A Case of Gastroduodenal Fistula Caused by Ingested Magnetic Foreign Bodies (자석 이물에 의한 위-십이지장 누공 1예)

  • If multiple magnets are ingested, the potential exists for the magnets attracting one another across the gastrointestinal tract and inducing pressure necrosis, perforation, fistula formation, or intestinal obstruction. We report the case of a 12-year-old boy who suffered from a fistulous communication between the lesser curvature of the mid-body of the stomach and the duodenal bulb, caused by 4 ingested magnets (Singing Magnets, China). The patient presented with moderate mental retardation, a one-year history of cyclic vomiting, and abdominal discomfort. We present the findings of simple abdominal radiography, esophagogastroduodenoscopy, computed tomography, and upper gastrointestinal series. An emergency exploratory laparotomy was performed, which revealed a gastroduodenal fistula. Fistula repair and the removal of 4 magnetic toys were subsequently performed. We emphasize that clinicians who care for children should be aware of the hazards of magnetic toy ingestion.

Primary Cardiac Lipoma Combined with Chest Pain Like Angina Pectoris (협심증 증상을 동반한 심낭내 원발성 지방종)

  • A 60-year-old male was admitted because of dyspnea and angina like chest pain. Noninvasive findings showed a mass at the posterior aspect of the left atrium. The mass was removed under cardiopulmonary bypass. An encapsulated adipose mass, which originated from the left atrium without any invasion to the pericardium, was completely excised. Histological examination showed mature adipose tissue with partial necrosis, confirming the diagnosis of lipoma. We report a rare of case of massive left atrial lipoma occupying the pericardial space.

  • The Korea Institute of Oriental White Stork Rehabilitation Research (KIOWSRR) is breeding oriental white storks for residential rehabilitation in South Korea. Three oriental white storks were referred with anorexia and depression. Two storks were died before examinations and one stork was died after 1 day of fluid therapy. On necropsy, salmon-colored subcutaneous and visceral fats were examined in all storks. Multiple focal necroses on liver surface were also examined. In gizzards, many gravels and grasses were severely tangled; therefore, it produced a large mass that blocked digestive system of the storks. Small intestinal segments of the stork, which was dead after the fluid therapy, were severely adhesive. On histopathology, diffuse necroses with/without inflammation were examined in the liver, gizzard and intestine in all storks. Bile stasis was additionally examined in the liver. According to the results of the examinations, all storks were diagnosed as gizzard impaction caused by parorexia.

  • Hyperthermia supplies RF high-frequency energy above 1MHz to the tumor tissue through the electrodes. And the temperature of the tumor tissue is increased to $42^{\circ}C$ or more to cause thermal necrosis. A mathematical model can be derived a human body model for absorption and transmission of electromagnetic energy in the human model and It is possible to evaluate the distribution of temperature fields in biological tissues. In this paper, we build the human model based on the adult standard model of the geometric shape of the 3D model and use the FVM code. It is assumed that Joule heat is supplied to the anatomical model to simulate the magnetic field induced by the external electrode and the temperature distribution was analyzed for 0-1,200 seconds. As a result of the simulation, it was confirmed that the transferred energy progressively penetrates from the edge of the electrode to the pulmonary tumors and from the skin surface to the subcutaneous layer.

Histopathology of a acanthocephalan infection in swamp eel, Monopterus albus (드렁허리, Monopterus albus의 구두충 감염예에 대한 병리조직학적 관찰)

  • Since March in 2013, Inland Aquaculture Research Center, NFRDI has cultivated 1,000 wild swamp eel(Monopterus albus) for species conservation research. While cultivating, 100 fishes showed clinical sign that darkness color, mucus hypersecretion and anus rubor. Even some of them were died. Result of anatomical test, all 100 fishes were infected with intestinal parasite, acanthocephalan. So we were going to determine the case of acanthocephalans infection in swamp eel(Monopterus albus) as histopathologically. Acanthocephalan was founded in alimentary canal only. Parasite were confirmed 19 unit in individual fish, averagely. Heavy infected fishes were confirmed enterocleisis by acanthocephalans. Worms were attachment in submucosa layer of alimentary canal by invading proboscis. Characteristic symptoms were observed in stomach and intestine, including hyperemia in mucous epithelium, infiltration of eosinophills in submucosa layer, inflammation, parasitic granuloma. Some fishes showed vacuolization of gastricgland epithelium, necrosis of intestinal mucosa. Other organs, excluding alimentary canal, were not found lesion. The results of this study, the effect of the acanthocephalan infection on swamp eel(Monopterus albus) could find and it seems to be a big help in the future swamp eel(Monopterus albus) cultivation.

