Features and Trends of IEC Particular Standards for Medical Equipment Related to Diagnostic X-ray Based on IEC 60601-1:2005 Ed. 3.0 (IEC 60601-1: 3판이 적용된 진단용 X선장치 관련 개별규격의 동향 및 특징)

  • IEC publications have applied in many countries all over the world such as Europe or Japan and these also have been published as in dustrial standards (KS) and notifications of Korea Food and Drug Administration (KFDA) in Korea. As the general standard of IEC 60601 series for medical electric (ME) equipment was revised as $3^{rd}$ edition in 2005, additional and particular standards have been revised or established newly. Under these circumstances, it is importance for manufacturing and assembling companies or authorized testing companies to understand the trend for revisions of IEC publications. Therefore in this study, the latest version of 3 IEC standards related to medical X-ray equipment : IEC 60601-2-44 for X-ray equipment for computed tomography (CT), IEC 60601-2-45 for mammographic X-ray equipment and IEC 60601-2-54 for X-ray equipment for radiography or radioscopy were covered and analyzed for trends and features accompanied by revision based on IEC 60601-1 $3^{rd}$ Ed. As KFDA notifications in force have referred to the particular standards based on 2nd edition of IEC 60601-1, those revised version of 3 particular standards were compared to KFDA notifications in force. The features of the latest standards applying IEC 60601-1 $3^{rd}$ Ed were shown as following: 1) Requirements for mechanical hazards, especially (motorized) moving parts were emphasized. 2) Indication and recording of patient dose were required. 3) Risk management process was introduced and enabled to monitor potential risks systematically. 4) DR system (digital radiography system) as well as analogue system (film-screen system) was included in the scope. Presently, KFDA will revise the notifications applying the particular standards based on IEC 60601-1 $3^{rd}$ Ed in a few years. Therefore the features of particular standards applying IEC 60601-1 $3^{rd}$ Ed was expected to help manufacturers, assemblers or testing companies of medical electric equipment understand IEC publications or KFDA notifications slated to be published.

Research For Reducing Patient Dose that Low Extremity CT Angiography Using Automatic Current Selection (하지동맥조영 전산화단층촬영에서 자동선량 조절장치를 이용한 환자선량 감소 연구)

  • Purpose : Fixed way of mAs previously Low Extremity Computed Tomography Angiography(LECTA) examination were used. Automatic Current Selection(ACS) to use for the purpose of reducing the dose when Low Extremity Computed Tomography Angiography examining patients. Materials and methods : Were analyzed from July 2011 to July 2012 MDCT examination of Dose Length Product(DLP) LECTA 116 Case. It was defined as previous inspection methods(Old protocol). CT workstation is set to 100 mAs and 150 mAs protocol based on the patient's weight 70kg examined by LECTA. We defined as 'New protocol' that applies to ACS. The data collection period are 76 cases from October 2012 to January 2013 Results : 1. Average Total DLP of 'Old protocol' is 3602.943 $mGy^*cm$. 2. Average Total DLP of 'New protocol' is 1762.977 $mGy^*cm$. 3. Due to the 'New Protocol' use of Total DLP was reduced by approximately 51 %. Phase-specific dose reduction is as follows. Pre(33.62 %), Artery(64.63 %), Delay(49.0 %) 4. Using One way ANOVA Analysis of fluctuations obtained DLP is as follows. 'Old protocol', 'New protocol' a value of P < 0.001, P = 0.882 values were obtained. Conclusions : Dose reduction of 51 % is a useful study that proves. The results obtained using the ACS, the effects of a dose reduction of 51 % was obtained. Therefore, it has been proven to be a useful way. Statistics using SPSS version came out of the 'Old protocol' P-value P < 0.0001. This result means that the DLP a large difference values. On the other hand, The results of the 'New protocol' was P = 0.882. These results means to that small and regularly was fluctuations of the dose. The use of ACS, you can get a reduction of the dose and will able to get the effect of reducing the dose errors.

Stereotactic Radiosurgery for Recurrent Glioblastoma Multiforme using Yeungnam Localization Device - Technical note and Clinical trial - (뇌정위적 방사선 절제술에 필요한 위치선정용기구 제작과 치험 2예)

  • Authors performed a stereotactic radiosurgery with multiple noncoplanar convergent photon beams of linear accelerator (NELAC-1018 18 MeV, NEC) using a specially designed Yeungnam localization device for two patients with recurrent glioblastoma multiforme. One patient had 2 cm sized and the other 4 cm sized mass on the CT images. After single session of treatment with 15 and 20 Gy, headache was improved in a few days after radiosurgery with no remarkable untoward reactions. Our experience with these two patients were encouraging and we found that our localization device, which is easily adjustable and inexpensive, could be a valuable tool for stereotactic radiosurgery particularly in the treatment of recurrent brain tumor.

