Conversion of implant overdenture to an implant assisted removable partial denture in maxilla: case report (상악 임플란트 피개의치에서 임플란트 보조 국소의치로의 전환: 증례보고)

  • The long-term use of an implant overdenture shows problems, such as wear of attachment, concentrated occlusal forces on the anterior teeth, fracture of artificial teeth and rotation of denture. By transitioning to an Implant-assisted removable partial denture (IARPD) using additional implant placement, the problems can be solved. In this case report, a transition was made from implant overdenture to IARPD utilizing surveyed crowns to distribute occlusal forces concentrated on anterior teeth and to prevent denture rotation in a skeletal Class III patient. Design of definitive prosthesis with appropriate function and aesthetics was determined through several stages of provisional restorations. In this case, appropriate posterior occlusion and maintenance of peri-implant bone level of definitive prosthesis were observed.

  • Oculodentodigital syndrome(ODD) was first reported by Lohmann in 1920 and termed by Meyer Schwicketath, which they called "dysplasia oculo-dento-digitalis" in 1957. It is somewhat rare heritable disease. ODD is generally inherited in an autosomal dominant pattern with a complex phenotype. The characteristic features are : (1) unique facial features, (2) microphthalmos, (3) syndactyly and camptodactyly of 4th and 5th fingers, (4) osseous anomalies of the middle phalanges of 5th fingers and toes, (5) enamel hypoplasia, (6) dry lusterless hair. We found several occlusal wearing and yellow discoloration of succedaneous teeth, multiple caries lesions, premature loss and pulpal involvement of primary teeth with associated enamel abnormalities caused by generalized enamel hypoplasia in a fairly constant oral finding. Occasionally partial anodontia, microdontia and cleft lip and palate can be manifested. This case, a 9-year-old female with repaired bilateral syndactyly was referred to pediatric dental clinic, Pusan National University Hospital for evaluation of severe attrition of teeth and caries lesions. She had most of the above mentioned typical manifestations of the syndrome. Dental treatment including caries control, stainless steel crown were performed.

  • Amelogenesis imperfecta is defined as a genetically determined effect affecting enamel formation and may be associated with other ectodermal or systemic disorders. It is entirely an ectodermal disturbance, since the mesodermal components of the teeth are basically normal. The presentation of diverse clinical manifestations in 1:14,000 to 1:16,000. Classification of the AI types considers mode of inheritance and clinical manifestations. The most widely accepted classification system recognize three major groups; i.e., hypoplastic(thin enamel), hypocalcified(primary mineralization defect), hypomaturation(defect in enamel maturation). The treatment is that at first, genetic counselling must be practiced, and in anterior teeth, composite resin veneer or jacket crown for esthetics, and in posterior teeth, stainlees steel crown or gold onlay.

  • This study was designed to investigate the attrition pattern in Angle Class III malocclusion with facial asymmetry. The sample consisted of three groups, the 20 subjects of normal occlusion group(Group I), the 12 subjects of class III malocclusion without facial asymmetry group(Group II) and 17 subjects of Class III malocclusion with facial asymmetry group(Group III). Attrition areas from canine to second molar on both sides in upper and lower arch, totally twenty, was marked by pencil and mesured by computer system(INTERGRAPH CO. USA) 2 times and the average value was used for date processing. Total attrition area in Group I was larger than in Group II and III. 2. There was no significant difference in attrition area between right and left side in each group, but attrition area in Group III was larger than in Group I and II. 3. In Group I, Maxillary attrition area was larger than mandibular attrition area, but in Group ll and III, there was no significant difference in attrition area between maxilla and mandible. 4. In Group III, the attrition area of deviated side was target than undeviated side 5. There was no significant difference in attrition area between chewing side and non-chewing side in each group. 6. The total attrition area was unaffected by gender.

  • In partial edentulous patients, implant-assisted removable partial denture which provide additional retention and support by placing a small number of implants in strategic positions might be suitable treatment. This case of patient with loss of maxillary posterior teeth and moderate to severe wear of residual dentition, three implants were placed in the maxillary anterior edentulous area and then surveyed bridges were made including remaining anterior natural teeth. Posterior edentulous area was restored with distal extension removable partial denture (RPD). In addition, the worn mandibular natural teeth were restored with fixed prostheses. As a result, reduced vertical dimension and collapsed occlusal plane were rehabilitated, and improved functionally and aesthetically. The purpose of this case was to report the results of three-year follow-up of full mouth rehabilitation with anterior implant surveyed bridges and distal extension RPD.

  • Dens evaginatus is a tooth with cylindrical enamel projection which forms a nodule on occlusal surface. It could be explained as outward overgrowth of inner enamel epithelium or localized hyperplasia of pulpal mesenchymal tissue during tooth development. A problem is that it is likely to be worn out or fractured by mastication ensuing pulpal inflammation. It is occasionally found on the lingual surface of upper anterior teeth as well, called talon cusp. Dens invaginatus is a tooth with deep lingual pit made by invagination of lingual enamel epithelium during tooth development while it is considered normal in terms of size and shape. Radiographically, a part of cervical enamel shows inward growth forming cavity and it is reasonable to say that the base is possibly open to pulpal cavity since they are very close. Talon cusp and dens invaginatus are relatively common abnormality of shape. However it becomes the opposite if the two exist in the same tooth. Once the talon cusp is broken by occlusal force or fissure between cusps is decayed, the complicated structure of canals makes the pulpal treatment difficult. Preventive treatments such as occlusal equilibrium and sealant, and regular oral examination should be preceded and thorough understanding of canal shape, using radiography, is required when pulpal treatment is necessary. This report is about a 9- year-old boy(lower left central incisor), a 8-year-old girl(upper right central incisor), and a 7-year-old boy(upper right central incisor), who have dens invaginatus and talon cusp in the same teeth. The first and the second patients are under pulpal treatments, and the last one is being observed showing no pathologic impressions.

Prosthetic rehabilitation in a Class III malocclusion patient with increasing occlusal vertical dimension (3급 부정교합 환자의 교합수직고경 증가를 동반한 보철 수복)

  • Class III malocclusion with mandibular protrusion can be divided into skeletal and pseudo malocclusion due to tooth displacement. For skeletal malocclusion, favorable treatment results can be obtained by establishing an appropriate vertical and horizontal intermaxillary relationship in order to secure a restoration space and obtain aesthetic and functional results. In this case, complete mouth rehabilitation was performed using an implant and a fixed prosthesis in a patient with mandibular protrusion and anterior teeth wear and reduced occlusal vertical dimension. After cast analysis and digital diagnosis, a provisional restoration with increased vertical dimension was fabricated to secure posterior support and evaluate stable centric occlusion. With the definitive prosthesis reflecting the provisional restoration, favorable function and aesthetics were obtained.
