절삭유 취급 근로자의 피부질환에 관한 연구 (A study on dermatologic diseases of workers exposed to cutting oil)

  • 절삭유의 사용과 피부질환의 관계를 알아보기 위하여 한 자동차공장의 엔진 및 구동부서 근로자 1,004명을 대상으로 피부 이상 증세에 대한 기초설문조사를 실시하여 응답을 해준 근로자 667명을 대상으로 분석 및 피부과적 검진을 실시하였다. 또한 절삭유의 노출 정도를 알기 위한 방법으로 공기 중 오일 미스트를 측정하였다. 이 조사의 결과를 요약하면 다음과 같다. 1. 공기 중 오일미스트는 3개 부서로 나누어서 측정 하였는데, 전체 52건의 측정건수 중에서 8시간가중 평균 기준치인 $5mg/m^3$을 넘는 건수는 9건(17.3%)이었고, 로그정규분포상에서 95% 신뢰한계의 전체 상한치는 기준치 미만이었다. 2. 조사한 부서는 엔진가공부(258명), 가솔린 엔진조립부(210명), 디젤엔진조립부(96명), 구동부 가공반(86명), 구동부 조립반(17명)의 5개 부서였으며, 조사한 667명의 평균 나이는 34.5세였고, 근무연수의 중앙값은 7년 7개월이었다. 3. 이들 부서 중 엔진가공부와 구동부 가공반(A군)은 절삭유를 공정상 많이 사용하는 부서로 근로자 344명 중 298명 (86.6%)이 한 가지 이상의 절삭유를 사용하고 있는 반면, 가솔린엔진 조립부, 디젤엔진 조립부, 구동부 조립반의 근로자 323명(B군) 중에는 31명 (9.1%)만이 절삭유를 사용하고 있었다. 4. 피부 이상 증세의 경험자 227명 중 213명에 대하여 피부과전문의의 진찰을 시행하였는데, 이중에 172명이 검진당시 증세를 호소했는데, 1인당 평균 호소증세 건수가 1.5건 정도였다. 가장 많은 증세는 가려움증(32.5%), 구진(21.6%), 인설(15.7%), 소수포(12.5%), 반점(9.4%)의 순서였다. 5. 검진을 받은 근로자 231명 중 정상소견이나 뚜렷한 병변이 없는 사람으로 분류된 경우는 53명이었고, 나머지 160명에서 피부과적 질환이 발견되었다. 가장 많은 것은 접촉 피부염으로 전체 질환자 중 92명 (57.5%)을 차지하였고, 이들 중에는 여드름이나 낭포염 등 기타 다른 피부질환을 동반한 경우가 23명 (25%) 이었다. 기타 낭포염, 여드름만 있는 경우도 각각 9명(5.6%), 8명(5.0%)이었으며 , 각질화와 색소 이상도 1명 씩 발견되었다. 기타의 수부 습진은 비특이성 수부습진으로 분류되었는데 이들은 47명이었다. 6.피부질환이 있다고 진단받은 160명에서 발견된 피부과적 이상부위는 1인당 1.3부위였다. 이상이 발견된 부위는 손바닥(27.6%), 손가락과 손톱(22.7%), 상박부(16.0%), 손등(11.8%), 손가락 사이(10.3%)의 순이었고, 이들 수부 및 상박부위의 이상이 전체 이상의 약 88.7%였다. 7. 전체 피부질환의 유병상태는 각 부서별로 차이가 많았는데, 특히 접촉 피부염의 경우 엔진가공부는 1,000명당 213명, 가솔린엔진 조립부는 1,000명당 29명, 디젤엔진 조립부는 1,000명당 73명이었고, 구동부 가공반은 1,000명당 267명, 구동부 조립반은 1,000명당 59명으로 부서별로 차이가 매우 켰다. 이러한 경향은 절삭유 사용에 따라서 특히 수용성 절삭유만 사용하는 군과 두 종류의 절삭유를 모두 사용하는 근로자에서 마찬가지로 높게 관찰되었다. 8. 감별진단이 필요하다고 판단된 접촉 피부염 환자 49명은 첩포 검사를 시행하였다. 첩포 검사 결과, 기본 표준시료에 양성이 나타난사람이 12명, 기본표준시료와 오일시료에 모두 양성이 나타난 사람 3명, 그리고 오일 시료에만 양성이 나타난 사람 3명이 있었고, 기본 표준시료와 유기용제시료에 양성이 나타난 사람, 유기용제 시료에만 양성이 나타난 사람이 각각 1명 있었다. 9. 전체 피부질환의 유무에 대하여, 근로자의 나이와 근무기간의 영향을 통제한 후 근로자들이 사용하는 절삭유의 종류에 따른 위험도를 알아보기 위하여 다변량 분석을 하였다. 분석결과 절삭유를 전혀 사용하지 않는 군과 비수용성 절삭유만을 사용하는 군과는 유의한 차이가 없었으며 수용성 절삭유만 사용하는 사람은 $1.4\sim3.3$배(교차비의 95%신뢰한계), 두 종류의 절삭유를 모두 사용하는 사람은 $1.9\sim4.1$배 정도로 위험도가 높았다. 10. 알레르기성 접촉 피부염을 종속변수로 같은 분석을 했을 때도 결과는 같았으나, 수용성 절삭유만 사용하는 군이 $2.7\sim9.7$배, 두 종류를 모두 사용하는 군에서는 $3.7\sim12.6$배 정도로 위험도가 증가한 것을 관찰할 수 있었다.