Osteonecrosis of Maxilla in Wegener's Granulomatosis: Case Report (상악골 괴사를 동반한 베게너 육아종증: 증례보고)

  • Wegener's granulomatosis (WG) is an autoimmune disease of unknown etiology characterized by the triad of necrotizing granulomatous lesion in the upper and lower respiratory tracts or both, disseminated vasculitis involving both small arteries and veins, and necrotizing glomerulonephritis. The most common oral lesions associated with WG are ulceration and strawberry gingivitis. A 47-years old man in medical care associated WG was consulted our Division of Oral and Maxillofacial Surgery for the chief complaint of toothaches. Pre-operative panorama showed the alveolar radiolucency and the loss of lamina dura regarding the left upper teeth. An oropharyngeal magnetic resonance imaging also revealed the increased bone marrow signal intensity on the left maxilla. Under the impression of maxillary osteonecrosis due to WG, maxillary saucerization with removal of involved teeth was performed. We obtained good results and report the first case of WG in Korea, with the review of literatures regarding oral and general systemic features.

The histological structure and the pathological lesions of gill in teleosts (어류의 아기미의 조직학적 구조와 병변)

  • The delicate histological structure of gill in teleosts can be easily affected by a variety of biological, chemical or physical detrimental agents because it is directly exposed to the surrounding water. The epithelium of secondary lamella is thin to allow efficient gaseous exchange and this also renders it particularly vulnerable to various pathogens. As well as the main respiratory role, the gill has other various important functions such as acid-base balance, osmoregulation or the excretion of nitrogenous waste products. Thus destruction of epithelial integrity such as epithelial necrosis or thickening can render a fish very vulnerable to respiratory, secretory and excretory difficulties. This article was tried to describe in detail the common processes of pathological responses correlated to the normal histological structures of the gill in teleosts.

  • Hybrid procedure was performed for a thirteen-day-old girl with a functionally single ventricle, who weighed 2.2 kg and had been prematurely born at $32^{+5}$ weeks of gestation. She underwent bilateral pulmonary artery banding using 3.5 mm Gore-Tex graft, ductal stenting using balloon expandable stent, and reverse Blalock-Taussig shunt with 3.5 mm Gore-Tex vascular graft. After discharge, she was followed up for 4 months, and underwent 2nd stage operation(extensive arch reconstruction with Damus-Kaye-Stansel anastomosis, atrial septectomy, bilateral pulmonary artery angioplasty, bidirectional cavopulmonary shunt). She has been followed up for 4 months after the 2nd operation with an excellent clinical condition.

Extravasation Injury of Contrast Media in the Neck and Thorax During MDCT Scanning with 3D Image Reformation Findings (CT검사에서 조영제의 혈관외유출에 의한 목 및 흉부 손상의 3차원 재구성 영상)

  • Contrast media may cause tissue injury by extravasation during intravenous automated injection during CT examination. Here, we present a study in which contrast media extravasation was detected and localized in the neck and thorax by three-dimensional(3D) CT data reformation. The CT studies of the extavasation site were performed using a 3D software program with four different display techniques axial, multi planar reformation(MPR), maximum intensity projection(MIP), and volume rendering displays are currently available for reconstructing MDCT data. 3D image reconstructions provide accurate views of high-resolution imaging. This paper introduces extravasation with the MDCT and 3D reformation findings of contrast media extravasation in neck ant thorax. The followed injection of the external jugular vein into an existing intravenous catheter and a large volume of extravasation was demonstrated on by 3D MDCT.

Vibro ordalii, the causative agent of massive mortality in cultured rockfish(Sebastes schlegeli) larvae (양식 조피볼락(Sebastes schlegeli) 치어의 대량폐사 원인인 비브리오병에 관하여)

  • A specific disease syndrome, which led to massive mortality on larve of rockfish(Sebastes schlegeli) in marine hatcheries at Chungnam area during the period 1995~1996 was studied. The causative agent isolated from the diseased or dead larvae was identified as Vibrio ordalii on the basis of biochemical and biological characteristics. In the experimental challenges aganist 0 and 1 summer fish conducted at two different temperatures as $18^{\circ}C$ and $25^{\circ}C$, Vibrio ordalii showed higher virulence to no summer fish at $18^{\circ}C$ than 1-summer fish at $25^{\circ}C$. These results were consistent to field data obtained during epizootic outbreaks in the farms. Moribund and died larvae presented telangiectasis of secondary gill lamella and brain, dissecting of respiratory epithelium, atrophy of hepatic cells and necrosis of kidney associated with the presence of the bacteria. But the digestive tissue of these fish showed no significant change.
