Iatrogenic Iliac Vein Injury Following Extracorporeal Membrane Oxygenation Cannulation in a Patient with May-Thurner Syndrome: A Case Report and Literature Review (May-Thurner 증후군 환자에서 체외막산소공급 삽관 중 발생한 의인성 장골 정맥 손상: 증례 보고 및 문헌고찰)

  • A 53-year-old woman presented with dyspnea. She had undergone extended thymectomy for an invasive thymoma two months prior. CT revealed numerous small nodules in the lung. After that, she deteriorated owing to acute respiratory distress syndrome (ARDS), and the vascular surgeon planned veno-venous extracorporeal membrane oxygenation (ECMO). During percutaneous cannulation through the left femoral vein, a vascular injury was suspected, and the patient's vital signs became unstable. Diagnostic angiography showed a ruptured left common iliac vein, and the bleeding was stopped by placement of a stent-graft. May-Thurner syndrome was diagnosed on abdominal CT. Here, we report a rare case of ECMO-related vascular injury in a patient with an unrecognized anatomical variant, May-Thurner syndrome.

The Effect of Lung Volume on the Size and Volume of Pulmonary Subsolid Nodules on CT: Intraindividual Comparison between Total Lung Capacity and Tidal Volume (전산화단층촬영에서 폐 반고형결절의 크기와 용적에 호흡이 미치는 영향: 개인 내 전폐용량과 일호흡량 간 비교)

  • Purpose To examine the effect of lung volume on the size and volume of pulmonary subsolid nodules (SSNs) measured on CT. Materials and Methods A total of 42 SSNs from 31 patients were included. CT examination was first performed at total lung capacity (TLC), and a section containing the nodule was additionally scanned at tidal volume (TV). The diameter and volume of each SSN, as well as the cross-sectional lung area containing the nodule, were measured. The significance of the changes in measurements between TLC and TV within the same individuals was evaluated. Results The lung area and the diameter and volume of SSNs decreased significantly at TV by 12.7 cm2, 0.5 mm, and 46.4 mm3 on average, respectively (p < 0.001), compared to those at TLC. Changes in lung area between TV and TLC were positively correlated with the change in SSN diameter (p = 0.027) and volume (p = 0.014). However, after correction (by considering the change in lung area), the changes in SSN diameter (p = 0.124) and volume (p = 0.062) were not significantly different. Conclusion SSN size and volume can be significantly affected by lung volume during CT scans of the same individuals.

Morphological Analysis of the Mental Foramen and Anterior Loop of the Mandibular Canal using Computed Tomography (전산화단층촬영 방사선영상을 이용한 이공과 하악관 전방고리의 형태학적 분석)

  • The mental foramen and anterior loop of the mandibular canal are important landmarks for mandibular surgical procedures. The purpose of this study was to analyze the shape and position of the mental foramen and anterior loop of the mandibular canal on the computed tomography (CT) images, and apply the results clinically. CT images of 96 patients (33 male, 36 female, age range 17~43 years, mean $24.6{\pm}4.99$ years) were enrolled. The horizontal and vertical position of the mental foramen, as well as the distance from the root apices were measured. The distance of the anterior loop of the mandibular canal to the root apices, and the buccal angle were measured. The mental foramen was found mostly below the second premolar observed in 81 cases (46.0%), between the first and second premolars in 67 cases (38.0%), and between the second premolar and first molar in 19 cases (10.2%). The mean distance between the mental foramen and the lower border of the mandible was $12.20{\pm}1.77$ mm, the mean distance between the mental foramen and root apex was $5.16{\pm}0.98$ mm. The mean distance of the anterior loop of the mandibular canal was $5.80{\pm}2.00$ mm. The buccal angle measured at $47.7{\pm}9.07^{\circ}$. The distance between the root apex and mental foramen measured as $5.16{\pm}0.98$ mm on panoramic radiography, and $6.2{\pm}3.07$ mm on CT. The mean distance between the mental foramen and mandibular canal was $5.39{\pm}1.62$ mm. When performing surgical procedures such as installing dental implants, it is important to minimize surgical trauma, especially the risk of damage to the mental nerve. To optimize the surgical outcome, a careful assessment of the shape and position of the mental foramen and the anterior loop of the mandibular canal must be made. CT images are useful for finding such anatomic structures.

Improvement in Reconstruction Time Using Multi-Core Processor on Computed Tomography (다중코어 프로세서를 이용한 전산화단층촬영의 재구성 시간 개선)

  • The reconstruction on the computed tomography requires much time for calculation. The calculation time rapidly increases with enlarging matrix size for improving image quality. Multi-core processor, multi-core CPU, has widely used nowadays and has provided the reduction of the calculation time through multi-threads. In this study, the calculation time of the reconstruction process would improved using multi-threads based on the multi-core processor. The Pthread and the OpenMP used for multi-threads were used in convolution and back projection steps that required much time in the reconstruction. The Pthread and the OpenMP showed similar results in the speedup and the efficiency.

