한국 의과대학·의학전문대학원의 전문직 간 교육 현황과 요구 분석 (The Current Status and Needs Analysis of Interprofessional Education in Korean Medical Colleges)

  • The purpose of this study was to investigate the current status of interprofessional education (IPE) and the efforts required to promote, popularize, and implement it in Korea. The IPE status of 40 medical colleges was investigated using a survey with questions regarding the details of IPE, the future plans and necessary support required, and the reasons for not implementing IPE. Thirty-two medical colleges responded, of which 10 are implementing or have implemented IPE. Most of these colleges started IPE in 2018, and the duration of IPE was less than 9 hours. All medical colleges held classes with nursing students. As for the type of IPE, there were independent courses for IPE, one-time special lectures, or partial sessions in one course. Lectures, discussions and presentations, role playing, and high-fidelity simulations were mainly used as educational methods. The support and interest of the dean was the most important facilitating factor. No medical colleges were currently preparing to implement IPE, four colleges had planned IPE but failed to implement it, and 16 had no plans for IPE at all. All medical colleges cited scheduling or cooperation with other majors as the most significant barrier. All the colleges listed their requirements for educational materials, cases, guidelines, and teaching and learning methods for IPE from external institutions. To activate IPE, it is necessary to create an appropriate atmosphere and conditions for developing IPE competencies and a model suitable for the domestic situation. External medical education support organizations should distribute IPE development guidelines and educational materials, form a network between medical colleges with IPE experience, and make efforts to promote the importance of IPE.

인제대학교 의과대학 의예과 교육과정 개발과 편성 사례 (Development of Premedical Curriculum in Inje University's College of Medicine)

  • One important purpose of premedical education is to help students attain core competencies during the two-year premedical program. Since premedical curriculum is the foundation of medical education, it is necessary to develop the premedical curriculum in accordance with the overarching curriculum of a medical school. Inje University's College of Medicine (IUCM) has attempted to integrate competency-based medical education into the premedical education curriculum. The revised premedical curriculum aims to develop students' intrinsic competencies during premedical years. In an effort to do so, IUCM defined competencies for premedical education, analyzed and designed premedical courses according to the defined competencies, and suggested learner-centered instructional methods. As a result of implementing the revised premedical curriculum, students have reported both positive experiences and difficulties. Based on the results, it will be necessary to continuously improve the premedical curriculum as well as develop appropriate methods for evaluating student achievement of the defined competencies.

외측 발목 불안정성을 치료하지 않으면 어떻게 되는가 (What Happens without Treating Chronic Lateral Ankle Instability)

  • The term chronic lateral ankle instability (CLAI) embraces a spectrum of conditions that involve functional and mechanical instability, both in athletes and lower-demand patients. The hallmarks of CLAI are recurrent sprains, persistent pain, and repeated episodes of the ankle giving way. CLAI is the main complication of acute ankle sprains, which can cause discomfort in daily and sports activity. Approximately 10% to 30% of patients with acute ligament ruptures will develop chronic instability over the course of a year from the index injury. An accurate diagnostic approach and successful treatment plan should be established based on a comprehensive understanding of the concept of functional and mechanical instability. Unfortunately, the optimal modality for the management of CLAI is unclear. Even after conservative treatment or surgical intervention, it could result in degenerative changes to the ankle joint in the long term. Thus far, the incidence of ligamentous posttraumatic ankle osteoarthritis was reported to be 13% to 78%. The mean latency time between injury and osteoarthritis was at least 10 to 34.3 years. CLAI is an important pathological condition that can cause discomfort or dysfunction in daily activity in the short term, resulting in joint destruction in the long term. Therefore, it is important to understand the various complications that can occur when CLAI is not treated properly.

기본의학교육에서 동료지원학습의 활용과 효과 (Utilization and Effects of Peer-Assisted Learning in Basic Medical Education)

  • This review of the literature explored the experiences and effects of peer-assisted learning in basic medical education. Peer-assisted learning is most commonly utilized to teach clinical skills (including technical skills) and medical knowledge (76.4%). It has also been used, albeit less frequently, to facilitate small-group discussions including problem-based learning, to promote students' personal and professional development, to provide mentoring for career development and adaptation to school, to give tutoring to at-risk students, and to implement work-based learning in clinical settings. Near-peer learning is a common type. The use of active learning techniques and digital technology has been increasingly reported. Students' leadership had frequently been described. Student tutor training, programs for teaching skills, institutional support, and assessments have been conducted for effective peer-assisted learning. There is considerable positive evidence that peer-assisted learning is effective in teaching simple clinical skills and medical knowledge for tutees. However, its effects on complex skills and knowledge, small-group discussions, personal and professional development, peer mentoring, and work-based learning have rarely been studied. Additionally, little evidence exists regarding whether peer-assisted learning is effective for student tutors. Further research is needed to develop peer-assisted learning programs and to investigate their learning effects on student tutors, small-group discussion facilitation, personal and professional development, peer mentoring, and peer-led work-based learning in the clinical setting in South Korea. Formal programs and system advancement for a student-led learning culture is needed for effective peer-assisted learning.

