  • The purpose of this study supplies the basic data to plan the optimum mental health service that is reflected needs of the elderly people with mental illness by checking how their demographic-sociological distinctive feature and function level have an effect on their needs of mental health service by being absent situation of a study of the elderly people with mental illness. As results of analysis, the residental type, the disease period, and the decision person of admission-discharge control of demographic-sociological distinctive feature is much effect on needs of programs and also effect commonly on the needs of the health promotion program and the discharge support program. Generally, the function level of the elderly people with mental illness is favorable and the order of function level is the group activity program, the counseling program, and the resource network program.

  • The crimes and the vandalism committed by mental patients such as the burning of Syungremoon and Daegu's subway have become a social insecure factors. In the paper, the criminal status of mental patients is analyzed and the range of probabilities at sea investigated. The countermeasures to these crimes are proposed.

An Influence of Psychosocial Rehabilitation Services to Independent Living Competency of People with Mental Illness (정신재활서비스가 정신장애인 자립생활 역량에 미치는 영향)

  • The purpose of this study is to examine the effect of psychosocial rehabilitation services on the independent living of the mentally ill people. The study was conducted using mentally ill people who were using services offered by psychosocial rehabilitation service center during the study period, have lived independently, and used the service center for three months or longer. The data analysis included 231 surveys collected from 42 agencies and then perform hierarchical regression analysis. As a study result, the number of types of psychosocial rehabilitation service used by the mentally ill people and the greater the satisfaction with the psychosocial rehabilitation service, the more independent living competency of the mentally ill people was received. This means that community-based integrated psychosocial rehabilitation service have the effect of contributing to strengthening the competency of independent living with the mentally ill people. Based on the research results, this study proposed the development of integrated mental rehabilitation service to improve the independence competency of the mentally ill people.

  • REM sleep behavior disorder is parasomnia characterized by unpleasant dreams and dream-enactment behaviors associated with excessive electromyography activity in REM sleep. This may appear idiopathic or secondary to other neurological or medical conditions. REM sleep behavior disorder, which appears to be idiopathic, most often implies the possibility of later neurodegenerative diseases due to synucleinopathy, so accurate diagnosis is important in predicting prognosis. For the diagnosis of REM sleep behavioral disorder, REM sleep without atonia, which appears in the polysomnography, is essential. Obstructive sleep apnea, trauma-related sleep disorders, and vigorous periodic leg movements during sleep are known as diseases that show dream enactment behavior in elderly patients. Effects of Discrimination Experience and Social Support on Physical and Mental Health among the Disabled - Mediation Effect of Disability Identity and Multiple Group Comparison by Degree of Disability - (고령장애인의 차별경험과 사회적 지지가 신체적 건강 및 정신적 건강에 미치는 영향 - 장애정체감의 매개효과와 장애정도에 따른 다중집단 분석 -)

  • In this study, we explore whether and how the experience of discrimination and social support affects physical health and mental health through disability identity among the disabled aged 50 and over. We also examine such relationship differs by the degree of disability. For the empirical test, we utilized the data from the Panel Survey for the Disabled conducted by our research team in 2016. According to structural equation model analysis, social support has a direct effect on disability identity. It also has a direct effect on physical health. As for the mental health, there are found direct effects of discrimination experience, social support, disability identity, and physical health, Furthermore, social support influences the mental health through disability identity. This finding suggests that social support for the disabled in old age increases disability identity and, in turn, it contributes their mental health. In the meanwhile, multiple group comparison analysis reveals the effect of discrimination experience on disability identity is found for those with lower level of disability but not for their counterparts. This seems to result from that the level of common ground which constitutes disability identity is low for the severely disabled. Thus, it requires our efforts to enhance disability identity focusing on common ground for the severe disabled.

  • The College entrance examination stress syndrome is a kind of anxiety disorder. The underlying cause of this disorder is not a test anxiety itself. One's hidden inner and familial conflicts are more likely attributed to this disorder. Patient's the most common complaints are various psychosomatic symptoms but in severe cases underlying psychopathology may be activated and progress to major psychosis. In a broad sense adolescent's delinguent behavior, drug abuse, school drop-out and sucide are closely related to this syndrome. In clinical management of these patients, considering the special situation of impending examination, the therapist must access to central conflict theme in a short time without severe resistance. The authors classified this syndrome into 5 clinical subtypes ; 'the anxious group', 'the exhaustion group', 'the despair group', 'the emptyness group' and 'the boredome group'. Typical case of each subtype and it's management methods were presented briefly.

