Selection of epitope for development of H5N1 specific diagnostic kit based on bioinformatics (생명정보학 기반 H5N1에 특이적인 진단키트 개발을 위한 epitope 선별)

  • 인플루엔자 A 바이러스의 아형인 H5N1은 고병원성으로 조류 독감을 일으킨다. H5N1 바이러스는 원래 조류끼리만 감염되는 독감이고, 사람에게는 전염되지 않는다고 알려져 있었으나, 2003년에 베트남과 중국을 시작으로 현재까지 168명의 사망자가 기록되고 있다. 그러나 현재 시판되고 있는 진단키트(Rapid diagnostic kits)들은 H5N1 에 특이적인 것이 아니라 influenza A virus 모두를 진단한다. 따라서 influenza 감염여부는 확인 할 수 있지만, 이것이 H5N1 인지는 확인 할 수가 없다. H5N1은 전염성이 강하기 때문에 빠르게 진단하여 감염조류를 살 처분 하여야 더 많은 경제적 손실을 줄일 수 있다. 따라서 H5N1 에만 특이적인 epitope를 네트워크 기반으로 예측하여 진단제에 응용할 수 있도록 하고자 한다. 각 서열 정보는 Openflu (http://openflu. vital-it.ch/browse.php)에서 얻었다. H5N1은 H1N1에서 유래되었기 때문에 두 subtype의 차이점을 알아보고자 TCOFFEE에서 multiple sequence alignment를 수행한 결과 N-terminal 부분이 상이하였다. 상이한 H5N1의 N-terminal 부분이 H5N1 virus에 감염된 모든 host에서 존재하는지 알아보기 위해 host가 사람인 경우와 조류인 경우를 TCOFFEE에서 alignment 하였다. 그 결과 H5N1의 N-terminal 부분은 사람과 조류에서 보존적이었다. 따라서 H5N1의 N-terminal이 다른 subtype과 유사하지 않고 H5에만 특이적이기 때문에 진단키트 제작을 위한 epitope로 사용할 수 있을 것으로 기대된다.

Clinical Usefulness of Rapid Antigen Test on the Diagnosis of Influenza (인플루엔자 진단을 위한 신속 항원검사의 유용성)

  • Purpose : Influenza is a respiratory disease which prevails widely every year and shows high morbidity and mortality among not only chronic invalids and the old, but also among infants and young children. To prevent community-acquired influenza infection, to facilitate prompt antiviral therapy and to avoid unnecessary use of antibiotics, an easy, rapid diagnostic method for the influenza virus is needed. We evaluated a lateral-flow immunoassay(QuickVue Influenza Test), compared to viral culture. Methods : During two consecutive years from Jan. 2004 to June 2004 and from Feb. 2005 to Jan. 2005, 408 patients who were suffering from fever, cough and/or sore throat and myalgia were enrolled in our study. A total of 408 patients were tested with $QuickVue^{(R)}$(Quidel Co., San Diego, USA) influenza rapid antigen test and virus cultures at the same time. Results : Of the 408 patients tested, children who showed positive results at the virus culture numbered 77; among them, 55(71.4 percent) were type A/H3N2 and 22(28.5 percent) were type B. QuickVue influenza test had a sensitivity of 71.4 percent and a specificity of 95.8 percent. The positive and negative predictive values were 79.7 percent and 93.5 percent, respectively. Conclusion : In our study, this test had comparable high sensitivity and high specificity and many advantages, such as being easy to perform and simple to interpret, and showing rapid results. If rapid influenza antigen tests are widely applied in the clinic, we can begin treatment more rapidly and reduce influenza complications and the abuse of antibiotics.

Clinical Characteristics and Comparison of the Various Methods Used for the Diagnosis of the New Influenza A Pandemic in Korea (한국에서의 2009 신종 인플루엔자 A의 임상양상과 다양한 진단 방법들의 비교)

  • Background: Laboratory diagnosis of new influenza A (H1N1) is crucial for managing patients and establishing control and prevention measures. We compared the diagnostic accuracies of the real time RT-PCR (rRT-PCR) test recommended for the confirmation of the new flu and the viral culture method used conventionally for viral disease with that of the rapid antigen test (RAT). Methods: We performed RAT, R-mix culture, and real-time PCR by using 861 respiratory samples collected from December 2009 to January 2010 and evaluated the abilities of these methods to detect new influenza A. The relationship among the positive rates of RAT, grades of culture, and the cycle threshold (Ct) values of rRT-PCR was also evaluated. Results: Of the 861 patients, 308 (35.8%) were diagnosed with new influenza A. The sensitivities, specificities, positive predictive values, and negative predictive values of the tests were respectively as follows: 59.7%, 99.5%, 98.4%, and 81.6% for RAT; 93.2%, 100%, 100%, and 96.3% for R-mix culture; and 95.8%, 100%, 100%, and 97.7% for rRT-PCR. Samples with weak positive grade in culture and those with Ct values of 30-37 in rRT-PCR showed positivities as low as 25.3% and 2.3% in RAT, respectively. The hospitalization rate and death rate of the confirmed patients were 3.2% and 0.3%, respectively, and gastrointestinal symptoms were observed in 7.2% of the patients. Conclusions: R-mix culture and rRT-PCR tests showed excellent reliability in the diagnosis of new influenza A and could be very useful, especially for samples with low viral load.