Stopping Power Ratio Estimation Method Based on Dual-energy Computed Tomography Denoising Images for Proton Radiotherapy Planning (양성자치료계획을 위한 이중에너지 전산화단층촬영 잡음 제거 영상 기반 저지능비 추정 방법)

  • Computed tomography (CT) images are used as the basis for proton Bragg peak position estimation and treatment plan simulation. During the Hounsfield Unit (HU) based proton stopping power ratio (SPR) estimation, small differences in the patient's density and elemental composition lead to uncertainty in the Bragg peak positions along the path of the proton beam. In this study, we investigated the potential of dual-energy computed tomography image-based proton SPRs prediction accuracy to reduce the uncertainty of Bragg peak position prediction. Single- and dual-energy images of an electron density phantom (CIRS Model 062M electron density phantom, CIRS Inc., Norfolk, VA, USA) were acquired using a computed tomography system (Somatom Definition AS, Siemens Health Care, Forchheim, Germany) to estimate the SPRs of the proton beam. To validate the method, it was compared to the SPRs estimated from standard data provided by the National Institute of Standards and Technology (NIST). The results show that the dual-energy image-based method has the potential to improve accuracy in predicting the SPRs of proton beams, and it is expected that further improvements in predicting the position of the proton's Bragg peak will be possible if a wider variety of substitutes with different densities and elemental compositions of the human body are used to predict the SPRs.

Evaluation of Dose and Image Quality of Lens according to Baseline during Brain CT Scan (두부 전산화단층촬영 시 기준선에 따른 수정체 선량과 화질 평가)

  • It is important to minimize the exposure dose during an examination and obtain good quality images at the same time. This study compared the beam harding effect according to the baseline superior orbito meatal line(SOML), orbito meatal line(OML), inferior orbito metal line(OML) and measured the exposure dose of the lens, especially in brain CT examinations, which generally apply to head diease patients. The beam harding effect assessment of each image along the baseline was performed quantitatively using the Image J program, and the exposure dose of the lens was detected by OSLDs and compared. As a result, As a result, when the SOML was used as the reference line, the dose of the lens was decreased by 85.08% at 80 kV and by 79.7% at 80 kV, compared to when IOML was used as the baseline. If the gantry angle at brain CT was parallel scan to SOML, there were no significant differences in the exposure to the lens and between the OML and IOML. Therefore, this study has shown that it is efficient to have a parallel scan on SOML as a protocol during Brain CT examinations.

  • Background: Pulmonary thromboembolism(PTE) is a life threatening disease that needs early diagnosis. Spiral CT angiography depict thromboemboli in the central pulmonary vessels with greater than 90% sensitivity and specificity, which approaches the results of pulmonary angiography in the Prospective Investigation of Pulmonary Embolism Diagnosis(PIOPED) study. This study was performed to evaluate the findings and the diagnostic value(clinical utility) of the spiral CT angiography with 2D image(multiplanar reformation) and 3D images(Shaded surface display, Minimal intensity projection) in the pulmonary thromboembolism. Methods: We retrospectively analysed spiral CT angiography and pulmonary angiography, lung scan and clinical recordings of 20 patients who had PTE diagnosed by spiral CT angiography(n=19 cases) or pulmonary angiography(n=l case) from September 1997 to August 1998. Among 20 patients who had underwent spiral CT angiography, 14 patients could be performed lung perfusion scan at the same time. We analyzed the vascular and parenchymal change in spiral CT angiogram. Efficiency Evaluation of CT Simulator QA Phantom (전산화 단층촬영 모의치료기 정도관리 팬텀의 유용성 평가)

  • Purpose: The purpose is to evaluate efficiency of the CT simulator QA phantom manufactured for daily QA. Materials and Methods: We made holes ($1{\times}100{\times}1\;mm$) to verify accuracy between image and real measurement in polystyrene phantom and made 1 mm holes to verify table movement accuracy at superior and inferior 100 mm to the center of the phantom and inserted radiopacity material. To evaluate laser alignment, we made cross mark on the right and left side at phantom and to evaluate CT number accuracy we made 3 cylindrical holes and inserted equivalence material of bone, water, air in them. After CT scanning the phantom, We evaluated accuracy between image and real measurement, accuracy of table movement, laser, and CT number using exposed image. Results: It was measured that the accuracy between image and real measurement was ${\pm}0.3\;mm$, table movement accuracy was ${\pm}0.3\;mm$, laser accuracy was ${\pm}0.5\;mm$ from 7th January to 7th March in 2008 as within the reference point ${\pm}1\;mm$. In the CT number accuracy of bone was ${\pm}10\;HU$, air was ${\pm}5\;HU$, water was ${\pm}5\;HU$ as within the reference point is ${\pm}10\;HU$. Conclusion: We was able to perform CT simulator QA and laser equipment QA more conveniently and fast using manufactured phantom at the same time. We will be able to make more accurate treatment plan that added to QA procedures using images at previous daily QA.
