CT Scan Findings of Rabbit Brain Infection Model and Changes in Hounsfield Unit of Arterial Blood after Injecting Contrast Medium (토끼 뇌감염 모델의 CT 소견과 조영제 주입 후 동맥혈의 Hounsfield Unit의 변화)

  • This paper explores CT findings of a rabbit brain infection model injected with Escherichia coli and investigates the changes in Hounsfield unit (HU) of arterial blood over time. The brain infection model was produced by injecting E. coli $1{\times}10^7$ CFU/ml, 0.1 ml through the burr hole in the calvarium; 2~3 mm in depth from the dura mater, and contrast-enhanced CT, dynamic CT and arterial blood CT images were gained. It was found that various brain infections such as brain abscess, ventriculitis and meningitis. The CT image of brain abscess showed a typical pattern which the peripheral area was strongly contrast-enhanced while the center was weakly contrast-enhanced. The CT image of ventriculitis showed a strong contrast-enhancement along the lateral ventricle wall, and the CT image of meningitis showed a strong contrast-enhancement in the area between the telencephalon and the diencephalon. In dynamic CT images, the HU value of the infection core before injecting contrast medium was $31.01{\pm}3.55$. By 10 minutes after the injection, the value increased gradually to $40.36{\pm}3.76$. The HU value in the areas of the marginal rim where was hyper-enhanced showed $47.23{\pm}3.12$ before contrast injection, and it increased to $63.59{\pm}3.31$ about 45 seconds after the injection. In addition, the HU value of the normal brain tissue opposite to the E. coli. injected brain was $39.01{\pm}3.24$ before the injection, but after the contrast injection, the value increased to $49.01{\pm}4.29$ in about 30 seconds, and then it showed a gradual decline. In the arterial blood CT, the HU value before the contrast injection was $87.78{\pm}6.88$, and it increased dramatically between 10 to 30 seconds until it reached a maximum value of $749.13{\pm}98.48$. Then it fell sharply to $467.85{\pm}62.98$ between 30 seconds to 45 seconds and reached a plateau by 60 seconds. Later, the value showed a steady decrease and indicated $188.28{\pm}25.03$ at 20 minutes. Through this experiment, it was demonstrated that the brain infection model can be produced by injecting E. coli., and the characteristic of the infection model can be well observed with contrast-enhanced CT scan. The dynamic CT scan showed that the center of the infection was gradually contrast-enhanced, whereases the peripheral area was rapidly contrast-enhanced and then slowly decreased. As for arterial blood, it increased significantly between 10 seconds to 30 seconds after the contrast medium injection and decreased gradually after reaching a plateau.

Image Analysis on Upper Gastrointestinal(UGI) Series of Gastric Cancer (위암환자의 위장조영검사 영상분석)

  • Despite significant advance in laparoscopy, upper gastrointestinal (UGI) series is still generally carried out for preoperative localization of gastrectomy. The aim of the study was to compare the accuracy of UGI series and postoperative pathological findings in the diagnosis and localization of gastric cancer. A retrospective review was carried out for 102 patients who underwent gastrectomy between October 2007 and April 2009. Preoperative reports of UGI series were compared with postoperative pathology reports and the accuracy of the preoperative reports was calculated. Two radiographer retrospectively reviewed the analysis of UGI series. postoperative pathology reports of the surgical specimens, were compared with the preoperative reports of the location and extent of the tumor were correct in 81 case("sure", 79.4%) and incorrect in 21 case("failed, 20.6%). In 92 case UGI series revealed intestinal metaplasia (90.2%) at consensus review and these results demonstrate the limitation of the UGI series in the diagnosis of type IIb gastric cancer with size less than 1.0cm and the poor detection of gastric cancer is that the overlying mucous membrane often appears to be normal in these patients. In conclusion, UGI series is accurate the detection of the tumor localization and diagnosis of intestinal metaplasia. However, for the overcome with the limitation of UGI series should be used accurate technique for the region of the stomach. To achieve this goal, it is necessary to determine the changes of the mucus membrane of the stomach and UGI series is gaining acceptance as a standard method for preoperative gastric cancer screening.

Study on Shielding using CT Contrast Medium (CT 조영제를 이용한 차폐체에 대한 연구)

  • Currently, shields for shielding medical radiation during medical examinations in the medical environment are lead robe and lead glass. Lead, the main component of this shielding, has limitations in lead poisoning and light weight, and high price. Iodine, which is used as contrast medium instead of lead shield, is expected to be effective as a shield because it has radiation absorbing properties. The purpose of this study was to evaluate the effectiveness of shielding by using acrylic plate filled with CT contrast agent for clinical use instead of conventional lead glass. As a result, it was found that the acrylic plate filled with the CT contrast agent showed a shielding effect of 7 times or more when the scattering ray dose was not shielded. Therefore, CT contrast agent composed of iodine is expected to be used as a shield instead of conventional lead glass.

