Estimation Algorithm of Bowel Motility Based on Regression Analysis of the Jitter and Shimmer of Bowel Sounds (장음 특징 변수의 회귀 분석을 통한 장 운동성 추정법)

  • Bowel sounds (BS) are produced by the movement of the intestinal contents in the lumen of the gastro-intestinal tract during peristalsis and thus, it can be used clinically as useful indicators of bowel motility. We devised an estimation algorithm of bowel motility based on the regression modeling of the jitter and shimmer of BS signals measured by auscultation. Ten healthy males ($23.5\pm2.1$ years) were examined. Consequently, the correlation coefficient, coefficient of determination and standard error between the colon transit times (CTT) measured by a conventional radiograph and the values estimated by our algorithm were 0.98, 0.96 and 2.86, respectively. Also, through k-fold cross validation, the average value of the absolute differences between them was $5.0\pm2.5$ hours. This method could be used as a complementary tool for the non-invasive measurement of bowel motility.

Effect of Bojoongikki-tang, Sipjeondaebo-tang, Youngkaechulgam-tang on Gastric Contractility Estimated by Bowel Sound in Rats (보중익기탕(補中益氣湯), 십전대보탕(十全大補湯), 영계출감탕(苓桂朮甘湯)이 장음으로 평가된 흰 쥐의 위(胃) 수축력(收縮力)에 미치는 영향)

  • Objectives : The aims of this study were to evaluate whether gastric contractility could be measured by bowel sound of rats, and whether gastrointestinal side effect of any herbal restoratives would be related with their inhibition of gastric contraction. Methods : Streptozotocin (STZ)-induced diabetic rats were used as a gastric hypocontraction model. At the time of 6 weeks after induction of diabetes, 2mL of normal saline (NS) and 2mL of extract solution each containing 125mg/kg of Bojoongikki-tang (BJ), Sipjeondaebo-tang (SJ), Youngkaechulgam-tang (YG) were given to Normal (NR) and diabetic rats (DR), respectively. Bowel sound was recorded for 30 minute in fasting, and 120 minute of their administration. Gastric motility index was serially calculated with the ratio of accumulated potentials of post-administration/fasting state every 10 minutes. Results : Gastric contractility between NR and DR could be significantly distinguished by bowel sound auscultation. BJ had a decreasing effect, SJ had a bidirectional effect, and YG had an increasing effect on gastric contractility measured by bowel sound auscultation. Conclusions : BJ showed the possibility of inhibitory effects on gastric contraction. So, before administration, a gastric motility test should be recommended to prevent possible side effects in patients with gastric hypocontractility.

A Combined Acoustic Feedback and Noise Cancellation Algorithm for Digital Hearing Aids (디지털 보청기를 위한 음향궤환 몇 잡음 제거 알고리즘)

  • This paper proposes a new algorithm to cancel the acoustic feedback and noise signals in digital hearing aids. The proposed algorithm combines the feedback canceller to remove acoustic feedback signals and the noise canceller to reduce background noises. The feedback canceller is implemented by normal adaptive FIR filter, and the noise canceller is implemented by using the Wiener solution in frequency domain. This noise canceller has the transfer function presented by the power spectral density of signals. To verify the performances of the proposed algorithm, the simulations were carried out for the system. As the results of simulations, it was proved that we can advance 10.85dB output SNR on the average for the forward path gain of 0dB, and 11.04dB output SNR on the average for the forward path gain of 6dB, in the case of using the proposed algorithm.

Correlation between HRV and Bowel Sound Parameters in the Evaluation of Gastrointestinal Parasympathetic Nerve Activity from Patients with Functional Dyspepsia (기능성 소화불량증에서 위장관 미주신경 활성 평가를 위한 심박변이지표와 장음지표의 상관성 연구)

  • Objectives : The aim of this study was to investigate the evaluation of gastrointestinal parasympathetic nerve activity by comparing between HRV and bowel sound parameters in patients with functional dyspepsia. Methods : 62 patients (male 22, female 40) were enrolled. HRV was measured in all patients. and TP (total power), LF (low frequency density: 0.04$\sim$0.15Hz), HF (high frequency density: 0.15$\sim$0.4Hz). and LF/HF ratio were analyzed. HF band is a reflection of parasympathetic activity. and the LF-to-HF ratio is considered a marker of sympathovagal balance. Bowel sounds were recorded at the day after, and postprandial %BS was used to indicate the gastric vagal activity after eating. HRV and bowel sound parameters were compared. and correlation between them was analyzed. Results : There was no difference in LF/HF ratio between patient group and healthy group of other report. According to values of %BS, each patient was classified into a normal (>6 %BS) or hypoactivity (<6 %BS) of gastric vagus nerve group. HF component of the hypoactivity group was not significantly higher than that of the normal group. There was no correlation between HRV and bowel sound parameters. Conclusions : Because no correlation was found between HRV and bowel sound parameters in patients with functional dyspepsia, we concluded that it is not proper to evaluate gastric vagus nerve activity by HRV measurement.

