The Clinical Aspects of Pulmonary Tuberculosis Patient Failed in Retreatment (재치료실패 폐결핵환자의 임상 양태)

  • Background: There are many retreatment failure patients admitted in National Kongju Tuberculosis Hospital. But there is not satisfactory treatment method for them at present. We think that more attentions and active measures for them are needed. Method: We reviewed sex and age, duration of illness, previously used antituberculosis drugs, drug resistance, extent of disease, reasons for early stopping or irregular medication and schooling of 50 retreatment failure patients admitted in National Kongju Tuberculosis Hospital from April 1992 to February 1993. Results: 1) The male to female ratio was 3:2 and 62% of the patients were between 21 and 40 years of age. 2) Twenty eight cases (56%) had the duration of illness over 10 years. 3) All cases had used most of the antituberculosis drugs. 4) Drug sensitivity test showed resistance to RMP in 46 cases (96%), INH in 40 cases (83%) and other drugs in 3-32 cases (6-67%). 5) Forty eight cases (96%) had far advanced disease on chest P-A film. 6) Twenty eight cases (56%) in primary chemotherapy and twenty one cases (42%) in retreatment had the histories of premature stopping or irregular ingestion of the drug. The reasons for premature stopping or irregular ingestion of the drug were as follows; in primary chemotherapy, 29 cases (75%) were due to 'having no symptoms', while in retreatment, 6 cases (29%) were due to 'having no symtoms', 6 cases (29%) were 'too, busy' and 3 cases (14%) were for 'financial problem'. 7) Twenty seven cases (54%) had at least graduated from high school. Conclusion: Greater efforts are needed to prevent tratment failure. More supports and admission treatment for retreatment failure patients are needed to prevent infection and to treat properly.

Trend of Emergency Department Visits for Elderly Patients with Chronic Diseases : 2014-2019 (만성질환을 동반한 노인 응급환자 추이: 2014-2019)

  • The purpose of this study was to determine the change trend of emergency department visits among elderly patients with chronic diseases. Using the National Emergency Department Information System data, from January 1, 2014 to December 31, 2019, the selected patient data were evaluated for the emergency department discharge main diagnosis codes for eight chronic diseases. The incidence of elderly chronic diseases, emergency department visits, and admission rates were analyzed. Since 2014, there has been a consistent increase in the number of elderly patients visiting the emergency department, especially among those aged over 85 years. The number of emergency department visits among the elderly chronically ill patients also increased, with a significant increase in ischemic heart disease and arthrosis cases. Furthermore, there was a significant difference in the number of chronically ill patients in each year from 2014-2019 (P<0.001). With respect to the trend of admission rates to the emergency department by chronic disease, most diseases showed an increasing trend (P<0.001). however, hyperlipidemia showed a continuous decreasing trend in all age groups since 2014 (P<0.001). Among the elderly chronically ill patients, a greater increase in the admission rate following emergency department visits was noted in those over 85 years of age, with a significant difference in all diseases, except for hyperlipidemia, hypertension, and tuberculosis (P<0.001). As the aging population grows, the emergency department admission rates among the elderly chronically ill patients will rise rapidly. This could create issues with respect to the use and consumption of emergency medical resources. Hence, it is necessary to manage chronic diseases effectively in the elderly.

  • The purpose of this study was to identify nurses' communication style and the extent of their health literacy and to discern the impact of nurses' communication and health literacy on self-care behaviors, as perceived by elderly diabetic patients in comprehensive nursing-care service wards. This study provides basic data for establishing an efficient nursing-care service-system environment through high-quality self-management. The measurement and improvement of an accurate health literacy level of elderly diabetic patients is an important intervention method to increase the self-care behavior of patients. Therefore, by developing a distinguished nursing intervention plan, early treatment of diabetic patients and quality of care may be possible.

  • The Illness Management and Recovery(IMR) Program was designed to improve self-management and recovery of people with mental illness. The purpose of this study was to identify the effects of the Korean version of the IMR program on recovery, knowledge against mental illness, hope among inpatients with severe mental illness. 92 participants diagnosed with schizophrenia, bipolar disorder and schizoaffective disorder were assigned to either an IMR group(n=51) or the control group(n=41) without randomization. Data collection had been conducted from June 2015 to December 2016. Mental Health Recovery Scale, Knowledge Scale, and Hope Scale was administered to participants before and after the intervention. Participants in the IMR program showed significant improvement in progress toward their recovery and knowledge about their mental illness compared with those receiving treatment as usual. However, changes in hope were not significant in both groups. This is the first evaluation of the effects of the Korean version of the IMR program. Our findings support potential benefits of implementing the Korean version of the IMR program.

A Clinical Study for 157 Cases of CVA by Sasang Constitutional Medicine (사상의학적(四象醫學的) 중풍치험(中風治驗) 157례(例)에 대(對)한 임상적(臨床的) 연구(硏究))

Background and Purpose: We intended to make the standards of CVA management of Sasang Constitutional Medicine and be helpful to understand the constitutional symptoms through a clinical study of CVA. 2. Methods: We studied 157 CVA inpatients(79 men and 78 women) who were in constitutional clinic of Dongguk Pundang Oriental Hospital during 1 year from March 1997 to February 1998. We investigated and analysed the characteristics, past histories, symptoms and progresses of patients. 3. Results: The results of constitutional analysis showed 88 Taeumin(56.1%), 48 Soyangin(30.6%) and 21 Soeumin(13.4%). The frequency of strokes was much the same between male and female and the greater part of cases were 50-70 of age. The CVA cases were classified into 119 cerebral infarction(75.8%), 11 cerebral hemorrhage(7.0%), 1 subarachnoid hemorrhage(0.6%) and 26 trasient ischemic attack. From the standards of symptom management, there were abnormality of a coated tongue(63.1%) in most cases, sleeping disorders(48.4%), excremental disorders(44.7%) and urinary disorders(36.9%) on admission. With regard to the rate of improvement in the standards of CVA management, excremental disorders improved 87.1% and emotional disorder 81.0%, swallowing disorder 78.7% and so on.

