신뢰할만한 임상시험연구결과를 얻기 위해서는 통계학자가 임상시험연구의 준비단계에서부터 실험계획 및 연구의 운용, 분석은 물론 결과의 보고에 이르기까지 총체적으로 관여해야 한다. 의학전문가들과의 원활한 공동연구를 위해서는 통계학자들이 의학분야에서의 용어나 관례, 임상시험이 이루어지는 절차등에 대한 명확한 이해가 필요하게 된다. 이 논문에서는 임상시험에서 진행되는 제반문제에 대해 통계학자가 숙지해야 할 사항들에 대해 논의 하였다.

식품의약품안전청은 산발적으로 흩어져 있던 임상시험 관련 정보를 통합하여 서비스하기 위한 방안으로 임상시험정보방을 개설하게 되었다. 임상시험승인현황 등 원시자료는 종이형태로 생산, 관리된다. 이를 통계자료로 활용하기 위해서 식품의약품안전청의 관련 시스템인 KiFDA 시스템에 입력을 하게 된다. 이 시스템에 입력된 데이터는 실시간으로 임상시험관련 웹서비스 시스템에 구축된다. 이때에 실시간 통계정보의 정확성이 무엇보다 중요하다. KiFDA 시스템의 데이터베이스의 실시기관명과 임상시험계획승인제도(IND) 승인리스트 원본의 실시기관명이 틀린 것도 있었다. 이는 동일한 실시기관도 화면에서는 서로 다른 실시기관으로 보일 수 있다는 것을 의미한다. 결국 통계자료가 부정확하게 표출되는 데는 이런 원인들이 있었던 것이다. 본 논문에서는 기존의 문제점을 개선하기위해 임상시험정보방의 효율적인 통계모델링을 설계하여 물리적 데이터베이스를 구축하였다.

The purpose of clinical trials is to find evidences for the effects of experimental new drugs or treatments on human. For the successful clinical trials, it is not sufficient to use statistics only for the analyses of collected data, but it is necessary to extend the usage of statistics in various ways. At the beginning of the study, one needs to use statistics for systematically and concretly planning the study. For this, we discussed the usage of statistics in defining the effect, determining the sample size, statistical analyses, and so on.

There are many clinical trials where a large portion of the patients are cured of disease. In such a case, one might be more interested in testing the differences in cure rates rather than other types of differences in failure distribution. For ethical and economic reasons, clinical trials must be repeatedly monitored for evidence of treatment benefit or harm. In this article, we examined by simulation the properties of nonparametric group sequential methods for comparing the cure rates between two treatment groups during the trial in a wide range of alternatives, censoring rates and cure rates.

환자에 대한 기록은 일정한 형식을 가지고 환자 내원 초기부터 환자 및 의료진의 요구와 관리목적에 부합한 효과적인 평가, 치료계획, 치료적 중재를 수립할 수 있게 만드는 필수적인 일련의 상호관련학적 의사소통 도구임과 동시에 치료사가 환자관리에 포함된 사고 과정을 분석하는 데 도움을 주며 문제해결에 대한 사고를 체계화하는데 있어 중요한 역할을 한다. 본 연구의 목적은 물리치료사의 일반적 특성과 임상기록의 실태, 임상기록의 중요성을 인식하고 임상활동의 문제점을 개선하며 기록의 체계화를 이루어 임상기록의 활성화와 나아가 물리치료의 질적 향상과 물리치료 서비스에 대한 효율성을 높이는데 있다. 이 연구는 전국 의료기관이나 보건관련기관에서 근무하는 물리치료사를 선정하였다. 2005년 7월1일부터 8월30일 사이에 구조화된 설문지 320매를 직접 전달해 치료사가 작성한 설문지 314매를 직접 수거하였다. 분석 방법으로는 백분율과 빈도, Kuskal-Wallis Test. Pearson Chi-Square, Mann-Whitney U로 검정하였다. 잘못 표기한 응답자 1명을 제외한 응답자 총 313명 중 남자 142명 (45.2%), 여자 171명 (54.5%) 이었으며, 25세 이상 29세 이하 연령층이 119명 (37.9%)으로 가장 많이 분포하였다. 근무처에 따른 근무 개월, 일일 평균 환자수는 통계학적 검정으로 유의한 차이가 있는 것으로 나타났다(p<.05). 근무처에 따른 임상기록 작성에서도 통계학적 검정이 유의하였다(p<.05) 임상기록 작성이 효율적인 치료. 치료사 만족도에도 통계학적으로 유의한 결과가 나타났다(p<.05).

Clinical vaccines studies (that include cancer prevention vaccines and therapeutic vaccines) are ongoing to improve the quality of life and lengthen the human lifespan. Recently clinical trials and research on vaccines have become more active due to the prevalence of new viruses such as the A(H1N1) virus that freighted the whole word in 2009. In this paper we will describe the statistical aspects of clinical vaccine trials and outline the current situation of domestic and international vaccine development.

정규분포에 관한 검정에 있어서 P-P 플롯과 Q-Q 플롯의 가시적인 변동을 이용한 통계량을 제시하고 이 통계량들과 Shapiro-Wilk의 W 통계량과의 비교를 정확도(accuracy)의 측면을 고려하여 실시하였다. 또한, 의학이나 임상에서 척도의 우수성을 검정하기 위해 많이 사용하는 Receiver Operating Characteristic (ROC) 분석 기법을 이용하여 제시된 통계량들에 관한 Power와 Accuracy는 물론 Best Cut-Off 측면에서의 효율성을 검정하였다.
