Partial Anomalous Pulmonary Venous Connection to the Superior Vena Cava

  • Background: Surgical correction of partial anomalous pulmonary venous connection to the superior vena cava has been associated with postoperative venous obstruction and sinus node dysfunction. In this paper we describe our current approach and its short-term results. Material and Method: Between April 1999 and January 2000, 5 consecutive patients, ranging from 2 months to 66 years old, underwent corrective operation for partial anomalous pulmonary venous connection to the superior vena cava at Sejong General Hospital and Daegu Catholic University Medical Center. Surgical correction involved diversion of the pulmonary venous drainage to the left atrium using a right atrial flap(2 patients) or prosthetic patch(3 patients) with division of the superior vena cava superior to the restore site of the pulmonary veins and reimplantation on the right atrial appendage to restore systemic venous drainage. Result: All patients were discharged between postoperative day 9 and 15 without complications. One Russian boy returned to his country, therefore, he was lost to follow-up after discharge. Remaining 4 patients were asymptomatic and in normal regular sinus rhythm at a mean follow-up of 17.75$\pm$4.27 months. Follow-up echocardiographic study (range, 12 to 24 months) revealed no incidence of narrowing of the venous pathways or of residual shunt. Conclusion: Our current approach is relatively simple and reproducible in achieving unobstructive pulmonay venous and SVC pathways. By avoiding incision across the cavoatrial junction, surgical injury to the sinus node and its artery may be minimized. The presented surgical technique can be safely and effectively applied to the selected patients.

  • Many policy makers believe that geographically clustered business activities, in order to stimulate regional innovation, are the keys to development. Underlying the assumption between cluster and development, mere co-located forms engaged in innovative process helped from knowledge transmission locally. To address the constraints of creative economy, creative milieu require networking to support innovation activities. This paper examines the development of the mini cluster reconsidered in creative milieu that shifted environment development through network activities. It shows that the powerful node get more powerful, weak node get weaker in institutional network. Moreover, the significant of cluster in creative milieu is that brings out synergy to spill over knowledge through inter-relationship based on face-to-face meeting. Creative milieu benefits from and contributes to the competitiveness of the city, they are also embedded in its network activities.

Sleep and Suicidal Risk Factors in Korean High School Students (고등학생 청소년들의 수면과 자살위험요인)

  • Objectives: Sleep problems has been consistently reported as a suicidal risk factor in adults and, recently, also in adolescents. In this study, dividing study subjects by the previous suicidal behaviors (suicidal vs non-suicidal), we compared the group differences of suicidal risk factors, and examined the possibility of sleep as a suicidal risk factor. Methods: Study subjects were 561 (271 boys and 290 girls) from a community sample of high school students. Suicidal Risk Behavior Checklist, Center for Epidemiological Study-Depression (CES-D), Symptom Checklist-90-Revision (SCL-90-R) Anxiety and Aggression subscale, Pittsburg Sleep Quality Index (PSQI) were done. Results: Forty six students (8.1%) reported previous actual self-harm behavior as a suicidal attempt, 181 students (32.4%) reported having suicidal thought only. Three hundred thirty four students (59.5%) reported no previous suicidal behavior (thought and attempt, both). Suicidal behavior group showed higher score on risk behaviors such as school violence, substance use and internet addiction. CES-D, SCL-90-R, PSQI showed significant group difference. Logistic regression analysis showed suicidal risk were significantly associated with depression, stress in suicidal risk factors and sleep latency, daytime dysfunction in PSQI. Analysis of variance (ANOVA) shows the most prolonged sleep latency and increased index of all PSQI components except sleep efficiency in suicidal attempt group. Conclusions: Sleep Problems had a strong association with the suicidal risk behavior in adolescents. Sleep problems, especially, prolonged sleep latency, daytime dysfunction might be important markers for suicidal behavior. Screening for sleep problems in adolescents are encouraged for the parents, school teachers, and related medical physicians.