Study on accuracy of panorama and CBCT through length measurement of wisdom teeth (사랑니 길이 측정을 통한 파노라마와 CBCT의 정확도 연구)

  • Panorama of dental radiation generators can observe the wide anatomical structures of oral and maxillofacial areas but there can be distortion of lengths, angles, or shapes. CBCT can diagnose 3D images and get the ones whose errors by superposition and interference are remarkably smaller between anatomical structures. But the quality of the images by movement of subjects can be lowered as it takes long to diagnose them. And if there are impermeable radiation objects like metal in mouths, impermeable radiation lines can radially appear with the objects as center. This study tries to analyze accuracy of panorama and CBCT and get useful anatomical information in dental treatment by comparing the length of wisdom teeth which were measured by Panorama and CBCT with the teeth which were actually extracted and analyzing distortion of the teeth. The test result could be found that Panorama is expanded by average 7.3% as the errors of Panorama and Digital Vernier Caliper range from 110.7% to 103.9%. The length of wisdom teeth which were measured in CBCT and Digital Vernier Caliper could be found that the error range is 1.3%. And the length of wisdom teeth which were measured in Panorama and Digital Vernier Caliper has found that the error range shows 7.3%. So it could be found that the images of CBCT is about 6% more exact than those of Panorama. It could be found that CBCT shows the more exact images than those of Panorama. But because the examination expenses of CBCT are higher than those of Panorama and exposure dose of CBCT is much more than that of Panorama, it is thought to find proper ways in examination.

Computed Tomographic Simulation of Craniospinal Irradiation (전산화 단층 촬영 장치를 이용한 뇌척수 조사의 치료 계획)

  • The aim of this study is to improve the accuracy of field placement and junction between adjacent fields and block shielding through the use of a computed tomography(CT) simulator and virtual simulation. The information was acquired by assessment of Alderson Rando phantom image using CT simulator (I.Q. Xtra - Picker), determination of each field by virtual fluoroscopy of voxel IQ workstation AcQsim and colored critical structures that were obtained by contouring in virtual simulation. And also using a coronal, sagittal and axial view can determine the field and adjacent field gap correctly without calculation during the procedure. With the treatment planning by using the Helax TMS 4.0, the dose in the junction among the adjacent fields and the spinal cord and cribriform plate of the critical structure was evaluated by the dose volume histogram. The pilot image of coronal and sagittal view took about 2minutes and 26minutes to get 100 images. Image translation to the virtual simulation workstation took about 6minutes. Contouring a critical structure such as cribriform plate, spinal cord using a virtual fluoroscopy were eligible to determine a correct field and shielding. The process took about 20 minutes. As the result of the Helax planning, the dose distribution in adjacent field junction was ideal, and the dose level shows almost 100 percentage in the dose volume histogram of the spinal cord and cribriform plate CT simulation can get a correct therapy area due to enhancement of critical structures such as spinal cord and cribriform plate. In addition, using a Spiral CT scanner can be saved a lot of time to plan a simulation therefore this function can reduce difficulties to keep the patient position without any movements to the patient, physician and radiotherapy technician.

Analysis of Exposure Dose According to Chest and Abdomen Combine CT Exam Method (CT 흉·복부 통합검사 시 선량분석)

  • The purpose of this study is confirmed to usefulness between division exam and combine exam of chest and abdomen according to comparing chest and abdomen radiation dose of division exam and combine exam in CT exam method. This study was conducted on patients who were admitted to the E hospital from July 2013 to March 2014 underwent CT studies for the diagnosis of chest and abdomen disease. In study result, male dose were more higher than female dose according to gender analysis of exposure dose that combine exam effective dose were male $33.10{\pm}2.75mSv$, female $31.66{\pm}3.12mSv$ and chest exam effective dose were male $9.07{\pm}2.62mSv$, female $8.30{\pm}2.18mSv$(p<0.05). And, division exam dose and combine exam dose were similar in gender comparison (p>0.05). And, combine exam effective dose, only chest exam effective dose, only abdomen exam effective dose were more higher than DRL(Diagnostic Reference Level) in comparison of patient exposure dose with DRL (p<0.05). In conclusion, chest-abdomen combine exam dose and division exam dose were similar. The chest-abdomen combine study can be used as follow-up and emergency trauma patients. That study will be reduce exam time and the occurrence risk of side effect of the contrast medium.

