 없고, 대형의 재료에 대해 서 한정된 부위의 피복이 가능하며, 모재의 열영향이 적고, 피막의 형성속도가 다른 피막법에 비해 빠른 장점을 가지고 있다. 그 예로 알루미나(Al$_{2}$O$_{3}$)를 피복할 경우 화학증착(CVD)법에 의해서는 피막형성 속도가 약 2 * $10^{-4}$mm/min 인데 비해 용사법에 의해서는 약 7.5 * $10^{-1}$mm/min로 매우크다. 이와같은 많은 장점을 갖고있는 용사법을 이용한 표면개질에 대해 본 기술보고에서 서술하고자 한다.

Effects of HX106N, a Water-Soluble Botanical Formulation on Scopolamine-Induced Memory Impairment in Mice (식물성 열수 추출물 HX106N이 스코폴라민으로 유도한 생쥐 기억력 저하에 미치는 효과에 관한 연구)

  • HX106N은 용안육, 맥문동, 단삼 및 천마 등의 4가지 식물로 구성된 추출물로서, 선행 연구에서 amyloid ${\beta}$ peptide에 의한 생쥐의 기억력 저하 및 산화 손상을 억제하는 것으로 밝혀졌다. 이 연구에서는 HX106N이 비선택적 무스카린 수용체 길항제로 잘 알려진 스코폴라민(scopolamine)으로 유도한 콜린성 건망증(cholinergic amnesia)에 어떤 영향을 미치는지를 평가하였다. ICR 생쥐에게 스코폴라민(1 mg/kg body weight, i.p.)을 주입하기 1시간 전에 HX106N(100 mg/kg body weight, p.o.)을 투여하였다. 30분 후 수행한 Y-미로 시험(Y-maze test) 및 수동 회피 시험(passive avoidance test)에서 HX106N는 스코폴라민에 의해 감소되는 자발적 변경 행동(spontaneous alternation) 및 지체시간(step-through latency)을 유의미하게 억제하여 건망증을 개선시키는 것으로 나타났다. 또한 HX106N을 투약 1시간 후 생쥐의 해마와 대뇌피질 부위의 아세틸콜린에스테라제(acetylcholinesterase; AChE)의 활성을 측정한 결과 통계적으로 유의미한 정도의 활성 감소가 관찰되었다. 이러한 결과들을 종합할 때 HX106N은 AD에서 관찰되는 콜린성신경전달 장애로 인한 기억력 저하 억제에 사용될 수 있는 가능성을 가진 것으로 판단된다.
