A Comparison of Empowerment, Job Satisfaction, Organizational Commitment between Advanced Practice Nurses and Registered Nurses of Hospitals (전문간호사와 일반간호사의 임파워먼트, 직무만족, 조직몰입 비교)

  • The purpose of the study was to identify and compare levels of recognition of empowerment, job satisfaction, organizational commitment between Advanced Practice Nurses(APN) and Registered Nurses(RN) of hospitals. Data collection was done by the 85 APN and 120 RN of six large hospitals, four medium sized hospitals in Seoul and Incheon. T-test and ANOVA using SPSS 18.0 were performed. There were no significant differences of empowerment between APN($3.56{\pm}0.53$) and RN($3.50{\pm}0.54$)(t =-. 861, p = 0.390). job satisfaction between APN($3.36{\pm}0.53$) and RN($3.25{\pm}0.46$)(t =- 1.575, p = 0.117), organizational commitment between APN($3.11{\pm}0.34$) and RN($3.05{\pm}0.36$)(t =- 1.301, p = 0.195). Empowerment, job satisfaction, organizational commitment of APN and RN showed no significant differences. There are some reasons why it resulted in no significant difference between APN and RN. APN's role in Korea is not clearly established and distinguished from RN, lack of appropriacy of employment and reward, and lack of the institutional foundation for APN's operation and efficiency.

A Comparative Law Study on the Professional Work of Nurses: Focusing on Legal Basis and Standardization (간호사의 전문적 업무에 대한 국가 간 비교법적 연구: 법적 근거와 업무 범위 표준화를 중심으로)

  • This study attempted to examine the ambiguity of work from the legal, social perspective, and actual performance differences in domestic nursing work compared to foreign countries. We compared the historical background of nurses' expanded work through overseas situations, compare and analyze the legal basis for nurses' qualifications and work in each country, and what changes they have undergone to clarify their work. Through this, we would like to consider the current status of the absence of a legal basis for professional work of domestic nurses and seek a direction for the development of domestic medical care. This study applied the case study method as one of the comparative institutional research methods. Among OECD countries, developed countries such as the United States, Australia, and Japan were selected and compared among developed countries that are solving medical gaps using nurses. In the United States, Australia, Japan, and Korea, nurses' professional work has been created by changes in the medical environment due to an aging society, chronic diseases, and lack of doctors. We looked at the start of their professional work, the establishment of legal grounds, the timing of qualification recognition, the development of the credential system and scope of work. Foreign countries have legal grounds for their roles and tasks, but domestic countries are before legislation. The country still has not narrowed the gap between the position of the legislative and judicial branches and actual work, and the current status of the domestic healthcare system has been measured through overseas development cases.

Study on the Subjectivity about Nursing Student's Image of Professional Nurse before Clinical Practice (임상실습 전 간호대학생의 전문 간호사 이미지에 관한 주관성 연구)

  • This study designed to nursing student's image of professional nurse before clinical practice using Q-methodology. Using Q-methodology, 43 nursing student sorted 34 statements selected professional nurse type. The statement positions on the sorting continuum were scored from +4(representing the highest level of agreement) through 0(at the neutral position on the continuum) through -4(representing the highest level of disagreement). Factor analysis of the data was undertaken using PC QUANL software to determine if any patterns were discernible. The three factors extracted eigenvalues greater than 1.0 and explained approximately 53.94% of the variation in responses(41.63, 7.41 and 4.9% respectively). The categories were labeled 'Nightingale's nursing philosophy type', 'Consideration of patient type' and 'Self-development type'. Twenty-two students who located to factor 1 "Nightingale's nursing philosophy type". Eleven students who located to factor 2 "Consideration of patient type". Three students sho located to the factor 3 "Self-development type". The significant differences found in the subjective structure of nursing student's image of professional nurse. The study results can be utilized in both an effective nursing education and clinical practice.