Evaluation the absorbed dose in brain of dental radiography (치과방사선 검사에서 두부(brain)의 흡수선량 평가)

  • This study was aimed to evaluate the absorbed dose in brain of dental radiography. For radiographic exposure, PLD(photoluminescence dosimetry) chips placed in Rando phantom to measurement the absorbed dose to pituitary gland, orbit, maxillary sinus and submandibular glands, thyroid gland, esophagus. Equipments were used Kodak 2200, Kodak 8000C dental radiographic systems and computed tomography(Lightspeed VCT). The absorbed doses were measured at the same exposure parameters and distance by the clinical factor(kV, mA, sec). The result were as follows ; The absorbed dose for intra-oral radiography were 0.02~2.47cGy, the greatest absorbed dose was 2.47cGy for thyroid gland in maxillary right molar projection. the lowest adsorbed dose was 0.02cGy for submandibular glands in lower anterior projection. The absorbed dose for extra-oral radiography were 0.36~3.44cGy of cephalometric method, 0.14~12.82cGy of panoramic method, 8.17~253.63cGy of computed tomography, the greatest adsorbed dose was 253.63cGy for submandibular glands in maxillary CT scan. the lowest adsorbed dose was 0.14cGy for orbit in panoramic method. As a result, extra-oral radiography was measured more than intra-oral radiography. In particular, method which used computed tomography was measured more than 100 times than intra-oral radiography highly. Therefore, you must show a guideline in extra-oral radiography and an effort to reduce absorbed dose is demanded.

Assessment of the Effective Dose to the Human Body and Estimation of Lifetime Attributable Risk by CT Examination (CT 검사별 노출되는 유효선량과 생애 암 귀속 위험도 평가)

  • The number of CT scans is increasing every year due to the improvement of the medical standards of the public, and thus the annual dose of medical radiation is also increasing. In this study, we evaluated the effective dose of the human body exposed to CT scans and estimated LAR. First, five region were selected from the CT diagnostic reference level guideline, and the effective dose of human body exposed to each examination was evaluated by clinical CT device. Second, the human organs and effective dose were calculated using the ALARA-CT program under the same conditions. Third, lifetime attributable risk (LAR) estimated by the effective dose exposed through the previous CT scan was estimated. As a result, the most effective dose was 21.18 mSv during the abdomen 4 phase scan, and the dose level was below DRL for all other tests except for the abdominal examination. As a result of evaluating effective dose using a dose calculation program under the same conditions, the results showed about 1.1 to 1.9 times higher results for each examination. In the case of organ dose, the closer the organ to the scan site, the higher the scattering ray. The lifetime attributable risk to CT radiation dose in adults was gradually decreased with age, and the results were somewhat different according to gender.

Comparative study on the Shape between a Customized Finger Made by 3D Printing Technology, Real Small Finger, a plaster Small Finger, Based on CT Data (CT data 기반 3D 프린팅으로 제작된 Small Finger, 실제 Small Finger 그리고 석고 Small Finger 형상 비교 연구)

  • The purpose of this study is to compare and analyse the differences between a customized small finger made by 3D printing technology, a real small finger, and the other made from plaster of an orthotic company. The areas and the volumes of each cross-section were measured by Computer tomography(CT) and a 3D scanner and analysis of variance was performed to find out the differences of each shape. The areas of the point of 15.69mm, Distal Interphalangel Joints, were measured 30 times respectively using the caliper toll function of Picture Archiving Communication System(PASC) program. The volumes were measured by Configure Units of Meshmixer Program. There was no significant difference in the areas between three of them and there was 0.2 mm gap in the volume, which was more than the significance probability. Therefore, the result of this study shows the availability of finger orthoses made by 3D printing technology in the medical field.

Changes in CT Number and Noise Level according to Pitch in Spiral Image Acquisition (나선형영상획득에서 Pitch에 따른 CT 감약계수와 잡음의 변화)

  • In this study, a self-made customized phantom was used to quantitatively measure the change in CT number and noise according to the change of pitch. In order to acquire an image using the phantom, the inside of the phantom was filled with sterile distilled water. Inside the glass tube, a solution obtained by diluting the ratio of normal saline and contrast medium to 100%(NS), 400:1, 200:1, 100:1, 50:1, respectively, was placed and imaged. At this time, the pitch was divided into steps of 0, 0.35, 0.7, 1.05, and 1.4 for each dilution ratio of the solution and imaged, respectively. One-way ANOVA analysis were performed to verify whether the mean of the CT number and noise values measured in all ROIs by dilution ratio showed a significant difference according to the change in pitch. As a result of the experiment, there was no statistically significant difference in the change of the CT number according to the change in the pitch for each dilution ratio, but the noise value tended to increase with the increase of the pitch, and showed a statistically significant difference. In the spiral image acquisition of CT, noise can be changed to a significant level depending on the pitch. Therefore, it will be necessary to set the quality evaluation items and criteria for CT images using the spiral image acquisition method.
