Prosthetic full mouth rehabilitation of patient with mandibular prognathism and asymmetry: a case report (하악의 전돌 및 비대칭을 가진 환자에서 보철적 전악 구강회복 증례)

  • Severe cases of mandibular prognathism can be treated with orthodontic therapy or surgical correction, but there are situations where only prosthetic improvement is possible. An understanding of class III patients is needed, including a vertical chewing pattern and the absence of anterior guidance. Additionally, it is relatively easy to increase occlusal vertical dimension to correct the anterior crossbite, but this approach can sometimes lead to unfavorable results, necessitating careful diagnosis and treatment planning. In this case report, oral rehabilitation was conducted in a patient with mandibular prognathism and asymmetry, utilizing implants and fixed dental prosthesis. Through a step-by-step treatment approach, the existing occlusal vertical dimension was maintained, and the final fixed dental prosthesis restoration was completed. Accordingly, it shows functional and aesthetically appropriate results, and reports on the patient's diagnosis and treatment process.

A study on the dental arch characteristics of bialveolar protrusion patients using a three-dimensional digital model (3차원 디지털 모형을 이용한 양악 치조골 전돌자의 치열궁 특성 연구)

  • Recent beauty trends have caused an increase in the number of bialveolar protrusion patients seeking treatment of lip protrusion. But studies of these patients are not common. Studies using their dental models are especially rare. Dental models have been measured manually or through a computer by digitizing two dimensional images of models. Nowadays, we are able to study dental models more easily and accurately by utilizing the three-dimensional (3-D) laser scanner in dentistry. An investigation was carried out to evaluate the characteristics of the dental arch in a bialveolar protrusion group in comparison with a normal group using 3-D digital models. The normal group was composed of 20 subjects who were selected from students of the School of Dentistry, Kyungpook National University. The bialveolar protrusion group was composed of 20 subjects who visited for treatment at the Department of Orthodontics, Kyungpook National University Hospital. Tooth size, arch width and arch length were measured digitally, and arch shape and the shape of the palate were drawn. Based on the results of this study, the differences of the arch characteristics in the bialveolar protrusion group were that the size of the teeth was larger, mandibular canine width and 1st premolar width were wider, and arch length was longer than in the normal group. And there were differences in the arch shape and the shape of palate between the bialveolar protrusion and normal groups.

Hard and soft tissue profile changes following anterior subapical osteotomy in bimaxillary dentoalveolar protrusion patients (양악성 치조 전돌 환자의 근첨하 분절 골절단술 후 경조직 및 연조직 측모 변화)

  • The purpose of this study is to evaluate hard and soft tissue changes following the subapical osteotomy in bimaxillary dentoalveolar protrusion patients requiring maximal retraction adult female patients was selected. Surgical procedures were performed by the same surgeon, anterior subapical osteotomy techniques were employed on the maxilla and cephalometric radiograms were traced and superimposed using the best-fit method and two reference The results were as follows 1. The bodily movement of the maxillary anterior segment was achieved in a posterior moved posteriorly with a slight correction of the lower incisors. 2. The horizontal soft tissue measurements changed significantly after treatment, but Nt and Sn vertical soft tissue measurements indicated that Ls moved inferiorly and Li superiorly. 3. The correlation between hard and soft tissue changes indicated that ${\Delta}HId/{\Delta}HLi,\;{\Delta}HId/{\Delta}LL-Eline,\;{\Delta}Hpt.B/{\Delta}HILS,\;and\;{\Delta}UI-FH/{\Delta}NL$ were significant. 4. More lower lip relative to upper lip retraction was demonstrated in relation to Rickett's E-line. The ratio between upper lip displacement was $50\%$, and between the lower incisor and lower lip displacement was $60\%$. We conclude from the results that the anterior subapical osteotomy is an efficient treatment severe dentoalveolar protrusion and desire rapid results.

DENTOFACIAL CHANGES IN CLASS I PROTRUSION PATIENTS TREATED WITH PREMOLAR EXTRACTIONS (제 1 소구치 발치가 수반된 Class I전돌 증례의 치료 전후 변화)

  • The purpose of this study was to evaluate the dentofacial characteristics and the fost-treatment dentofacial changes of those treated by four premolar extractions and to investigate the factors affecting extraction decision. The sample consisted of 35 patients (27 females, and 8 males) with no more than 7.0mm crowding, diagnosed as Class I protrusion. Pre-treatment and post-treatment lateral cephalograms were evaluated. Computerized statistical analysis was carried out using SPSS/PC+ program. The results were as follows. 1. There was no significant change in skeletal pattern after treatment while there was significant change in dentoalveolar and soft tissue pattern. 2. In pre-treatment skeletal pattern, a tendency toward vertical discrepancy was found. 3. In pre-treatment dental pattern, interincisal angle was $113.11^{\circ}$, U1 to FH was $117.78^{\circ}$ and L1 to A-Pog was 7.94mm. Pre-treatment upper and lower lip position was 2.88mm and 5.43mm to E line. 4. After treatment, interincisal angle increased $14.46^{\circ}$ and upper and lower lip moved back 2.45mm and 3.2mm to E line.(p<0.001) 5. The EI was 138.71 before treatment and 148.2 after treatment.

