연구목적 : 남북한의 정신의학 체계나 내용이 매우 다를 수 있지만 현재까지 북한의 정신의학에 대한 정보가 거의 없다. 본 연구에서는 북한의 정신의학의 현황에 대해 알아보려 하였다. 방 법 : 남한에 거주하고 있는 북한이탈주민 중 북한에서 10년 이상 의사 경력이 있던 3명을 면담하였다. 정신건강의학과 환자 진료 경험, 북한 정신의학의 실상, 북한의 정신건강의학과적 치료, 일반 북한 주민들의 정신장애 인식도, 북한의 자살에 대해 반구조화 면담이 진행되었다. 결 과 : 북한의 정신건강의학과는 49호라고 불리며, 대부분 확연한 정신병적 증상이 있는 경우에만 정신건강의학과에서 진료를 하였으며, 비정신병적 우울증이나 불안장애는 정신건강의학과에서 주로 다루지 않는 편이었다. 정신장애의 병인은 유전적 이유 등 생물학적 요소에 집중되었으며, 사회심리적 요소나 정신역동적 요소는 정신장애의 병인으로 중시되지 않았다. 정치이념적 요소와 정신의학은 별개로 간주되었다. 주로 입원과 생물학적 치료를 시행하였으나, 인슐린 혼수 요법과 같이 상당히 뒤떨어진 치료법이 사용되고 있었다. 사회 전반적으로 정신장애에 대한 편견은 심하였다. 북한에서는 자살을 민족 반역으로 간주하며 자살이 드물었다. 결 론 : 북한의 정신의학 현황은 남한의 정신의학 현황과는 큰 차이를 보였다. 과거 사회주의 국가의 정신의학과 유사한 점이 많지만, 북한 고유의 정신건강의학과적 특성도 가진 것으로 판단된다.
Objective : The objective of this study is to determine the effects of fear of Middle East respiratory syndrome (MERS) on socio-psychological health during an outbreak of MERS and the post-traumatic stress as a mediator on the relationship between stress and socio-psychological health. Methods : Visual Analog Scale, Impact of Event Scale -Revised, Psychosocial well-being index short form was implemented for 150 medical persons who worked at the hospital in which exposure to MERS cases had been confirmed and 96 ordinary people. A Pearson correlation coefficient and a hierarchical multiple regression was carried out to confirm the effect of fear of MERS and the mediating effect of post-traumatic stress between fear and socio-psychological health. Results : The higher the fear, the lower the socio-psychological health in both healthcare workers and the public (r=0.32, p<0.01) and the higher post-traumatic stress (r=0.32, p<0.01). But, the research results showed that only healthcare workers had a partially mediating effect of post-traumatic stress in the relationship between fear and socio-psychological health (${\beta}=0.45$, t=6.33 p<0.001), (${\beta}$ value : 0.39>0.26). Conclusion : This study demonstrated that the post-traumatic stress can indirectly lead to a negative effect on the socio-psychological health of healthcare workers when under the fear of MERS and shows adverse effects on psycho-social wellbeing. This suggests that clinical intervention and psycho-social approach aiming at reducing post-traumatic stress is important to maintain mental health during crisis development.
Objective : The aims of this study were to investigate the prevalence of co-morbid depression and related factors of depression in mild cognitive impairment (MCI) patients. Methods : Nine hundred and six MCI individuals were included in this study. Depression was defined as major and minor depressive disorder according to the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV). Depression-related demographic and clinical factors were also explored through multivariate logistic regression analysis. Results : In MCI patients, the prevalence of depression as defined according to the DSM-IV diagnostic criteria was 1.8% for major depressive disorder, 11.4% for minor depressive disorder, and overall 13.2% for both. Multivariate logistic regression showed that increased prevalence of overall depression was associated with female gender and non-amnestic subtype of MCI. Conclusion : Our findings based on a large number of MCI subjects who visited memory clinic indicated that more than ten percent of MCI patients have comorbid depression. In addition, female and non-amnestic subtype of MCI patients seems to be more vulnerable to depression.
