An Examination of Theories of Health Behaviors for Guiding Research on Mammogram Screening Practices for Korean Immigrant Women (건강행위이론의 사회문화적 적합성에 대한 논의;한인여성의 유방암 방사선 검사 행위와 관련하여)

  • 연구 목적: 1980년대 이후 미국 여성들의 유방암 조기 진단을 위한 방사선 검진율은 급속히 증가하였음에도 불구하고 유색 인종의 여성들은 여전히 조기 검진의 혜택을 받지 못하고 있다. 유색인종 여성들의 낮은 검진율을 설명하기 위해 여러 건강행위이론을 이용한 관련 요인들이 연구되어 왔다. 이 논문은 미국 보건 의료관련 연구에서 가장 많이 쓰이는 다섯 건강행위 이론을 유색 인종 여성, 특히 한국 이민 여성들의 유방암 조기검진 이행에 적용하기 위해 사회문화적 적합성을 평가하기 위해 고안되었다. 연구 방법: 네 종류의 데이터 베이스(CINAHL, MEDLINE, PsycINFO, Sociological Abstracts)를 이용한 심층적 문헌 고찰을 통해 각각의 이론으로 유방암 조기 검진을 설명한 연구들을 모두 분석하였다. 연구 결과: 각 이론들의 배경, 주요 요인, 그리고 유색인종의 유방암 조기 검진에서의 적용 연구 등을 분석하였다. 결론: 서양 문화권 속에서 개발된 각 이론들이 한국적 정서와 행동을 설명하는데 명확한 한계가 있으며 이러한 한계를 극복하기 위해서는 기존의 이론들을 면밀하게 재분석하여 한국적 특성을 담아낼 수 있는 새로운 이론의 도출이 요구된다.

Analysis of Nursing Needs of Home-dwelling Breast Cancer Patients Based on Counselling Contents (상담일지에 근거한 재가 유방암 환자의 간호요구도 내용분석)

  • Purpose: This study was performed to identify the nursing needs of home-dwelling breast cancer patients based on counseling contents. Methods: Descriptive research was conducted with content analysis. This study included 185 patients who underwent treatment for breast cancer in a tertiary hospital. The data were collected using personal counseling via telephone or face-to-face between March 2011 and July 2013. A total of 536 counseling contents were used in the analysis. Inductive content analysis was used to analyze the contents related to nursing counselling. Results: According to the results. the most frequently reported nursing needs was symptom management, followed by information needs and supportive intervention needs; symptom management needs included symptoms related to chemotherapy, daily living, surgery, and medication side effects; information needs included treatments, medication, clinical tests, and alternative therapies; and supportive intervention needs were related to emotional and social aspects. Conclusion: Our findings suggest that nursing interventions for home-dwelling breast cancer patients should be designed and provided with a consideration to these relevant nursing needs.

Mass Analysis of Isoflavones in Chungkookjang (청국장에 존재하는 Isoflavone의 Mass분석)

  • Fermented soybean, Chungkookjang contains microorganism, enzyme, and diverse bioactive compounds. Isoflavones in Chungkookjang might suppress breast and prostate cancers. Using HPLC and Mass analyses, it was found that 100% ethanol extract of Chungkookjang contains aglycone forms such as genistein, daidzein, and glycitein. 8-OH-genistein, 8-OH-daidzein were not found. There are two estrogen receptors, $ER{\alpha},ER{\beta}$. $ER{\alpha}$ might stimulate proliferation of breast and prostate cancer cells, and $ER{\beta}$ might suppress their growth. Using yeast transactivation assay under the control of human ER expression, it was demonstrated that isoflavones in Chungkookjang can stimulate $ER{\beta}_{1}$, selectively.

Satisfaction and Needs of Cancer Patients and Caregivers under Home-based Cancer Patients Management in Jeju (제주지역내 재가암환자관리를 받은 환자와 보호자에게 제공되는 프로그램 만족도와 요구)

  • This study was conducted to improve the quality of home-based cancer management programs through a survey of satisfaction and needs of the program provided by a regional cancer center. From March 2015 to October 2015, we conducted face to face surveys of patients and caregivers enrolled in home-based cancer management. A total of 101 patients, 59 patients and 41 caregivers, were enrolled. Breast cancer was the main cancer and 51 (86%) patients and 36 (85%) caregivers were satisfied with the number of visits for home-based cancer management. For the service application route, 22 patients (37.29%) obtained information through the cancer center publicity paper and 11 caregivers (26.19%) received recommendations from acquaintances. Except for treatment provided directly to the patients, psychological counseling was the most preferred, and satisfaction regarding picnic and cancer education were also high. Satisfaction with the program provided by regional cancer center was relatively high ($4.14{\pm}1.21$ on a 5-point scale). Additionally, satisfaction of the patients was higher than that of the caregivers, but this difference was not significant ($4.29{\pm}1.11$, $3.93{\pm}1.31$, p = 0.141). Self-esteem was higher among caregivers than patients, but this difference was not significant. To improve the quality of life of cancer patients and caregivers, it is necessary to develop customized programs considering patients' economic situations and need for psychological counseling.

