"장애의 정도가 심해 집안에서 생활하는 재가 장애인들은 지식의 충족은 물론 사소한 정보로부터도 철저히 소외돼 있는 경우가 대부분입니다. 장앤이라 해서 지식과 정보에 대한 욕구가 정상인과 틀리지는 않습니다. 이 도서실은 이같이 재가장애인들의 절박한 문제를 해결해줄 목적에서 설립되었습니다.

본 연구는 지역사회 내에 거주하는 재가 장애인의 신체적, 심리적, 사회 환경적 상황을 종합적으로 평가하여 그에 적합한 서비스를 제공하기 위한 욕구 중심의 사정도구를 개발하고, 개발된 도구를 활용하여 재가 장애인 200명의 사정 데이터를 수집한 후 데이터마이닝의 의사결정 나무분석 기법을 활용하여 욕구에 적합한 서비스제공을 위한 욕구 추출 알고리즘을 구성하였다. 본 연구는 2006년 6월부터 10월까지 5개월간 이루어졌으며, 크게 사정도구 개발과 개발된 도구를 활용한 욕구추출 과정으로 나뉠 수 있다. 도구개발은 문헌고찰을 통하여 기본적인 틀을 구성하였고, 포커스집단과 전문가들을 통하여 사정도구의 주관적 호소와 욕구 문항을 개발하였으며, 도구의 타당도를 확인하기 위해 통계적인 검증과정을 거쳤다. 검증결과 본 도구는 <표 2>와 <표 3>의 결과처럼 타당도와 신뢰도를 확보하였으며, 이 도구를 활용하여 욕구추출 알고리즘 요약을 <표 5>와 같이 제시하였다. 본 연구의 결과로 제시한 사정도구와 알고리즘은 재가 장애인의 객관적 욕구를 사정하고 확인함으로써 체계적인 사례관리를 수행하는 자료로 활용될 수 있다.

본 연구는 광주 나주 지역에 거주하는 20세 이상 재가장애인의 일상생활활동과 삶의 질을 조사하였다. 기본적 일상생활활동과 수단적 일상생활활동 수행력과 삶의 질의 정도를 조사하였고 이들의 상관관계를 알아보기 위해 이변량 상관관계 분석을 실시하였다. 그 결과 일상생활활동 수행력이 낮을수록 삶의 질이 낮게 나타났고, 수단적 일상생활활동이 삶의 질과 관련성이 더 높은 것으로 나타났다. 이 연구를 통하여 앞으로 재가장애인들이 보다 독립적이며, 질 높은 삶을 영위 할 수 있도록 도움이 되고자 한다.

본 연구의 목적은 역학적 요인(우울), 행위적 요인(일상생활 수행능력), 소인요인(자기효능감), 강화요인(가족지지, 전문가 지지), 촉진요인(자원이용가능성, 접근성)이 농촌지역 재가 장애인의 삶의 질에 미치는 영향요인을 파악하기 위한 것이다. 연구의 개념적 기틀은 Green과 Kreuter의 PRECEDE 모형을 기반으로 하였다. ${\bigcirc}$군 보건소에서 실시한 지역사회중심 재활사업 기초조사 결과 자료를 근거로, 기초조사에 참여한 190명중 186명이 최종분석에 포함되었다. 선택된 요인들과 삶의 질 간의 직 간접 경로를 파악하기 위하여 AMOS 19.0을 이용하여 자료를 분석하였다. 모형 검증 결과 농촌지역 재가 장애인의 삶의 질은 85.5%의 설명력을 가지며, 역학적 요인(우울)과 강화요인(가족지지, 전문가지지)에 의해 직접적 영향을 받은 반면, 행위적 요인(일상생활수행능력), 소인요인(자기효능감), 촉진요인(자원이용가능성, 접근성)을 통하여 간접적 영향을 미치는 것으로 나타났다. 본 연구 결과는 농촌지역 재가 장애인의 삶의 질 증진을 위한 재활사업 계획 시 우울을 감소시키거나 가족이나 건강전문가로부터의 지지를 강화해야 할 필요성을 조명하였다. 뿐만 아니라 자기효능감 향상을 위한 전략을 세우고 지역의 보건기관과 전문 의료기관과의 연계를 확대하는 것도 농촌지역 재가 장애인을 위한 지역사회 중심 재활서비스에 유용할 수 있음을 제안하였다.

The Purpose of this study was to review the necessity of home care service for the handicapped. The Present momentum for home care for the handicapped become when the circulating rehabilitation service center for the handicapped was designated by the government in Korea. That was 1992. Home care service is a burgeoning segment of Korea health Service, driven by factors which include an expanding elderly population and increasing the disabled persons. To ensure quality of life for the disabled persons with diminishing financial resources allocated to health me, it is essential that community based rehabilitation and home care service be extended.

This policy alternatives for establishment of rehabilitation health care delivery system for the disabled in the community were developed based on the data of current health status and situations of health care management for disabled persons in Korea. This research was conducted with secondary data analysis for identifying health status and current situations of managing health of disabled persons, and discussed current issues for establishing rehabilitation health care delivery systems in the community. Observing the health status and current situations of managing health of disabled persons, scope and target population of disabled person extended. so prevalence rates increased. and the severity of disability intensified and specified. The summary of issues of health management for disabled persons included; 1) absence of comprehensive and systematic policy in rehabilitation health care systems. 2) absence of consumer based rehabilitation health care facilities and delivery systems that are considered as the characteristics of disabled persons 3) fixed form of projects based on the provider and lack of variety in the programs. Hereafter. to overcome these problems. policy alternatives should 1) establish a comprehensive rehabilitation health care policy for disabled persons. 2) establish comprehensive and specific community based rehabilitation health care delivery systems that can promote preventing disability. providing medical care for disabled persons, establishing rehabilitation management for disabled persons and health care when returning to society. 3) provide training and secure manpower for rehabilitation, but the training case managers who will take the roles as an expert rehabilitation nurse mediators for multidisciplinary team work are needed immediately. and 4) include efficient connection and provision of independent services for welfare rehabilitation service and health care. Conclusively. a community based rehabilitation health care delivery system should be comprehensive policy vision of the government on rehabilitation health care delivery System rehabilitation service has to be constructed systematically under suitable facilities con consumer characteristics and rehabilitation health care policy. By doing this, consumer comprehensive community rehabilitation health care delivery system could be constructed disabled person.

본 연구는 장애인들의 일상생활 경험에서 어려움을 알아보고자 하였다. 이를 위해 서울지역 재가 장애인 8명을 심층면접 실시하여 합의적 질적(CQR) 방법으로 분석하였다. 연구 결과는 3개의 범주에서 14개 영역 및 그에 따른 48개의 내용에 따라 빈도분석을 하였다. 장애인들의 일상생활에서 심리 정서적 경험의 어려움은 자신감 결여, 건강문제 염려, 결혼(재혼) 및 이성문제, 가족관계 어려움, 외로움, 노후문제 염려로 나타났다. 사회 경제적 경험의 어려움은 관계의 어려움, 생계문제, 출산 양육문제, 직장생활 어려움, 외출 시 번거로움으로 나타났다. 이러한 경험들을 대처하는 과정으로는 종교기관 모임, 지역사회체계 이용, 주변사람들의 도움을 이용하였다. 본 연구 결과에 의하여 장애인들이 일상생활에서 어려움을 잘 극복할 수 있도록 실질적인 도움이 되는 사회복지서비스와 지역사회체계를 통하여 지속적인 지원이 필요함을 제언하였다.