Electroconvulsive therapy (ECT) is indicated for various mental disorders (e.g., major depressive disorder, schizophrenia, and bipolar disorder) and the behavioral and psychological symptoms of dementia in elderly patients. Furthermore, ECT is a useful first-line treatment in emergency and crisis situations such as suicide risk, violent behavior, catatonia, and food refusal, which are more frequent in elderly patients. ECT is also effective in the treatment of the motor symptoms of neurological disorders, such as Parkinson's disease and Huntington's disease. Due to the high risk of various physical diseases, the comorbid physical conditions of elderly patients should be individually controlled to optimize ECT treatment. Compared to young adults, in elderly patients the seizure threshold is higher, the seizure duration is shorter, and the anesthetic dose is lower. On the contrary, the response rate in the elderly is both faster and higher. Considering potential cognitive decline and the prevention of further deterioration of cognitive function in elderly patients, in the absence of significant comorbidities, twice weekly sessions and right unilateral electrode placement with a lower seizure threshold and less cognitive effect are preferred to bilateral electrode placement, which has a high risk of adverse cognitive effects. After an acute course of ECT, continuation and maintenance of ECT, combined with prescription of therapeutic drugs, may prevent possible relapse or recurrence of mental disorders. In conclusion, ECT can be used to treat mental disorders in elderly adults, with safety and effectiveness comparable to that in young adults.
Objectives : The aim of this study was to examine associations between quality of life in patients with schizophrenia and their family relationships. Methods : A total of 68 patients with schizophrenia participated in the study. Socio-demographic and clinical data were collected, as well as results from the Subjective Well-being under Neuroleptic Treatment Scale-Short Form(SWN-K), Family Emotional Involvement and Criticism Scale(FEICS), a visual analogue scale for evaluating the degree of perceived criticism(VAS), the Family Adaptability and Cohesion Evaluation Scale(FACES), Beck Cognitive Insight Scale(BCIS), and Rosenberg Self Esteem Scale(R-SES). Psychiatrists administered the Positive and Negative Syndrome Scale and the Calgary Depression Scale for Schizophrenia(CDSS), and collected a history of previous suicide attempts. Results : The SWN-K total scores showed significant negative correlations with scores on the FEICS perceived criticism, CDSS, and VAS measures and significant positive correlations with scores on the R-SES, FACES, and BCIS. Multiple regression analysis revealed that FEICS perceived criticism, FACES, and BCIS scores were significantly associated with SWN-K scores. The FACES total score was significantly lower in patients with a history of previous suicide attempt. Conclusion : The quality of life of patients with schizophrenia was negatively associated with greater perceived familial criticism and positively associated with better family cohesion and higher patient insight. In patients with a history of suicide attempts, family adaptability and cohesion were significantly low.
연구목적 본 연구는 코로나19가 소아청소년 자해의 발생 및 심각도에 미치는 영향을 조사하고자 한다. 방 법 건양대학교병원 응급실을 내원한 만 18세 이하 환자를 대상으로 의무기록을 후향적으로 검토하여 코로나19 발생 전후 자해 시도로 응급실을 내원한 환자의 비율, 인구학적, 임상적 요인 및 자해 관련 양상을 비교하였다. 결 과 코로나19 발생 후 건양대학교 병원에 내원한 자해 소아청소년의 실인원과 연인원의 비율은 발생 전에 비해 유의미하게 증가하였다(𝛘2=14.397, p<0.001; 𝛘2=24.156, p<0.001). 코로나19 발생 전후로 응급실에 내원한 자해 소아청소년의 자해 과거력 및 정신건강의학과적 과거력, 타과 입원 비율은 유의미한 차이를 보였다(p<0.05; p<0.05; p<0.05). 결 론 코로나19 상황에서 소아청소년의 자해로 인한 응급실 내원 및 타과 입원 비율이 증가하는 것으로 나타났으며, 이전 자해 과거력 및 정신건강의학과 과거력이 있는 소아청소년에서 자해 발생이 유의미하게 증가한 것으로 나타났다. 범유행 상황에서 소아청소년 정신건강 고위험군의 평가와 개입에 대한 추가적 연구가 필요할 것으로 사료된다.
Objective : The purpose of this study was to compare and analyze effectiveness of various psychotherapies of depressive symptoms in patients with anxiety disorders by means of a systemic review. Methods : A systematic search using key words was conducted using EMBASE, MEDLINE and CENTRAL. Results : Five studies were included in this systematic review. The Scottish Intercollegiate Guidelines Network (SIGN) was used for quality assessment. Diagnostic-specific cognitive behavioral therapies for various anxiety disorders (ds CBT) were also effective in reducing comorbid depression. Transdiagnostic cognitive behavioral therapies targeting anxiety and depression (tCBT-AD) were not superior than ds CBT for anxiety disorders. Mindfulness-based stress reduction (MBSR) was more effective than traditional cognitive behavioral therapy. Conclusion : ds CBT relieves anxiety symptoms as well as depressive symptoms comorbid with anxiety disorders effectively. In addition, MBSR is more successful for anxiety disorders with co-occurring depression by focusing broadly on the shifting relationship to internal experience rather than focusing narrowly on specific symptoms.