Case Report on Improvement of Reproduction Rate in Hanwoo Farms (한우 농장별 번식기록 분석을 통한 번식률 제고 사례 연구)

  • This work was conducted to study the improvement of reproduction rate from the breeding data collected from four farms from January 2007 to October 2010. The average numbers of service per conception were 1) A farm $1.7{\pm}0.1$ times, 2) B farm $1.5{\pm}0.1$ times, 3) C farm $1.5{\pm}0.1$ times, 4) D farm $1.4{\pm}0.1$ times. The average days from calving to conception was $77.4{\pm}4.8$ days in A farm, $150.8{\pm}11.2$ days in B farm, $90.4{\pm}4.5$ days in C farm, and $71.4{\pm}2.5$ days in D farm. Number of artificial insemination (AI) service per conception was higher at the 30 days before first AI ($2.1{\pm}0.2$ times) than at the 31 days after first AI, and the days from calving to conception were shorter at the 90 days before first AI than at the 91 days after first AI. After timed AI (TAI) treatment, the pregnancy rate was 60.3% for the 58 cows with reproductive disorder. In order to improvement of reproduction rates, the farms has to improve the accuracy of estrus detection, pregnancy diagnosis, check-up for reproductive health, and control of day for first AI periods after calving. The result suggests that farmers need the careful management and reproductive examination of farm animals to improve of reproductive efficiency.

  • Chemoradiation therapy used on pediatric oncology patients often causes dental developmental anomalies that affect future dental care. Defects noted include tooth and root agenesis, root thining and shortening, and localized enamel defects. The effect of radiotherapy usually are confined to the radiation site, but the effects of chemotherapy may be more wide spread because of its systemic distribution and structures and organs unrelated to the primary tumor may be affected. Many pediatric cancers are treated with a combination of radiation and multiagent chemotherapy to create synergic and additive effects. Dental treatment affected by chemoradiation damage to developing teeth includes orthodontic tooth movement, prosthetic abutment considerations, periodontal health, space maintenance, requirements for home fluoride regimens to protect hypomineralized areas, restoration options for hypoplastic/hypomineralized teeth, and endodontic procedures. The following case demonstrate chemoradiation therapy effects on the dental development.

Depression after Traumatic Brain Injury (외상성 뇌 손상이후의 우울증)

  • Traumatic Brain Injury(TBI) of any severity can result in broad and persisting biopsychosocial sequelae. Depression after TBI occur at a greater frequency than in the general population, with estimates approaching 25% to 50% for major depression, and 155 to 30% for dysthmia. Acute onset depressions are related to lesion location and may have their etiology in biological response of the injured brain, whereas delayed onset depressions may be mediated by psychosocial factors, suggesting psychological reactions as a possible mechanism. Anxious depressions are associated with right hemisphere lesions, whereas major depressions alone are associated with left dorsolateral frontal and left basal ganglia lesions. However, there is insufficient information to postulate a specific neuroanatomic model for TBI-related depression.

  • P450 CYP2D6 enzyme(=debrisoquine hydroxylase) is known to metabolize many neuroleptics and some genetic polymorphisms in the CYP2D6 gene were reported to be associated with tardive dyskinesia(TD). We investigeted the association of two genetic polymorphisms in the CYP2D6 gene, $CYP2D6^*4$ and $CYP2D6^*10$, with TD in Korean schizophrenic subjects. Subjects consisted of 71 Korean schizophrenics and TD was evaluated using the Abnormal Involuntary Movement Scale (AIMS). There were no statistically significant differences in the demographic variables of age, male to female percentage and the current antipsychotic(CPZ equivalent) dose between the group with TD and the group without TD. But the duration of antipsychotic drug exposure was significantly higher in the group without TD(p=0.000, by independent t-test). The mean AIMS score in the group with TD was $11.2{\pm}6.6$(S.D.). Genotypings for the presence of $CYP2D6^*4$ and $CYP2D6^*10$ were done using PCR amplifications and endonuclease digestions. There were no statistically significant genotypic and alleleic associations between TD and $CYP2D6^*4$(by chisquare tests), and between TD and $CYP2D6^*10$(by chi-square tests). These results indicate that the $CYP2D6^*4$ and $CYP2D6^*10$ polymorphisms have no significant roles in the causation of TD.

