The Relationship of Functional Health Status, Satisfaction of Customized Home Visiting Health Service, and Quality of Life in Vulnerable Elderly (취약계층 재가노인의 기능적 건강상태와 맞춤형 방문건강관리 서비스 만족도 및 삶의 질과의 관계)

  • This study attempted to identify the relationship of functional health status, satisfaction of customized home visiting health service, and quality of life in vulnerable elderly. This research people 399 total in the object. Data gathering period was from June to September of 2010 and measurement tool used for this study was the house visiting health service recording sheet, recommended by the Ministry of Health and Welfare. Data were analyzed using t-test, one-way ANOVA, Pearson correlation coefficients with SPSS/WIN 17.0 version. Functional health status and satisfaction of customized home visiting health service positively correlated with quality of life. Functional health status not correlated with satisfaction of customized home visiting health service. Results of this study can be leveraged as complementary information for the effective management of customized home visiting health service subjects.

A Study on the Characteristics and Satisfaction of the Recipients of Home Care (재가복지대상자의 특성과 서비스 만족도)

  • The objectives of this research are (1) to identify characteristics and needs of the recipients of home care, and type of services (2) to examine the factors that predicts the degrees of recipients' satisfaction. This study uses the data based on the responses of 490 recipients selected randomly from 22 home care centers. The results show that recipients have higher frequencies in age of 60+, disabled status, difficulty with ADL, IADL items, no family, and lowest class. And the type of services offered by home care centers have highest frequencies in home maker services. Recipients' attitude toward home care services is "helpful," but they are not satisfied with the frequency of visit. The study found that disabled status, difficulty with ADL, IADL, health services, and home maker services are negatively related with degrees of recipients' satisfaction. Based on the findings, the study would conclude that home care policy should be recipients' need-based, developing systematic: need assessment tool. Expanded home health caring must be required for the disabled elderly.

Effects of the In-Home Visitation Care Facility Directors' Leadership Types on Organizational Effectiveness and the Moderating Role of Organizational Trust (재가방문요양시설장의 리더십유형이 조직유효성에 미치는 영향과 조직신뢰의 조절역할: 요양보호사를 중심으로)

  • The purpose of this study was to investigate the effects of the in-home visitation care facility directors' transformational and transactional leadership on organizational effectiveness and the moderating role of organizational trust between leadership types and organizational effectiveness. In this study, job satisfaction and turnover intention were used as organizational effectiveness variables. For this study, structured questionnaires were used and data were collected from 290 caregivers from 50 in-home visitation care facilities in Seoul. Collected data were analyzed using hierarchical regression technique. Results showed that directors' transformational leadership had a positive effect on caregivers' job satisfaction and a negative effect on their turnover intention, whereas directors' transactional leadership had no effect on caregivers' job satisfaction, and turnover intention and that caregivers' organizational trust moderated between directors' transformational leadership and caregivers' job satisfaction.

The Effect of Choice and Service Quality on User Satisfaction among Long-Term Home Care Service Recipients (장기요양 재가서비스 서비스 품질과 선택권 실현이 이용자 만족도에 미치는 영향)

  • The purpose of this study is to examine the moderating effect of choice realization between service quality and user satisfaction to elderly home care services. For this purpose, we analyzed 258 respondents who received elderly home care service in Jeollabuk-do. Considering the assumption that the moderating effect of choice realization would be different by rural and urban areas because of the disparity of the elderly welfare infrastructure, the urban and rural areas were analyzed separately. The analyses showed that choice realization had a moderating effect on the relationship between service quality and user's service satisfaction for the service clients resided in urban areas. However, there was no significant moderating effect of choice realization for the service clients in rural areas. This result implies that the moderating effect of selection realization is different in rural and urban areas because of the poor elderly home care facilities in rural areas. In order for aged people to actively express the effect of service user choice, it is necessary to solve welfare imbalance between regions through expansion of welfare infrastructure and policy support to rural area.

