  • A structure of musculotendon model with a fatigue profile is investigated. The Hill-type musculotendon model can predicts the decline in muscle force for a given fatigue profile. It consists of nonlinear activation and contraction dynamics based on the physiological concepts. It is normalized for generalization to deal with the various muscles. Muscle force generated by continuous tetanic electrical monophasic pulsewidth modulation stimulation is decreased in time. A fatigue profile is expressed by a function of intramuscular acidification and applied to the relationship between muscle force and shortening velocity in contraction dynamics. The results of computer simulation are well matched with data in a literature which are isometrically performed for knee extension muscles. Also change in optimal fiber length has an effect only on muscle time, constant not on the steady-state tetanic force.

Effects of TENS and EA on the Expression of NT-3 on Lumbar Spinal Cord after Crush Injury of Extensor Digitorum Muscle (전침자극과 경피신경전기자극이 장지신근의 압좌손상 후 척수수준에서의 NT-3 발현에 미치는 영향)

  • Purpose: The purpose of this study to investigate the effect of TENS and electroacupuncture (EA) on injured muscle recovery and to compare the difference between TENS and EA. Methods: 18 S-D rats were produced surgically for crush injury on Extensor digitorum. TENS and EA stimulation were applied on acu-point that Zusanli (ST36) and Taijun (Liv3) of each hindlimb as approximitely 2Hz for 15 minutes. The rats were sacrificed on 1, 3, 7 post operation day, and the spinal level was immunohistochmical stainied. Results: The expression of NT-3 on the spinal level related with injured muscle between Ex-1 group and Ex-2, there is no significant difference. But between Con and Ex-1/Ex-2, there was little difference. In the same indivudual, we confirmed more NT-3 expression on the same side of injured muscle than on another side of gray matter in posterior horn. Conclusion: There is no difference, which is more effective to recover of injured muscle, between EA and TENS on injured muscle.

  • This study was undertaken to evaluate the clinical usefulness of magnetic motor evoked potential (MEP) in the diagnosis of stroke and predicting the motor improvement following stroke. The cortical, cervical and lumbar stimulations were performed in the case of 24 healthy controls and 24 ischemic stroke patients. The central motor conduction time (CMCT) was represented by the difference of latency to a target muscle between after transcranial stimulation and after cervical or lumbar stimulation. There was no case showing no response in controls. But in 11 out of 24 ischemic patients, we could not get cortical MEP. Mean CMCT of abductor pollicis brevis muscle was not significantly different in controls and stroke patients in whom MEPs were recorded. There were significant differences between mean CMCT of normal controls and that of stroke patients showing MEPs in AH Muscle. MEP Results from testing the stroke patients were correlated with site of lesion, degree of motor weakness and motor improvement after 1 to 2 months. These results suggest that magnetic MEP is easy and useful in electrophysiological test of central motor pathway and is useful indicator for representing the motor weakness and predicting the motor outcome in acute ischemic stroke patients.

  • 후두의 생리적 기능은 하기도를 보호하는 방어기능과 호흡기능 그리고 발성기능으로 대별할 수 있는데 이 가운데 계통발생적으로 가장 원시적이지만 중요한 기능은 하기도 방어기능으로 이는 다른 기능과 달리 전적으로 불수의적이고 반사적으로 이루어진다. 이 기능은 후두내 점막에 존재하는 촉각 수용체(tactile receptor)가 자극되면서 후두근육이 수축 반사를 일으켜 성문이 닫히는 성문폐쇄반사(glottic closure reflex)로서 다접합뇌간반사(polysynaptic brain stem response)이다. 현재까지 후두의 신경장애에서 그 부위나 정도 또는 신경재생 상태 등을 검사하는 방법으로 근전도검사가 주로 쓰여져 왔으나 그것이 주는 정보가 극히 제한되어 있다. 그러나 최근 청각뇌간유발반응과 같이 후두뇌간유발반응 이라 명명된 wave가 존재한다는 사실이 밝혀져 이에 대한 연구가 이루어지고 있어 이것이 임상에 쓰여질 수 있다면 현재 성문폐쇄반사의 소실이나 이상이 원인으로 사료되는 idiopathic laryngospasm, gastroesophageal reflux, spastic dysphonia, stuttering, sudden infant death syndrome과 같은 질환의 진단과 치료에 커다란 진전이 있을 것이다. 이에 저자들은 고양이 6마리를 이용하여 상후두신경을 전기적으로 자극하여 유발되는 반응을 far field recording을 이용 평균 가산법으로 그 wave를 측정하여 다음과 같은 결과를 얻었다. 1. 상후두신경자극(2㎃, stimuli frequency 3/s, Band filter 320-1000, 0.2 ㎳ duration)에 의한 반회신경에의 유발 반응을 기록하였고 그 잠복시간은 평균 8.2 ㎳ 였다. 2. 상후두신경을 자극하여 후두뇌간유발반응을 기록하였고 후두뇌간유발반응은 4개의 양 wave와 4개의 음 wave로 구성되었다. 3. 각 wave의 평균 잠복시간은 P1은 0.8㎳, P2는 2.3㎳, P3는 3.6㎳, P4는 4.3㎳였고 N1은 1.5㎳, N2 은 2.7㎳, N3는 3.9㎳, N4는 5.5㎳ 였다.

