Anterior implant case report using digital guided implant template (임플란트 가이드를 활용한 전치부 수복증례)

  • Guided implant surgery is useful in anterior implant case in terms of fixture installation and temporary crown delivery. For the aesthetic prosthetics in anterior implant, the position of the implant fixture is crucial. Guided surgery is the top-down procedure and we designed prosthetics first and then determine the position of the fixture. Guided surgery can reduce the stress of dentist with difficult anterior implant case.

A study on the anterior tooth size discrepancies among orthodontic patients with varying malocclusions (부정교합자의 전치부 치아크기 부조화에 관한 연구)

  • Bolton analysis is widely used to predict tooth size discrepancy. but its accuracy has been challenged. The purpose of this study was to describe true anterior tooth size discrepancies among orthodontic patients and to evaluate the factors that affect true anterior tooth size discrepancies. The subjects consisted of 80 patients with varying malocclusions (Class I. Class II. Class III. and Class III surgery) who were treated orthodontically. Pre-treatment models. set-up models from post-treatment models. and lateral cephalometric radiographs were analyzed The results were as follows. The means. the standard deviations. and ranges of anterior Bolton ratio in the present study were somewhat higher than those of Bolton's samples and Korean normal samples. The number of patients showing maxillary deficiency was larger than that of patients showing maxillary excess in view of true anterior discrepancies. There was a significant difference between anterior Bolton discrepancy from pre-treatment models and true anterior discrepancy from set-up models (p < 0.05) There was no significant difference in true anterior discrepancies among malocclusion groups (p > 0.05). And there was also no significant difference between the male and female groups (p> 0.05). Overbite and the incisal edge thickness of maxillary anterior teeth have little relationship with true anterior discrepancies. Multiple regression analysis showed that true anterior discrepancy was mainly determined by anterior Bolton ratio, upper incisor to occlusal plane angle after treatment. interincisal angle after treatment. and upper right lateral incisor width.

Three-dimensional finite element analysis of initial tooth displacement according to force application point during maxillary six anterior teeth retraction using skeletal anchorage (골격성 고정원을 이용한 상악 6전치 후방 견인시 힘의 적용점 변화에 따른 치아 이동 양상에 관한 유한 요소법적 분석)

  • The purpose of this study was to investigate the micro-implant height and anterior hook height to prevent maxillary six anterior teeth from lingual tipping and extruding during space closure. We manufactured maxillary dental arch form, bracket and wire, using the computer aided three-dimensional finite element method. Bracket was $.022'{\times}.028'$ slot size and attached to tooth surface. Wire was $.019'{\times}.025'$ stainless steel and $.032'{\times}.032'$ stainless steel hook was attached to wire between lateral incisor and canine. Length of hook was 8mm and force application points were marked at intervals of In. Four micro-implants were implanted on alveolar bone between second premolar and first molar. The heights of them were 4, 6, 8, 10mm starting from wire. We analyzed initial displacement of teeth by various force application point applying force of 150gm to each micro-implant and anterior hook. The conclusions of 4his study are as the following : 1. When the micro-implant height was 4m and the anterior hook height was 5mm and below, anterior teeth were tipped lingually. When the anterior hook height was 6mm and above, anterior teeth were tipped labially. 2. When the micro-implant height was 6mm and the anterior hook height was 6mm and below, the anterior teeth were tipped lingually. When the anterior hook height was 6m and above, the anterior teeth were tipped labially. But lingual tipping of anterior teeth decreased and labial tipping Increased when the micro-implant height was 6mm, compared with 4mm micro-implant height. 3. When the micro-implant height was 8mm and the anterior hook height was 2mm, the anterior teeth were tipped lingually. When the anterior hook height was 3mm and above, labial tipping movement of the anterior teeth increased proportionally. 4. When the micro-implant height was 10mm and the anterior hook height was 2mm and above, labial tipping of the anterior teeth increased proportionally. 5. As the anterior hook height increased, aterior teeth were tipped more labially. But extrusion occurred on canine and premolar area because of the increase of wire distortion. 6. Movement of the posterior teeth was tipped distally during maxillary six anterior teeth retraction using micro-im plant because of the friction between bracket and were Based on the results of this study, we could predict the pattern of the tooth movement according to position of micro-implant and height of anterior hook. It seems that we can find the force application point for proper tooth movement in consideration of inclination of anterior anterior teeth, periodontal condition, overjet and overbite

  • This study was undertaken to compare each maximum biting force and to investigate its relationship with the facial skeketal form, number and position of tooth contact between anterior openbite and normal occlusion adults, using the T-scan system and the lateral cephalogram. The subjects of this study consisted of a group of 25 individuals with normal occlusion and another group of 14 with anterior openbite. The obtained results of this study were as follows : 1. The maximum biting force of anterior openbite adults was less than that of normal occlusion adults. 2. In anterior openbite adults, there were negative correlations between the maximum, biting force and SN/MP, FMA, PP/MP mesurement of lateral cephalogram. 3. In anterior openbite adults, as the mesial angulation of lower first molar against the occlusal plane increased, the more the biting force decreased. 4. In both groups, the greater the number of tooth contact, the more the biting force increased. 5. In both groups, the center of effort for anteroposterior occlusal contact was located on the first molar region.