마치현(馬齒莧)이 DNCB로 유발된 생쥐의 알레르기성 접촉피부염에 미치는 영향 (Effects of Portulacae Herba (PH) on Allergic Contact Dermatitis (ACD) Induced by DNCB in Mice)

  • Objective : Portulacae Herba (PH) can clear away heat, detoxicate, cool down blood heat. The present study was designed to investigate the effects of PH on ACD induced by 2,4-dinitrochlorobenzene (DNCB) in mice. Method : In this experiment, the effects of PH on changes in body weights, thicknesses and weights of ear, thicknesses of dorsum skin, degree of symptoms on the dorsum skin, histopathological changes of ear and dorsum skin, spleen weights, TNF-${\alpha}$ and IL-10 in serum were observed. And the effects on the proliferation rates of splenocytes were also investigated in vitro and in vivo. Results : 1. PH spread (PHS) group and PH spread plus administered (PHS+Adm) group didn't show any changes in weight and thickness of ear. 2. But in PHS and PHS+Adm group, thickness of dorsum skin decreased significantly. 3. And PHS and PHS+Adm group showed meaningful effectiveness on ACD symptoms like erythema, desquamation and keratinization. 4. In histopathological observation, hyperplasia and edema of spongy tissues were remarkably diminished in PHS and PHS+Adm group. 5. And PH reduced the proliferation rates of splenocytes in vivo and vitro study. Conclusions : PH effectively decreased symptoms of ACD in this study, therefore PH can be one of medicinal ingredients to treat ACD.

자외선A로 손상된 DNA의 회복과 DNCB에 의한 알러지성 접촉피부염에 대한 표고버섯과 침향 추출 혼합물의 효과 (The Effects of Lentinula edodes and Aquilariae agallocha Extracts Combination on the Repair of UVA-Damaged DNA and DNCB-Induced Allergic Dermatitis)

