Clinical Results of Arthroscopic Anterior Cruciate Ligament Reconstruction using Hamstring Tendon (슬괵건을 이용한 관절경적 전방십자인대 재건술)

  • Purpose : We intend to review clinical results after Anterior cruciate ligament(ACL) reconstruction using hamstring tendon arthroscopically. Materials & Methods : Sixty two patients who had underwent arthroscopic ACL reconstruction using hamstring tendon from Mar. 1996 to Mar. 1997 were reviewed. The average follow-up was 16 months and average age at operation was 27 years old. Clinical results were evaluated with physical examination, Lysholm Knee score and instrumented anterior laxity test with Telos Results : The average preoperative Lysholm knee score was 57.0 and postoperative average of that was 91.8. All cases of 62 patients had normal range of motion of knee and were able to walk with no problems at follow-up. On the Lachmann test, there were mild(+) instability in 24 cases, moderate(++) in 24 cases, severe(+++) in 14 cases preoperatively and 48 cases were converted to negative, 14 cases mild postoperatively. On instrumented anterior laxity test with Telos, difference between normal and affected knee on 20 lb which was 13.4mm preoperatively was decreased to 4.7mm at follow-up and anterior stability was regained(P<0.05). Parapatellar complications such as crepitus in 18 cases(29.5%), atrophy of quadriceps in 23 cases(36.5%) were observed. There were 2 cases of inaccuracy of guide pin of semifix screw intraoperatively and 3 cases of malposition of semifix screw postoperatively. Conclusion : ACL reconstruction using hamstring tendon seems to be a effective procedure to establish the stability of knee joint but is technically demanding procedure and leaves some parapatellar complications.

  • This paper introduces the simulation concepts and technical approach of wire-guided torpedo performance analysis simulator, as a consequence, provide a framework for understanding overall attack procedures and effectiveness of tactics to torpedo operator. It described the mathematical models of simulation components and weapon engagement principle, especially it derived the closed-form solution of time consumption and leading angle problem of torpedo attack situation based on geographical assumption. In addition, it adopted the proportional navigation guidance at final stage of torpedo attack and also consider the tradeoff relation between target ship speed(propeller noise level) and detection probability, so that it improves the fidelity of physical realism. Simulator is developed with high degree of freedom in the perspective of tactical situation, and it helps user to understand the overall situation and tactical effectiveness.

Arthroscopic Double-Bundle Reconstruction of Anterior Cruciate Ligament (관절경을 이용한 전방 십자 인대의 이중 다발 재건술)

  • Purposes: The purpose of this study was to report surgical technique of double bundle anterior cruciate ligament(ACL) reconstruction and to compare the short-term clinical results between arthroscopic single-bundle and double-bundle ACL reconstruction. Materials and Methods: From May 2005 to May 2006, ninety-eight patients were underwent ACL reconstruction. We designed prospective study with sixty-one patients who were revealed isolated ACL injury. We serially checked clinical and radiologic data preoperatively and postoperatively. We compared single-bundle with double-bundle ACL reconstruction patients with preoperative datas and postoperatively 1-year data. There were 30 single bundle reconstruction and 31 double bundle reconstruction. Stability was assessed objectively by anterior stress radiographs with the $Telos^{(R)}$ device and the maximal manual test with the KT-2000 arthrometer. The clinical results were assessed by IKDC(International Knee Documentation Committee) and OAK(Orthopadische Arbeitsgruppe Knie) scores. Also, we evaluated postoperative thigh circumference and range of motion. All of operations were done by only one surgeon. Results: At single-bundle reconstruction group, preoperative AP instability which was checked by $Telos^{(R)}$ device and the maximal manual test with the KT-2000 arthrometer was $7.9{\pm}3.3$ and $7.4{\pm}2.0$, respectively. At double-bundle reconstruction group, it was $8.3{\pm}3.5$ and $7.9{\pm}3.2$, respectively. Residual AP laxity checked at 1 year after operation was $1.9{\pm}1.2$ and $2.2{\pm}1.6$ in single-bundle reconstruction group, and $1.1{\pm}0.9$ and $1.0{\pm}1.0$ in double-bundle reconstruction group. So, double-bundle reconstruction had better results in both anterior stress radiographs with the $Telos^{(R)}$ device and the maximal manual test with the KT-2000 arthrometer, and there were significant differences in statistics. But, clinical results such as IKDC(International Knee Documentation Committee) scores, OAK(Orthopadische Arbeitsgruppe Knie) scores, thigh circumference and range of motion had no significant difference between two groups. Conclusions: On the basis of stability, the side-to-side anterior laxity of double-bundle ACL reconstruction was significantly better than that of single-bundle reconstruction, although there were no significant differences in the other clinical measures among them.