The Comparison of Health Status, Functional Status, and Home Environment Hazards in Terms of the Fear of Falling in Community-dwelling Older Adult (재가 노인의 낙상공포 여부에 따른 건강상태, 신체기능상태 및 가정환경 위험 비교)

  • The purpose of this study was to compare the health status, functional status, and home environment hazards by the fear of falling and explore the factors that contribute to the fear of falling. This was a descriptive study. Data were collected for two years with 973 older adults who enrolled in a health center at A city. Trained nurses visited subjects' home and collected data using Resident Assessment Instrument MDS-HC. The average age of the subjects was 76.8 (±7.6) and female was 74.8%. 20.7% of subjects reported fear of falling. In terms of the fear of falling, two groups were compared. There were significant differences in vision, urinary incontinence, pain that interrupts daily activity, unstable gait, comorbidities, frequency of depressive or anxious sign. The greater the age, IADL and ADL dependency, comorbidities, unstabled gait, the more fear of falling. In conclusion, this study reassured the importance of assessing the major factors related to fear of falling such as age, IADL and ADL, comorbities, gait among older adults. This study will contribute to establish strategies for preventing fall. Reducing the fear of falling through the comprehensive nursing intervention will ultimately improve the quality of life in the elderly.

The Moderating Effects of Social Support and Self-Efficacy on the Relationship between Resilience and Burnout of Visiting Caregivers in Seoul (서울지역 방문요양보호사의 회복탄력성과 직무소진의 관계에서 사회적 지지와 자기효능감의 조절효과에 관한 연구)

  • As the demand for elderly care service has been on rise due to the increasing of the aging population, the role of visiting caregivers becomes increasingly important. In this context, this study aims to examine whether there are moderating effects of social support and self-efficacy on the relationship between resilience of visiting caregivers and their burnouts. The hierarchical regression analysis was used for statistical analysis. The findings from the study are as follow. First, resilience has a negative effect on burnout. As the resilience increases, the burnout decreases. Second, social support has moderating effects on the relationship between a resilience sub-variable(positivity) and burnout. Third, self-efficacy has a moderating effect on the relationship between a resilience sub-variable(self-regulation) and burnout. The findings of the study suggest that nursing facilities pay attention to psychological characteristics of visiting caregivers as well as their job characteristics in designing burnout prevention programs for them.

Fall Risk in the Community-dwelling Elderly who Received Home Care Services: Focused on Residential Environment and Perception of Fall Risk (방문간호를 받는 재가노인의 낙상위험)

  • Purpose: The purpose of this study was to investigate the factors increasing fall risk in the residential environment risk and the perceived fall risk among the older adults who received home care services to provide information for developing a comprehensive falls intervention program. Methods: The subjects were 227 community-dwelling elderly aged 65 years and over who were taken care of by home-visiting nurses of the national health centers. The data were collected from July to August in 2012 using the Choi's residential environmental risk scale (2010) and the Hong's fall risk scale (2011). Results: Requires an assistive devices to walk, modified residential environment, health security, approval certificate of LTC, residential safety perception, residential environment risk, and perception of fall risk were statistically significant risk factors. A multiple logistic regression analysis showed that room & kitchen, physical perception, medication & ADL perception, floor-related environmental perception, and daily living tool-related perception were statistically significant predictors of fall. Conclusion: The results showed that the residential environment and the perceived fall risk were associated with fall experiences among the elderly. It is necessary to develope multifactorial intervention programs considering both environmental and perceived risk factors as well as physical risk factors to reduce and prevent falls among the elderly.

  • The purpose of this study was to understand the meaning of experiences in home visiting nursing service among the elderly in community provided by nursing students. Method: This is a phenomenological study to investigate the experience of community-based patients. Data were collected through in-depth interviews from the 4th of March to the 15th of December 2003. Individual interviews were conducted with 8 patients on subjective experiences. Data were analyzed through Colaizzi's method, in which meaningful statements were extracted. Result: Four categories were identified from nine theme clusters. The four categories were 'thankfulness', 'dependence', 'sorrowfulness' and 'the sense of being'. Conclusion: These results show the importance of visiting nursing service for the elderly in community