CLINICAL EVALUATION OF CHILDREN WITH INATTENTION AND HYPERACTIVITY IN A PSYCHIATRIC CLINIC (주의산만과 과잉운동을 주소로 하는 정신과 내원 아동들의 임상 평가)

  • The aim of this study is to examine the diagnostic profiles and related clinical variables of children with attention and hyperactivity in psychiatric outpatient clinic. Seventy one children with age range of 5 to 14 were diagnosed by DSM-IV, and assessment battery including KEDI-WISC, KPI-C, ADS(ADHD Diagnostic System) were completed. The subjects were divided into 3 diagnostic groups:ADHD only(n=17), ADHD comorbid(n=27), Other diagnosis(n=27). The results were as follows:In ADHD comorbid group, tic disorder, developmental language disorder, borderline intellectual function, oppositional defiant/conduct disorder, and learning disorder were combined in descending order. Other diagnosis group consisted of tic disorder, borderline intellectual function, depression/anxiety, oppositional defiant/conduct disorder, and others. There were significant differences in IQ, PIQ, and VIQ among the three groups, and ADHD only group showed higher scores of IQ and VIQ than ADHD comorbid group. On the KPI-C, there were no significant differences in all subscales among the three groups. On the visual ADS, omission error and sensitivity showed significant differences among the three groups, and ADHD comorbid group represented higher omission error and lower sensitivity than other diagnostic group. The findings indicated that the inattention and hyperactivity symptoms could be diagnosed into diverse psychiatric disorders in child psychiatry, and ADHD children with comorbidity will show more problems in academic performance and school adjustment.

A Modified Attribution-Affection Model of Public Discrimination against Persons with Mental Illness -Model comparisons among schizophrenia, depression and alcoholism- (정신장애인의 사회적 거리감에 대한 수정된 귀인정서모형 적용 - 정신장애 유형별 모형비교 -)

  • Recently, the many anti- stigma program use the 'mental illness is an illness like any other biogenetic illness' approach. This is based on Weiner's attribution affection theory. However, mental illness is difficult to be applied with attribution affection premise because attributing no blame for mental problem(biogenetic cause) leads to fear and dangerousness. We proposed a modified attribution affection model that explains the relations between biogenetic causal belief and social distance. Our model assumed that attributing personal responsibility for each mental problem leads to anger and social distance. And attributing no blame for mental problem(biogenetic causal belief) reinforces perception of dangerousness and social distance. This study presented typical vignettes of schizophrenia, depression and alcoholism according to the diagnosis criteria of DSM-IV to 768 university students randomly. Path analysis was used to test modified attribution affection model. The major findings are, First our original model modified partially for fit index. So final model assumed that i) The more respondents believed personal responsibility, the more anger, the more anger reaction corresponded closely with more social distance. ii) biogenetic causal beliefs leads to a worsening of dangerousness and perception of dangerousness leads to a increasing of social distance. Second, multi-group analysis was conducted to verify how a modified attribution affection model would be applicable to three groups. The result is that there is no difference among three groups. Finding from this research suggest to change anti-stigma program that use medical model.

Dysfunctional Breathing in Anxiety and Depressive Disorder (불안-우울 환자에서 역기능 호흡)

  • Objectives : Although dysfunctional breathing is a common symptom in general population and affects qualities of life, it is still underdiagnosed. There are some studies of prevalence of it in astma, but few studies in anxiety and depressive disorders. The purposes of this study were to explore the prevalence of it in anxiety and depressive disorders, and to investigate whether anxiety and depressed mood influence it. Methods : 135 patients diagnosed with anxiety or depressive disorders, and 124 controls were recruited. Nijmegen questionnaire was used to assess dysfunctional breathing, and Hospital anxiety depression scale was used. Results : The prevalence of dysfunctional breathing in anxiety or depressive disorders was higher than that in control. In the linear regression model, anxiety accounted for 59.6% of dysfunctional breathing, but depressed mood did not. With covariate adjusted for anxiety, scores of dysfunctional breathing in anxiety or depressive disorders were higher than in controls. Conclusions : Dysfunctional breathing in anxiety or depressive disorders is higher than that in control. Adjusting anxiety, its difference is still. Anxiety affects dysfunctional breathing, but depressed mood does not.

The Judgment of Criminal Liability and Psychiatric Evaluation for Mentally Defective Person (정신장애자의 형사책임능력 판단과 정신감정)

  • The Korean Criminal Code ${\S}10$ (1) provides that "The act of a person who, because of mental disorder, is unable to make discriminations or to control one's will, shall not be punished". Therefore, it'll not be able to be given criminal punishment if a mentally defective person is determined to lack the criminal liability. The problem about judging the criminal liability for the mentally defective person exists in areas where the criminal law intersects with psychiatry. Although the supreme court ultimately judges whether the criminal liability by mental defectiveness exists or not, the judgment of mental defectiveness, which is biological element, needs psychiatric knowledge and it is no wonder to rely on this. In particular, a change is required in the procedure and contents of mental examination for a mentally defective person as implementation of the Civil Participation in Criminal Jury Trial. It is needed the improvement of procedure to submit more accurate mental examination and the result of it in order that jurors are able to understand the result of mental examination and make an decision. This is because jurors consisting of ordinary citizens take part in trial. For guaranteeing the precise result of mental examination in the criminal justice procedure, it is necessary to establish the pool of manpower consisting of psychiatrists or psychologists who have completed the specific educational programs about the criminal justice and legal psychiatry, and it is desired to carry out the psychiatric test with selecting appraisers who belong to a pool of manpower. Furthermore, it is required to draw up and submit the written appraisal of mental examination which is easy to be known because of considering the nonprofessional of jurors consisting of ordinary citizens in the Civil Participation in Criminal Jury Trial. In order to gain a fair verdict of the jury about whether mental defectiveness exists or not, it is recommended the prompt submission of the written appraisal of mental examination, the presentation of the written appraisal of mental examination summarizing the important contents, and making out the written appraisal of mental examination for jurors to understand it easily.
