  • 견관절은 우리 몸에서 가장 넓은 운동 범위를 나타내는 반면, 가장 불안정성한 구조를 가지고 있는 관절이다. 견관절을 이루는 해부학적 구조물들은 견관절 운동에 관여하지만 안정성 유지에도 기여하는 역할을 한다. 다음은 견관절을 구성하는 해부학적 구조물들을 주로 기능과 병리 측면에서 알아보고자 한다.

측두악관절 내장증의 외과적 치험례

  • 우측 측두악관절에 동통과 연발음을 수반한 개구장애 및 저작장애를 호소하는 환자에 있어서 측두 악관절조영술을 하므로써 관절원판부의 천공과 관절원판의 전방전위의 악관절내장증으로 진단하였으며, 악관절원판부의 성형술과 측두악관절 결절의 절제술을 시행하였다. 수술후 8주부터 최대개구가 4.5cm이였으며 측두악관절의 원활한 운동으로 별 불편없이 저작 및 개구운동이 가능하게 되었다.

Functional Electrical Stimulation for Rehabilitation of a Shoulder Joint (견관절 재활훈련을 위한 기능적 전기자극)

  • This study deals with experimental studies on electromyogram (EMG) measurements and functional electrical stimulation (FES) for the rehabilitation of a shoulder-joint. Based on the structure, motion, and main functions of the musculoskeletal system in a shoulder-joint, the muscles playing a major role for the motion in the sagittal plane were selected for the experiment. First, the surface electromyogram of the main muscles was measured according to the joint angle. The results showed that the change in the surface EMG was linearly proportional to the change in the joint angle. Second, the joint angle was measured during FES at shoulder muscles. The results showed that the joint angle increased as the FES current increased in a certain range of FES. It was confirmed that the willingness of muscles to move could be detected by measuring EMG and that the generation of muscle tension could be assisted by FES for active rehabilitation.

  • 이 연구의 목적은 대상자의 팔과 다리를 연구자가 잡아당기고, 저항을 준 상태로 대상자가 대상자의 몸통으로 가져가는 수동적 가동범위의 운동인 '저항굴곡MS운동'이 코어를 강화시켜주는 운동임을 증명한다. 또한 본 연구의 목적은 누군가의 작은 도움이 근육강화와 유연성은 물론 관절의 기능회복에 도움을 주며 류마티스 관절염 통증완화에 미치는 영향을 알아보고자하는데 있다.

  • Type I, II, III are regarded as "true" joint receptors, type IV is considered a class of pain receptor. Type I, II and III mechanoreceptors, via static and dynamic input, signal joint position, intraarticular pressure changes, and the direction, amplitude, and velocity of joint movements. Type I mechanoreceptor subserve both static and dynamic physiologic functions. Type I are found primarily in the stratum fibrosum of the joint capsule and ligaments. Type I receptors have a low threshold for activation and are allow to adapt to changes altering their firing frequency. Type II receptors have a low threshold for activation. These dynamic receptors respond to joint movement. Type II receptors are thus termed rapidly adapting. Type II joint receptors are located at the junction of the synovial membrane and fibrosum of the joint capsule and intraarticular and extraarticular fat pads. Type III receptors have been found in collateral ligaments of the joints of the extremities. Morphologically similar to Golgi tendon organ. These dynamic receptors have a high threshold to stimulation and are slowly adating. Type IV receptors possess free nerve ending that have been found in joint capsule and fat pads. They are not normally active, but respond to extreme mechanical deformation of the joint as well as to direct chemical or mechanical irritation. Small amplitude oscillatory and distraction movements(joint mobilization) techniques are used to stimulate the mechanoreceptors that may inhibit the transmission of nociceptors stimuli at the spinal cord or brain stem levels.

The effect of cervical stabilized exercise and joint mobilization on maximum muscle strength and static muscle endurance of cervical region (경추안정화운동과 경추관절가동술이 경부의 최대근력과 정적근지구력에 미치는 영향)

  • The purpose of this study was to identify cervical stabilized exercise and joint mobilization, the difference between them, and the maximum muscle strength and static muscle endurance of each group after the enforcement to general people. Cervical joint mobilization group refers to interarticular exercise (traction, pressure, glide) using with Kaltenborn technique. 3 classes were divided into 20 people each, cervical joint mobilization and stabilized exercise, and the comparison groups were randomized for the study. Both cervical stabilized exercise and joint mobilization increased maximum muscle strength and static muscle endurance. Patients should be able improve muscle stabilization and deep cervical muscle by using joint mobilization when the patient is unable to exercise on their own.