Magnetic Resonance Imaging Factors Predicting Re-excision in Breast Cancer Patients Having Undergone Conserving Therapy (유방보존술을 시행받는 유방암환자에서 재절제 예측의 자기공명영상소견)

  • Purpose : The aim of this study was to determine the magnetic resonance imaging (MRI) features associated with re-excision due to the presence of a positive margin after breast conserving therapy (BCT) in breast cancer patients. Materials and Methods: We reviewed the records of 286 consecutive breast cancer patients who received BCT between January 2006 and December 2007. Among 246 patients who had undergone BCT, 38 (15.4%) underwent immediate further surgery due to positive margin status. We analyzed the MRI findings using ${\chi}^2$ test, Fisher's exact test and t tests. Multivariate logistic regression was conducted for prediction of re-excision. Results: Tumor size (p < 0.001), lesion multiplicity (p = 0.003), and non-mass-like enhancement (NMLE) type on MRI (p < 0.001) were associated with margin involvement in BCT. On preoperative MRI, larger size (${\geq}5cm$) (odds ratio = 2.96), NMLE (odds ratio = 3.81), and multifocal lesions (odds ratio = 2.54) were positively associated with re-excision. In cases involving NMLE, segmental distribution was associated with a greater likelihood of immediate re-excision. Conclusion: Larger size, multiplicity, and NMLE on MRI are significantly associated with re-excision after BCT in breast cancer patients. For NMLE lesions, the segmental distribution pattern was predictive of re-excision.

Case of Remission of Progressive Breast Cancer Treated by Capecitabine with Integrative Medicine Therapy (카페시타빈과 통합의학 프로그램 치료 병용을 통해 호전된 진행성 유방암 1례)

  • This report is aimed to investigate the effect of Integrative Medicine Therapy (IMT) in treating breast cancer metastasized to several portion. A 55-year-old woman who was diagnosed breast cancer and treated by breast conservation surgery (BCS) IN 2002. (pT2M0N0), adjuvant chemotherapy and radiotherapy till Jan. in 2013. But cancer had metastasized to sacrum (2009), liver (2012) and ribs (2013). Finally cause of several side effects of Xeloda(neuralgia and vommiting), she gave up to be treated by Xeloda and started to receive IMT including Abnoba Viscum, Vitamin C therapy Xeloda again, Korean herbal medicine and pharmacopuncture from Jul. in 2013. The effect was evaluated with Positron Emission Tomography and Computed Tomography (PET-CT) and Abdomen Computed Tomography (CT). The metastatic tumor in liver was disappeared and cancers to ribs, their size decreased after 12 months and pain of sacrum maintained VAS 2 continually. These results suggest that IMT is a therapeutic method to treat metastatic tumor originated from breast cancer.

Cloning and Expression of Mammaglobin Gene (Mammaglobin 유전자 재조합 및 발현에 관한 연구)

  • In this study, I attempted to develope the expression and purification system of human mammaglobin proteins in Escherichia coli and to produce anti-human mammaglobin rabbit antibody for the detection of human mammaglobin protein in the peripheral blood of breast cancer patients. Human mammaglobin gene was cloned and sequenced from m-RNAs purified from donated breast cancer tissues using RT-PCR. The cloned gene was inserted into pET30, pET22, and pET32 plasmid. The cloned gene in pET30 yields insoluble proteins which was difficult to purify from the cells extracts. The mammaglobin gene in pET32 was strongly expressed soluble proteins which were isolated using Ni-NTA affinity chromagraphy and DEAE-ion exchange chromatography, followed by enterokinase digestion of the purified proteins. The isolated proteins had enough purity to use as a antigen for the production of anti-mammaglobin antibody in rabbits. The polyclonal antibody produced against the isolated mammaglobin showed a specificity to mammaglobin after Westernblot immuno assay. In conclusion, the isolated mammaglobin protein and the anti-mammaglobin rabbit antibody may be used for diagnosis of breast cancer as well as development of anti-breast cancer drug.

Internal Mammary Lymph Node Irradiation after Breast Conservation Surgery: Radiation Pneumonitis versus Dose-Volume Histogram Parameters (유방보존술 후 내유림프절 방사선 조사: 방사선 폐렴과 체적-선량 히스토그램 변수들)

  • Purpose: To evaluate the association between radiation pneumonitis and dose-volume histogram parameters and to provide practical guidelines to prevent radiation pneumonitis following radiotherapy administered for breast cancer including internal mammary lymph nodes. Materials and Methods: Twenty patients with early breast cancer who underwent a partial mastectomy were involved in this study. The entire breast, supraclavicular lymph nodes, and internal mammary lymph nodes were irradiated with a dose of 50.4 Gy in 28 fractions. Radiation pneumonitis was assessed by both radiological pulmonary change (RPC) and by evaluation of symptomatic radiation pneumonitis. Dose-volume histogram parameters were compared between patients with grade <2 RPC and those with grade ${\geq}$2 RPC. The parameters were the mean lung dose, V10 (percent lung volume receiving equal to and more than 10 Gy), V20, V30, V40, and normal tissue complication probability (NTCP). Results: Of the 20 patients, 9 (45%) developed grade 2 RPC and 11 (55%) did not develop RPC (grade 0). Only one patient developed grade 1 symptomatic radiation pneumonitis. Univariate analysis showed that among the dose-volume histogram parameters, NTCP was significantly different between the two RPC grade groups (p<0.05). Fisher's exact test indicated that an NTCP value of 45% was appropriate as an RPC threshold level. Conclusion: This study shows that NTCP can be used as a predictor of RPC after radiotherapy of the internal mammary lymph nodes in breast cancer. Clinically, it indicates that an RPC is likely to develop when the NTCP is greater than 45%.
