치조능확장술과 자가치아골이식술을 이용한 치조능 수평증대술: 증례보고 (Horizontal Ridge Augmentation using Ridge Expansion and Autogenous Tooth Bone Graft: A Case Report)

  • 점차적으로 폭경이 증가하는 나사들을 이용하여 치조능확장술을 시행하고 임프란트를 식립하였다. 임상적으로 양호한 결과를 얻었으며 수술 중 협측 피질골판 골절은 발생하지 않았다. 임프란트 주변 결손부와 협측 피질골판 상방에 자가치아골이식재를 이식함으로써 부가적인 치조능 확장 효과와 동시에 협측 피질골판 흡수를 보상하였다. 치조능 폭경이 협소한 부위에 임프란트를 식립할 때 나사를 이용한 확장술은 성공적이고 예측 가능한 술식이며 자가치아골이식재는 치조능증대술과 골유도재생술을 위해 사용될 수 있다.

다양한 골이식재료를 이용한 상악동 골이식 4개월 후 조직형태계측학적 평가 (Histomorphometric Study of Sinus Bone Graft using Various Graft Material)

  • 본 연구는 상악동골이식술에 사용된 다양한 골이식재와 자가치아골이식재의 효율성을 비교하고 조직형태계측학적으로 평가하는 것이다. 2007년 10월부터 2009년 9월까지 분당서울대학교병원에서 상악동골이식술을 시행 받은 24명의 환자들을 대상으로 하였다. 환자들의 평균 나이는 $52.51{\pm}11.86$세였다. 모든 증례들은 사용된 골이식재의 종류에 따라 3그룹으로 분류되었다. 1군: 자가치아골이식재, 2군: OrthoblastII(integra lifescience corp. Irvine, US)+Biocera(Osscotec, seoul, korea), 3군: DBX(Synthes, USA)+BioOss(Geistlich Pharm AG, Wolhusen, Switzerland). 총 37개의 임프란트가 식립되었다(1군: 7, 2군: 10, 3군: 20). 신생골 형성, 직골과 층판골의 비율, 신생골과 골이식재의 비율을 조직형태계측학적으로 평가하였다. 통계처리는 SPSS Ver. 12.0을 사용하여 Kruskal-Wallis test를 시행하였다. 신생골 형성 비율은 1군 $52.5{\pm}10.7$, 2군 $52.0{\pm}23.4$, 3군 $51.0{\pm}18.3$였고 통계적으로 유의성 있는 차이는 없었다. 직골과 층판골의 비율은 1군 $82.8{\pm}15.3$, 2군 $36.7{\pm}59.3$, 3군 $31.0{\pm}51.2$였다. 신생골과 잔존골이식재의 비율은 1군 $81.3{\pm}10.4$, 2군 $72.5{\pm}28.8$, 3군 $80.3{\pm}24.0$였다. 4개월의 치유기간 후에 모든 군에서 양호한 신생골 형성이 이루어졌다. 본 연구의 제한적인 범위 내에서 자가치아골이식재는 상악동골이식에 사용될 수 있는 새로운 골이식재료라고 사료된다.

  • Horizontal and vertical ridge augmentation was performed using autogenous tooth bone graft block and powder in 44-year old male patient. Excellent bony healing was obtained 2~4 months after ridge augmentation. Implant treatment was performed successfully.

  • An extraction socket was preserved and reconstructed using an autogenous tooth bone graft powder and block in two patients. The grafted site was healed 3 to 3.5 months after surgery. Implant treatment was successfully completed.

  • This study evaluated the mineral crystalline structure of an autogenous tooth bone graft material. The crystalline structures of the autogenous tooth bone graft material enamel (AutoBT E+), dentin (AutoBT D+), xenograft (BioOss), alloplastic material (MBCP), allograft (ICB) and autogenous mandibular cortical bone were compared using XRD. The XRD pattern of AutoBT dentin and ICB was similar to that of autogenous bone.

  • The authors installed implants combined with guided bony regeneration (GBR) using autogenous tooth bone graft material in the patients. In one patient, GBR and simultaneous implant placement were performed. In two patients, GBR was performed and the implants were placed after 6 months. All patients achieved favorable clinical outcomes. Excellent osteoconductive bony healing was observed in the 6 month histology examination after the bone graft.

  • Horizontal and vertical ridge augmentation with implant placement was performed, using a block type of autogenous tooth bone graft in a 37-year old male patient. This material was very useful for the case of severe alveolar bone resorption of a single tooth. After 13 months, excellent bony healing was obtained and final restoration was performed successfully.

자가치아골이식재를 이용한 골이식술의 임상적 평가 (Development of autogenous teeth bone graft material and clinical evaluation)

  • The author have been conducting research on the development of biomaterials using human teeth since 1993, and we recently reported the results of several of our advanced studies. Based on previous studies, new bone graft material using autogenous tooth(AutoBT) was developed. The author performed implant placement combined with GBR and sinus bone graft using AutoBT during the period of June 2009 to September 2009. During the postsurgical healing period, clinical and radiological evaluations were performed. For patients who consented to histological testing, tissue samples were obtained and histological examination was performed two months and four months after GBR and four months after sinus bone graft. Serious complications pertinent to bone graft materials were not observed, and it was shown that the level of the crestal bone in the vicinity of implants was relatively well maintained. In histologic examination after two months and four months, favorable new bone formation was observed.
