Stiffness and Elasticity of the Masticatory and Facial Expression Muscles in Patients with the Masticatory Muscle Pain (저작근통 환자에서 저작근 및 안면표정근의 경도와 탄성도 평가)

  • This study aimed to assess stiffness and elasticity of the masticatory muscle in the patients with the masticatory muscle pain using a tactile sensor and to investigate whether the masticatory muscle pain affects the facial expression muscles. From those who visited Department of Oral Medicine in Dankook University Dental Hospital, 27 patients presenting with unilateral muscle pain and tenderness in the masseter muscle (Ms) were selected (mean age: $36.4{\pm}13.8$ years). Exclusion criterion was those who also had temporomandibular joint (TMJ) disorders or any neurological pain. Muscle stiffness and elasticity for the muscles of mastication and facial expression was investigated with the tactile sensor (Venustron, Axiom Co., JAPAN) and the muscles measured were the Ms, anterior temporal muscle (Ta), frontalis (Fr), inferior orbicularis oculi (Ooci), zygomaticus major (Zm), superior and inferior orbicularis oris (Oors, Oori) and mentalis (Mn). t-tests was used to compare side difference in muscle stiffness and elasticity. Side differences were also compared between diagnostic groups (local muscle soreness (LMS) vs myofascial pain syndrome (MPS) and between acute (< 6M) and chronic ($\geq$ 6M) groups. This study showed that Ms and Zm at affected side exhibited significantly increased stiffness and decreased elasticity as compared to the unaffected side.(p<0.05) There was no significant difference between local muscle soreness and myofascial pain syndrome groups and between acute and chronic groups. The results of this study suggests that masticatory muscle pain in Ms can affect muscle stiffness and elasticity not only for Ms but also for Zm, the facial expression muscle.

Evaluation of Quality of Life in Patient with Temporomandibular Disorders (측두하악관절장애를 가진 환자에서의 삶의 질의 평가)

  • Objectives: To explore the quality of life in patients with temporomandibular disorders and to evaluate it in terms of source and duration of the pain. Methods: A total of 61 patients with temporomandibular disorders participated in this study. According to pain source, they were divided into 2 groups, masticatory muscle pain (MMP) group and intracapsular pain (ICP) group. And each group was divided into acute phase group (pain duration <6 months) and chronic phase group (pain duration=6 months). The Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36) was used to measure patients' quality of life. The scores for eight-scale profile and the physical component summary (PCS) and mental component summary (MCS) of the SF-36 were compared between groups (MMP vs. ICP and acute vs. chronic). Student t-test was used to analyze the difference of the scores of the SF-36 between MMP and ICP groups. Results: MMP group showed significantly lower score in the 3 scales of the SF-36 (Role limitations due to emotional problems, Vitality, Bodily pain) when compared to ICP group. In acute phase there was no significant difference between MMP and ICP group in PCS as well as MCS scores, but in chronic phase MMP group showed significantly lower MCS score than ICP group. Conclusions: The masticatory muscle pain in patients with temporomandibular disorders,negatively influences the quality of life especially in chronic phase, and the mental components of quality of life are significantly interfered in the TMD patients with chronic masticatory muscle pain.

Effects of Noise on the Masticatory Muscles (소음이 저작근에 미치는 효과)

  • The purpose of this study was to investigate short-term masticatory muscle reactions in response to simulated noise and music sound. Hypothesis of this study was that loud noise would cause increased stiffness and decreased elasticity of the masticatory muscles compared to low level of noise or identical sound level of music. Fifteen male volunteers were recruited for the study. The sound levels of noise and music used here were 60 dB and 100 dB. The experiment comprised 4 sessions, Session 1 with 100 dB of noise for the 1st day of experiment: Session 2 with 100 dB of music for the $2^{nd}$ day: Session 3 with 60 dB of noise for the $3^{rd}$ day: Session 4 with 60 dB of music for the $4^{th}$ day. Stiffness and elasticity on the anterior temporalis and superficial masseter muscles were measured with tactile sensor before and 2, 4 and 6 minutes after exposure of sound. The study indicated that, in short-term exposure of sound, there was no significant difference between noise and music at both 60 and 100 dB of sound level, but that there were partially significant differences between 60 and 100 dB of sound level regardless of sound type. This suggest that high level of sounds like 100 dB used in this study, in spite of short term exposure of several minutes, would lead to masticatory muscle contraction, especially in the masseter muscles.

