Clinical Usefulness on K-MBI for Decision of Driving Rehabilitation Period in Patients with Stroke: A pilot study (뇌졸중 환자의 운전재활 시기 결정을 위한 K-MBI의 임상적 유용성: 예비 연구)

  • Background & Object: Basic daily activity screening tool such as the Modified Barthel Index (MBI) has been used commonly in rehabilitation clinic and community based rehabilitation setting. Previous studies have shown the significant relations between the level of daily activities and driving ability on stroke or elderly people. However, there is a lack of studies to investigate the usefulness of MBI on prediction of driving ability for stroke patient. This study was to predict driving abilities of stroke survivor using Korean version Modified Barthel Index (K-MBI). Methods: A sample of 48 patients with stroke in rehabilitation hospital was recruited. All participants were tested level of basic daily activities using K-MBI. The driving ability of participants was tested using virtual reality driving simulator. The predictive validity was calculated of the K-MBI among pass or fail group of driving simulator test using receiver operating characteristics curves. Results: The cut-off score of >86.5 on the K-MBI is proper sensitivity to predict on driving performance ability. Conclusion: This pilot result offers clinical reference to therapists and caregivers for reasoning on driving recommendation period during rehabilitation stage of stroke survivors. Further studies need to identify prediction using real on-road test in a large population group.

Statistical Analysis for Risk Factors and Prediction of Hypertension based on Health Behavior Information (건강행위정보기반 고혈압 위험인자 및 예측을 위한 통계분석)

  • The purpose of this study is to develop a prediction model of hypertension in middle-aged adults using Statistical analysis. Statistical analysis and prediction models were developed using the National Health and Nutrition Survey (2013-2016).Binary logistic regression analysis showed statistically significant risk factors for hypertension, and a predictive model was developed using logistic regression and the Naive Bayes algorithm using Wrapper approach technique. In the statistical analysis, WHtR(p<0.0001, OR = 2.0242) in men and AGE (p<0.0001, OR = 3.9185) in women were the most related factors to hypertension. In the performance evaluation of the prediction model, the logistic regression model showed the best predictive power in men (AUC = 0.782) and women (AUC = 0.858). Our findings provide important information for developing large-scale screening tools for hypertension and can be used as the basis for hypertension research.

Clinical usefulness of rapid antigen test to detect respiratory syncytial virus infection (Respiratory syncytial virus 감염진단을 위한 신속항원검사의 유용성)

  • Purpose : Respiratory syncytial virus (RSV) is the most frequent cause of lower respiratory infections in infants and young children. Early detection allows quarantining of infected inpatients to prevent nosocomial transmission and to choose a treatment. To achieve rapid reporting, to facilitate prompt antiviral therapy, and to avoid unnecessary use of antibiotics, an easy, rapid diagnostic method for RSV is needed. We evaluated a lateral flow immunochromatography (RSV Respi-Strip test) and EIA (Enzyme immuno assay) compared to RT-PCR. Methods : From April 2007 to March 2008, 112 consecutive respiratory specimens (nasopharyngeal aspirates, throat swabs, tracheal aspirates, sputum) from patients who were suffering from the clinical signs and symptoms of respiratory tract infection were enrolled in Busan. A total of 112 patients were tested with RSV Respi-Strip (Corio-BioConcept, Belgium), EIA, and RT-PCR at the same time. Results : Of the 112 specimens tested, the number of children who showed positive results at RT-PCR and Respi-Strip were 45 and 42, respectively. The Respi-Strip rapid antigen test had a sensitivity of 88% and a specificity of 94%. The positive and negative predictive values were 90% and 92%, respectively. The agreement was 83%. Conclusion : In our study, the rapid antigen test had as much sensitivity as any method for detection of RSV. The test has many advantages such as easy performance, simple interpretation, and rapid results. If the rapid antigen test is widely applied in the clinical setting, the may be useful for diagnostic and epidemiological studies of RSV infection.

  • Symbolic data analysis extends the data mining and exploratory data analysis to the knowledge mining, we can suggest the SDA tree model on clinical and genomic data with new knowledge mining SDA approach. Using SDA application for huge genomic SNP data, we can get the correlation the availability of understanding of hidden structure of HCC data could be proved. We can confirm validity of application of SDA to the tree structured progression model and to quantify the clinical lab data and SNP data for early diagnosis of HCC. Our proposed model constructs the representative model for HCC survival time and causal association with their SNP gene data. To fit the simple and easy interpretation tree structured survival model which could reduced from huge clinical and genomic data under the new statistical theory of knowledge mining with SDA.

Usefulness of Low Risk Criteria for Serious Bacterial Infection Among Febrile Infants Younger than Three Months of Age (생후 3개월 이하의 발열이 있는 환아에서 세균성 감염의 예측을 위한 저위험 예측기준의 유용성)

  • Purpose : A retrospective study was undertaken to evaluate the usefulness of low risk criteria for identifying febrile infants younger than three months unlikely to have serious bacterial infection. Methods : We conducted a retrospective study of 527 infants younger than three month with a axillary temperature ${\geq}37.4^{\circ}C$. If they met the following all four criteria, appear well, WBC $5,000-20,000/mm^3$, urine stick WBC(-) and nitrite(-), CSF WBC < $10/mm^3$, they were considered at low risk for serious bacterial infection(SBI). SBI was defined as a positive culture of urine, blood, or cerebrospinal fluid. The sensitivity, specificity, negative predictive value and positive predictive value of the low risk criteria were calculated. Results : Of 527 febrile infants, 110(21.0%) had serious bacterial infections. The 2.7% who met the low risk criteria had SBI and negative predictive value was 97.3%. SBI was diagnosed in 103 infants(38.6%) who didn't meet the low risk criteria including urinary tract infection(78.6%), most commonly, bacteremia(16.5%), bacterial meningitis(8.7%), Salmonella gastroenteritis(1%), osteomyelitis( 1%), septic arthritis of hip joint(1%). There were no differences in the sensitivity and negative predictive value according to the monthly-age-group. Conclusion : This low risk criteria to identify infants unlikely to have SBI early is available, however low risk infants must be carefully observed.

