Patients complaining with somatic symptoms are very common in clinical practice, and are often consulted to medical department. But it is difficult to treat well. The treatment of somatic symptom disorder is multi-modal as none of the methods on their own provide a satisfactory outcome. The treatment of somatic symptoms disorders is complicated by lack of boundary, conceptual clarity, and overemphasis on psychosocial causation and effectiveness of psychological treatments. In clinical practice all classes of psychotropics are used to treat somatic symptoms disorder. Drugs such as tricyclic antidepressants, serotonin reuptake inibitors(SSRI), serotonin and noradrenalin reuptake inhibitors (SNRI), atypical antipsychotics are studied. The evidence indicates that these drugs are effective in somatic symptom disorders. All classes of antidepressants seem to be effective against somatic symptom disorders. SSRIs are more effective against hypochondriasis and body dysmorphic disorder, and SNRIs appear to be more effective than other antidepressants when pain is predominant. The author suggest that psychiatrists should know how to treat patients complaining with somatic symptoms by using not only psychotherapeutic approach but also pharmacological treatment. It will be helpful to reduce suffering and increase quality of life of these patients.
Parkinson's disease (PD) is clinically characterized by a variety of motor and non-motor symptoms, including cognitive and neuropsychiatric symptoms. Integrating a large variety of symptoms into a small number of clinical subtypes could be valuable for appropriate and early therapeutic intervention. As a first step toward this aim, this study attempted to identify correlation patterns among motor, cognitive and neuropsychiatric symptoms in PD without dementia. One hundred four non-demented patients with PD underwent a comprehensive motor, neuropsychological, and neuropsychiatric assessments. Factor analysis was performed to identify correlation patterns among demographic, motor, cognitive and neuropsychiatric variables. The eight factors were extracted: 1 motor-related, 3 cognitive-related and 4 neuropsychiatric factors. We indentified that characteristics of correlation can have associated symptom pattern in the disease process of Parkinson's disease. The current results suggest that a broad range of motor and non-motor symptoms in PD may be reducible to a small number of clinical parameters, which may be useful for identifying clinical subtypes of PD for individual patients.

Objectives : This study was designed to investigate the effect of sleep quality on depression symptoms and the mediating effect of interpretation bias and anxiety symptoms in psychiatric patients. Methods : Data accumulated for outpatients and inpatients in the Department of Mental Health Medicine at Hanyang University Guri Hospital were used. The measurement tools were Pittsburgh Sleep Quality Index (PSQI), Ambiguous/Unambiguous Situations Diary-Extended Version (AUSD-EX), Beck Anxiety Inventory (BAI), Beck Depression Inventory-II (BDI-II). Correlation analysis and bootstrapping analysis were conducted using SPSS 25.0 and SPSS Macro based on 162 patient data. Results : As a result of the study, the double mediating effect of interpretation bias for Ambiguity and anxiety symptoms was significant in the relationship between sleep quality and depression symptoms. Conclusions : In this study, it was confirmed that low sleep quality sequentially affects anxiety and depression symptoms through interpretation bias for ambiguity. Based on this, it is expected that the development of other psychiatric symptoms can be prevented by preferentially performing therapeutic intervention on preceding symptoms.

Objectives : Although subclinical depression symptoms are associated with suicidal idea, most research have focused on clinical depression such as major depressive disorder or dysthymia. The aim of this study is to investigate network structure of depressive symptom and to reveal which symptoms are associated with suicidal ideation. Methods : We used part of data from the seventh Korea National Health and Nutrition Examination Survey. Participants were between 19 and 65 years of age (N=8,741). Network analysis with Isingfit model is used to reveal network structure of depressive symptoms and most central symptom and edges assessed by patient health questionnaire (PHQ-9). Results : The most two central symptoms were psychomotor activity and suicidal ideation. The strongest edge was psychomotor activity-suicidal ideation. Suicidal ideation also has strong association with depressive mood and worthlessness. Conclusions : These results suggest that psychomotor activity and suicidal ideation can serve as treatment target for subclinical depression and psychomotor activity, worthlessness and depressed mood may be important factor for early intervention of suicidal ideation.
This study examines the effects of recovery-oriented practices on psychiatric symptoms of people with mental disorder and further explores the impact of recovery-oriented practices in each subcategories on psychiatric symptoms, thereby demonstrating the intent of restoring the community mental health rehabilitation center. For this, a survey was conducted on 378 people with mental disorder who used community mental health rehabilitation center and correlation and hierarchical regression analysis were conducted. The study found that among the five subcategories of recovery, including age and duration of illness, 'Life Goal,' 'Involvement,' and 'diversity of treatment options' alleviate symptoms of people with mental disorder. Based on these findings, community mental health rehabilitation center has presented the combined significance of psychiatry and social welfare programs to facilitate the recovery of the mentally handicapped by reflecting the value and factors of recovery-oriented practice.
Objectives: The purpose of this study was to investigate whether longitudinal changes in positive and negative symptoms affect cognitive functioning in chronic schizophrenia. Methods: Sixty-eight patients diagnosed with DSM-IV schizophrenia were examined on two occasions over 6 months for symptoms and cognitive changes. Symptoms were measured by PANSS. Cognitive functions were examined for sustained attention, executive function, concentration and attention, and verbal memory and learning using Degraded Stimulus Continuous Performance Test, Wisconsin Card Sorting Test, Digit Span, and Rey Auditory Verbal Learning Test, respectively. Twenty control subjects were assessed to compare the cognitive scores of remitted schizophrenic patients. Results: Patients showed significant improvement in symptoms and all cognitive tests after 6 months treatments. Significant improvements in positive and negative symptoms did not predict improvements in any aspect of cognitive functioning measured. Normal controls performed significantly better than remitted schizophrenic patients on all cognitive tests. The results show no relationship between change in symptoms and change in cognition in chronic schizophrenia. Conclusion: We suggest that symptomatic and cognitive impairment may be a distinct construct. These findings highlight the importance of treating cognitive impairment in addition to the clinical symptoms of schizophrenia.
Objectives : Aims of the study were to investigate the physical and mental symptom profiles related with perceived life stress in normal populations. Methods: The study subjects were 186 subjects who visited the Health Promotion Center at National Health Insurance Corporation Ilsan Hospital between July and December, 2000. We measured Life Stress Assessment Scale (LSAS), SCL-90-R (Symptom Check List-90 Revised), and Physical Health Check Questionnaires to evaluate perceived life stress for one year, and physical and mental symptoms. The physical markers related with stress were also measured. According to LSAS scores, we selected two groups from all subjects : Group I (LSAS score with lower 25 percentile) and Group II (LSAS score with higher 75 percentile). Each number of subjects was 46 in Group I and 47 in Group II. We analyzed stasistical significances of study variables between Group I and II. Results : Group II subjects showed higher scores in all subscales of SCL-90-R compared to Group I subjects. Comparing to Group I, Group II subjects complained more physiological symptoms related with anxiety and depression. But there were no statistical differences in the physical markers related stress between two groups. Conclusion: We found that past year-life stress caused various mental and physical symptoms, which didn't develop any physical illness related life stress in normal populations yet.