The Influence of Botulinum Toxin Type A Masticatory Efficiency (보툴리눔 A형 독소가 저작효율에 미치는 영향)

  • This study was aimed to evaluate the masticatory efficiency after botulinum toxin type A (BTX-A) injection during 12 weeks using objective and subjective test. Also, we compared the difference of masticatory efficiency between group that injected into the masseter muscle only (M-group) and group that injected into the masseter and temporalis muscle (M-T group). The mixing ability index (MAI) was used as the objective indicator, and visual analogue scale (VAS) and food intake ability (FIA) index were used as the subjective indicators. It was concluded that masticatory efficiency was significantly lowered after a BTX-A injection into the masticatory muscle, but it gradually recovered in a predictable pattern by the 12 weeks. The disturbance of subjective masticatory efficiency was lasted longer than objective masticatory efficiency. The masticatory efficiency was lower in M-T group than M group. It was statistically significant in the VAS and FIA at 4 weeks, but the MAI showed no significancy. After 4weeks, there was rapid recovery of muscle function in M-T group, and the difference between two groups was not significant. It could be concluded that there will be no serious disturbance of mastication compared to injection is done only into the masseter muscle, even if injection is done into the masseter and temporalis muscle in dose of this study. According to the food properties, it was confirmed that people feel more discomfort on taking hard and tough foods after BTX-A injection and not only hard foods, but also intake of soft and runny foods were influenced by botulinum toxin injection.

Effects of Oral Parafunction on the Stiffness and Elasticity in the Muscles of the Mastication and Facial Expression (구강악습관이 저작근 및 안면표정근의 경직도 및 탄성도에 미치는 영향)

  • The purpose of this study was to evaluate the effects of oral habits on the muscles of mastication and facial expression by means of two parameters: muscle stiffness and elasticity. 10 healthy, fully-dentate male subjects in their twenties were selected for this study; all had normal Class I occlusal relationships. Muscle stiffness and elasticity were measured with a tactile sensor(Venustron, Axiom Co., JAPAN) while subjects were asked to relax and perform various parafunctional activities such unilateral clenching(biting the bite force recorder with a force of 50kg on each subject's preferred side), jaw thrusting and lip bracing. The following muscles were examined: temporalis anterior(Ta), masseter(Mm), frontalis(Fr), inferior orbicularis oculi(OOci), zygomaticus major(Zm), superior and inferior orbularis oris(OOrs and OOri) and mentalis(Mn). Paired t-test, Correlation Coefficients, ANOVA and Multiple Comparison t-tests were used for statistical analysis. Unilateral clenching was highly correlated with bilateral stiffness and elasticity of all the muscles tested. Mm was affected by all three oral habits; Ta was affected by unilateral clenching(p<0.05); Zm was affected by unilateral clenching and OOrs, OOri and Mn were most affected by lip bracing(p<0.05). This study indicates that not only the masticatory muscles but also the muscles of facial expression, mainly circumoral muscles, can be significantly influenced by parafunctional activities such as unilateral clenching and lip bracing.

A Study on the Changes in Motor Unit Action Potential, EMG Power Spectrum, and Pressure Pain Threshold of Masticatory Muscles during Sustained Fatiguing Contraction (피로를 유발하는 지속적인 근수축 동안 저작근의 운동단위전위, 근전도 power spectrum, 압력통각역치 변화에 대한 연구)