Studies on the Detections of Congenital Genetic Disorder in Holstein Proven and Candidate Bulls (Holstein 보증종모우 및 후보종모우의 선천성 장애 유전좌위 검색에 관한 연구)

  • This study was performed to discriminate defective loci by detection of congenital genetic disorder, to offer basic data for selection and improvement of Korean dairy cattle using frozen semen of Holstein bulls(16 proven and 93 candidate). The results obtained were as follows ; By the detection of DUMP(deficiency of uridine monophophate synthase) for 109 Holstein bulls(16 proven and 93 candidate), DUMP carrier was not found in whole animals. Also, it was possible to early detection of DUMP carrier by using PCR-RFLP(AvaⅠ). As the results of detection for BLAD(bovine leukocyte adhesion deficiency), BLAD carrier was not found in 16 proven bulls. But 5 candidtae bulls are discriminated to BLAD carrier, and it could be predicted to transmitted pathway of inherited loci by pedigree identification. Also, when digesting PCR products using restriction enzyme, results from TaqⅠ restriction enzyme were more efficient than that of HaeⅢ. After detection test of citrullinaemia, it was concluded that proven and candidate bulls were not. However, wide range of research and citrullinaemia genotyping should be performed. As a result of this study, the wide and various research should be performed in genetic disease of animal. And in the selection and breeding of animal, the breeding scheme by completely and continuously management of pedigree should be established.

Effect of Crude Saponin from Korean Red Ginseng on Clinical Chemical Parameters of Ovariectomized Rat (난소절제 흰쥐의 임상화학지수매 미치는 홍삼조사포닌의 영향)

  • This study was carried out to investigate the effect of crude saponin from Korean red ginseng on clinical chemical parameters in ovariectomized rats. The crude red ginseng saponin was prepared by Diaion HP-20 adsorption chromatography and spirit. Tota1 of 50 rats were divided into 5 groups: normal control (NC), sham-ovariectomized (SO), ovariectomized (OR), ovariectomized and saponin treated (OS) and normal control treated with saponin (NS). Saponin was intraperitonally administered for 12 weeks since 1 week before ovariectomy: The body weight of ovariectomized rats showed no significant change but that of NS group showed significant increase when compared with NC group. Platelet counts of serum showed significant increase when treated with saponin regardless of ovariectomy. Triglyceride content of serum in NC group was 152.1 mg/㎗, while that of OR group was decreased to 99.9 mg/㎗ However, when saponin was administered, the content was increased to 138.0 mg/㎗. The weight of spleen also showed significant increase when treated with saponin, while the other organs showed no weight changes. On the other hand, ovariectomy in rats induced decrease in femur weight by 10% when compared with NC group. However, administration of crude saponin in ovariectomized rats recovered the weight of the femur to the similar level of NC (e<0.01 0.05). In addition, femur weight of NS group indicated 10 to 16% higher value than that of NC. These results suggest that Korean red ginseng saponin attenuates phyiological disorders induced by malfunction of ovary.

  • This study has investigated the status and overview of design characteristics of customized shoes, which receives much recognition at domestic and overseas markets. Based on the market research on current consumer lifestyle and needs of the custom-shoes(which is an abbreviation of customized shoes), this study has provided its development directions, views and demands. The design types of custom-shoes have been broadly divide into two: firstly 'style centered custom-shoes'; secondly 'function centered custom-shoes'. Style centered custom-shoes are specifically divided into 3 types; first, formal custom-shoes; second, sports and casual custom-shoes; and lastly, bespoke shoes. Function centered custom-shoes design refers to enhancing the function of personal soleprint which has to be designed by delicately detailed insole and outsole, rather than alignment of exterior parts of the shoes. These are used in professional sports shoes and also include orthopedics footwear and grafted footwear for people who have disabled on foot. Custom footwear market has been highly expanded and has entered value-added market according to increasing interest in foot health and shopping trends on shoes buying like taking shoes as personal expression in accordance with a variety of places and situations. In the near future, custom-shoes can be produced in a couple of days due to the technological advance of 3D scanners and 3D printers. This phenomenon will wants more demands of specific shoes experts such as custom-design consultants or shoes fitting experts, who can overcome limitations of online custom-shoes stores.
