A Study on the conceptualization of disability of the Mental Health Act and Related Laws (정신장애인 관련 법령의 정신장애개념과 정신보건법의 정신장애개념 개정방향에 관한 연구)

  • The purpose of this study was firstly to analyse the conceptualization of disability of the Mental Health Act and related laws and to suggest revision of the definition of psychiatric disability of Mental Health Act to improve the protection of human rights and welfare of the people with psychiatric disability. For these purposes this paper reflected on the disability models of WHO(1980, 2001) and multiple paradigm of disability of Priestley(1998) and suggested a new conceptualization of disability consists of impairment, functional disability, and social interactional disability. Based on the analyses of conceptualizations of psychiatric disability of related laws, this study suggested revision of Mental Health Act to distinguish between mental disorder and psychiatric disability, to introduce the definition of mental capacity, and to expand the conceptualization of social interactional disability.

  • The movement disorders in psychiatry have been neglected, though it is an important psychiatric dimension to exert unfavorable influence on patients'quality of life. The etiologies of movement disorders in psychiatry can be classified as primary neurological disorders, psychiatric comorbidities of neurological disorders, manifestations of primary psychiatric disorders, drug-induced movement disorders and psychogenic movement disorders. For the rapid and proper treatment for movement symptoms and signs easily observed from psychiatric patients, psychiatrists' ability toward precise disgnosis and differential diagnosis of movement disorders should be preceded.

Factor Structure of the Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile in Infants and Toddlers With Mental Disorders and the Difference Among Mental Disorders (정신장애 영유아에 대한 K-CBCL 1.5-5 (Korean-Child Behavior Checklist 1.5-5) 조절곤란 프로파일의 요인구조와 정신장애 간 차이검증)

  • Object : This study aimed to identify the factor structure of Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile (K-CBCL 1.5-5 DP) in infants and toddlers with mental disorders and verify differences in K-CBCL 1.5-5 DP among the diagnosis groups. Methods : The participants were 265 mothers of infants and toddlers with mental disorders who completed K-CBCL 1.5-5 DP. The data was analyzed using AMOS 25.0 and SPSS 25.0. Results : First, the bifactor model was the most suitable for the factor structure of the K-CBCL 1.5-5 DP. Second, there were significant differences among the diagnosis groups, such as communication disorders, pervasive developmental disorders, emotional disorders, and developmental delays. It was confirmed that the pervasive developmental disorder and emotional disorder groups showed significantly higher dysregulation compared with the communication disorder group. Conclusion : This study confirmed that infants and toddlers had dysregulation problems. Using the bifactor model, the multidimensional nature of the K-CBCL 1.5-5 DP was assessed. It was also meaningful that dysregulation could contribute to onset and deepening of symptoms of pervasive developmental disorders and emotional disorders in infancy.

The Discourse associated with mental illness on TV documentaries : The Completion of Distinction (TV 다큐멘터리가 생성한 정신장애 담론 : 구별짓기의 완성)

  • This paper discusses the type of discourse associated with mental illness and individuals with mental illness in the context of TV documentary. Discourse is an linguistic product which prescribes and interprets the reality and reconstructs the reality systematically. Therefore, TV documentary contents illuminate the dominant discourse associate with mental illness through the diverse types of representation. We picked four TV documentaries from each public channels and analyzed these documentaries using Fairclough's Critical Discourse Analysis. Faircough suggests the analysis frame consisting of three level. The analysis reveals that TV documentaries produce the discourse "the Completion of Distinction" associated with mental illness and individuals with mental illness. TV documentaries suggest the reason why we distinct them from us in textual level. In discourse practice level, they suggest the method and the principal agent of distinction. For social practice, TV documentaries reinforce the dual attitude of viewer. Alternative discourse associated with mental illness and individuals with mental illness will be constructed when individuals with mental illness recovers the status of principal agents and produces strong voices about themselves.