  • In this paper, we suggested the possibility of using an efficient classifier for the dependency analysis of retinal disease. First, we analyzed the classification performance and the prediction accuracy of GBN (General Bayesian Network), GBN with reduced features by Markov Blanket and TAN (Tree-Augmented Naive Bayesian Network) among the various bayesian networks. And then, for the first time, we applied TAN showing high performance to the dependency analysis of the clinical data of retinal disease. As a result of this analysis, it showed applicability in the diagnosis and the prediction of prognosis of retinal disease.

Assessing a Body Shape Index and Waist to Height Ratio as a Risk Predictor for Insulin Resistance and Metabolic Syndrome among Korean Adults (한국 성인의 인슐린저항성 및 대사증후군 위험 예측인자로서 체형지수와 허리둘레/신장 비율의 효용성)

  • The WHtR (waist to height ratio) and ABSI (a body shape index) are indicators that reflect abdominal obesity. This study examined the insulin resistance and metabolic syndrome prediction ability of ABSI and WHtR. In this study, 4,395 people aged 20 years or older, who underwent physical examinations at a General Hospital in Gyeonggi-do from January 2017 to September 2017 were assessed on a cross section survey. Metabolic syndrome was defined according to the criteria of the AHA/NHLBI. Insulin resistance was judged to be insulin resistance when the HOMA-IR value was 3.0 or more. Both men and women showed a stronger correlation between WHtR and the metabolic risk factors than ABSI. The AUC value of WHtR and ABSI was 0.849 and 0.676, respectively (p<0.001). The AUC value of WHtR and ABSI for predicting insulin resistance was 0.818 and 0.641, respectively (p<0.001). In conclusion, the ABSI has low predictive power of insulin resistance and metabolic syndrome whereas the WHtR has good predictive power for metabolic syndrome and insulin resistance.

  • The purpose of this study is to make and validate landslide susceptibility map using artificial neural network and GIS in Kangneung area. For this, topography, soil, forest, geology and land cover data sets were constructed as a spatial database in GIS. From the database, slope, aspect, curvature, water system, topographic type, soil texture, soil material, soil drainage, soil effective thickness, wood type, wood age, wood diameter, forest density, lithology, land cover, and lineament were used as the landslide occurrence factors. The weight of the each factor was calculated, and applied to make landslide susceptibility maps using artificial neural network. Then the maps were validated using rate curve method which can predict qualitatively the landslide occurrence. The landslide susceptibility map can be used to reduce associated hazards, and to plan land use and construction as basic data.

Study on the effects of Menstrual Attitudes, Life Stress and Clinical Practice Stress on premenstrual syndrome in Nursing Students (간호대학생의 월경태도, 일상생활 스트레스, 임상실습 스트레스가 월경전증후군에 미치는 영향)

  • This study identified the effect of menstrual attitude, life stress and clinical practice stress on premenstrual syndrome of nursing students. Data were analyzed with the SPSS 22.0 program using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and simple linear regression. Premenstrual syndrome was significantly negative correlated with menstrual attitude (r=.28, p=.004), positive correlated with life stress (r=.31, p=.001) and clinical practice stress (r=.29, p=.003). The factors that influence premenstrual syndrome were regular exercise (β=.-.18, p=.006) and dysmenorrhea (β=.51, p=.000), which explained 38.0%.

Diagnostic Performance for Detection of Hezicobacter Pyzori Infection in Gastric Biopsy Specimens with No Gold Test: Non-linear Regression Approach (위 조직 생검 시료의 Helicobacter pylori 균 검출에 사용되는 진단검사의 특성을 추정하기 위한 비선형 모형의 응용)

  • The selection of a test as a reference with no perfect sensitivity and specificity may lead to bias, yielding distortion of the diagnostic performance. This means it is inappropriate to use imperfect diagnostic tests as a reference method to identify infected patients in clinical environments. In this study, diagnostic performance of rapid urease test, polymerase chain reaction (PCR), and histology of gastric biopsy specimens for diagnosing Helicobacter pylori infection separately and in combination was estimated by using non-linear regression. Based on this approach, the sensitivity, specificity and likelihood ration positive and negative values for each test were as follows: urease test 99.9%, 99.9%, 99.9%, 99.6%, respectively; PCR 88.6%, 99.9%, 99.9%, 70.5%, respectively; histology 78.3%, 97%, 78.3%, 97%, respectively. Predictive values for positive and negative changes with varying Combination of three diagnostic tests employed in the study gives no substantial benefit for practitioners to screen infected patients, and urease test or PCR represents an appropriate single test in clinical environments.