  • The effects of extracts from Lentinula edodes (L. edodes) and Aquilariae agallocha (A. agallocha) on the DNA damage response in ultraviolet A (UVA)-exposed HaCaT cells and on the allergic contact dermatitis caused by 2,4-dinitro-chlorobezene (DNCB) were investigated. When UVA-exposed cells were incubated for 24 hours in medium containing L. edodes or A. agallocha extract, the level of 8-OHdG and CPD decreased in a concentration-dependent manner. The combined treatment with both extracts potentiated the decrease in UVA-induced 8-OHdG and CPD levels as compared with those following treatment with a single extract. In addition, the two extracts showed preventive effects against the UVA-induced reduction in collagen levels. Furthermore, the blood levels of IgE, IL-6, and histamine decreased more significantly upon combined treatment with L. edodes and A. agallocha extracts as compared with those following treatment with single extracts in DNCB-induced allergic contact dermatitis in the ICR mouse. The results of the present study suggest that the components with in the extracts of L. edodes and A. agallocha can help to prevent of UVA-induced genomic instability via a decrease in DNA damage, and to decrease the DNCB-induced allergic dermatitis via modulation of relevant proteins including IgE and IL-6. Further study is needed to clarify the purified components related to the preventative effects of the two extracts against UVA- or DNCB-induced genomic damage.

생지황(生地黃) 추출물(抽出物)이 DNCB로 유발(誘發)된 생쥐의 Allergy성(性) 접촉피부염(接觸皮膚炎)에 미치는 영향(影響) (The effect of Rhemanniae radix extract on allergic contact dermatitis on Mice induced by DNCB)

  • Allergic contact dermatitis is a common skin disease resulting from specific immunologic to topically applied various allergen. After Dinitrochlorobenzene (DNCB) secondary sensitization, the ICR mice administered Rhemanniae radix extract (RRE) was observed to ascerstain the effect of RRE on allergic contact dermatitis. Purpose of this study was to investigate contact hypersensitivity assay, abdominal skin morphologic changes. Including mast cells and cell-surface glycoconjugates. The change of interleukin 2 (IL-2) receptor (CD25R). ICAM in abdominal skin, lymph node of inguinal region, and electro microscope-morphologic changes of abdominal skin. The results of this study were as follows: 1. The contact hypersensitivity assay, the ear swelling in the RRE had lesser probability than in the ACD Group. 2. In the general morphologic change of skin, hyperplasia of keratinocytes, distribution of vasculogenesis and epidermal lymphocytes infiltration were decreased in the RRE group compared with the ACD group. In epidermal basal layer and prickle layer, cell damage was decreased in the RRE group compared with the group painted with ACD. 3. MasT-cell in dermis was decreased in the RRE group compared with the group painted with DNCB. 4. Distribution of interlukin-2 Receptior positive cell and ICAM positive cell in dermis was decreased in the RRE Group compared with the ACD group. 5. Distribution of helper T-lymphocyte and cytotoxic T-lymphocite in inguinal nodes was decreased in the RRE group, and was observed well in paracortical area and cortical cord. 6. Distribution of apoptotic cell was appeared in the RRE group compared with the ACD group, in skin, dermis. in inguinal nodes, paracortical area observed well. With above results, the restarint of immunosuppression occuring in Allergic contact dermatitis is resulted in the slow progress the effect of Allergic contact dermatitis, and it is thought that ithis fact has a series of relation with apoptosis.

알레르기성 접촉피부염 치험 1례 (A Case of Allergic Contact Dermatitis Treated with Korean Medicine)

  • Objective This study is designed to investigate the effect of Korean medical therapy on Allergic contact dermatitis patient. Method This is a case report on a female patient who has been suffering from allergic contact dermatitis. To reduce her symptoms, we provided with internal herbal medicine 3 times a day, acupuncture therapy two time a day and Carbon phototherapy one time a day. Results After series of treatments, the symptoms of allergic contact dermatitis were remarkably improved. SCORAD Index was changed from 77.3 to 13.6. Conclusions This study shows us that Korean medical treatments has remedial values for dermatitis patients. Hence, more studies should be demanded in Korean medicine for elevation of treatment rate.