  • 당뇨병과 정신건강은 어떤 관련성이 있을까? 당뇨병이 스트레스나 우울증과 관련이 있다는 것은 비교적 잘 알려져 있는데 다른 정신건강 문제와도 관련이 있을까? 당뇨병을 꾸준히 관리하는 것만도 수월하지 않은 당뇨병 환자에게 정신건강 관리를 하라는 것은 또 다른 부담을 가중시킬 수 있을 것이다. 그러나 효과적인 당뇨병 관리를 위해서는 정신건강에 관심을 기울이고 함께 관리하지 않을 수 없다. 건강하고 안정적인 정신건강 상태는 당뇨병 관리의 뒷받침이 되므로 당뇨병의 효과적인 관리를 위해 선행되어야 할 요소이다. 종종 정신건강 문제는 당뇨병 관리의 방해요인이 되기 때문에 이를 방지하기 위해서도 정신건강 관리는 당뇨병 관리와 함께 이루어져야 한다. 스트레스, 우울, 불안 등 정신건강 문제들이 어느 정도 해결될 때, 비로소 당뇨병 자가관리를 지속할 수 있으며 그 효과를 거둘 수 있는 것이다. 대부분의 당뇨병 환자는 진단기준을 충족시키지는 않더라도 적어도 가끔은 정신장애 증상을 경험할 수 있다. 일부 환자는 진단기준에 부합하는 장기간의 심각한 정신과적 증상으로 고통을 받기도 한다. 당뇨병 진단 후 당뇨병을 심리적으로 수용하고 관리하는 과정에서 정신장애를 경험하게 될 가능성이 높아진다. 정신장애도 만성질환의 속성을 가지므로 정신장애를 동반한 당뇨병 환자는 만성질환 관리의 부담이 가중될 수 있다. 따라서 정신장애를 최대한 예방하고 적시에 적절한 치료를 통해 심화되지 않도록 하는 것이 중요하다. 이를 위해 당뇨병과 관련 있는 정신장애에 대한 이해와 지식을 갖추는 것이 필요하다.

  • 본 연구는 정신적 장애인의 소득수준과 직업능력에 따른 취업욕구를 살펴보고 그에 따른 취업 활성화를 방안을 제안하고자 하였다. 이를 위해 '2008년~2010년 장애인 고용패널조사'에 3년간 참여한 정신적 장애인 298명(가중빈도적용:154,348명)을 연구하였다. 연구결과, 직업교육을 희망하는 정신적 장애인이 남성, 중졸 이상, 자격증 소지자, 그리고 정신적 장애인 유형중 지적장애일때 교육참여 희망률이 높았다. 한편, 취업의사를 가진 정신적 장애인은 남성, 가구주, 중졸이상, 자격증 소지자, 육체활동 능력이 높아질 때, 그리고 정신적 장애인 유형 중 자폐장애일 때 취업의사가 높았다. 이러한 결과를 토대로 정신적 장애인의 경제활동참여를 높이기 위하여 정신적 장애인의 의사를 반영한 직업재활서비스제공, 사업주 주도적 직업활동을 제안하였다

Latent Means Analysis of Parenting Competency, Parenting stress, Resilience, Social support according to the disability types among disabled women (여성장애인의 장애유형별 자녀양육역량, 양육스트레스, 회복탄력성, 사회적 지지에 대한 잠재평균분석)

  • This study aimed to examine disabled women to determine whether differences existed in parenting stress, resilience, social support, and parenting competency based on the disability type using an latent means analysis. The research data was sampled from 167 mentally disabled women and 132 physically disabled women. Parenting stress and social support had higher latent means in the mentally disabled women. Parenting competence and resilience had higher latent means in the physically disabled women. The results of this study suggested that differentiated, practical intervention approaches should be implemented for each disability type.