Relationship of Perception of Clinical Ladder System with Professional Self-Concept and Empowerment based on Nurses' Clinical Career Stage (간호사의 임상경력단계에 따른 경력개발제도 인식과 전문직 자아개념, 임파워먼트와의 관계)

  • 연구목적: 본 연구는 간호사의 임상경력단계에 따른 경력개발제도에 대한 간호사의 인식을 파악하고, 경력개발제도에 대한 인식, 전문직 자아개념과 임파워먼트와의 관계를 파악하여 간호사 경력개발제도의 개선의 근거 마련하고 전문직 자아개념과 임파워먼트의 증대 방안 모색을 통한 인적자원관리에 기여하고자 시도 된 서술적 상관관계 연구이다. 연구방법: 서울시 소재 일 상급종합병원에서 근무하는 중환자실, 수술실, 응급실 간호사 162명을 대상으로 설문지를 이용하여 경력개발제도에 대한 인식, 전문직 자아개념, 임파워먼트를 측정하였다. 수집된 자료는 SPSS WIN 18.0 프로그램을 활용하여 서술적 통계, t-test, ANOVA, Pearson's Correlation Coefficient, Multiple linear Regression을 시행하였다. 연구결과: 간호사의 임상경력단계에 따른 경력개발제도에 대한 인식은 전임 2 간호사가 신입 간호사, 일반 간호사, 전임 1 간호사보다 높은 인식을 가지고 있었다. 경력개발제도에 대한 인식과 전문직 자아개념, 임파워먼트에는 통계적으로 유의한 양의 상관관계가 있었다. 다중회귀분석을 실시한 결과 경력개발제도에 대한 전반적 이해, 경력개발제도에 대한 기대효과, 최종학력, 임상경력단계가 전문직 자아개념의 42% 설명하는 것으로 나타났고, 경력개발제도에 대한 전반적 이해, 전문적 활동 참여에 대한 인식, 경력개발제도에 대한 기대효과, 임상경력단계가 임파워먼트를 42% 설명하였다. 결론: 전문직 자아개념과 임파워먼트에 영향을 미친 변수로 나타난 경력개발제도에 대한 인식을 향상시킬 수 있는 방안을 개발하여 적용을 통한 효과 검증이 요구되며, 간호 관리자들의 제도 운영과 관련된 장애요인의 파악 및 세심한 제도 개선이 필요하다.

The Influence of Nursing Image, Job Value on Nursing Professionalism of First Year Nursing Students (간호학과 신입생의 간호사이미지, 직업가치관이 간호전문직관에 미치는 영향)

  • This study examined the influence of nurse image, job value on the nursing professionalism of first year nursing students. Data were collected from May 7 to May 14, 2018 for 306 first year nursing students. The collected data were analyzed using SPSS 22.0. The average degree of nursing image at 4.12, job value score of 3.12, and nursing professionalism score of 3.98. Nursing professionalism was positively correlated with the nursing image(r=.823, p<.001) and job value(r=.492, p<.001). The significant variables explaining nursing image(${\beta}=.760$, p=.000) and job value(${\beta}=.134$, p=.000) These factors explained 69% of nursing professionalism. It is necessary to provide education that consider the nursing image, job value of nursing students to help them establish an acceptable view of nursing professionalism.

  • The importance regarding the notion of immediate medical treatment and nursing for the victims of sexual crimes is not widely recognized yet in Korea. Throughout this research is focused on the nursing systems and the system of foreign countries in reference to sexual crime victims, and the conclusion is as follows. In Korea, there are currently no educational or medical institutes for the medical handling for the victims of sexual crimes, and the role of nurses in the medical institutes is insufficient. In addition, the basis for the standard of operation regarding the initial response, evidence collection, pursuit of systematical evidence and data is not established even in the sexual assault counseling/advocacy center. Therefore, it is necessary to foster the sexual assault nurse examiner for the sexual crime victims, and to develop adequate training programs based on the present situation in Korea. Though the cooperation among government, medical institutes, and social movement groups, it is essential to initiate effective and publicly-trusted specialized training programs for forensic nurses who will be fully charged with the treatment of sexual crime victims. By introducing the sexual assault nurse examiner system, it can be possible to develop advanced treatment for the child or adolescent victims of sex crimes.
