Multiple Schwannomas in the Peripheral Nerve - 3 Cases Report - (말초신경에 생긴 다발성 신경초종 - 증례 보고 3예 -)

  • This journal reports three cases diagnosed with schwannomatosis in which no clinical symptoms of type 2 neurofibromatosis. The chief complain was pain. In adolescence and adult group, all masses were found. The locations were brachial plexus, popliteal fossa and hand. No hearing impairment, vertigo, tinnitus and visual disturbance was observed in any of the case. Family history was negative. In all cases, there was no evidence of vestibular schwannoma on cranial MRI imaging study. In all cases, Tinel sign was positive. Pathologic diagnosis was positive for schwannoma. Further study and case collection is needed to idenity the clinical manifestation, clinical course and genetic characteristic of schwannomatosis.

A Case Report of Referral Pain on Mandibular Toothache Originated from Myofascial Pain (근막동통으로 인한 하악 구치부 연관통의 임상증례)

  • Majority of toothache is caused by abnormality of pulpal or periodontal tissues. However, there are numerous nonodontogenic sources that may be responsible for pain felt in the tooth. Nonodontogenic toothache may result from muscle, maxillary sinus, neuropathic, neurovascular, even cardiac and psychogenic problems. Myofascial pain is one of the most common abnormality characterized by palpation of a hard band which acts as the trigger point that causes stiffness and fatigue of the muscle, referral pain in tooth, tension-type headache, and hyperalgesia. Masseter muscle particularly induce referral pain in maxillary and mandibular molars. This case reported the treatment of the pain in right mandibular molar originated from myofascial pain of the right masseter. The pain is improved by general and reversible treatments such as muscle exercise, physical therapy, and medication. Nonodontogenic toothache should undergo proper differential diagnosis in order to avoid unnecessary dental treatments, such as endodontic, periodontic treatment, and tooth extraction, which would fail to alleviate the symptom of the patient.

  • 최근 국내의 각종 공사장부근에서 일어나는 가축에 대한 환경분쟁(소음, 진동, 먼지 등)피해를 호소하는 농가가 지속적으로 늘고 있으나, 피해 농가의 피해를 입증할 수 있는 객관적인 자료가 부족한데 따라 보고서를 작성하는 데 여간 어려웠던 적이 한, 두번이 아님에 많은 아쉬움을 가졌다. 각종 공사로 인해 가축 사육 장소인 목장(농장)내 도로개설 및 관통으로 인한 목장의 지속여부, 겨울철 수렵시기에 총성으로 인한 한우 성장지연, 육질 저하 등에 미치는 영향, 태양광 발전소가 축사 인근에 있어 사육 한우가 성장지연 등의 피해를 호소하는 농가가 있으나, 이에 대한 관련 연구자들의 많은 연구가 이뤄졌으면 한다. 그런 가운데서 우리 임상수의사들의 관련 자료나 정보를 입수하여 보다 적극적으로 대처하여 피해 축산농가들에 대한 보상에 도움을 줘야 하나, 피해 목장내 사육가축의 진단서나 소견서등 발부요청에 미온적이거나 회피하는 경우들을 적잖게 목도를 하고 실망감을 감출 수가 없었다. 진단서 등의 각 항목의 기재 내용의 누락, 여러 마리를 한 장(1두 1매 원칙)에 작성한다거나 유 사 조산시 정확한 월(일)령의 비기재, 환경분쟁과 관련된 피해사실의 누락이나 전혀 무관한 병명으로 발급하는 경우 및 수의사법 등에서 승인된 양식을 이탈하여 임의로 작성하는 등의 예를 간혹 보아왔는 터라 개선이 시급하다 하겠다. 임상 수의사들이 발급하는 진단서나 소견서 등은 어디까지 관련 전문가나 법원 등의 감정시 어디까지나 참고용으로 활용되고, 발급에 따른 제 비용은 인정될 경우에는 피해농가들에게 되돌려주며, 절대적으로 피해를 주는 일이 없다는 것을 강조해두고 싶다. 따라서 필자는 환경분쟁사건을 현지조사내지는 보고서를 작성하면서 아직도 우리 임상 수의사들은 새로운 분야에 대한 능동적인 대처가 더더욱 요구되는 시기에 수동적이거나 미온적으로 대하는 사례에 안타까움을 호소하며, 환경분쟁시 가축에 미치는 영향과 최근 새로이 제시된 배상액의 산정기준을 소개하여 도움을 주고 싶다.

