Initiation and Termination of Electromyographic Activity in the Early Hemiparetic Wrist (초기 편마비 환자의 손목에서 근수축 개시 및 종료의 지연)

  • 본 연구는 초기 편마비 환자의 손목에서 표면근전도 분석을 통해 근수축 개시 및 종료의 특성들을 알아보고, 임상적인 치료방법의 기초를 제안하고자 실시하였다. 연구대상자는 원주기독병원에 뇌졸중으로 입원한 환자 중 발병 후 3개월 미만인 13명과 원주시에 거주하는 대조군 7명이었다. 근수축 개시 및 종료의 지연은 표면근전도를 이용하여 손목굽힘근과 손목펴짐근에서 손목관절의 굽힘과 폄동작 시 3초의 근전도 신호음에 따라 가장 빠르고 강하게 최대 등척성 수축과 이완을 하여 신호를 수집하였다. 그 결과 편마비 환자의 환측은 건측과 대조군에 비해 손목관절 굽힘과 폄동작에서 근수축 개시및 종료가 유의하게 지연되었으며, 개시보다 종료가 더 유의하게 지연되었다. 따라서 초기 뇌졸중 환자의 근약화는 근육의 개시 및 종료의 반응시간 지연에 영향을 준다고 볼 수 있다. 앞으로 운동조절과 연관되어 기능적인 회복을 유도할 수 있는 근수축의 민첩성 훈련과 근섬유 동원의 효율성을 증진시키기 위한 치료방법들이 연구되어야 할 것이다.

A Home Network System for General Paralyzed Patients based on an Eye Mouse (안구 마우스를 이용한 전신마비 환자용 홈 네트워크 시스템)

  • 본 연구에서는 전신마비 환자들이 안구 마우스를 사용하여 전등, 선풍기, 창문 제어 및 침대 각도 조절을 할 수 있고, 보호자에게 소변통 알림과 응급메시지 알림을 보낼 수 있는 홈 네트워크 시스템을 설계하고 구현한다. 또한 전신마비 환자들뿐만 아니라 일반 사용자들도 이용할 수 있도록 동일한 기능을 모바일 어플리케이션으로 구현한다. 본 연구의 입력장치인 안구 마우스는 삼성전자 창의개발연구소에서 개발한 eyeCan을 사용하였다. 전체적인 시스템은 Arduino를 사용하여 제어하고, 무선 통신은 Xbee모듈을 사용하여 구현하였다. 기존의 홈 네트워크 시스템은 일반 사용자들만 이용할 수 있지만, 본 연구에서는 루게릭 환자처럼 전신을 움직이지 못하는 환자들에게 최적화된 홈 네트워크 시스템을 구축하여 소외계층의 스마트 기기에 대한 접근성을 높이고, 자립심을 키울 수 있도록 도와준다는 점에서 의의가 있다.

  • Gastroparesis is rare in children and is defined as delayed emptying of gastric contents into the duodenum without mechanical obstruction. We experienced a case of gastroparesis in a 7-year-old boy after a viral illness. He was admitted because of excessive abdominal bloating and diffuse abdominal pain, and was diagnosed by clinical manifestations and measurement of the gastric emtpyting time. He recovered after dietary management and with commbined medication of erythromycin and domperidone in 50 days of illness.

A Study on the Relationship between Body Function and Prelusive Movement to Falls to Promote Wellness in Chronic Stroke Patients (만성뇌졸중 환자의 웰니스 증진을 위한 신체기능과 낙성전조동작의 관련성 연구)

  • This study was conducted to investigate the effects of a participatory rehabilitation program on sit-rise and rise-to-walk test performances, and perception and motor skills in adults with medically vulnerable individuals and, adults with developmental disabilities in particular. Seventeen adults with developmental disabilities participated in a participatory rehabilitation program using resistance bands and exercise balls, for 60 minutes once weekly over 13 weeks. Their performances were measured before and immediately after the intervention, and 12 weeks after. The findings were as follows. In the sit-rise test, the number of times rising from sitting posture increased after the intervention versus before, but the difference was not statistically significant. In the rise-to-walk test, the performance showed statistically significant difference over time, and the post-hoc test showed a significant effect after the intervention versus before. There was no significant difference in perception and motor skills. In sum, the participatory rehabilitation program positively influenced dynamic balancing related to functional activities but had no significant effect on perception and motor skills, which is related to motor control and motor learning. It is suggested that to increase the participation in community activities, reduce fall risk, and improve dynamic balancing abilities in adults with developmental disabilities, participatory rehabilitation programs should be utilized to promote the physical wellbeing.

A Systematic Review of Occupational Therapy Interventions for Children With Cerebral Palsy: Focus on Single-Subject Research Design (뇌성마비 아동을 위한 작업치료 중재에 대한 체계적 고찰: 국내 단일대상연구를 중심으로)

  • Objective : The purpose of this study was to identify the characteristics of a single-subject research study and qualitative levels in which occupational therapy interventions were performed on children with cerebral palsy. Methods : This study targeted papers, published in Research Information Sharing Service (RISS), National Digital Science Library (NDSL), Koreanstudies Information Service System (KISS), and E-article from May 20 to 29, 2022. The search terms were 'cerebral palsy' AND 'single subject research design' OR 'individual subject study'. Eleven papers, were finally selected and analyzed. Results : Most of the studies were medium in methodological quality, and the subjects were pre-school age and spastic hemiplegia. Among the single-subject designs, intervention-removal designs were the most common, and among them, ABA designs were the most common. Interventions included assistive devices, constraint-induced therapy, neurodevelopmental therapy, and sensory integration therapy were 2, and upper extremity exercise, interactive metronome, and CO-OP were 1. Dependent variables were measured with 2 to 4 measurement tools, Significant improvements were found in postural control ability, gait and balance, hand function, and upper extremity function. Conclusion : This study confirmed that it is helpful to apply cerebral palsy occupational therapy by presenting the characteristics of cerebral palsy, intervention sessions and effects, measurement tools and methodological quality levels.