Comparison of Alexithymia among Patients with Psychosomatic Disorders, Anxiety Disorders and Depressive Disorders (정신신체장애, 불안장애 및 우울장애 환자들 간의 Alexithymia의 비교)

  • A comparison was made regarding the degree of alexithymia among patients with psychosomatic disorders, anxiety disorders and depressive disorders. The author examined the degree of alexithymia in three groups : 100 psychosomatic patients(including 47 patients with tension headache), 52 outpatients with anxiety disorden, and 50 outpatients with depressive disorders. Alexithymia was assessed by Alexithymia provoked Response Questionnaires(APRQ) developed as a semi-structured interview form. No significant difference was found in the degree of alexithymia among Patients with Psychosomatic disorders, anxiety disorders, and depressive disorders. On the other hand, patients with tension headache were significantly more alexithymic than patients with anxiety disorders and depressive disorders, respectively. However, there was no significant difference in degree of alexithymia between patients with anxiety disorders and those with depressive disorders. Multiple regression analysis revealed that demographic variables such as set age, education level, and marital status did not make a significant influence on alexithymia scores. These results suggest a greater degree of alexithymia in patients with a specific group of psychosomiatic disorders such as tension headache than in patients with emotional disorders, unlike the previous report that in general, psychosomatic patients are alexithymic. Thus, it is necessary to develop special forms of interview which can induce and encourage expression of emotion as a therapeutic strategy for patients with tension headache.

Perception of Mental Health Recovery of Mentally Disordered Offenders Hospitalized at the National Forensic Psychiatric Hospital : Contents Analysis (국립법무병원에 입원 중인 정신장애 범죄자의 정신 건강 회복에 대한 인식: 내용 분석)

  • The purpose of this study was to identify perception of mental health recovery of mentally disordered offenders hospitalized at the national forensic psychiatric hospital. The participants were 91 mentally disordered offenders hospitalized at the national forensic psychiatric hospital in C province. The participants were provided with a written questionnaire about mental health recovery so that they could freely describe it, and collected data were analyzed by two researchers according to Krippendorff's content analysis method. As a result, meaning of mental health recovery were derived from 4 categories and 17 themes, and four categories were 'liberate from psychiatric symptoms', 'stable daily and social life is possible', 'living a hopeful and meaningful life' and 'life without crime after discharge'. mental health recovery facilitators and barriers were derived from 4 categories and 14 themes, 'treatment factors', 'personal factors', 'interpersonal factors' and 'environmental factors'. The results of this study can be used as basic data for education and intervention development for mental health recovery of mentally disordered offenders at the national forensic psychiatric hospital.

Judgment on the Criminal Responsibility of Perpetrators with Mental Disorders and Their Mental Examination (정신장애 범죄인의 책임능력 판단과 정신감정)

  • This article focuses on §10 of Criminal Act of the Republic of Korea to discuss how to determine criminal responsibility of a perpetrator suffering from mental disorders, while reviewing existing process and standards of mental examination at each stage of the criminal procedure as well as exploring suggestions regarding how they should be complied. The determination of the sanity or criminal responsibility of the mentally ill as defined in the §10 of Criminal Act, by its nature, cannot be approached with a traditional, clear-cut dichotomy of biology by medical practitioner and psychology·normative science by lawyer. Looking into the actual procedure of determining mental and physical disorder with special consideration of mental illness reveals the inevitability of collaboration between lawyers and psychiatrists. In the meantime, the process and standards of mental examination at each stage of the criminal procedure must be definitive and clear. First of all, during the investigation stage, examination prior to prosecution should be more actively encouraged, considering that judging sanity of the perpetrator at the time of committing a criminal act is important. During the trial stage, the mandatory examination must be conducted depending on the sensitivity and gravity of the case. Next, medical examination to determine criminal responsibility and the one to order treatment and custody must be separately conducted in order to properly execute medical treatment and custody. Obligatory mental examination could be considered both during the stage of request for and execution of the treatment and custody. Lastly, the procedure of examination and format of examination documents need to be standardized for better objectivity and reliability.
