Fast Marker-based Registration of 3D CT and 2D X-ray Fluoroscopy Images (3차원 전산화 단층촬영영상과 2차원 X-선 투시영상간 표식기 기반 고속 정합)

  • This paper proposes a novel technique of marker-based 2D-3D registration to combine 3D information obtained from preoperative CT images into 2D image obtained from intraoperative x-ray fluoroscopy image. Our method is divided into preoperative and intraoperative procedures. In preoperative procedure, we generate CT-derived DRRs using graphics hardware and detect markers automatically. In intraoperative procedure, we propose a hierarchical two- step registration to reduce a degree of freedom from 6-DOP to 2-DOF which is composed of in-plane registration using principal axis method and out-plane registration using minimal error searching method in spherical coordinate. For experimentation, we use cardiac phantom datasets with confirmation markers and evaluate our method in the aspects of visual inspection, accuracy and processing time. As experimental results, our method keeps accuracy and aligns very fast by reducing real-time computations.

  • Caries remains prevalent throughout modern society and is the main disease in the field of dentistry. Although studies of this disease have used diverse methodology, recently, X-ray microtomography has gained popularity as a non-destructive, 3-dimensional (3D) analytical technique, and has several advantages over the conventional methods. According to X-ray source, it is classified as monochromatic or polychromatic with the latter being more widely used due to the high cost of the monochromatic source despite some advantages. The determination of mineral density profiles based on changes in X-ray attenuation is the principle of this method and calibration and image processing procedures are needed for the better image and reproducible measurements. Using this tool, 3D reconstruction is also possible and it enables to visualize the internal structures of dental caries. With the advances in the computer technology, more diverse applications are being studied, such automated caries assessment algorithms.

Assessment of DRL for Computed Tomography in Local Hospital (지역병원에서의 전산화단층촬영 검사에 대한 DRL 평가)

  • In the field of imaging medicine, computed tomography is one of the most common test methods and one of the most frequently used test methods in hospitals. However, it is accompanied by a very high radiation exposure compared to other test methods. In order to reduce exposure, CT scans should be performed only when absolutely necessary, and even if the tests are performed because they are absolutely necessary, a protocol that serves the purpose of the test and allows the test to be performed in a small dose should be used. In this study, we wanted to learn about the most up-to-date radiation dose usage information used by the region's leading general hospitals and develop a diagnostic reference level (DRL). In the experimental results, the Head CT and Abdomen CT tests showed that DLP was higher than the NRPB (U.K) and Korean DRL. The DLP values used by Chest CT were low for all 3 types of CT devices. The hospital found that efforts to reduce exposure should be made during CT examinations, and in particular, Head CT and Abdomen CT determined that efforts to reduce exposure were necessary.

Noise Properties for Filtered Back Projection in CT Reconstruction (필터보정역투영 CT 영상재구성방법에서 잡음 특성)

  • The filtered back projection in the image reconstruction algorithms for the clinic computed tomography system has been widely used. Noise of the reconstructed image was examined under the input noise for parallel and fan beam geometries. The reconstruction images of $512{\times}512$ size were carried out under 360 and 720 projection by the Visual C++ for parallel beam and fan beam, respectively, and those agreed with the original Shepp-Logan head phantom very much. Noise was generated because of intrinsic restriction (finite number of projections) for the image reconstruction algorithm, filtered back projection, when no input noise was applied. Because the result noise was rapidly increased under 0.5% input noise ratio, technologies for reducing noise in CT system and image processing is important.

  • 단일광자방출전산화 단층촬영술을 이용한 영상정보를 효과적으로 얻기 위하여 고려되어야 할 사항으로는 1) 조준기(collimator)의 선택, 2) 기질(matrix)의 크기, 3) 회전각의 수 (number of angles), 4) 360도 또는 180도 획득(acquisition), 5) continuous 또는 step& shoot, 6) 원형 또는 비원형회전 등이 있다. 저자들은 비원형회전으로 검체와 검출기 사이의 거리를 단축시킴으로써 직선성, 균일성, 대조도, 해상력에 미치는 영향을 알아보기 위하여 원형회전 방법과 비교하여 다음과 같은 결과를 얻었다. (1) 비원형회전을 하여도 균일성(uniformity)과 직선성(linearlity)을 유지한다. (2) 균일성, 대조도(contrast), 해상력(resolution)들이 비원형 회전을 한 경우에 보다 더 개선되었다. (3) 영상 획득시간은 비원형회전인 경우에 더 소요되었다. (매스캔 당 10분) 따라서 검사자는 영상 화질의 개선효과와 상반되는 보정(calibration)과 설치(set-up)에 소요되는 시간(매스캔당 10분이상)을 비교하여 자료획득(data acquisition) 회전방법을 선택하여야한다.