Speech Rate and Pause Characteristics in Speaker with Flaccid Dysarthria (이완형 마비말장애 화자의 말속도와 쉼 특성)

  • The purposes of this study were to investigate the characteristics of speech rate and pause in patients with flaccid dysarthria. For this purpose, 15 patients with flaccid dysarthria and 15 normal speakers matched in gender and age participated as subjects. The overall speech rate, the articulation rate, the inter-sentence and the intra-sentence pause duration and pause frequency were measured during reading the standardized passage "Autumn"(Kim, 1996). As a result, the overall speech rate and articulation rate of patients with flaccid dysarthria were significantly slower than normal speakers and intra-sentence pause duration and frequency of patients with flaccid dysarthria were significantly higher than normal speakers, but those of inter-sentence weren't. The results from this study provides the speech rate index of flaccid dysarthria and indicates that to control the speech rate of flaccid dysarthria have regard to not only the overall speech rate and the articulation rate but also the intra-sentence pause duration and the frequency.

A Systematic Review of Task-Oriented Training to Improve the Physical Function and Activities of Daily of Living of Children With Cerebral Palsy (뇌성마비 아동의 신체기능 및 일상생활 향상을 위해 적용된 과제 중심 훈련의 체계적 고찰)

  • Objective : This study aimed to analyze and classify the task-oriented training methods used to improve the daily lives of children with cerebral palsy. Methods : The search period from January 2008 to August 2020, and the CINAHL, MEDLINE and PubMed was databases were used. A total of 18 studies was selected, and the PICO method was applied to conduct the systematic review. The training methods were then classified according to task, practice, and feedback as strategies to learn or to exercise control based on prior research. Results : Of the selected studies, 13 reported significant results in the area of motor skills, two of which related to Activities of Daily Living. Discrete and closed were the most common forms of task, and the most used types of practice were whole, part, and blocked, in that order. Finally, feedback was not specifically provided in most studies, followed by extrinsic feedback. Conclusion : Based on this review, task-oriented training for children with cerebral palsy can be made more effective in clinical practice in the future through the systematic selection of techniques that promote exercise control and the presentation of specific methods.

Effect of Intensive Trunk-Pelvic Stabilization Training Using Sling on Muscle Activity and Balance in Hemiplegia (편마비 환자에서 슬링을 이용한 체간-골반 안정성 집중 훈련이 근활성도 및 균형능력에 미치는 영향)

  • This study was to investigate the effect of intensive trunk-pelvic stabilizing training using sling exercise on trunk muscle activity and balance in patients with hemiplegia. Twenty hemiplegic patients(6 month ${\leq}$, 1 years >) participated were divided into two groups randomly. Control group(n=10) had only general rehabilitation training and experimental group(n=10) had both the general rehabilitation training and intensive trunk-pelvic stabilizing training using sling. They were treated for 30 min./5 times/4 week. I measured ability of trunk control, muscle activity, and balance which were done before and after the intensive training using sling. In significant difference after training, show a significant difference in TIS(p<.001), in particular static(p<.05), dynamic(p<.01), coordination(p<.05) and muscle activity of RA(p<.001), EO(p<.001), ES(p<.001), Multifidus(p<.05), FMA(Balance)(p<.01), and static(p<.001) & dynamic(p<.001) balance ability on MTD-Balance system. As this study proved effective therapeutic exercise of trunk muscle activity and balance ability on intensive trunk-pelvic stabilizing training using sling, it could identify importance of trunk-pelvic stabilizing and control on balance.

A comparative study of the acoustic characteristics of the vowel /a/ between children with spastic and dyskinetic cerebral palsy (경직형과 불수의운동형 뇌성마비아동의 /아/ 모음 음향학적 비교)

  • This study aims to compare the acoustic characteristics of vowel phonation in children with spastic and dyskinetic cerebral palsy (CP). Thirty-four children aged 4-12 years with CP participated in the study (spastic 26, dyskinetic 8). Voice samples for the acoustic analysis were extracted from a sustained vowel /a/. All acoustic measures were made using Praat. Group differences were compared by an independent t-test or Welch-Aspin test, if the equivalence assumption was not met. The results of this study are as follow. First, maximum phonation time(MPT) was significantly shorter for the dyskinetic CP than for the spastic CP. Second, shimmer percent was significantly increased in the dyskinetic CP than in the spastic CP. Lastly, there were no significant group differences in both the first formant and the second formant. These findings indicate that the dyskinetic CP has a poorer respiratory capacity and poorer laryngeal function than the spastic CP. On the other hand, both groups have a comparable ability to articulate the vowel /a/. The results of the present study help speech language pathologists identify the speech motor control ability of children with two types of CP (spastic and dyskinetic) and help to make an intervention plan associated with a specific type of CP.
