Clinical Research Trends of Gut Microbiome for Respiratory Diseases (호흡기질환에 대한 장내 미생물의 임상 연구 동향)

  • Objectives: This study aimed to review the clinical research of the gut microbiome for respiratory diseases to assist the design of trials for respiratory diseases by regulating the gut microbiome with herbal medicine later. Methods: We searched three international databases (PubMed, CENTRAL and EMBASE) to investigate randomized controlled trials (RCTs) of the gut microbiome for respiratory diseases. The selected trials were analyzed by study design, subject diseases, inclusion/exclusion criteria, sample size, study period, intervention group, control group, outcome measures, and study results. Results: A total of 25 studies were included and published from 1994 to 2021 mostly in Europe and Asia. Subject diseases were many in the order of respiratory tract infection, cystic fibrosis, allergy, and so on. As outcome measures, the gut microbiome in a fecal sample was analyzed by 16S rRNA sequencing analysis method, and symptom assessment tools related each disease were used. Major intervention drugs were probiotics and the results were mostly improved in the composition and diversity of the gut microbiome. Conclusion: Clinical studies of the gut microbiome for respiratory diseases have confirmed various effects and this review provides basic data for a well-designed clinical study for respiratory diseases by regulating the gut microbiome with herbal medicine.

Pyogenic Spondylitis Caused by Nontyphoidal Salmonella in an Immunocompetent Child (정상 면역 소아에서 발생한 비장티푸스 살모넬라 화농성 척추염)

  • Pyogenic spondylitis, an extremely rare complication of nontyphoidal Salmonella infection in immunocompetent children, occurs more commonly in patients with hemoglobinopathies such as sickle cell disease or in immunocompromised patients. In this study, we report a case of pyogenic spondylitis in a previously healthy 13-year-old immunocompetent adolescent, who presented with prolonged fever and right upper quadrant pain. Stool culture results comfirmed nontyphoidal Salmonella infection, and the patient was diagnosed with pyogenic spondylitis caused by nontyphoidal Salmonella based on additional imaging studies performed for evaluation of the patient's prolonged fever. This case highlights the importance of culture studies for evaluation of patients with fever of unknown origin and also the importance of detailed investigations for early detection of complications of extra-intestinal infections in patients with an unusual clinical course of salmonellosis.

Moisturizing and Dryness Reduction Effect of Face Cream Containing Persicaria Perfoliata (L.) Extract (며느리배꼽추출물을 함유하는 페이스 크림의 보습 및 건조함 감소 효과)

  • Purpose : This study was aimed to analyze the effects of cosmetics containing Persicaria perfoliata water extract (PPWE) on the skin moisturizing and improvement of skin condition in clinical trials. Methods : Clinical trial was conducted for five weeks after IRB approval at Dong-Eui University. Out of a total of 64 people, 15 people each were assigned to four groups as follows; control group A, B, C and the experimental group A that using cosmetic containing PPWE. Skin condition was measured two times, before and after clinical trial, by a professional skin analyzer, SDM (skin diagnosis system). Moisture and oil value of participants was analyzed twice, each morning and evening, using a portable device on their cheeks. In addition, the survey was investigated subjective satisfaction on change in skin condition and the satisfaction on the use of cosmetics. Result : The experimental group exhibited subjectively significant changes before and after clinical trials on skin its dryness (p=.039), blush (p=.017), and redness (p<.001). In addition, subjective evaluation was also the highest satisfaction in aspects of number of application (p=.003), amount of application (p=.002), moisture maintenance, and skin scratching frequency. The satisfaction on the use of cosmetics was the highest in the intention to repurchase (p=.045), recommendation willingness to others (p=.020), and intention to use various products (p=.001). Skin moisture of the clinical trial participants using the SDM, moisture level and elasticity of the experimental group increased by 12.94 and 10.28. Moisture level, which was measured by a portable device, was the most potently increased in the experimental group. Conclusion : Consequently, PPWE containg cosmetics exhibited the effects of moisturization and attenuated skin dryness in clinical trials, which might be utilized as a fundamental data to develop numerous lines of cosmetics.

