A study was carried out in 81 retreatment patients with sputum positive pulmonary tuberculosis who had previously been treated with first-line drugs for more than one year at Health Center. The Following results were obtained; 1. Of the total 81 case of retreatment patients, male patients occupied 63(77.8%) and 18(22.2%) were female. Age group of $30{\sim}49$ years was 54.3% of total cases. 2. By extent of disease, moderate advanced cases were 53.1% and far advanced cases were 35.8%. 3. Of 81 patients admitted to the study. 65(80.3%) completed 1 year treatment 16(19.7%) patients discharged prematurely before 1 year. 4 patients terminated their treatment during $9{\sim}11$ months after registration. 4. Completment rate of chemotherapy was highest (90%) at age of under 30 years. 5. Intractable patients with persistant positive sputum test for A.F.B. even after 12 months of retreatment were occupied 9(13.8%) of total retreatment cases.

Prosthetic valve replacement has resulted in marked improvement in the functional status of many patients with valvular heart disease. But valve failure from various causes has necessitated re-replacement of prosthetic valve in some of these patients. This selective group of patients has many inherent problems. This report presents our experience with seven patients who received re-replacement of prosthetic valve in the Seoul National University Hospital from 1981, to 1983. The patients included one woman and six men in the range of 9 and 49 years old. As the first valve operations, there were five MVR using tissue valves, one DVR [Bjork-Shiley and Ionescu-Shiley valve] and one case of modified Bentall operation with composite graft [Ionescu-Shiley valve]. Reoperations on prosthetic valve failure were performed 17 to 54 months after the first operation [mean 34 months]. Amon8 seven patients, there were two cases of prosthetic valve endocarditis and five cases of primary tissue failure. Gross calcification of the xenograft was found in two children with Ionescu-Shiley valve. All except one had relatively successful operative results. The unsuccessful one died intraoperatively from low output syndrome.

We performed a reoperation of failed tricuspid mechanical valve in a 63-year-old female patient because the overgrown endothelial pannus had entrapped the prosthetic leaflets. Four years ago, the patient underwent mitral and tricuspid valve replacements with 31 and 33 mm Carbomedics, respectively. The patient showed symptoms of neck vein distention, abdominal distention and peripheral edema. The chest film, echocardiography and cineangiography confirmed the diagnosis of tricuspid valve. During the operation, we found the entrapped leaflets of the tricuspid valve in a partially closed state and the endothelial pannus had overgrown into the leaflets. Carpentier-Edward bovine pericardial valve was inserted and the patient was discharged with no significant events.

영화관의 구조설계에서 중요한 고려 인자 중에서 음향적 관점뿐만 아니라, 영사기의 진동으로 인한 영상의 떨림 현상은 관람석에서 미치는 구조물의 동적 특성 문제에 있다. 이는 영사기-스크린-관람객 사이의 3인자에 내한 상대진동허용변위에 의해 지배됨을 고려하여, 이를 만족시키는 변수 등에 따른 진동허용규재치를 결정하는 절차가 필요하다. 본 연구에서는 영화관의 진동허용규제치를 이들 3인자에 대한 변수들을 고려한 결정 과정을 공학적인 차원에서 정리하였으며, 복합상가 건물내의 영화관 영사실에 이를 만족하기 위한 진동 저감설계 자료에 활용하였다.

Moliere completed classical French comedy by combining farce and commedia dell\`arte. Moliere believed that it was a plays obligation to give a lesson and entertainment. Moliere recreated commedia dell\`arte\`s typical acting patterns and characters in his work. He created with Lully comedie ballet that combined ballet and comedie for taste of Louis 14. Les precieuses ridicules critically displayed women at that time. Both contemporary high status women\`s custom and behavior were the targets of the author\`s synical criticism. There are three notable clothing imageries are shown in this work. First, Moliere used a variety of items in order to show emptiness of noble at that time. Items are ribbon and loop decorated rhingrave, feather decorated hat, perfumed wig and glove, lace covered canon and flower decorated shoes. Second, the author showed people\`s stutus through their clothes. Last, the author used clothing used clothing as a metaphor for hypocrisy. Thus when he said “take off the clothes” he meant take off hypocrisy and return truth.

인공판막 혈전에 의한 관상동맥 폐쇄 후 발생하는 급성심근경색은 드물지만 치명적인 질환으로 환자들의 생존율을 높이기 위해서는 빠른 진단과 적절한 치료가 중요하다 본원에서는 기계승모판막 혈전에 의한 좌전하행지의 폐쇄로 급성심근경색이 발생한 환자 1예에서 중재시술 및 항혈전요법 후 승모판 재치환술을 성공적으로 시행하였기에 증례 보고하는 바이다.

This article described a procedure for fabricating an esthetic gingival porcelain restoration as an implant-supported fixed prosthesis for edentulous maxilla. Alternative treatments for fully edentulous patients include an implant-supported overdenture or a fixed implant-supported prosthesis with bilateral distal cantilevers. But, from a functional and biomechanical point of view, the fixed implant-supported prosthesis with posterior cantilevers or implant-supported tissue-borne overdenture do not significantly improve masticatory effectiveness compared with a distributed implant restoration as a fixed implant-supported prosthesis. The fact that the prosthesis is supported by distributed implants over eight for edentulous maxilla in general, provides increased masticatory efficiency as a fixed restoration and similar gingival appearance with esthetic gingival porcelain. It is also detachable by dentist to allow easier after-care of soft tissue and the prosthesis.

일반적인 보철치료 과정에서 치과의사와 치과기공사 간의 의사소통 부족으로 인한 재치료 사례가 빈번하게 일어나고 있다. 대규모 치료의 성공은 작은 치료에서 부터 시작한다. 이 보고서에서는 그것과 관련한 간단한 사례를 통해 커뮤니케이션의 중요성을 되새긴다. 좀 더 나은 색과 형태를 재현하기 위해서 치과의사는 치과기공사에게 구체적이고 정확한 정보와 의견을 전달해야 하고 치과기공사는 이것을 올바로 분석하고 제대로 표현할 수 있는 기술을 보유해야 한다. 이 과정에서 반드시 서로가 의견교환을 통해 좀더 나은 보철을 제작할 수 있는 환경을 구축해야 한다.

기계판막의 비구조적 기능이상은 판막 치환 후 생길 수 있는 심각한 합병증 중의 하나이다. 이러한 기계판막의 비구조적 기능이상은 일반적으로 1) pannus나 조직 혹은 봉합물질에 의한 장애 2) 치환 판막 주위의 틈 3) 부적절한 판막의 크기나 위치, 판막 삽입시의 잘못 등에 의해 생긴다. 저자들은 상대적으로 작은 판막륜을 가진 환자에서 치환 판막의 잘못된 위치로 야기된 pannus 형성에 의한 판막의 기능부전을 경험하였으며, 이 환자에 대해 pannus 제거 후 판막륜 확장술(닉스 술식, Nicks procedure)을 포함한 판막 재치환술을 성공적으로 시행하여 이를 보고하는 바이다.

복부 인조혈관 감염증은 매우 심각한 상태로서 문합부 파열로 심한 출혈이나 급성 말초 색전증으로 하지 허혈을 유발하여 사망률이 높고 치료하기 어려운 질환으로 치료에 대한 논란이 많은 질환이다. 저자는 복부대동맥류 수술 후 약 1년 후에 발생한 인조혈관 감염증례를 양 허벅지 심부정맥인 표제성 대퇴정맥을 사용하여 재치환하여 좋은 결과를 얻었기에 문헌과 함께 보고한다.