Validation of Exercise Effect in Rehabilitation during Knee Extension of the Lower Limbs using Pneumatic Exercise System (공압운동기기의 하지 슬관절 운동 시 재활운동 효과의 유효성에 관한 연구)

  • Our study is to verify validation of exercise effect that developing exercise and rehabilitation instrument using pneumatic actuators available rehabilitation and healthcare, while at the same time safer and more convenient for the user to select the desired exercise. Shares subject to the usual exercise is conducted in less than a week in the knee extension and flexion at the undergraduate level there is no more than 10 of the 20 patients were female. Subject performed knee exercise 3 set a day with 12times a set using fluid type exercise instrument and pneumatic exercise instrument that build in this study. Experiment has progressed in 4-weeks with 3-days a week. Our exercise load consist of two type. One is active exercise load with developed instrument in our study and the other one is passive exercise load with fluid instrument. Exercise program based on real-time muscle activation patterns and muscle activity before and after exercise by comparing the effects of exercise for rehabilitation has been verified. The results showed that muscular activity is higher in active exercise load than passive load, and improvement of muscle strength more increased too. Equipment more user-centered exercise more effectively exercise may enhance muscle strength was found. This study can adapt rehabilitation program and exercise for patient and weaker. Also our system can provide easily and convenient prescription of rehabilitation for expert related rehabilitation.

Effect of Whole Body Vibration Training in Drop Landing Jump (전신진동운동이 드랍랜딩점프 동작에 미치는 영향)

  • Objective: This study was conducted to investigate the effects of WBVE on biomechanical factor analysis of drop landing jumps before and after a four week training program. Methods: Participants were divided into two group: VEG (n=5, age: $25.7{\pm}2.3yrs$, height: $170{\pm}7.6cm$, weight: $69.3{\pm}8.3kg$) and OEG (n=5, age: $24.6{\pm}3.4yrs$, height: $164{\pm}4.9cm$, weight: $58.8{\pm}9.2kg$). Ten infrared cameras (Vicon, UK) with a sampling rate of 100 Hz were used in two GRF measurement systems (AMTI, USA). Results: The variability of the center of mass was largest at the falling phase, and there was a great risk of injury when landing after the jump. Second, the jumping heights of the VEG (vibration exercise group) were higher than those of the OEG (only exercise group), regardless of training period. Third, there were significant differences in the hip joint P1 (flexion) and knee joint P2 (extension) between the exercise groups after 4 weeks of training. However, there were no significant differences among training periods or phases. Conclusion: regardless of training periods, the VEG showed increased jump height compared with the OEG, but the consistency between the jump height and the lower joint power could not be determined.

  • Biomechanical analysis of arm motion during steering was performed using a motion analysis technique. Three-dimensional position data for each part of arm are fed into an interactive model combining a musculoskeletal arm model and the mechanical steering system to calculate joint angles and torques using inverse kinematic and dynamic analyses, respectively. The analysis shows that elbow pronation/supination, wrist flexion/extension, shoulder adduction/abduction, and shoulder flexion/extension have significant magnitudes. Sensitivity analysis of the arm joint motion with respect to seating posture and steering wheel configuration is carried out to investigate the qualitative influence of the seating posture and driver's seat configuration on the steering behavior.

  • The purpose of this study is to see how the sling lumbar stabilization exercise affects muscle body type and subjective pain degree of 17 middle-age men who are diagnosed with sacroiliac joint syndrome and investigate the effectiveness. The subjects of this study are patients who have been diagnosed with sacroiliac joint syndrome through tests like X-ray in S Orthopedics in C City and have had chronic lumbar pain for more than 6 months and the before and after results about subjective pain degree through three dimension Moire Muscle Body Type Tester and subjective pain degree through VAS Pain Awareness after sling lumbar stabilization exercise 3 times a week, 30 minutes a time, total 24 times for 8 weeks have revealed the following: The change of muscle body type has been comprehensively improved in the statistics before and after the exercise and the subjective pain degree has also comprehensively decreased in the statistics. Therefore, sling lumbar stabilization exercise is proved to give positive affect to muscle body type and pain improvement of the patients with sacroiliac joint syndrome.
