THE SPACE OF CONGENITALLY MISSING OF PRIMARY CANINE WITH ODONTOMA (치아종을 동반한 선천적 결손된 유견치의 공간)

  • Space loss of dental arch can appear when the proper position of teeth within the dental arch changes by a certain cause, because the balance of force makes changes about tooth position as well as alignment. The causes of space loss include proximal caries, early extraction, congenital missing of a tooth and hypodontia, etc. Among those causes of space loss, congenital missing of a tooth is more rarely observed in the primary dentition than in the permanent dentition. Congenital missing in the primary dentition is associated with that in the permanent dentition. Furthermore, it can cause space problem, such as mesial tilting or drift of adjacent teeth, space loss for permanent successors and dental arch constriction, etc. Primary lateral incisors is the most commonly involved, in the maxilla rather than in the mandible, but primary canine is rarely reported. In this patient, who visited the department of pediatric dentistry at Yonsei university dental hospital, it was observed that the maxillary right primary canine was congenitally missing and an odontoma was found insteadly. However, neither the space loss for the congenitally missing primary canine nor midline deviation is remarkable during the 2-year-10-month observation period. In addition, any clinical or radiographical symptom did not occur in spite of odontoma. Therefore, surgical enucleation of odontoma is planned according to the eruption of permanent lateral incisor or canine, unless eruption failure of permanent lateral incisor or canine nor cystic change around the odontoma is occurred. Through further evaluation, space maintainer or orthodontic treatment may be necessary.

  • Early childhood caries which mainly affect maxillary anterior area, defined as 'the presence of 1 or more decayed, missing, or filled tooth surfaces in any primary tooth in a child 71 months of age or younger'. Extraction of teeth when early childhood caries affected in maxillary primary incisors often develops progressively, result in severe destruction of crowns, acute or chronic pulpitis, and periapical abscess formation. Maxillary primary incisors are need to preserve as possible, because the early loss of maxillary primary incisors may lead to various functional, esthetical, and psychological problems. It is necessary to the availability of an easy to perform technique capable of providing efficient, durable, functional, and esthetic restorative methods. Polyethylene fiber-reinforced post can be used in strengthen of composite resins that is esthetic and good physical and mechanical properties. $Ribbond^{(R)}$ is made from an polyethylene fiber, has numerous usages, its surface is treated to enhance adhesion to resins, ease of manipulation, relatively cost effective. We report this case, had restored of maxillary primary incisors with severe coronal destruction due to affecting severe early childhood caries, using polyethylene fiber-reinforced posts, composite resin cores, and celluloid crowns. We could obtain good result of treatment.

  • The term 'impaction teeth' is used to designate a tooth which remains unerupted in jaw beyond the time at which it should normally be erupted. Most cases of impacted teeth reported in the literature are permanent teeth. The impaction of primary teeth occur rarely whereas impaction of second primary molars is more numerous than all other impactions. Failure of eruption of primary teeth may cause a number of complications, such as interference with development and eruption of successive permanent teeth, malocclusion, cystic change of tooth follicle. The clinican should consider the various treatment option available (a) No treatment and observation, (b) surgical extraction (c) space regainer. Proper treatment plan should be established after thought consideration of impacted tooth and it's relation with successive permanent tooth.

  • In the traumatized teeth, the major complications are pulp necrosis, and root resorption. The factors influencing to prognosis are the state of root development, type of injuries, quality of fixation. There are good prognosis in the case of immature teeth, minor luxation injuries. To minimize of occurrence of these complication, it is very important to determine the pulp vitality. This can help us decide whether or not to treat the teeth endodontically. But, it is difficult to determine of pulp vitality in immature teeth or traumatized teeth. In this case, pulp vitality test was done periodically on the traumatized upper central incisors, and the results were different among cold test, electric pulp test, and laser Doppler flowmetry. The conclusions were obtained from this case can be summarized as follows; 1. Pulp vitality of traumatized teeth should be negative early, but with time going, pulp vitality could be recovered. 2. The positive response of pulp vitality test was detected earlier in laser Doppler flowmetry. 3. Between cold test and electric pulp test, cold test was more reliable in determining pulp vitality.

  • Ectopic eruption is caused by an abnormal direction of eruptive path, most common in maxillary first molar, mandibular lateral incisor, and maxillary canine, and sometimes mandibular first molar. Ectopic eruption of first molar leads to abnormal root resorption of second deciduous molar, which, if left untreated, could cause premature loss of second deciduous molar; mesial tilting and rotation of first permanent molar; lack of space for eruption of second premolar; and occlusal problems. Therefore early treatment is advised when diagnosed as ectopic eruption. Treatment of ectopic eruption in the first permanent molar involves providing proper guidance for the direction of eruption using interproximal wedging and distal tipping methods while preserving second deciduous molar. This case report shows satisfactory results of the ectopic eruption of mandibular first molars in young patients who were treated with Humphrey appliance and Halterman appliance.