A Case of the Large Colon Impaction in an Adult Donkey (성숙한 당나귀의 큰잘록창자 폐색 증례)

  • The purpose of this report is to describe the diagnosed case of a large colonic impaction in an adult donkey. An adult female donkey (Equus africanus asinus; 6-years-old) was admitted to the Equine Hospital of Jeju Race Park with signs of anorexia, which had started 1 day previously. Clinical examination revealed: weak gut sounds, dehydration, and severe abdominal distension, but normal respiratory and heart rates and no signs of pain. Transrectal palpation identified an impaction in the colon. The patient was treated with nasogastric siphonage, fluid therapy, and nonsteroidal anti-inflammatory drugs. Over the course of a day, her condition worsened; she started trembling, could not stand, and 2 hours later she died. Upon necropsy, a vast amount of ingesta was found in the large colon, particularly in the left ventral colon. The cecum and small colon were normal. Eleven days prior to presentation, the stable management and diet of the donkey had changed. We suspect that the change of feed, reduced water consumption, and lack of exercise precipitated the illness. However, the exact cause of the sudden death of the donkey requires further investigation.

On the relationship between the phonetic realizations of the allophones of the Korean liquid /l/ and their prosodic status (한국에 유음 /l/의 변이음들의 음성적 실현과 운율적 위상과의 상관관계에 관하여)

  • The purpose of this study is to investigate phonetic realization of flap [r], one of the allophones of Korean /l/. Phonetic realization of a segment is affected by not only its neighboring segments but also its prosodic position in an utterance. This study examined how various prosodic positions affect the phonetic realization of [r]. Effects of the four prosodic positions on the phonetic realization of [r] were examined: utterance initial, Intonation Phrase initial, Accentual Phrase initial, and Accentual Medial positions. Word positional effect was also examined: word initial, medial, and final positions. Acoustic and statistical analyses showed that flap [r] was realized in a variety of phonetic forms: from sonorant(the most reduced form) to short stop(the least reduced form). It was shown that generally. word-initial position is stronger than word-medial position. It was also shown that in many cases, utterance-initial position and intonation-phrase-initial position are stronger than accentual-phrase-initial and accentual-phrase-medial positions. Sonorants were observed more often in the prosodically weaker portions. VOT duration was also shorter in accentual-phrase-initial and accentual-phrase-medial positions.

A Study on the Existence, Marketing Behavior and Function of Urban Weekly Markets in Taegu City (도시 요일장의 형성과 이용 및 기능에 관한 연구)

  • The purpose of this study is to identify the existence, personal characteristics and behavior of market-participants, and function of urban weekly markets in Taegu City through observation, interview and questionnaire survey. There are four weekly periodic markets such as Seongso(Monday), Chilgok(Wednesday), Siji(Thursday), and Chilgok(Friday) which have recently developed near the new apartment areas on the outskirts of Taegu. The major findings of this study can be summarized as follows: These markets originated from the agricultural cooperative's Friday markets, but were substantially established by mobile traders or merchants for the sake of their livelihood. Therefore, the existence of these markets is not explained by Stine's central place perspective but partially explained by Hay's economic locational and Bromley's socio-cultural perspective. Most traders are male, aged 30 to 40 years and have an educational level higher than high school. Most of them are living in Taegu and itinerantly visit more than two markets among the four markets by small truck. And most of them sell agricultural products and other food materials, except some who sell manufactured goods. Most consumers are housewives, aged 20 to 30 years and have an educational level higher than high school. Most of them are living within 1,000m and go there on foot. Most of them visit the markets around 3 to 5 p.m. on every market day. And they mostly buy vegetables, fruits, and fishes. Weekly markets provide employment opportunities for those who want to be merchants, and also contribute to the increase of farmers' income, because some traders(18.8%) are farmers who periodically visit markets to sell agricultural products. On the other hand, they function as a general food materials markets to neighboring residents. From these facts, both traders and consumers want weekly markets sustained. Therefore, these markets need to be protected by institutional assistance in spite of being informal markets.
