신생아집중치료실 간호사의 임종간호 스트레스, 대처방식 및 심리적 안녕감 (Stress due to End-of-Life Care, Coping Strategies, and Psychological Well-being among Nurses in Neonatal Intensive Care Units)

  • Purpose: This study aimed to identify stress levels due to end-of-life care, coping strategies, and psychological well-being among nurses in neonatal intensive care unit, and to investigate the effect of stress levels and coping strategies on their well-being. Methods: A total of 128 nurses in the neonatal intensive care units of general hospitals in B city participated. The data were collected using a self-report questionnaire. The collected data were analyzed using descriptive statistics, the t-test, ANOVA, the Pearson correlation coefficient, and hierarchical regression with SPSS version 22.0. Results: The coping strategy that nurses most often used was seeking social support. The factors affecting the well-being of the participants were wishful thinking, problem-focused coping and seeking social support, in order. Those 3 variables explained 21 % of the total variance in psychological well-being. Problem-focused coping and seeking social support were positively associated with psychological well-being, while wishful thinking showed a negative association. Conclusion: In order to improve the psychological well-being of nurses in neonatal intensive care units, it is necessary to provide nurses with a program to build a social support system and to improve their problem-based coping skills.

요양병원 입원 노인의 좋은 죽음 인식, 사전의료의향서에 대한 태도 및 임종치료선호도 (Good Death Awareness, Attitudes toward Advance Directives and Preferences for Care Near the End of Life among Hospitalized Elders in Long-term Care Hospitals)

  • Purpose: This study was done to examine good death awareness, attitudes toward advance directives (ADs), and preference for care near the end-of-life (PCEOL) of hospitalized elders in long-term care hospitals. Relevant characteristics were investigated as well as correlation of the variables. Methods: This descriptive research study involved 161 hospitalized elderly patients in long-term care hospitals. A self-report questionnaire was used to measure Good Death Scale, ADs Survey, PCEOL Scale, and general characteristics. Collected data were analyzed using descriptive statistics, t-test, ANOVA, and Pearson correlation with SPSS/WIN 23.0. Results: In terms of good death awareness, a significant difference was observed; in according to age (F=3.35, p=.037), payer of treatment costs (F=3.98, p=.021), mobility (F=3.97, p=.021), heard discussion about ADs (t=-3.89, p<.001), and willing to complete ADs (t=2.12, p=.036). As far as attitudes toward ADs, the participants presented significant difference depending on religion (t=2.38, p=.018), average monthly income (F=3.91, p=.022), duration of hospital admission (F=5.33, p=.006), person to discuss ADs (t=-2.76, p=.006). On PCEOL, there was a significant difference, depending on religion (t=-3.59, p<.001) and perceived health status (F=3.93, p=.022). Finally, as for how the variables were related to each other, good death awareness and attitudes toward ADs had a weak positive correlation with PCEOL. Conclusion: To help seniors staying in nursing homes face a good death and enjoy autonomy, there should be educational and support systems that reflect each individual's sociodemographic characteristics so that the seniors can choose what kind of care they want to receive near the end-of-life.

신생아중환자실 간호사의 임종 환아 돌봄 경험 (Neonatal Intensive Care Unit Nurses' Experience in Caring for Infants Who Are Dying)

  • 목적 본 연구의 목적은 신생아중환자실에서 근무하는 간호사의 임종 환아 돌봄 경험을 이해하고 개인의 생생한 경험의 전체적인 의미 구조를 기술하기 위함이다. 방법 본 연구는 현상학적 방법론을 적용한 질적 연구이다. 본 연구의 참여자는 서울시 S대학병원의 신생아중환자실에서 근무하면서 임종 환아 돌봄 경험이 있는 간호사로서 눈덩이 표집 방법을 통해 모집되었으며 자료가 충분히 포화되었다고 판단되는 시점까지로 정하여 총 8인이 선정되었다. 본 연구의 자료는 2012년 12월부터 2013년 2월까지 참여자와의 개인 심층 면담을 통하여 수집되었으며 수집된 자료는 Colaizzi의 현상학적 연구 방법을 근거로 분석되었다. 결과 신생아중환자실 간호사의 임종 환아 돌봄 경험의 구조는 '사투하는 환아의 생명존중과 안위존중의 갈림길', '환아의 죽음에 대한 정서적 고통', '환아의 죽음에 대처하며 적응하기', '임종 환아 돌봄에 대한 새로운 기대'의 4개의 주제모음으로 확인되었다. 결론 본 질적 연구를 통하여 신생아중환자실 간호사의 임종 환아 돌봄 경험을 좀 더 깊이 있게 살펴보고 이해할 수 있었다. 본 연구 결과를 토대로 신생아중환자실에 근무하면서 환아의 임종을 경험하는 간호사들이 느끼는 정서적인 갈등과 어려움을 다루고 지지해 줄 수 있는 전문적인 지지 프로그램의 개발과 임종 환아 돌봄에 대한 교육의 활성화와 더불어 임종 환아의 부모와 의료인을 위한 시간과 공간이 제도적으로 마련되었으면 한다.