Allergy성(性) 접촉피부염(接觸皮膚炎)에 대(對)한 고삼(苦蔘), 황연(黃連)의 Iontophoresis 치료(治療) 효과(效果) (The Effects of Sophorae Radix and Coptidis Rhizoma - Iontophoresis in Allergic Contact Dermatitis -)

  • Objective : This study was performed to investigate the effects of Sophorae Radix and Coptidis Rhizoma-iontophoresis in allergic contact dermatitis. Methods : Contact hypersensitivity assay, video microscope, melanin-erythema levels, pH levels, hydration levels, WBC count, RBC count, neutrophil ratio, lymphocyte ratio and total IgE levels were measured. Results : At contact hypersensitivity assay, the right ear swellings in Sophorae Radix and Coptidis Rhizoma-iontophoresis group were decreased compared with control group, but have no statistical significance. At observation of skin morphologic change, many papules were seen in control group and keratins were seen in Sophorae Radix-iontophoresis group. At melanin levels, Sophorae Radix and Coptidis Rhizoma-iontophoresis groups showed no significant difference compared with control group. At pH levels, Sophorae Radix-iontophoresis group showed significant decreased with control group. At hydration levels, Sophorae Radix and Coptidis Rhizoma-iontophoreses groups showed to significant difference compared with control group. WBC count, RBC count and neutrophil ratio were significantly increased in Sophorae Radix and Coptidis Rhizoma-iontophoresis group compared with control group. Lymphocyte ratio and total IgE levels were significantly decreased in Sophorae Radix and Coptidis Rhizoma-iontophoresis groups compared with control group. At morphology of skin, inflammation was decreased and the thickness of epidermis was well preserved in Sophorae Radix and Coptidis Rhizoma-iontophoresis group. Conclusion : Sophorae Radix and Coptidis Rhizoma-iontophoresis had some anti-allergy, anti-inflammatory effects on allergic contact dermatitis, but had no effects on melanin-erythema levels, pH levels and hydration levels of skin.

우방해기탕(牛蒡解肌湯)의 알러지성 접촉피부염(接觸皮膚炎)에 대한 소염효과(消炎效果) (The Anti-Inflammatory Effects of WooBangHaeGiTang on the Allergic Contact dermatitis)

  • Objective : This study was performed to investigate the Anti-Inflammatory Effects of WooBangHaeGiTang on the Allergic Contact dermatitis. Method : Contact hypersensitivity assay, video microscope, WBC count, RBU count, neutrophil ratio, lymphpcyte ratio and GOT & GPT were measurd. Result : 1. At Contact hypersensitivity Aasay, at 24 hours after Control group & Sample group showed appreciably more ear swelling than Normal group, at 48 hours after Control group showed more ear swelling than Normal group & Sample group, at 72 hours after Control group & Sample group showed more ear swelling than Normal group 2. Regarding the number of WBC, at 24 or 48 hours after There are no significant difference among Normal group, Control group. Sample group, at 72 hours after Sample group showed more WBC than Control group. 3. Regarding the number of RBC, at 24 hours after Control group showed more RBC than Normal group, at 48 hours after Control group & Sample group showed more RBC than Normal group. at 72 hours after Control group showed more RBC than Normal group. 4. Regarding the percentage of neutrophil in WBC, at 24 & 48 hours after Control group & Sample group showed higher percentage than Normal group. 5. Regarding the percentage of Lymphocyte in WBC, at 24 hours after Sample group showed lower percentage than Normal group, at 48 hours after Sample group & Control group showed lower percentage than Normal group. 6. Regarding the GOT & GPT in blood, the GOT & GPT are in Normal scale, so there are no Hepatotoxicity in Normal group, Control group, Sample group. Conclusions : According to above experiments, WooBangHaeGiTang have some anti allergy effects on allergic contact dermatitis.