췌장 양성 종양의 내시경적 치료 (Endoscopic Therapy for Pancreatic Benign Neoplasms)

  • 내시경초음파가 1980년대에 개발된 이후, 내시경초음파는 췌장 종양 관리에 있어서 진단적인 역할뿐만 아니라 치료적인 역할로 그 기능이 확대되고 있다. 췌장 양성 종양에 대한 전통적인 치료방법은 수술적 절제를 시행하거나 주기적인 경과관찰을 하는 것이었다. 그러나 내시경초음파는 그 대안으로 췌장 양성 종양을 가진 환자에게 최소 침습적인 치료법을 제공한다. 본고에서는 췌장 양성 종양치료에 있어서 내시경초음파의 역할에 대해 알아보고자 한다.

의료시스템과학의 개념과 교육 필요성 고찰 (The Concept of Health Systems Science and Educational Needs in the Korean Context)

  • Physicians should be able to address health-related issues of patients and populations from a multidimensional perspective. Therefore, medical schools have a social responsibility to develop and implement curricula that enable trainees to acquire the competencies needed to improve all aspects of patient care and healthcare delivery. This study explored the concept of health systems science concept as the third pillar of medical education (the other two are basic science and clinical medicine) in the Korean context, as well as related educational needs. The theoretical foundation of health systems science is the biopsychosocial conceptual model, which emphasizes the biological, psychological, and social factors surrounding patients. We concluded that the three domains (core functional, foundational, linking) and 12 subcategories of health systems science proposed by the Association of American Medical Colleges could be applied to Korean medical education. Health systems science education must be emphasized to solve the various healthcare problems facing Korea today and to train physicians to provide medical services in line with society's needs. Introducing a health systems science curriculum will be challenging in the Korean medical environment, which has traditionally emphasized basic science and clinical medical education. Health systems science education should begin in the basic medical education phase, where physicians' professional identity is formed, and continue through graduate medical education. It is essential to understand related educational needs, develop curricular content, conduct faculty development programs, and provide financial resources for the development of an integrated curriculum.

유기인제 중독에 의한 호흡부전 (Respiratory Failure of Acute Organophosphate Insecticide Intoxication)

  • 연구배경: 유기인제는 국내에서 1950년대부터 농업용 살충제로 널리 사용되고 있으며, 최근 공업용 화공약품으로 사용이 증가하면서 이로 인한 중독 환자가 증가하고 있다. 아트로핀, PAM 또한 호흡부전 발생시 인공호흡기를 이용한 적극적인 치료로 사망률은 과거보다 감소하였으나, 호흡부전으로 인한 사망은 유기인제 중독의 주요한 사망 원인이다. 저자들은 유기인제 중독시 호흡부전의 발생 빈도 및 그 유발 인자를 관찰 분석하여 유기인제 중독으로 인한 사망률을 줄여 보고자 하였다. 방 법: 영남대학교 의과대학 부속병원에서 유기인제 중독증으로 진단 받은 111례를 대상으로 하였으며, 특히 호흡부전이 발생한 31례의 임상 소견 및 치료 방법, 혈중 cholinesterase 활성도 등을 조사하여 호흡부전 발생과 관계가 있는 요인들을 알아보았다. 결 과: 유기인제 중독환자 111례 중 자살 목적인 경우가 81례로 가장 많았고, 중독 정도도 가장 심하였다. 환자 중 15례에서 사망하였으며, 사망률은 14%였다. 호흡부전이 발생한 31례 중 23예가 중독 후 24시간이내, 8례는 25시간에서 96시간이내 발생하였으며, 중독 정도가 심할수록 호흡부전 발생 빈도가 높았다. 호흡부전이 발생한 31례 중 15례에서 사망하였고, 호흡부전이 발생하지 않은 경우에서는 사망한 예가 없었다. 16예의 환자에서 폐렴이 발생하였으며 이중 14례에서 호흡부전이 생겨, 폐렴이 생긴 대부분의 환자에서 호흡부전이 발생하였다. 심혈관 허탈은 5례에서 발생하였으며 이중 2례에서 호흡부전이 생겼으나 통계적으로 유의하지 않았다. 첫 24시간동안 아트로핀의 평균 요구량은 $56{\pm}65mg/dl$이고, 급성 호흡부전인 경우 아급성 호흡부전이나 호흡부전이 발생하지 않았을 때보다 아트로핀 요구량이 더 많아 중독 정도가 심할수록 아트로핀 요구량이 더 많았다. PAM으로 치료하였던 95례 중 26예와, 치료하지 않았던 16예 중 5례에서 호흡부전이 발생하여 PAM 치료 여부와 호흡부전 발생간의 유의한 차이를 보이지 않았다. 혈중 cholinesterase 활성도는 호흡부전이 발생하였을 때보다 인공호흡기 이탈시 7배정도 높았으며, 인공호흡기로 치료한 기간은 급성 호흡부전이 생겼을 때 유의하게 길었다. 결 론: 급성 유기인제 중독에 의한 호흡부전은 중독 후 첫 96시간 이내 주로 발생하며, 중독 정도와 폐렴 발생이 호흡부전 발생의 중요한 유발 인자로 생각된다. 따라서 중독 후 첫 96시간 동안 세심한 관찰이 필요하고 적극적인 아트로핀 투여와 기도 확보 및 흡인성 폐렴 방지로 호흡부전 발생 빈도를 줄일 수 있을 것으로 생각된다.