EVALUATION OF INTERNAL DERANGEMENT OF TMJ BY ANALYZING TMJ SOUNDS (측두하악관절잡음을 이용한 악관절내장증 진단)

  • The purpose of this study was to evaluate the diagnostic value of $SONOPAK^*$ in internal derangement of TMJ by comparing the spectral analysis data of TMJ sounds recorded by SONOPAK with the results of MRI. From the patients who came to Department of Orthodontics, Seoul National University Dental Hospital for treatment of malocclusion, eighteen adult patients (five males and thirteen females) with TMD symptoms were selected. After joint sounds were checked by a pediatric stethoscope, they were recorded and analyzed by the SONOPAK. The spectral analysis of the SONOPAK provided SONOPAK INTERPRETATION data about the stage of internal derangement, which were compared with the results of MRI. Among the patients whose disc positions were diagnosed as 'normar by MRI, there were no false positive diagnosis by the SONOPAK INTERPRETATION, But in the cases of anterior disc displacements (reducible and/or nonreducible), most of SONOPAK INTERPRETATION data did not coincide with MRI results. In conclusion, it is not adequate to try to differentiate reducible and non-reducible anterior disc displacements on the basis of joint sounds. And it is recommended not to determine the stage of internal derangement according to the nature of sounds. We suggest that the diagnostic value of the SONOPAK will be enhanced when clinicians combine some other informations such as clinical signs/symptoms and other supplementary diagnostic tools), and that more data be incoporated into the SONOPAK INTERPRETATION software.

  • Rock mass includes natural discontinuities such as joints and faults during its formation. Discontinuities are also referred as planes of weakness because of their weak mechanical characteristics. In the design of underground structures, it is necessary to consider the properties of discontinuities to insure the stability. During the excavation of a tunnel, these discontinuities have to be identified as early as possible so that proper change in excavation method or support design can be made accordingly. The excavation of the tunnel in a stable rock mass causes a 3-dimensional arching effect around the excavation face. It was revealed by previous studies that the existence of a weak zone or a fault zone ahead of tunnel foe induces a typical displacement tendency of convergence. For better understanding of the meaning of influence/trend lines of various displacement components, three-dimensional numerical analyses were conducted while varying deformation moduli, thicknesses and orientations of discontinuities. Numerical results showed that the changes in influence/trend lines of various displacement components were very similar to those by measurements. The discrepancies from the expected values were dependent on the physical properties, thicknesses and orientations of discontinuities.

Electrical surveys at the Okmyung waste landfill of Pohang (포항 옥명 폐기물 매립장에서의 전기탐사)

  • Schlumberger soundings, dipole-dipole survey and electrical conductivity mappings were carried out inside and in front of the entrance of the Okmyung waste landfill in August, 1997 and January, 1998. Inside and in front of the landfill, 11 and 4 electrical soundings and 1 dipole-dipole survey were carried out, respectively. Electrical conductivities were measured at 164 points along the 4 lines in front of the entrance of the landfill. Interpretations of survey data show that low resistivity zones of 0.3∼3 Ωm extend down to 65 m depth from the surface in the 6th landfill, which indicates subsurface contamination by leachate and leachate level at 3∼6 m depth from the surface. In the 9th landfill, low resistivity zones below 2 Ωm appear at 11∼15 m depth from the surface, which indicates a very slim chance of subsurface contamination. On the other hand, electrical surveys and electrical conductivity mappings reveal low resistivities at shallow depths in front of the entrance of the landfill, indicating a high possibility of contamination of weathered zone in this area. It appears that southern part of this area close to the 6th landfill is more contaminated by leachate.