Clinical Characteristics of Asthmatic Patients Who Visited Emergency Room (응급실로 내원한 천식환자들의 임상적 특성)

  • Background : Despite remarkable progress of understanding the pathophysiology and therapy of bronchial asthma, asthma morbidity and mortality are on the rise. Also hospitalization and attending rates of emergency department for asthma have been increasing gradually. We analyzed clinical characteristics and prognosis of patients who visited emergency room due to asthma attack in order to define clinical characteristics of these group of patients. Method : We reviewed 105 adult asthmatic patients who attended emergency department of Korea University Hospital between August 1995 and July 1996, retrospectively. Results : 103 patients(56 female, 47 males, mean age : 48.6 years) attended-68 self referral, 18 practitioner referral and 17 OPD transfer- and 86 patients(83.5%) were admitted. Attending emergncy department was clearly more frequent in December(13.6%) and May(12.6%). Time lag between onset of asthmatic attack and arrival at the hospital was $14.2{\pm}15.5$ hour and initial peak expiratory flow rate was $166.7{\pm}68.3L/min$.(43.3% predicted) The commonest cause for visiting emergency room was aggravation of asthma due to upper respiratory tract infection in mild asthmatics. About half of them had history of previous ER visits. Their prognosis was not bad, but after discharge, about half of patients escaped from OPD follow-up. Conclusion : As a group they merit detailed attention and follow up arrangement. Clinician need to monitor and review the treatment plans, the medications, the patient's management technique, and the level of asthma control. For this group, plans for longer term treatment, including asthma education program and adjustment of overall treatment plan should be made.

Differences in Medical Care Utilization by Regional Economic Status (지역 소득수준에 따른 의료이용의 차이)

  • The purpose of this study was to identify the differences in medical care utilization by regional economic status using the National Hospital Discharge Patients Injury Survey. In order to determine economic status of each region, 234 cities and counties were categorized 5 quintiles according to their financial self-reliance ratio. The main results are as follows. First, low economic region has high age-standardized admission rate and standardized mortality rate. Second, of 16 major diseases, cerebrovascular and heart diseases, lung cancer, and stomach cancer reported greater changes in standardized mortality rate by regional economic status. Third, the rate of admission via emergency room in low economic region is higher than that of high economic region. Lastly, in the major illnesses, lower economic status led to an increase in average length of stay. Therefore, In order to bridge the gap in health inequality across regions, a regional medical policy tailored for each region and characteristics of the economic status should be established.

  • The aim of this study was to compare effects of oral health education on oral health behavior according to demographic and dental health-related characteristics in inpatient alcoholics. 62 alcoholic male patients were recruited to 32 patients at education group and 30 at non-education group. To search demographic and dental health-related characteristics, the self-administered structured questionnaires were used, and the survey was conducted before and after the oral health education. The oral health education program was consisted of 40 mins theoretical education and individual tooth brushing training once a week for 4 weeks. The oral health education for alcoholic patients had big effects on oral health behavior. And these results indicate that if oral health program was performed systematically in alcohol counseling centers or alcohol hospitals, more oral health promotion effects will appear.

  • This study aims to provide basic information necessary to find an efficient management plan for patients using auto insurance. The analysis was conducted on the five-year auto insurance medical expenses review data registered in the health care bigdata Hub from 2016 to 2020. As a result of the analysis, the number one composition ratio of auto insurance inpatient treatment expenses was treatment and surgery fees for Certified tertiary hospitals, hospitalization fees for general hospitals, hospitals and clinics, and treatment and surgery fees for oriental medical institutions and dental hospitals. outpatient treatment expenses was doctor's fee for medical institution, treatment and surgery fees for oriental medical institutions and dental hospitals. The ratio of medication, anesthesia, and special equipment significantly affected the cost of inpatient. And the ratio of physical therapy significantly affected the cost of outpatient.

The Effects of an Admission-Education Program on Knowledge, Self-Efficacy, Self-Care and Glucose Control in Type 2 Diabetes Patients (당뇨입원교육프로그램이 제2형 당뇨병 환자의 지식, 자기효능감, 자가관리 및 당 조절에 미치는 효과)

  • Purpose: This study was performed to explore the effects of an admission-education program on knowledge, self-efficacy, self-care and glucose control in type 2 diabetes patients. Method: A single group pre-post test design was used. Twenty-one patients participated in this program during 6 days which consisted of education and practice about exercise, diet, self-care and prevention of complications. Variables at baseline, 3 months, and 6 months were measured. Results: Knowledge level, self-care level and self-efficacy were significantly increased over time after the admission-education program. The $HbA_1C$ level and glucose level in urine were significantly decreased over time after the admission-education program. Conclusion: The admission-education program can increase levels of knowledge, self-care, self-efficacy and decrease glucose levels. Therefore, this admissioneducation program was shown to have a positive effect on the management of diabetes.