  • One of the main purposes in the treatment of developmental dislocation of the hip is to achieve and maintain concentric, congruent, and stable reduction. The arthrogram performs an important role in the diagnosis and treatment of developmental dislocation of the hip. The arthrogram provides much information about the soft tissue status of the hip joint. Limbus and ligamentum teres is exactly evaluated so that we can plan the reduction and treatment before operation. Eighteen preoperative hip arthrograms of 17 children treated for developmental dislocation of the hip from 1992 to 1998 were reviewed. The limbus, ligamentum teres and transverse acetabular ligament were compared with the pathoanatomy seen at the time of open reduction. Arthrography proved to be reliable in identifying the limbus and ligamentum teres. So we recommend that arthrography must be performed before closed or open reduction. Also, we recorded the radiographic parameters: acetabular index, acetabular floor thickness, center edge(CE) angle of Wiberg, and Y-coordinate. The center edge(CE) angle of Wiberg obtained from arthrography was measured more accurately than from simple roentgenograms because the ossification of the femoral head was frequently located eccentrically in the developmental dislocation of the hip.

A Study on the Heating of Lipiodol during Lymphangiography (림프관 조영술 시 리피오돌의 가온에 관한 고찰)

  • The study was conducted to improve the efficiency of the test and to reduce the exposure dose of patients and operators by analyzing the difference in the moving speed of Lipiodol according to the temperature during lymphography. The device for injecting Lipiodol at a constant pressure was self-made, and after inserting Lipiodol into the Connecting Tube, the moving speed of the contrast agent was photographed at temperatures of 26℃, 36℃, and 46℃ using a heat transfer device. Lipiodol movement time from the Support Catheter to 20cm was measured and analyzed, and statistical significance was confirmed. In the 46℃ environment, the average moving time was 11 seconds, at 36℃ the average was 13 seconds, and at 26℃ the average was 17 seconds. Lipiodol showed a significant difference in moving time with increasing temperature (p<.001), and it was confirmed that the higher the temperature, the faster the moving speed. In the case of lymphangiography, when heated to a certain temperature (46 degrees) rather than injecting Lipiodol at room temperature, the injection speed can be increased and the speed of movement in the lymphatic vessel can be improved.

  • Radiographic contrast medium may cause tissue injury by extravasation during intravenous automated injection during CT examination. A large - volume extravasation (140 mL) occurred in an adult during contrast-enhanced CT The patient had a swelling and injury on the dorsum right hand of intravenous catheter region. The extravasation injury site was determined by CT scanning. The extavasation compartment syndrome case was examined using four separate display techniques. These 3D MDCT findings might help to determine the best course of treatment for patient with contrast extravasation. 3D image reconstructions provide accurate views of high-resolution and soft-tissue imaging. This paper introduces extravasation with the radiography and 3D MDCT findings.

  • This paper proposes a method to extract coronary arteries effectively in the angiography, In general. DSA(Digital Subtraction Angiography) is a well-established technique for the visualization of coronary arteries, DSA involves the subtraction of a mask image, an image of a heart before the injection of contrast medium, from a live image, However, this technique is sensitive to the movement of background and can cause wrong detection due to the variance of background intensity between two images. Therefore, this paper solves the structural problem resulted from background movement by selecting an image which has the least difference of movement through the similarity analysis of background texture, and it extracts only the blood vessels effectively through local intensity correction of the selected images, Experimental results show that the proposed method has the lower false-detection rate and higher accuracy rate than existing methods.

Comparative Analysis of Quantitative Signal Intensity between 1.0 mol and 0.5 mol MR Contrast Agent (1.0 mol 과 0.5 mol MR조영제의 정량적 신호강도 비교분석)