Anti-influenza Virus Activity of Water Soluble Substance from Elfvingia applanata Alone and in Combinations with Interferons (잔나비걸상버섯 수용성물질의 항인플루엔자바이러스 작용과 인터페론과의 병용효과)

  • EA, the water soluble substance, was prepared from the carpophores of Elfvingia applanata (Pers). Karst. Anti-influenza A virus (anti-Flu A) activity of EA was examined of Vero cells by plaque reduction assay in vitro. And the combined antiviral effects fo EA with interferon (IFN) alpha and gamma were examined on the multiplication of Flu A with 50% effective concentration ($EC_50$) of 1.50 mg/ml. The results of combination assay were evaluated by the combination index (CI) that was analysed by the multiple drug effect analysis. The combination of EA with IFN alpha on Flu A showed more potent synergism with CI values of 0.50~0.52 of 50%, 70%, 90% effective levels than that with IFN gamma with CI values of 0.82~0.99.

Isolation and Identification of Influenza Virus from Pusan, 1998-1999 (1998-1999년 절기에 부산지역에서 유행한 인플루엔자 바이러스의 분리)

  • Investigate the epidemics for influenza outbreaks. The outbreak pattern of the internal patients housed in the 10 designated hospitals was monitered to investigate and the characteristics of the virus isolates are as follows. 232 strains of influenza virus was isolated from the oral specimen of 1,320 respiratory disease patients in Pusan from Oct. 1998 to Jun. 1999. Among these isolates, 222 strains were A-type and the rest were B-type. The outbreak pattern for sex-and age-groups is as follows. The male outbreak was similar to the female outbreak: male outbreak, 47.4% and female outbreak, 52.5%. Most of the patients were less than 10 years old. The monthly influenza outbreak was consistent from Dec. 1998 to Apr. 1999. and The 113 strains from the A-type isolates were A/ Sydney/05/97(H3N2)-like, the 109 strains were A/Beijing/262/95(H1N1)-like, and all of the 10 B-type isolates were B/Harbin/07/94-like.

  • The outbreak patterns of the internal and external and external patients in the 20 designated hospitals and in 16 health centers were monitored to investigate and the characteristics of the virus isolates were as follows. Two hundreds and thirteen strains of influenza virus were isolated from the oral specimens of 1,686 patients with respiratory disease in Pusan. 1999. Among these isolates, 203 strains were A-type and the rest were B-type. The outbreak patterns for sex and age group were as follows. Preparation of an Inactivated Influenza Vaccine Using the Ethanol Extracts of Medical Herbs (한약재 식물 에탄올추출물을 이용한 인플루엔자 불활화백신 제작)

  • As seen in the COVID-19 pandemic, unexpected emergence of new viruses presents serious concern on public health. Especially, the absence of effective vaccines or antiviral drugs against emerging viruses significantly increases the severity of disease and duration of viral circulation among population. Natural products have served as a major source for safe and effective antiviral drugs. In this study, we examined the virucidal activity of medical herb extracts with a view to discover novel antiviral agents with desired levels of safety and antiviral efficacy. Ethanol extracts of ten selected medical herbs were tested for antioxidant activity and in-vitro cytotoxicity in various animal cell lines. Of note, the herbal extracts showed broad and potent virucidal activities against rotavirus, hepatitis A virus, and influenza A virus. The extracts of Sorbus commixta and Glycyrrhiza uralensis showed strong virucidal activities against influenza A virus. We also examined whether the extracts of Sorbus commixta and Glycyrrhiza uralensis can be used as inactivating agents to prepare an inactivated viral vaccine. In a mouse model, influenza A virus inactivated by the extracts elicited high levels of neutralizing antibodies, and the vaccination provided complete protection against lethal challenge. These results suggest that herb-derived natural products can be developed to antiviral drugs as well as inactivating agents for preparation of inactivated viral vaccines.

Pathogenesis of Hong Kong H5N1 Influenza Virus in Immunodeficient Mice (Hong Kong H5N1 인플루엔자 바이러스의 면역부전 마우스에 대한 병리발생)

  • Virulent and avirulent H5N1 viruses were inoculated intranasally to BALB/c and immunodeficient mice, and compared the pathogenesis by histology and immunohistochemistry. All of mice infected with virulent virus died by systemic infection at 6 to 7 days postinfection (PI). BALB/c mice infected with avirulent virus survived from the infection, whereas immunodeficient mice showed nervous symptoms in addition to respiratory disease and died at 13 days PI. Viral positive antigens was detected from multiple organs including central nervous system in immunodeficient mice infected with avirulent virus. These results suggest that avirulent H5N1 influenza virus can aquire the multiple tissue tropism under immunosuppresed condition and host immune system is a important factor to protect the development of disease.

Laboratory Surveillance of Influenza B Epidemic in Seoul During the Winter of 1985-1986 (서울에서의 인플루엔자 B바이러스 유행감시, 1985-1986)

  • During 31 days (5 week) from December 9th 1985 to January 8th 1986, Influenza B epidemic were observed in Seoul city. Epidemic peak was shown at 52nd week in 1985. The Epidemic associated viruses were determined as B/yamanish/510/84 and B/Ann Arbor/1/86-like strain. From total of 124 specimens of influenza-like illness children virus isolation rate were average 4.0%, much lower than that of influenza A virus isolation experience. In the epidemic influenza B infection were predominantly involved in school age children.

  • Clinical vaccines studies (that include cancer prevention vaccines and therapeutic vaccines) are ongoing to improve the quality of life and lengthen the human lifespan. Recently clinical trials and research on vaccines have become more active due to the prevalence of new viruses such as the A(H1N1) virus that freighted the whole word in 2009. In this paper we will describe the statistical aspects of clinical vaccine trials and outline the current situation of domestic and international vaccine development.