The Degrees of Emotional Labor and the Its Related Factors among Clinical Nurses (간호사의 감정노동과 영향요인에 관한 연구)

  • Purpose: The objectives of this study were to investigate the degrees of emotional labor and the effects of Type A personality, emotional expressiveness, job stress, and social support on the emotional labor among clinical nurses. Methods: The subjects of this study were the clinical nurses working at the general hospital in Seoul. The subjects were selected by quota sampling in consideration of the length of employment and the working units. The self-reported questionnaires were administered to 286 clinical nurses and the data were analyzed with SPSS 12.0 software Results: The level of emotional labor of the subjects was revealed moderate to high. Significant explanatory variables of the degrees of emotional labor included psychological job load, the lack of job autonomy, and role conflicts. These three variables demonstrated the explanatory power of 11.4% of the emotional labor. Conclusion: The findings suggested that the intervention program intervening emotional labor should be developed in order to improve psychological job load, the job autonomy, and role conflicts among clinical nurses.

Clinical Study for Ten Cases, who Complains Abdominal Pain after Surgery -Laparoscopic Gynecological Surgery- (부인과 질환관련 복강경 수술 후 발생한 복통 환자 10예의 임상적 고찰)

  • Purpose: The purpose of this study was to report the effect of TKM (Traditional Korean Medicine) to abdominal pain after laparoscopic gynecological surgery(LGS). Methods: 10 patients who visited the department of gynecology in OO medical center from 1st August 2007 to 31st December 2008 with abdominal pains after laparoscopic operation. They complained abdominal pain and other pains such as back pain, shoulder pain and vaginal bleeding etc. We treated patients with herb medicine, acupuncture and moxibustion treatment. The progress of signs and symptoms was evaluated by checking the change of visual analog scale(VAS). Results: The mean age was 45.1 years(range 38-49), parity 2(0-3) and previous abdominal surgery case was 5. The mean of hospital admitting day was 20 days(range 9- 51) and taken for reducing VAS 10 to 3 were 10 days(range 4-24). After taking TKM, patient's signs and symptoms were alleviated or resolved and Hb, Hct were increased. Conclusion: After laparoscopic gynecological surgery, patients had taken pain such as abdominal pain, shoulder pain, back pain etc. TKM treatment is effective on the recovery after laparoscopic surgery.

Current Update of Cartilage Regeneration Using Stem Cells in Osteoarthritis (골관절염에서 줄기세포를 이용한 연골 재생의 최신 지견)

  • Osteoarthritis is a disease characterized by the progression of articular cartilage erosion, that increases pain during joint motion and reduces the ability to withstand mechanical stress, which in turn limits joint mobility and function. Damage to articular cartilage due to trauma or degenerative injury is considered a major cause of arthritis. Numerous studies and attempts have been made to regenerate articular cartilage. In the case of partial degenerative cartilage changes, microfracture and autologous chondrocyte implantation have been proposed as surgical treatment methods, but they have disadvantages such as insufficient mutual binding to the host cells, inaccurate cell delivery, and deterioration of healthy cartilage. Stem cell-based therapies have been developed to compensate for this. This review summarizes the drawbacks and consequences of various cartilage regeneration methods and describes the various attempts to treat cartilage damage. In addition, this review will discuss cartilage regeneration, particularly mesenchymal stem cell engineering-based therapies, and explore how to treat future cartilage regeneration using mesenchymal stem cells.

Nursing Students' Clinical Judgment and Performance in Simulation of Recognizing and Responding of the Deterioriating Patient ; a retrospective mixed-methods (악화환자 인지 및 대응을 위한 시뮬레이션교육에서 간호대학생의 임상판단력과 간호수행: 후향적 혼합연구)