  • The purpose on this research is quantitatively comparing and analyzing signal intensity of 1.0mol and 0.5mol contrast agent. For this study, two MR phantoms were produced. One of them is used with 1.0mol Gadobutrol. The other is used with 0.5mol Gadoteridol. These two phantoms respectively have been scanned by SE T1 sequence which is used to get a general contrast-enhanced image in 1.5T MRI and 3D FLASH sequence which is used as enhanced angio MRI. Signal intensity was measured by scanned images as per contrast agent dilution ratio. The results were as follow: RSP(Reaction Starting Point) of the two sequences(2D SE, 3D FLASH) was respectively 6.0%, 60.0% in 0.5mol contrast and 2.0%, 20.0% in 1.0mol contrast, which means in 0.5mol contrast, RSP was formed faster than the one in 1.0mol contrast. MPSI was respectively 1358.8[a.u], 1573[a.u] in 0.5mol contrast and 1374[a.u], 1642.4[a.u] in 1.0mol contrast, which means 0.5mol contrast's MPP (0.4%, 10.0%) was formed faster than 1.0mol contrast's MPP (0.16%, 1.8%). Lastly, RA as per contrast agent dilution ratio was 27.4%, 11.8% wider in 0.5mol contrast(20747.4[a.u], 23204.6[a.u]) than in 1.0mol contrast(12691.9[a.u], 20747.4[a.u]). According to the study, we are able to assure that signal reaction time of 1.0mol contrast is slower than the one of 0.5mol contrast in contrast-enhanced MRI at two different sequences(2D SE, 3D FLASH). Furthermore, owing to the fact that there are not any signal intensity differences between 1.0mol and 0.5mol contrast, it is not true that high concentration gadolinium MR contrast agent does not always mean high signal intensity in MRI.

A Study of The Correlation of The Area Dose with Residual CT Contrast Media and MRI Contrast Media during The Use of General Imaging Automatic Exposure Control System (일반촬영 자동노출제어장치 사용 시 잔존 CT 조영제와 MRI 조영제에 따른 면적선량의 상관성 연구)

  • The purpose of this study is to investigate the effect of CT contrast agent and MRI contrast agent on the area dose in the body when using automatic exposure control system in general radiography. After making rectangular holes in the center of the abdominal thickness paraffin phantom, CT contrast agent and MRI contrast agent were respectively diluted with physiological saline solution for contrast medium dilution ratio of 10:0, 9:1, 8:2, 7:3, 6:4, 5:5, 4:6, 3:7, 2:8, 1:9, 0:10%. Each experiment was set to 78 kVp, 320 mA, which is the proper condition for KUB photography, and thereafter a total of 30 inspections were made for each dilution ratio using an automatic exposure control device, and the area dose corresponding to the dilution ratio of each contrast agent, Average comparison and correlation analysis were performed on the exposure index. As a result, the CT contrast agent and the MRI contrast agent appeared different in area dose according to the dilution ratio(p<0.05), and as the dilution ratio increased, the area dose increased for CT contrast agent and MRI contrast agent(P<0.05). In each test, the exposure index showed the manufacturer's recommendation of 200-800 EI value, and the exposure index and area dose increased as the area dose increased(p<0.05). In conclusion, CT contrast agent and MRI contrast agent confirmed to increase the area dose by general imaging test using all automatic exposure control device. Therefore, it is considered that it is necessary to perform it after the contrast medium has been excreted sufficiently when using usual imaging test after using the contrast agent in CT and MRI examination.

Optimization of Protocol for Injection of Iodinated Contrast Medium in Pediatric Thoracic CT Examination (소아 흉부 CT검사에서 조영제 주입에 관한 프로토콜의 최적화)

  • The purpose of this study is to establish a physiological injection protocol according to body weight, in order to minimize amount of contrast medium and optimize contrast enhancement in pediatric patients performing thoracic CT examinations. The 80 pediatric patients under the age of 10 were studied. Intravenous contrast material containing 300 mgI/ml was used. The group A injected with a capacity of 1.5 times its weight, and groups B, C and D added 5 to 15 ml of normal saline with a 10% decrease in each. The physiologic model which can be calculated by weight about amount of injection of contrast medium and normal saline, flow rate and delay time were applied. To assess image quality, measured average HU value and SNR of superior vena cava, pulmonary artery, ascending and descending aorta, right and left atrium, right and left ventricle. CT numbers of subclavian vein and superior vena cava were compared to identify the effects of reducing artifacts due to normal saline. Comparing SNR according to the contrast medium injection protocol, significant differences were found in superior vena cava and pulmonary artery, descending aorta, right and left ventricle, and CT numbers showed significant differences in all organs. In particular, B group with a 10% decrease in contrast medium and an additional injection of saline showed a low degree of contrast enhancement in groups with a decrease of more than 20%. In addition, the group injected with normal saline greatly reduced contrast enhancement of subclavian vein and superior vena cava, and the beam hardening artifact by contrast medium was significantly attenuated. In conclusion, the application of physiological protocol for injection of contrast medium in pediatric thoracic CT examinations was able to reduce artifacts by contrast medium, prevent unnecessary use of contrast medium and improve the effect of contrast enhancement.