Effects of Exercise on Rat Skeletal Muscle Perfused with Glucose (포도당으로 Perfusion한 쥐의 다리근육에 운동(運勳)이 미치는 영향)

  • Muscular exercise induced by electrical stimulation of femoral nerves in perfused rat hindquarters(5 contractions per sec) in the presence of insulin and glucose effected a rapid increase(c. a. two-fold) in the level of citric acid cycle intermediates. The highest values were found within one minute of stimulation. The tissue concent ratios of lactate, pyruvate and alanine increased rapidly on initiation of exercese. Release of lactate also increased rapidly, whereas that of pyruvate was only moderately elevated. In the course of three minute exercese, the sum of alanine, glutamate and aspartate was only transiently elevated. A fall in creatine-p and ATP in the stimulated muscle was accompanied by increases in tissue level of AMP and release of ammonia into perfusing medium. However, the changes in glutamine were small. It is concluded that the pool of citric acid cycle intermediates is expanded during muscular work due (a) to an elevated level of pyruvate, leading to shifts in the levels of alanine and cycle intermediates vie trans-amination reactions and (b) to stimulation of the purine nucleotide cycle due to elevated AMP, resulting in generation of cycle intermediates and ammonia at the expense of aspartate.

  • Liquid crystal elastomer (LCE) fibers have been widely applied in various fields, such as soft robots and biomimetic actuators, in a one-dimensional form. LCEs possess the characteristics of both fluidity and solid order, as well as the elasticity of rubber, and exhibit stimulus-response based on these properties. In particular, by programming the responsiveness to various stimuli such as heat, light, electric fields, and magnetic fields in terms of shape-changing, various movements such as lifting, twisting, and rotating can be realized with high degrees of freedom. Therefore, LCE fibers have the potential for application in various fields such as artificial muscles, soft robots, wearable technologies, and sensing technologies. The research on liquid crystal elastomer fibers is evaluated to have high applicability in various fields in the Fourth Industrial Revolution as a smart material that can include various functionalities beyond simple fibers. In this review, we introduce the structure and basic characteristics of liquid crystal elastomer fibers, the latest research trends on orientation-based fabrication methods, and various applications such as artificial muscles, smart fabrics, and soft robots.

  • The exoskeleton robot to assist walking of hemiplegia patients or disabled persons has been studied in this paper. The exoskeleton robot with degrees of freedom of 2 axis has been developed and tested for joint motion. The obtained EMG signal from normal person was analyzed and the control signal was extracted from it for convenient and automotive performance of assistance robot to help hemiplegia patient walks as normal person does. the purpose of using FES(Functional Electrical Stimulation) for hemiplegia patient's walk is to restore damaged body function by this, but this could give fatal electrical shock to patients by wrong use or cause quick fatigue in muscle by continuous stimulation. The convenient movement of hemiplegia patients with minimum muscle fatigue was looked possibly by operation of assistance robot exoskeleton using control signal. and the walking assistance exoskeleton robot seemed works more efficiently than using FES stimulator. The experiment in this study was performed based on usual motion in our life like walking, standing-up, sitting-down, and particularly feedback control system using Piezo sensor along with button switch was applied for smooth swing motion in walking. The experiment also shows that hemiplegia patients can move conveniently by using electromyogram signal of healthy leg for the operation signal of assistance robot system attached at damaged symmetrical leg.