  • Several authors have proposed techniques and devices by which the correct position of the proximal segment can be maintained both during fixation and postoperatively. Schendel, Epker, Lake, Worms, Ive and Poulton have been discussed the problem of condylar distration in mandibular orthognathic surgery. This study described have showed the some advantages forward repositioning of the condyle head in vertical ramus osteotomy which used with the Mini-Hoffmann sets in external skeletal pin fixation extraorally in 19 cases.

  • LINGUAL ARCH APPLIANCE는 1908년 Mershon이 처음 창시한 교정장치로 band를 사용하는 교정장치중 가장 간단하면서도 여러가지 장점을 지니고 있다. 이 장치는 특히 성장발육기의 소아교정에 최적하여 치아의 기능을 제한하지 않으며 악골 및 치아의 자연적인 성장발육을 허용하고 자연적인 치아이동을 도모하게 한다. 또한 이 장치의 특징은 지속적인 교정치료보다는 간헐적인 치료를 한다는 사실이다.예를 들면 Angle씨 2 급부정교합 즉 상악전돌의 치료시는 상악에 Libial arch wire, 하악에 Lingual arch wire를 장착하고 여기에 악간고무를 이용하여 2급의 교합상태를 개선한후 수개월간 장치를 제거하여 치아의 자연적인 적응을 허용하고 다시 장치를 장착하여 치료를 계속하는 것이다.

  • Two patients, sought treatment for chief complaints of protruding frontal tooth and desired treatment to reduce the prominance of lips, were diagnosed as bimaxillary protrusion via clinical and cephalometric analysis. The authors corrected them by combined surgical and orthodonic treatment. As pre-surgical survey, paper and cast surgery were performed and wafer and resin sprint were constructed. We performed anterior maxillary and mandibular osteotomies in first premolar site to retract the maxillary and mandibular dentoalveolar segment in order to ; 1) decrease prominance of upper and lower lips. 2) create proper lower incisor intrusion. By use of intrtamaxillary fixation, prompt oral intake was possible. We made good result of esthetic improvement and there was no evidence of relapse and any complication.

Comparison of computer-based treatment prediction with true results in bimaxillary protrusion cases (치조성 양악 전돌 환자에서 컴퓨터를 이용한 치료 결과 예측의 정확성)

  • This study was performed to assess the accuracy of computer-based treatment prediction for soft tissue profile using Quick Ceph Image $Pro^{TM}\;&\;Quick\;Ceph\;2000^{TM}$ in bimaxillary protrusion cases. The Ore- and post-treatment lateral cephalograms of 21 female adults treated by low first premolar extraction were imaged and 9 landmarks and 27 specific soft tissue mesurements were digitized for comparing actual treatment results with computer simulations. The results of this study showed that Quick Ceph Image $Pro^{TM}\;&\;Quick\;Ceph\;2000^{TM}$ tends to overestimeate horizontal changes and underestimate vertical changes. In the computer simulation, upper lip showed rolling tendency. The upper lip measurements were disposed to be regular direction hut lower lip measurements were varied case by case even if it was statistically insignificant.

  • Floating-Harbor syndrome (FHS) is a rare genetic disorder. This report introduced in a patient with FHS. Distinctive facial characteristics, severe skeletal class 3 malocclusion with underdeveloped maxilla and protruded mandible, congenital missing teeth, microdontia and ectopic positions of maxillary teeth were presented in the patient. In his twin sister, mild skeletal class 3 malocclusion with protruded mandible was observed but congenital missing teeth and microdontia were not observed. High-arched palate, narrow V-shaped maxillary arch compared to wide and ovoid mandibular arch and inverse relationship between the maxillary and mandibular intermolar width resulted in posterior crossbite were confirmed by model analysis of the patient. These were not observed in the twins. Behaviorally, poor cooperation during dental treatment because of mental retardation was observed in the patient.

  • 악안면외과 교정술은 악골, 안면 혹은 구강에 어떤 기형증에 있을때 이를 외과적 처치에 의하여 개선하거나 치료하는 것을 말한다. 악안면, 구강영역에 발생하는 기형증은 여러가지가 있으나 대체로 비대칭성안모, 악골의 전돌증이나 후퇴증, 개교증, 토순, 구강파열 및 선천성안모발육부전증등 여러 가지가 있다. 본인은 지난 83년 2월부터 5월 초순까지 스위스 쥬리히 치대 악안면외과 교실을 방문하여 이곳 교수들과 접하고 의견을 나누었으며, 강연도 듣고, 하여 주기도 하였다. 또한 이곳 병원에서 외과적안모교정술 등 여러 가지 새로운 시술방법등을 견학하면서 새로운 시술방법을 연구한 바 있다. 지면과 시간이 허락되는 범위안에서 앞으로 수회에 나누어서 악안면외과 교정술에 대하여 구체적으로 소개하고저 한다.