Objective : The purpose of this study was obtaining data on the efficacy and safety of risperidone in child and adolescent psychiatric patients. Method : Thirty one children and adolescents (males n=18, females n=13, age ranged from 5.4 to 17.3 years) treated with risperidone were selected among child and adolescent psychiatric inpatients of Seoul National University Hospital from January, 2001 to June, 2002, and charts for them were reviewed retrospectively. Results : The primary psychiatric disorders treated with risperidone were schizophrenia and other psychosis, bipolar I disorder with psychotic features, Tourette's disorder, autism spectrum disorders, mixed receptive and expressive language disorder, attention deficit-hyperactivity disorder, conduct disorder and obsessive-compulsive disorder. twelve of these had comorbid mental retardation. Primary target symptoms of risperidone were psychotic symptoms (n=13 or $41.9\%$), behavioral symptoms (n=10 or $32.3\%$) including aggression, impulsivity, hyperactivity, stereotypy nonresponsive to other psychiatric treatments, and chronic and severe tics (n=8, $25.8\%$). The efficacy of risperidone was measured by clinical global improvement (CGI) for target symptoms, $67.7\%$ of subjects showed moderate or marked improvements and its therapeutic effect appeared to be maintained during at least 7.5 months. Mean daily dosage of risperidone was $0.05{\pm}0.01mg/kg$, the group with psychotic symptoms had significantly higher mean daily dosage (0.07mg/kg) compared with other two groups (0.04mg/kg) with behavioral symptoms or tics. A variety of adverse events were reported in this study : weight gain (n=23) most commonly reported, extrapyramidal symptoms (n=15), autonomic symptoms (n=6), sedation (n=5) and symptoms related to hyperprolactinemia (n=2) etc. Although there was no drug change related to the adverse events of risperidone, and $90\%$ of subjects at their last visits were maintained on it, thus its tolerability appeared good. Conclusions Results suggest that risperidone may be relatively safe and effective drug in managing a wide variety of child and adolescent psychopathologies such as psychotic symptoms, behavioral symptoms including aggression, impulsivity, hyperactivity and stereotypy nonresponsive to other psychiatric treatments, and chronic and severe tics. Controlled and long-term studies of efficacy and safety of risperidone treatment for children and adolescents are recommended in the future.

Objectives : Thyroid function is associated with psychiatric disturbance such as mood symptoms, cognitive functions, anxiety and sleep problem. We evaluate the effects of thyroid dysfunction on negative symptoms of schizophrenia, mood symptoms and somatic symptoms in patients with schizophrenia. Methods : Sixty five patients with schizophrenia were recruited. The patients were classified into two groups; the patients with the history of abnormal thyroid indices level(group of abnormal thyroid function) and the patients without the history of normal thyroid indices level(group of normal thyroid function). At baseline and 8 weeks later, psychiatric symptoms were assessed with the Scale for the Assessment of Negative Symptoms (SANS), the Scale for the Assessment of Positive Symptoms(SAPS), and Beck Depression Inventory(BDI), Somatosensory Amplication Scale(SSAS). Results : During 8 weeks, there were significant differences in the changes of SANS, BDI, SSAS between group of abnormal thyroid function and group of normal thyroid function. Compared to group of normal thyroid function, group of abnormal thyroid function showed greater reduction of the scores of SANS(23.3%), BDI (19.6%), and SSAS(16.2%), respectively. However, there was no significant correlation between the scores of SANS, BDI, and SSAS. Conclusion : Our study suggested that abnormal thyroid indices would predict the prognosis of negative symptoms, mood symptoms, and somatic symtpoms in patients with schizophrenia.