  • In the theory of Korean medicine, Angelicae Dahuricae Radix (ADR) can expel wind and relieve exterior syndrome, and eliminate dampness. Recently, ADR has been reported to have possibilities as anti-inflammatory agent and cosmetics. The purpose of this study is to evaluate the efficacy of ADR on contact dermatitis (CD). In order to investigate the anti-inflammatory effects of ADR on CD, we investigated the effects of ADR on ear thickness, ear weight, skin lesion and histopathological changes in mice with CD induced by 1-fluoro-2,4-dinitrofluorobenzene (DNFB). In addition, the effect on spleen weight was also measured. In our results, topical application of ADR lowered ear thickness and weight respectively. ADR treatment also improved skin lesions such as erythema and scale. In the histopathological observation, ADR-treated group showed diminished epidermal hyperplasia and immune cell infiltration in inflammed tissues compared to those of non-treated control group. In conclusion, These data suggest that ADR has anti-inflammatory action in inflammed skin tissue, resulting in improving skin lesion and histopathological abnormalities of CD.

오령산(五苓散) 위주의 한방 치료로 호전된 만성 알레르기성 접촉피부염 치험 1례 (A Case of Chronic Allergic Contact Dermatitis Treated with Oryeong-san-Based Korean Medicine)

  • Objectives : This study is to report a case of chronic allergic contact dermatitis, treated with Korean medicine, primarily focused on Oryeong-san. Methods : A 43-year-old female has suffered from dermatitis in axillary, lower abdominal, inguinal and popliteal area for more than 10 years. She was diagnosed with chronic allergic contact dermatitis accompanied by skin symptoms such as severe pruritus, erythema, skin pigmentation, papules and lichenification, and by decreased quality-of-life. She received Korean medicine treatment including herbal medicine, acupuncture and herbal external medication for 4 months. Results : The skin symptoms such as pruritus, erythema, skin pigmentation, papules and lichenification were remarkably improved. Also, the NRS of pruritus changed from 10 to under 1, and the DLQI score changed from 29 to 6. Conclusions : This case report suggests that Korean medicine, especially Oryeong-san may have a role in treating skin symptoms such as pruritus, erythema, skin pigmentation, papules and lichenification in the patients diagnosed with chronic allergic contact dermatitis. Also, the improvement of skin symptoms treated with Korean medicine may result in quality-of-life improvement in those patients.

인동등(忍冬藤)이 DNCB로 유발된 알레르기성 접촉피부염에 미치는 영향 (Effects of Lonicerae Caulis (LC) on Allergic Contact Dermatitis (ACD) induced by DNCB in mice)

  • Objectives : Allergic contact dermatitis(ACD) is a type IV delayed hypersensitivity reaction that results from cumulative exposures and subsequent sensitization to an environmental chemical. Lonicerae Caulis(LC) can clear away heat and relieve toxin, disperse wind and heat, dredge the channel. The present study was designed to investigate the effects of LC on allergic contact dermatitis(ACD) induced by 2,4-dinitrochlorobenzene(DNCB) in mice. Methods : In this experiment, the effects of LC on changes in body weights, ear and dorsum skin thicknesses, ear weights, clinical aspects on the dorsum skin, histopathological changes, spleen/body ratio, cytokines were investigated. In addition, the effects on proliferations of splenocytes were also investigated in vitro and vivo study. Results : LC spread(SPR) group and LC spread and administered(SPR+ADM) group showed diminished ear thicknesses. In SPR+ADM group, ear weights were lowered significantly compared to contact dermatitis control(CTL) group. LC treatment diminished erythema, desquamation and keratosis which were induced by repeated painting of DNCB. In histopathological observation, spongiosis and edema were diminished in SPR and SPR+ADM group. In cytokines, SPR+ADM group were increased in IL-10, and SPR and SPR+ADM group were decreased in TNF-${\alpha}$ compared with control group. Conclusions : These data suggest that LC can decrease symptoms of ACD, then LC is useful to treat patient with ACD.