노인의 임종과정에 대한 태도 유형 분석 (A Study on Attitude Patterns of Elderly toward Dying)

  • Purpose: This study was performed to identify and understand the nature of attitude patterns regarding dying process exhibited in the elderly persons who lived in the community setting. Method: Q methodology was applied. Ten elderly were selected from the senior centers and requested to answer open ended questions by using written questionnaires. Additionally, 12 other elderly from different senior centers were interviewed in depth, and their statements were recorded via voice pen by researchers to derive Q-population. From the 120 Q-population, 33 Q-sample were selected. 22 P-sample were rated by 1 to 9 point scale respectively for Q-sorting. For Q-type analysis, pc QUNAL was used. Result: Three types of attitude patterns were identified, which were valuing dignity, seeking provision of nature, and avoiding of dying process. Conclusion: The significance of this study is as follows : Discovery of 3 attitude patterns of elderly toward dying, better understanding about elderly's perspectives for favorable dying, and informations for possible development of nursing strategies for elderly who is facing death.

임종간호 교과구성의 요구에 대한 주관성 연구 (Contents Related to End-of Life Care in Nursing Curriculum: Q Methodological Approach)

  • Purpose: The purpose of this study was to analyze the subjectivity of contents related to death as a nursing curriculum subject for nurses and students. Method: The Q-methodology which provides a method for analyzing the subjectivity of each items was used. The 34 selected Q-statements sorted by each of the 36 participants were classified into a normal distribution by using a 9 point scale. The collected data were analyzed using the QUANL PC program. Results: Four types of content related to death nursing curriculum were identified. Type I is the psychologically sympathetic type, Type II is the clinical based experience type, Type III is the physically comfort seeking type, and Type IV is the human-understanding oriented type. Conclusion: The results of this study show that different approaches to education are needed to address the four types of content related to death as a nursing curriculum subject. Both contents and characteristics need to be considered.

임상실습경험에 따른 간호대학생의 죽음에 대한 태도와 자아존중감 및 삶의 만족도 비교 (Comparison of Attitudes of Nursing Students toward Death, Self-esteem and Life Satisfaction according to Clinical Experience)

  • 목적: 본 연구는 간호대학생의 임상실습 경험유무에 따른 죽음에 대한 태도, 자아존중감 및 삶의 만족도의 차이를 파악하기 위한 서술적 조사연구이다. 방법: 양산시에 위치한 2개 대학에 재학 중인 간호대학생 1,030명으로 대상으로 하였다. 자료수집은 2010년 10월 1일~10월 30일까지 강의시간 이외의 시간에 연구의 목적과 방법을 설명하고 설문에 대한 응답은 익명으로 처리됨을 알려 준 뒤 연구에 참여하기로 자발적으로 동의한 대상자에게 서면동의서와 설문지를 배부하고 개별적으로 설문내용을 작성하도록 한 후 현지에서 직접 회수하였고, 수집된 자료는 PASW statistics data editor 통계 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, 카이제곱 검정, t 검정, 분산분석, Scheffe 검정과 Pearson correlation coefficients로 분석하였다. 결과: 죽음에 대한 태도 점수는 임상실습 경험이 있는 간호대학생의 경우 2.85점, 실습경험이 없는 간호대학생의 경우 2.79점으로 중간수준이었으며, 임상실습 경험이 있는 간호대학생이 임상실습 경험이 없는 간호대학생에 비해 죽음에 대해 보다 긍정적인 태도를 가지고 있었고 자아존중감과 삶의 만족도가 높았다. 임상실습 경험이 있는 간호대학생의 죽음에 대한 태도는 자신이 지각하는 경제수준과 본인이 지각하는 자신의 건강상태에 따라 죽음에 대한 태도의 차이가 있었으며, 자아존중감과 만족도와 낮은 상관관계가 있었다. 결론: 간호제공자의 죽음에 대한 태도는 임종간호의 가장 중요한 요소로 간호의 질을 결정함을 감안할 때 미래의 간호사가 될 간호대학생의 죽음에 대한 태도를 보다 긍정적으로 변화시킬 필요가 있다. 특히 임상실습 경험이 없는 간호대학생이 죽음에 대해 보다 부정적인 태도를 가지고 있었고 자아존중감과 삶의 만족도가 낮았으므로 이들에게 보다 적극적인 관심이 필요할 것이다. 한편 임상실습 경험 유무에 따른 간호대학생의 죽음에 대한 태도, 자아존중감 및 삶의 만족도에 대한 선행연구가 부족하여 논의하는데 제한이 있으므로 반복연구가 필요할 것을 제언한다.