  • 본 연구는 구강안면동통 중에서 빈번히 나타나는 근육성 동통의 주 원인인 저작근의 과활성으로 유발된 근육의 피로 시에 운동단위전위, 압력통각역치, 근전도 power spectrum의 변화 양상과 이들 척도간의 연관성을 조사하기 위해 시행되었다. 두개하악장애의 병력 및 현증이 없고 정상적인 구치부 교합관계를 가진 평균연령 25.8세인 36명의 정상 성인(남자 26명, 여자 10명)을 대상으로 교근과 전측두근의 지속적인 등길이 수축 전후의 압력통각역치 및 운동단위전위를 측정하였고 인내시간까지의 근수축 동안 근전도 power spectrum을 분석하여 다음과 같은 결론을 얻었다. 1. 지속적인 등길이 수축 후 교근과 전측두근의 압력통각역치는 수축 전에 비해 유의하게 감소하였다. 2. 압력통각역치는 수축 전과 수축 후 모두에서 전측두근이 교근보다 유의하게 높게 나타났으며, 전체적으로 남성이 여성보다 높게 나타나는 양상을 보였으나 성별간의 차이는 전측두근의 수축 후 압력통각역치에서만 통계적으로 유의하게 나타났다. 3. 지속적인 등길이 수축말기의 중간주파수는 수축초기에 비하여 유의하게 감소하였고, 전측두근의 수축초기 중간주파수와 수축말기 중간주파수 모두 교근보다 유의하게 높게 나타났다. 4. 교근은 지속적인 등길이 수축 전에 비하여 수축 후의 운동단위전위의 지속시간,진폭, 면적, 상의 4가지 척도에서 유의한 증가를 보였고 전측두근은 진폭을 제외한 나머지 3가지 척도, 즉 지속시간, 면적, 상의 유의한 증가를 보였다. 5. 교근과 전측두근의 지속적인 등길이 수축 전의 압력통각역치와 운동단위전위 척도 사이에는 통계적으로 유의한 상관관계가 없었고 교근에서는 수축 후의 압력통각역치와 운동단위전위의 지속시간, 진폭, 면적, 상 사이에 유의한 상관관계가 존재하였다. 위의 실험결과를 통해 근육피로 검사에 압력통각역치, 근전도 power spectrum 검사 외에 근육수축의 기능적 최소 단위인 운동단위전위의 분석 또한 유용할 수 있고 추후 만성으로 진행된 근막동통환자와 정상 대조군간의 운동단위 수준에서의 비교연구와 근피로에 더욱 민감한 운동단위전위의 다른 척도에 대한 개발과 연구가 필요하다고 사료된다.

Surface Electromyographic Characteristics of a Myofascial Trigger Point of the Temporalis Muscle: A Case Report (측두근의 근막동통 발통점의 표면 근전도 특성: 증례 보고)

  • Myofascial pain is a condition associated with regional pain and muscle tenderness characterized by the presence of myofascial trigger points. In this case report, a subject complaining of nighttime bruxism was clinically assessed, and a latent trigger point of the anterior temporalis muscle was identified with manual palpation. A surface electromyographic (SEMG) exam of the anterior temporalis muscle harboring the latent trigger point demonstrated several SEMG features, including post-contraction irritability, delayed relaxation following contraction and accelerated muscle fatigue. It was concluded that a SEMG exam may detect abnormal masticatory muscle function and, therefore, assist in the evaluation of myogenous temporomandibular disorders.

  • 음식물을 씹거나 삼키는 정상적인 하악운동이외에 악안면 근육의 활동에 의해 상, 하악 치아가 접촉하거나, 비정상적인 근육활동이 있을 때 이 현상을 이갈이(Bruxism)이라 합니다. 이갈이 습관은 대개 수면중에 나타나나 깨어 있는 상태에서도 비정상적인 근육수축을 관찰할 수 있읍니다. 이러한 현상을 저작근의 근전도(Electromyogram, EMG)를 기록하거나, 구강내에 장치된 원격계측장치로 치아접촉을 관찰함으로써 정상적인 저작이나 연하작용과 비교할 수 있습니다. 이갈이의 발생빈도는 학자들에 따라 서로 다르게 보고되고 있으며, 수면중 이를 가는 사람들은 대부분 이갈이 습관을 알지 못하며, 단지 10%정도만이 이가는 소리를 알 뿐입니다. 본문에서는 이갈이와 수면중의 생리적 현상과의 관계, 저작근에 미치는 영향과 이갈이 발생에 대한 중치신경의 작용을 검토함으로써 이갈이를 이해하는데 도움이 되고저 합니다.