Chest CT findings and Clinical features in Mediastinal Tuberculous Lymphadenitis (종격동 결핵성 임파선염의 흉부전산화 단층촬영 소견과 임상 양상에 대한고찰)

  • Background: Recently there has been a trend of an increasing incidence of mediastinal tuberculous lymphadenitis(MTL) in adults. MTL often cause bronchial stenosis or esophago-mediastinal fistula. In spite of effective treatment, it is difficult to cure. Moreover, relapse frequently occurs. Authors analyzed chest CT findings and clinical features of 29 cases with MTL Methods: 29 cases with MTL were retrospectively studied with the clinical and radiologic features from April 1990 to March 1995 Results: 1) A total of 29 cases were studied. 12 cases were male and 17 cases were female. The male to female ratio was 1:1.4 Mean age was 29 years old. The 3rd decade(45%) was the most prevalent age group 2) The most common presenting symptoms and signs were palpable neck masses(62%) followed by cough(59%) and sputum(38%) 3) Except in one case of MTL, all patients had coexisting pulmonary tuberculosis, cervical tuberculous lymphadenitis, endobronchial tuberculosis and tuberculous pleurisy. Among the coexisting tuberculous diseases, Pulmonary tuberculosis was the most common(76%) 4) On simple chest X-ray, mediastinal enlargement was noted in 21 cases(72%), but it was not noted in 8 cases(28%). The most frequently involving site was the paratracheal node in 16 cases(72%). Rt side predominence(73%) was noted 5) Patterns of node appearance on a postcontrast CT scan were classified into 3 types. There were 19 cases(30%) of the Homogenous type, 30 cases(47%) of the Central low density type and 15 cases(23%) of the Peripheral fat obliteration type. The most common type was the central low density type. The most common lymph node size was 1~2 cm(88%) 6) The most frequently involved site was the paratracheal node in 26 cases(89%) by chest CT. Rt side(63%) was predominant 7) 9 cases(43%) had complete therapy and most common treatment duration was 13 - 18 months. 12 cases(57%) had incomplete continuing antituberculous medication and half of the cases had been treated above 19 months. Conclusion: Chest CT findings of MTL showed central low density area and peripheral rim enhancement, so this characteristic findings could differentiate it from other mediastinal diseases and help a diagnosis of tuberculosis. In spite of effective antituberculous medication, it is difficult to cure. Moreover, relapse frequently occurs. Further studies will be needed of the clinical features and the treatment of MTL.

슬개 대퇴 불안정성에 대한 진단적 접근 방법

  • 슬개 대퇴 불안정성의 진단은 정확한 해부학적 지식을 바탕으로 역동적인(dynamic)기전을 이해하여야 한다. 슬개골의 저명한 탈구는 쉽게 진단되지만, 경미한 외상으로 슬개골이 탈구되어 정복된 후에는 올바른 진찰(dynamic study)없이 방사선 검사(static study)만으로는 진단을 놓치는 경우가 많다. 그러므로 정확한 진단을 위해서는 병력, 이학적 검사, 방사선 검사, 전산화 단층 촬영, 자기 공명 영상 그리고 관절경 검사까지 다양하게 필요할 수 있다.

  • 관절경적인 접근이 필요한 고관절의 동통을 호소하는 젊은 환자의 감별에서 가장 중요한 것은 동통의 원인이 관절 내인지, 관절외인지를 확인하는 것이다. 이에는 다양한 임상 진찰이 사용되지만, 이 뿐 아니라 단순 방사선 검사, 전산화 단층촬영, 자기공명영상, 초음파, 관절조영술 등의 다양한 조합을 통한 관찰이 중요하다. 그러나 진찰이나 병력 등을 통해 어느정도 의심되는 질환을 생각하지 않고 검사를 진행한다면 아무 결과를 얻을 수 없거나, 검사를 하고서도 병변을 찾아내지 못하는 문제가 발생할 수 있다. 이를 피하기 위해 검사의 선택시 각 검사 방법의 특징과 의심되는 질환의 성격을 잘 이해하는 것이 무엇보다 중요하다.

  • Chemomechanical approach to caries removal was introduced in order to preserve the maximum amount of sound tooth structure. The efficacy of chemomechanical caries removal was assessed using microcomputed tomography which offers 3 dimensional data without destroying the tooth, and the V works program. In group 1, the density values of the sound dentin, carious dentin, and remaining dentin after chemomechanical treatment were analyzed. In group 2, the density values of the sound dentin, cavity wall prepared using high speed bur, and the remaining dentin after additional $Carisolv^{TM}$ gel application on the same cavity were analyzed. The results were as follows; 1. The density value of the remaining dentin after the $Carisolv^{TM}$ treatment was 81.8% of the sound dentin(p < 0.001). 2. The density value of the remaining dentin after the conventional rotary instrument showed no statistically significant difference from that of the sound dentin(p = 0.234).

  • Various diseases can affect the esophagus. Endoscopic ultrasound (EUS), which provides precise information about the layers of the esophageal wall, is the primary approach used to investigate esophageal diseases. However, CT is one of the most important imaging modalities for diagnosing esophageal diseases as it can elucidate mediastinal involvement, adjacent lymphadenopathy, and distant disease spread. These two modalities complement each other in the diagnosis of esophageal diseases. Although radiologists may be unfamiliar with EUS procedures and their interpretation, understanding them aids in the differential diagnosis of esophageal conditions. This pictorial essay illustrates the EUS and CT findings of various esophageal diseases originating in the esophageal wall.