임상간호사의 임종환자 간호체험 (The lived experience of nursing care for the dying patients in clinical nurses)

  • It is important for nursing managers to understand the lived experience of nursing care for dying patients in clinical nurses for the effective management of them. The purpose of this Phenomenological study was to explore the lived experience of nursing care for the dying patients in clinical nurses and identify the meaning and structure of their lived experience. This study was conducted from 1 of June, 2000 to 1 of November, 2000. Data were collected with several in-depth interviews until data were fully saturated, from 1 of June, 2000 to 10 of September, 2000. The Subjects were five nurses who had more than three-year job experience in caring for dying patients, three protestant christians and two atheists, one married and four unmarried persons. The range of their age was from 28 to 36. Data were analysed by the Colaizzi's methodology. Ten themes were extracted from fifty-one fomulated-meanings. Fomulated-meanings were extracted from the restatements and the significant-statements which were deriven from the raw data. Finally ten themes took form of five structures. Five structures of 'The lived experience of nursing care for the dying patients in clinical nurses' were : 1. Experiencing guilty feeling and anger due to their and other's manneristic and ignored attitude toward dying patients 2. Feeling heartily the necessity of the education of hospice care because of their incompetence due to lack of knowledge of hospice care 3. Recognizing the human rights of dying patient's thinking themselves and their families 4. Felling satisfaction with their nursing accomplishments and reflecting their life through nursing care of the dying patients 5. Experiencing low self-respect due to the other's negative perspective toward their job The results of the study would give useful information to nursing managers to understand the lived experience of nursing care for dying patients in clinical nurses and establish adequate strategies to support them.

가정전문간호사의 죽음인식과 죽음 불안이 임종간호태도에 미치는 영향 (Association of Perceptions and Anxiety of Home Health Nurses about Death, on their Attitudes to Terminal Care)

  • Purpose: The purpose of this study was to identify the extent to which in-hospital-based home health nurses' perceptions and anxiety about death following terminal care, affects their attitudes toward terminal care. Methods: The subjects were 128 advanced practice nurses working in hospital-based facilities for home health care, located in Seoul, Gyeonggi-do, and Incheon Metropolitan City. Data were collected from May 3, 2019, to June 3, 2019, using structured questionnaires, on terminal care and related variables based on the literature. Data were analyzed by performing the Student's t-test, one-way ANOVA, Pearson's correlation, and multivariable stepwise regression using the SPSS Version 25.0 program. Results: The scores of the attitudes toward terminal care of home health care nurses was 3.25 points out of a possible 4 points. Factors affecting nurses' attitudes toward terminal care were their concern about death(β=0.45, p<0.001), religion(β=-0.26, p=0.001) and the anxiety of others about dying(β=-0.23, p=0.003), which explained 32.0% of the observed variance concerning the factors affecting nurses' attitudes toward terminal care. Conclusion: Through this study, concern about death, anxiety about how other people process dying, and religion, were found to be associated factors. The more concerned the nurses were about death and the less their anxiety about how others process dying, the better the home health care nurses' attitude toward implementing terminal care.