Objective : The purpose of this study was to investigate the associations between psychosocial factors and the mask-wearing behavior after deregulation of COVID-19 quarantine guidelines among adults in Korea. Methods : We collected data (345 subjects) from online questionnaire survey. The questionnaire included the Korean version of the Patient Health Questionnaire-9, the Korean version of fear of COVID-19 Scale, the Korean version of the Patient Health Questionnaire-15, Korean versions of the Perceived Stress Scale, and measurement tools adapted from previous studies for COVID-19 risk perception, social stigma, and appearance interest of subjects. We analysed data using SPSS version 23.0 for descriptive statistics, chi-square test, and logistic regression analysis. Results : People with cohabitant or mask-wearing tendency before COVID-19 pandemic had a higher rate of mask-wearing than those who were not. Subjects reporting higher level of social stigma (OR=1.154, 95% CI 1.049-1.270) and COVID-19 anxiety (OR=1.072, 95% CI 1.007-1.141) were more likely to report maskwearing behavior. Conclusion : From the results, appropriate intervention to those who fear social stigma and are anxious to the infectious diseases will be needed. Additionally, providing policies and guidelines that consider cohabitants and offering continuous education with information of disease to the public are also expected to helpful for recovery of daily life from infectious diseases.
Objectives To initiate and develop a treatment guideline in multidisciplinary approaches for related professions who are either working and/or living with children and adolescents with neurodevelopmental disorders who show behavioral problems. Methods To collect and reflect opinions from multiple professions who assumedly have different interventions or mediations on behavioral problems, a self-report survey and Focus Group Interview (FGI) were conducted for a group of child and adolescent psychiatrists, behavioral therapists, special education teachers, social welfare workers, and caregivers. Results According to a self-report survey and FGI results from multiple professional groups, aggressive behavior is the mostly common behavioral problem necessitating urgent interventions. However, both mainly used intervention strategies and effective treatment methods were different depending on professional backgrounds, such as pharmacological treatment, parent training, and behavior therapy, even though they shared an importance of improving communication skills. In addition, there was a common understanding of necessity to include parent training in a guideline. Lastly the data suggested lack of proper treatment facilities, qualified behavior therapists, and lack of standardized treatment guideline in the field needed to be improved for a quality of current therapeutic services. Conclusion It is supported that several subjects should be included in the guidelines, such as how to deal with aggressive behavior, parent training, and biological aspects of neurodevelopmental disorders. Also, it is expected that publishing the guideline would be helpful to above multiple professions as it is investigated that there are lack of treatment facility and qualified behavioral therapists compared to need at the moment.
연구목적 코로나바이러스 사태로 인해 전세계가 보건 위기와 큰 혼란의 국면을 맞은 가운데, 특히 재난 상황에서 시민의 건강을 위해 노력하는 공공 근로자들의 신체적, 정신적 스트레스가 과중해지고 있다. 본 연구는 공공 근로자들의 정신적 스트레스를 분석하고자 했다. 방 법 본 연구에서는 광명시 정신건강복지센터 홈페이지를 통해 모집한 531명의 공공영역 종사자를 대상으로 한국판 지각된 스트레스 척 도(Perceived Stress Scale, PSS), 한국판 우울증 평가도구(Patient Health Questionnaire-9, PHQ-9), 한국판 사건 충격 척도 수정판(Impact Event Scale-Revised, IES-R-K)을 시행하였다. 결 과 분석 결과에 의하면 코로나 19 관련 종사자들은 중등도 이상의 지각된 스트레스 반응(85.2%), 우울증상(22.2%), 외상 후 스트레스 관련 증 상(38.8%)을 보여, 상당한 스트레스와 우울증상을 경험하는 상태로 나타났다. PSS 총점은 PHQ-9, IES-R 총 점 및 IES-R의 하위 척도별 점수와 유의한 상관관계를 보여, 주관적으로 지각된 스트레스 반응 정도가 우울 증상과 연관되어 있음을 시사하였다. PSS, PHQ-9, IES-R 총점은 각각 모두 연령과 음의 상관관계를 보였다. 즉, 연령이 직무 스트레스 반응과 번아웃에 대한 보호요인으로 작용할 가능성이 있다. 결 론 향후에 공공영역 종사자 들의 정신건강에 관한 추가적인 연구가 필요하겠으며 정신건강 관리 및 적절한 개입이 마련되어야 할 것으로 사료된다.