THREE-DIMENSIONAL EVALUATION OF IMPACTED MAXILLARY CANINES USING CONE BEAM COMPUTED TOMOGRAPHY AND PANORAMIC RADIOGRAPHS (Cone beam CT와 파노라마방사선사진을 이용한 매복 상악 견치의 3차원적 분석)

  • Normal eruption of the canine is important for the transition to the permanent dentition. Etiologies, including premature loss or delayed retention of deciduous teeth, neoplasm and abnormality of lateral incisor can cause impaction of the maxillary canine. Untreated canine impaction can result in malocclusion, cyst formation and obstacles in orthodontic treatment. The aim of this study is to evaluate location of the impacted maxillary canine and to identify correlation between location and management of the impaction including complications. Using panoramic radiographs and CBCT scan, images of 89 children diagnosed with impaction of the maxillary canine, location of impacted canines was evaluated. The choice of treatment and complications were investigated to identify correlation. Results show that the most commonly impacted location of the maxillary canine was in the mid-alveolar area, followed by buccal side and palatal side. Orthodontic traction was selected more frequently than the other treatments. As complications, displacement of adjacent tooth was occurred most frequently. Buccally impacted canines showed increased tendency towards displacement. The more buccally the canine was impacted, the less orthodontic traction was chosen as the treatment. The canine impacted mesially to the central incisor showed increased tendency to occur root resorption. Therefore, early diagnosis by periodic examination, appropriate treatment and intervention is required.

Transportation and Distribution Temperatures Affect Fruit Quality and Physiological Disorders in 'Wonhwang' Pears (수송 및 유통온도에 따른 '원황' 배 품질 및 생리장해 발생)

  • 'Wonhwang' pear ($Pyrus$ $pyrifolia$ Nakai) often showed physiological disorder and quality deterioration during long-term storage and shelf life although this cultivar has the advantage of a good appearance and better taste for overseas exportation. This study was conducted to investigate the effect of temperature during transportation and continuing market temperature on fruit quality, and the occurrence of physiological disorder to set up the appropriate transportation and distribution temperatures in the fruits harvested at different times. Unripe fruits harvested 120 days after full bloom maintained higher firmness until 21days of shelf life at both 18 and $25^{\circ}C$ than late harvested ones. Otherwise, ripe fruits harvested 130 days after full bloom showed high incidence of mealiness breakdown of flesh tissues in 21 days of shelf-life at $25^{\circ}C$ of market temperature, but not in the fruits stored at $18^{\circ}C$. Late harvested fruits showed much severe physiological disorders when compared with early harvested ones that showed relatively lower respiration rates. Fruit quality parameters were more highly affected by market temperature than transportation temperature. Results showed that unripe fruits harvested at 120 days after full bloom maintains high marketability until 21 days of shelf-life, while the maximum shelf-life of ripe fruits harvested 130 days after full bloom will be considered at within 7 days.

Experimental Evaluation of Fire Behavior of High-Strength CFT Column with Constant Axial Load (일정축력하에 고온을 받는 고강도 콘크리트 충전강관 기둥의 구조적 거동에 관한 연구)

  • Fire-resistant (FR) test data for a square concrete-filled steel tube (CFT) columns consisting of high-strength steel (fy>650MPa) and high strength concrete (fck>100MPa) under axial loads are insufficient. The FR behavior of square high-strength CFT members was investigated experimentally for two specimens having ${\Box}-400{\times}400{\times}15{\times}3,000mm$ with two axial load cases (5,000kN and 2,500kN). The results show that the FR performance of the high-strength CFT was rapidly decreased at earlier time (much earlier at high axial load) than expected due to high strength concrete spalling and cracks. In addition, a fiber element analysis (FEA) model was proposed and used to simulate the fiber behaviour of the columns. For steel and concrete, the mechanical and thermal properties recommended in EN 1994-1-2 are adopted. Test results were compared to those of numerical analyses considering a combination of temperature and axial compression. The numerical model can reasonably predict the time-axial deformation relationship.

SURGICAL TREATMENT OF HEAVY MANDIBULAR LABIAL FRENUM IN PRE-SCHOOL CHILD WITH A HISTORY OF SYNDACTYLY SURGERY : A CASE REPORT (합지증 수술 병력이 있는 어린이에서 하악 거대 협소대의 외과적 처치)

  • The lower labial frenum attached to the free gingival margin can promote local tension, resulting in tissue ischemia, promoting the development of gingival recession, as well as complicating oral hygiene, resulting in chronic inflammation. In this case, early diagnosis and surgical treatment is recommended. This is the case about surgical treatment of heavy mandibular labial frenum in pre-school child with a history of syndactyly surgery. A 5-year-old girl visited the clinic with the chief complaint of high labial frenum of the mandible. Hyperplastic lower labial frenum was attached to the free gingival margin on the primary mandibular lateral incisor area. After fifteen month follow-up, right after the eruption of the permanent lower right lateral incisor, 6 years old patient received lower labial frenectomy to prevent periodontal diseases in permanent teeth and to reestablish normal anatomic characteristics. After 2 years of follow-ups, there were no marked complications.

THE SUCCESS AND FAILURE OF TREATMENT BY USING LIP BUMPER (Lip Bumper를 이용한 치료의 성공과 실패)

  • In the mixed dentition, many children have mild crowding of anterior dentition on the mandibular arch and mesial tilting of mandibular molar. Lip bumper have been used to gain arch length for the alignment of mild to moderate crowded dental arches. As such, they may provide an alternative to extraction therapy. The claimed therapeutic effect of the lip bumper is bodily forward incisor movement, flaring of the lower incisors, and distal tipping of the molar. The dental changes can be attributed to removal of lip pressure on the lower anterior dentition and the distal forces exerted at the molar abutment. The purpose of this study is to show more easily method of treatment for mild anterior crowding of mandible.