  • Maxillary and mandibular anterior dental arches often have the problems of occlusal relation and esthetics by malformations of teeth, congenital missing, et at. Though the clinician usually use the anterior ratio to overcome this problems, he has the limitation of a direct application this ratio to the prediction of anterior occlusal relationship by the change of anterior ratio as dental arch form, intercanine width, segment depth and arch perimeter. So this study examine maxillary and mandibular anterior dental arch forms by least square method using Korean normal occlusion models(man : 20 casts, woman : 20 casts). Maxillary and mandibular anterior dental arches of Korean normal occlusion models are curve fitted to polynomial function, beta function, hyperbolic cosine function in order. And this accuracy of curve fitting is constant regardless of man/woman and maxilla/mandible. The relationships between intercanine width, segment depth, and arch perimeter based on this owe fitted dental arch form are acquired. This relationships will give the prediction of anterior dental arch form and the information of more accurate anterior ratio according to intercanine width.

  • A device called incisor inclination indicator to control the axial inclinations of the incisors in the diagnostic setup is introduced. It is used to control the retraction of the maxillary and mandibular incisors. In this article, we describe the use of the incisor inclination indicator to prescribe adequate torque into the anterior lingual brackets and evaluate the results of treatment in a bimaxillary dentoalveolar protrusion case that underwent orthodontic treatment with the pretorqued anterior lingual brackets. Retraction of the maxillary and mandibular incisors was achieved with careful control of the axial inclination. It is indicated that the use of the incisor inclination indicator is an effective adjunctive laboratory procedure for anterior torque control during retraction in lingual orthodontic treatment.

  • The bite collapses due to posterior teeth loss or wear results in inadequate space for restoration and esthetic concerns. Increasing the occlusal vertical dimension to improve space deficiency rotates the mandible posteriorly, creating a gap between the maxillary and mandibular anterior teeth, leading to loss of anterior guidance. To solve this problem, the prosthodontics or orthodontics treatments are the commonly used methods for proper anterior guidance. However, it is reasonable to assume that the anterior teeth can naturally relapse to their original position when the occlusal force is eliminated. Therefore, this case report aimed to test whether natural relapse could recover the lost anterior guidance to develop a less invasive and more convenient treatment method. Digital superimposition was used to evaluate the changes in anterior teeth alignment to confirm the change of the recovered anterior guidance. The appropriate indications for this new treatment method were defined and applied clinically.

Esthetic restoration in mandibular anterior region with one-piece implant and immediate loading (하악 전치부에서의 일체형 임플란트 식립 후 즉시부하)

  • Mandibular anterior region has high implant survival rates and better accessibility compared with any other region. Even if there are possibilities for perforation on lingual cortical bone due to improper drilling and bleeding caused by lingual artery damage, mandibular anterior region is a safe region because less amount of major anatomical structures exist compared with other regions. However, because of narrow bucco-lingual width of alveolar ridge, it is challengeable to obtain esthetic implant prosthesis. Although patients are less sensitive subjectively, mandibular anterior region is as difficult as maxillary anterior region in that implant placement location plays a critical role on the prognosis of implant prosthesis. One-piece implant is a very useful option for mandibular anterior region. Considering the narrow roots and thin alveolar bone of mandible, it is clinically difficult for implant diameter to be greater than 3mm In this case, we could approach the esthetic restoration in mandibular anterior region with one-piece implant and immediate loading.

  • When we deal with maxillary and mandibular anterior dental arches showing problems in occlusal relation and aesthetics caused by malformations of teeth and congenital missing, et al during the orthodontic treatment, we could not often decide the functional occlusion by only relying on the orthodontic treatment. If orthodontists can predict what kinds of treatments are needed for functional occlusion in maxillary and mandibular anterior dental arches, they can not only effectively treat patients but also facilitate the cooperation with other field during the treatment, Our previous research showed the correlation among intercanine width, segment depth and arch perimeter by using the Korean normal occlusion model. At this time, we produced the computer application program by taking advantage of this correlation. And then, we applied this program to setting up the treatment plans for 2 patients with the damaged maxillary and mandibular dentures. With the help of this program, we could not only easily acquire the information about the change of variables required by treatment plans but also intercanine width, segment depth and arch perimeter. Later, if we can the information about the relationship between the change of the angle of incisors depending on facial types and arch forms and, in addition, can acquire the appropriate intercanine width, we can have the ability to produce the 3 dimensional occlusogram for the anterior dental arch forms.

  • Anterior open bite is one in which the teeth in the anterior portion of the maxilla and mandible are vertically apart and lack the overlapping necessary for the incisive function when the mandible is in closed position. Anterior open bite is a result of the interaction of many different etiologic factors including thumb and finger sucking, lip and tongue habits, airway obstruction, skeletal growth abnormalities and its tendency may appear with any type of skeletal patterns, such as Class I, II or III malocclusion types. Though the treatment methods for anterior open bite are various, the conventional FR-4, designed by Rolf Fr$\"{a}$nkel, is known to be effective in treating open bite cases with Class I or II skeletal patterns. It is due to that an incidence of skeletal Class II is high in the Occidentals, and open bite is accompanied by these malocclusion type in many cases. However, an incidence of skeletal Class III is high in the Orientals, and open bite is sometimes accompanied by skeletal Class III in many cases. Although the use of the conventional FR-4 was effective in the treatment of open bite, skeletal Class III would be worsened. So, a modified FR-4(placing the labial bow in the lower, the labial pads in the upper) was designed for the treatment of patients showing skeletal Class III and open bite.