Spinal Syringomyelia Secondary to an Intracranial Microcystic Meningioma in a Dog (개에서 척수공동증을 동반한 소낭성 뇌수막종 1례)

  • A 5-year-old, 2.7 kg female maltese dog was presented a local veterinary clinic with one week history of hindlimb lameness associated with patellar luxation. Reduction of bilateral medial patellar luxation was operated using trochlear resection and lateral reinforcement technique. Three weeks after the surgery, the dog showed bending spine with pain in thoracic and lumbar region, continuous ataxia and intermittent convulsion. Magnetic resonance imaging scanning revealed a hyperintense mass in right frontal lobe of brain and abnormal cavitation from cervical cord to third lumbar cord. Histopathologically, neoplasm in brain composed of meningothelial cells showed loosely reticular or lace-like morphology with numerous extracellular cystic spaces of variable size and shape. Neoplastic cells were positive for vimentin and negative for neuron specific enolase and glial fibrillary acidic protein. Irregular shaped enlarge central canal-like cavity was existed in cervical and lumbar cords. In our best knowledge, this report described the clinical findings, imaging and histopathologic characteristics of unusual intracranial microcystic meningioma with secondary syringomyelia in a dog.

Trilogy of Fallot in a Dog (개에서 발생한 팔로삼징)

  • A 3 years-old female mixed dog (weighing 5.3 kg) was referred to veterinary teaching hospital of Kangwon National University with primary complaints of syncope, severe exercise intolerance, depression and lethargy. Diagnostic studies revealed polycythemia, right sided cardiac enlargement on thoracic radiography and right-to left atrial septal defect, severe pulmonary stenosis (~5 m/s of peak velocity) and right ventricular hypertrophy. Based on diagnostic findings, the dog was diagnosed as trilogy of Fallot. To improve clinical condition of this dog, diltiazem and enalapril were prescribed with weekly phlebotomy. To author's best knowledge, this is the first case of trilogy of Fallot in Korea.

Renal Adenoma with Hydronephrosis in a Cat (고양이에서 수신증이 동반된 신선종)

  • A 6-year-old castrated male domestic short hair cat with the clinical signs of anorexia and vomiting was admitted to the local animal hospital. Abdominal radiography and ultrasonography revealed renomegaly and severe hydronephrosis in the right kidney. Surgically excised right kidney was submitted for diagnosis. On the cut surface, two milky white masses and severe dilation of renal pelvis were observed. Most of the neoplastic masses were composed of uniform well differentiated tubules lined by a single layer of cuboidal to columnar cells and projected papillae into the lumen. The neoplastic cells were strong positive for cytokeratin (CK) MNF116, but negative for CK 7. Based on the clinical and gross findings, histopathology and immunohistochemistry, this case was diagnosed as papillary renal adenoma with hydronephrosis.

Proventricular Dilation Disease Concurrent with Ingluvitis in an Indian Ring-necked Parakeet (Psittacula krameri manillensis) (인디안 링넥 앵무새에서 발생한 소낭염을 동반한 선위확장증)

  • A six-year-old, intact female Indian ring-necked parakeet (Psittacula krameri manillensis) was presented due to chronic weight loss and anorexia. The bird was tentatively diagnosed as proventricular dilation disease (PDD) by radiographic evidences of dilated crop and proventriculus. The patient was symptomatically treated, however the bird was die two months after the first presentation. At necropsy finding, distinctive signs of PDD were noted, and PDD concurrent with ingluvitis was finally diagnosed according to histopatholgic examination.

Extrahepatic Biliary Obstruction Secondary to Traumatic Diaphragmatic Hernia in Dog (개에서의 횡격막 탈장에 의한 간외담도폐색)

  • A 3-year-old intact mixed hunting dog was referred to Gyeongsang Animal Medical Center with the clinical signs of vomiting, dyspnea, anorexia. The dogs had traumatic history that wounded at abdominal wall during the wild boar hunting 2 week ago. The dogs was diagnosed as extrahepatic biliary tract obstruction (EHBO) secondary to traumatic hernia. Diagnosis was based on radiographic features of a diaphragmatic hernia on survey radiography, dilated bile duct in abdominal ultrasonography and complete recovery due to the return of displaced organs into the peritoneal cavity and repair of the diaphragmatic rent. Diagnostic imaging and surgical intervention may result in a favorable outcome in dog with post hepatic obstruction secondary to diaphragmatic hernia. This case emphasizes the strangulation of bile duct by diaphragmatic rupture could be the cause of EHBO.

  • Immunoglobulin G4 (IgG4)-related disease is a systemic fibro-inflammatory disease characterized by pathologic findings in various organs. Imaging is critical for the diagnosis and treatment assessment of patients with IgG4-related disease. In this pictorial essay, we review the key features of multiple imaging modalities, typical pathologic findings, and differential diagnosis of IgG4-related disease. This systematic pictorial review can further our understanding of the broad-spectrum manifestations of this disease.

  • Inflammatory pseudotumors (IPTs) are uncommon, mass-forming lesions, predominantly involving the lung and orbit. Although the incidence of IPTs is rare in the abdomen and pelvis, they can be encountered as enhancing, soft-tissue lesions, mimicking malignancy or fibrosclerosing disease. Generally, they exhibit a wide range of nonspecific imaging features in various organs. Preoperative imaging diagnosis of IPTs in appropriate clinical settings may help determine proper patient management. In this article, we review radiologic findings of IPTs in the abdominopelvic cavity, including the liver, spleen, kidney, gastrointestinal tract, mesentery, pelvis, and retroperitoneum.