  • Purpose : This retrospective mixed-methods study aimed to explore key considerations for designing effective simulated education in nursing, focusing specifically on the recognition and response to deteriorating patients. Methods : Quantitative and qualitative data were analyzed to assess the clinical judgment and performance of the nursing students. Descriptive statistics were used to analyze quantitative data related to prior knowledge, simulation satisfaction, clinical judgment, and nursing performance during deteriorating patient simulations. Qualitative content analysis was conducted for the reflective journal entries of the participants. Results : Quantitative analysis showed that most participants demonstrated a "being skillful" level of clinical judgment (33.1%) in effective response. At the beginner level, clinical judgment varied across effective noticing(39.7-82.8%), effective interpretating(77.6-82.8%), effective responding(3.4-86.2%), and effective reflecting(90.0-95.4%). Nursing performance in assessing patient respiration or SpO2 after request from a physician ranged from 46.6-48.3%. Qualitative analysis indicated that 48.5% of the participants anticipated a deteriorating condition and initiated appropriate actions, while 70% noticed patient unresponsiveness for the first time. Conclusion : To design an effective simulation program for identifying and addressing deteriorating patient care, a framework for observation and interpretation is essential, along with regular simulated training. It is important to design and assess simulation programs and to conduct thorough interviews with nursing students to gain insight into their clinical decision-making.

A Novel SLC25A15 Mmutation Causing Hyperornithinemia-Hyperammonemia-Homocitrullinuria Syndrome (Hyperornithinemia-hyperammonemia-homocitrullinuria 증후군을 유발하는 SLC25A15 유전자의 새로운 변이)

  • Hyperornithinemia-hyperammonemia-homocitrullinuria syndrome (HHH syndrome) is a neurometabolic disorder with highly variable clinical severity ranging from mild learning disability to severe encephalopathy. Diagnosis of HHH syndrome can easily be delayed or misdiagnosed due to insidious symptoms and incomplete biochemical findings, in that case, genetic testing should be considered to confirm the diagnosis. HHH syndrome is caused by biallelic mutations of SLC25A15, which is involved in the urea cycle and the ornithine transport into mitochondria. Here we report a boy with spastic paraplegia and asymptomatic younger sister who have compound heterozygous mutations of c.535C>T (p.R179*) and c.116C>A (p.T39K) in the SLC25A15 gene. We identified that p.T39K mutation is a novel pathogenic mutation causing HHH syndrome and that p.R179*, which is prevalent in Japanese and Middle Eastern heritage, is also found in the Korean population.

Cost-Effective Neuro-Modulation Device for Medical Imaging (의료영상 촬영을 위한 비용-효율적인 신경조절 장비)

  • In this study, we aimed to develop a cost-effective neuro-modulation device for use in neuroscientific and clinical medical imaging applications. To achieve this, we designed and fabricated a brain neuro-modulation device with a material cost of less than $500, and subsequently evaluated its performance. The measured performance was found to be comparable to existing medical devices (TENS), with the developed device being compact in size, measuring less than 3 cm by 3 cm. The outcomes of this study are expected to be applicable in accelerating research and development in related fields, as well as in the rapid commercialization of neuro-modulation technology. Furthermore, it is anticipated that this work will contribute to advancements in functional radiological medical imaging technology and enhance accessibility to brain science and brain stimulation technology.

Trends in Domestic and International Clinical Research on Korean Medicine Treatments for Triangular Fibrocartilage Complex Injury (삼각섬유연골 복합체 손상의 한의학 치료에 대한 국내외 임상 연구 동향)

  • Objectives The purpose of this study is to analyze the trends in domestic and international clinical research on the Korean medicine for triangular fibrocartilage complex (TFCC) injuries and to contribute to future domestic research. Methods In this study, clinical studies on TFCC injuries treated with Korean medicine were searched through databases such as Research Information Sharing Service, ScienceON, DBpia, China National Knowledge Infrastructure, PubMed, Medline, and Embase. Fifteen clinical studies were selected, of which publication years, intervention methods, assessment tools, and various other factors were analyzed, including potential side effects. Results Among the selected 15 studies, total of 13 intervention methods were used. Electroacupuncture was the most frequently used method (40%), followed by herbal medicine (33%), acupuncture (26.7%), chuna manual therapy (26.7%), pharmacopuncture (20%), acupotomy (13%), herbal ointment (13%), hot medicinal compress therapy (13%) and etc. Most studies showed that Korean medicine treatments effectively alleviated TFCC injuries, with visual analogue scale being the most commonly used assessment tool. Conclusions Korean medicine treatments for TFCC injuries appears to be effective; however, further research is needed to enhance understanding. Large-scale clinical trials comparing the efficacy of Korean medicine treatments, along with investigations into side effects and recurrence rates are necessary. By doing so, the safety and efficacy of Korean medicine treatments for TFCC injuries can be further established.