The Effect of Mandibular Protrusion on Dynamic Changes in Oropharyngeal Caliber (하악의 전방이동이 구인두 내경의 동적 변화에 미치는 영향)

  • The purpose of this study was to determine the sites of narrowing/obstruction and to measure the regional severity of narrowing through the evaluation of dynamic changes in upper-airway of healthy subjects. The selected 9 subjects were proved not to have any sleep-related disorder such as snoring or obstructive sleep apnea through clinical examination, radiological examination, sleep study with a portable recording system. Afterward, the Electron Beam Tomography was performed during the waking and sleeping state of subjects, with their mandible in resting and protruded position. Intravenous injection of Dormicum$^{(R)}$ was used for the induction of sleep. The maximum and minimum cross-sectional areas at each airway level during tidal ventilation were measured and the Collapsibility Index for each level of cross-section was also computed. In a comparison with results under variable conditions, the result was showed that the significant difference between each airway level divided with upper, middle, lower region of upper airway is not observed in the average minimum cross-sectional areas and Collapsibility Index. The significant difference only between in wake and sleep state was observed in the average minimum cross-sectional area at the lower region. Also, in wake state, the significant difference between resting and protrusive position of mandible for the average minimum was also observed in cross-sectional area at middle region. In sleep state, no significant difference between resting and protrusive position of mandible was observed in cross-sectional area and the Collapsibility Index.

Theoretical and experimental studies on influence of electrode variations in electrical resistivity survey for tunnel ahead prediction (터널 굴착면 전방조사를 위한 전기비저항 탐사에서 전극의 변화가 미치는 영향에 대한 이론 및 실험연구)

  • Variety of tunnel ahead prediction methods have been performed for safe tunnel construction during tunnel excavation. Pole-pole array among the electrical resistivity survey, which is one of the tunnel ahead prediction method, has been utilized to predict water-bearing sediments or weak zone located within 5 times of tunnel diameter. One of the most important processes is the estimation of virgin ground resistivity and it can be obtained from the following process: 1) calculation of contact area between the electrodes and the medium, and 2) assumption of the electrodes as equivalent spherical electrodes which have a same surface area with the electrodes. This assumption is valid in a small contact area and sufficient distance between the electrodes. Since the measured resistance, in general, varies with the electrode size, shape, and distance between the electrodes, it is necessary to evaluate the influence of these factors. In this study, theoretical equations were derived and experimental tests were conducted considering the electrode size, shape, and distance of cylindrical electrodes which is the most commonly utilized electrode shape. Through this theoretical and experimental study, it is known that one should be careful to use the assumption of the equivalent half-spherical electrode with large ratio between the penetrated depth and radius of the cylindrical electrode, as the error may get larger.

  • It is reported that the causes of unaesthetic proportion of anterior teeth vary widely. Especially, when the unaesthetic tooth proportion of the mandibular incisors arises due to the wear of the anterior teeth accompanied by the compensation of the alveolar bone, it may cause serious functional and aesthetic problems. In such case, it should be considered that the evaluation of vertical dimension and tooth proportion as well as smile line, soft tissue and hard tissue morphology. And, increase of vertical dimension or clinical crown lengthening followed by prosthodontic restorations is needed to improve the interdental mesial/distal, width/length ratio considering the anterior guidance. This case report demonstrates functional and aesthetic improvements through systematic diagnosis and treatment procedures in a 48-year-old male patient with unaesthetic anterior teeth proportion because of tooth wear accompanied by the compensation of alveolar bone and defect of several central incisors due to chronic periodontitis.

A Study on Improvement of Launch Performance for Precision Guided Missile by Live-fire test results (사격시험 결과에 따른 정밀 유도무기 발사성능 개선에 관한 연구)

  • Precision Guided Missiles after production and militarization have various characteristics that enable the final performance to be identified by conducting live-fire tests after long-term storage. Likewise, the performance and reliability of ${\bigcirc}{\bigcirc}$ Missiles, which are currently used by the Korean Navy, are also verified consistently by conducting live-fire tests after militarization. Specially, the live fire test at '00 year, which was conducted by Korean Navy, showed the result that 'Ring', which is a component of the canister's front cover, was jammed with wings for propulsion and then broke away from the canister during the missile launch process. This situation led to an interruption of the deployment of wings and finally affected the missile's flight performance. The results of a survey of the canisters of those missiles whose live fire tests were conducted successfully, based on the live fire test at '00 year, showed the 'Ring's separation from canisters. This raises recognition for the need to solve the problems of 'Ring's separation and breakaway. This study suggests an improvement derived by the result of live-fire tests and introduces the effect of final launch performance of ${\bigcirc}{\bigcirc}$ Missiles and test result after applying the improvement.