A Case of Becker's Type Congenital Myotonia (Becker형 선천성 근긴장증 1례)

  • Congenital myotonia is a hereditary disorder of the skeletal muscle. The most characteristic features of the disease are myotonia and variable muscular hypertrophy. Molecular biologic investigations have revealed that mutations in the gene of the human skeletal muscle chloride ion channel protein are a cause of the disease. The Becker's type congenial myotonia is clinically similar to the autosomal dominantly inherited congenital myotonia (Thomsen's disease). Both disorders are characterized electrophysiologically by increased excitability of muscle fibers. reflected in clinical myotonia. In general, Becker's type congenital myotonia is more severe than Thomsen's disease in muscular hypertrophy and weakness. The authors recently experienced a 25-year-old female patient who has no family-related disease history and who has conspicuous muscular hypertrophy and the stiffness with muscles which occurred from the age of 3 or 4. Clinically she showed the authors a percussion myotonia. On electrophysiological study, exercise and repetitive stimulation of the abductor digiti quinti muscle disclosed a decline in the compound muscle action potential. Biopsy of biceps muscle revealed enlargement of muscle fibers with marked nuclear internalization. After the oral taking the Mexiletine, the patient showed a favorable turn a little with her stiffness of muscles. So we authors are reporting one case of Becker's type congenital myotonia with review of literatures.

Facial Motor Evoked Potential Techniques and Functional Prediction during Cerebello-pontine Angle Surgery (소뇌교각 수술 중에 안면운동유발전위의 검사방법과 기능적 예측인자)

  • Facial motor evoked potential (FMEP) by multi-pulse transcranial electrical stimulation (mpTES) can complement free-running electromyography (EMG) and direct facial nerve stimulation to predict the functional integrity of the facial nerve during cerebello-pontine angle (CPA) tumor surgery. The purpose of this paper is to examine the standardized test methods and the usefulness of FMEP as a predictor of facial nerve function and to minimize the incidence of facial paralysis as an aftereffect of surgery. TES was delivered through electrode Mz (cathode) - M3/M4 (anode), and extracranially direct distal facial muscle excitation was excluded by the absence of single pulse response (SPR) and by longer onset latency (more than 10 ms). FMEP from the orbicularis oris (o.oris) and the mentalis muscle simultaneously can improve the accuracy and success rate compared with FMEP from the o.oris alone. Using the methods described, we can effectively predict facial nerve outcomes immediately after surgery with a reduction of more than 50% of FMEP amplitude as a warning criterion. In conclusion, along with free-running EMG and direct facial nerve stimulation, FMEP is a useful method to reduce the incidence of facial paralysis as a sequela during CPA tumor surgery.

The Effects of Transcutaneous Electrical Nerve Stimulation on Sympathetic Nerve Activity in Delayed Onset Muscle Soreness (경피신경전기자극이 지연성근육통의 교감신경활동에 미치는 영향)

  • This study conducts eccentric exercise ti non-dominant elbow flexor of 20 persons in order to examine the effects of transcutaneous electric nerve stimulation on sympathetic nerve activity in delayed onset muscle soreness, induces delayed onset muscle soreness, divides them into 10 persons respectively as experimental and control groups. And a stimulation for 2 min. with 100 pps is given to elbow flexor after repeated three times of 10 minutes rest, temperature, blood pressure and pulse are measured and as a result of two-way ANOVA, change of temperature didn't show a significant difference according to the elapse of times(p>0.05) and systolic pressure and pulses in showed a significant difference between experimental and control groups(p<0.05). These results suggest that transcutaneous electrical nerve stimulation has a direct or indirect influence on sympathetic nerve activity in delayed onset muscle soreness under a restricted condition of electrical stimulation.