위암 및 위선종에서 내시경절제술 조직의 병리 판독 (Pathological Interpretation of Gastric Tumors in Endoscopic Submucosal Dissection)

  • Endoscopic submucosal dissection (ESD) is a minimally invasive and effective treatment for early gastric cancer. The criteria for determining the suitability and success of ESD have expanded, with changes in the histopathological classification of gastric carcinoma, including results from molecular research. Treatment methods have also diversified to include targeted therapies and immuno-oncology agents. To improve communication between clinicians and pathologists, it is crucial to understand the standardized diagnostic forms of gastric cancer. This study aims to examine the handling method of ESD specimens and describe the pathological findings of gastric tumors.

위 신경내분비종양의 진단과 치료 (Diagnosis and Treatment of Gastric Neuroendocrine Tumors)

  • The incidence of gastric neuroendocrine tumors (NET) has been increased with the improvement of endoscopy accessibility. The World Health Organization classified NET of low (G1), intermediate (G2), high (G3) grade and neuroendocrine carcinoma with poor differentiation by mitotic count and Ki-67 labeling index. Gastric NET are divided into three subtypes based on the pathophysiology, and treatment is determined according to the subtype and prognostic factors of tumor. For diagnosis, endoscopy with biopsy, endoscopic ultrasonography, abdominal pelvis computed tomography, and serum gastrin level measure are required. In general, type 3, size > 2 cm, deep submucosal infiltration, high histological grade, lymphovascular invasion and metastasis are poor prognostic factors. Type 1 or 2 without these factors are treated by endoscopic resection, and other tumors needs surgery. Endoscopic resection of early type 3 or type 1 and 2 tumors with poor prognostic factors still remains a challenge.

알츠하이머병과 경도인지장애에서 체질량지수와 인지기능과의 연관성 (Association of Body Mass Index and Cognitive Function in Alzheimer's Disease and Mild Cognitive Impairment)

  • 연구목적 본 연구는 알츠하이머병(Alzheimer's disease, AD), 경도인지장애(Mild Cognitive Impairment, MCI), 정상군에서 체질량지수(Body Mass Index, BMI)의 차이를 분석하여 낮은 BMI 수치와 인지기능 저하와의 연관을 확인하고자 하였다. 또한, 한국판 간이정신상태검사(Mini Mental State Examination-Korean version, MMSEK), 치매임상평가척도(Clinical Dementia Rating, CDR), 전반적 퇴화척도(Global Deterioration Scale, GDS)과 BMI 사이에 연관성이 있는지도 알아보고자 하였다. 방 법 총 257명의 연구대상자들이 이 연구에 포함되었고, AD 및 MCI 진단을 위해 병력 청취, 정신상태검사, 신경인지기능검사, 신체 검사가 시행되었다. 대상자의 성별 및 연령과 신체 질환도 함께 조사하였다. 전반적인 인지기능과 질병의 심각도 단계 평가는 MMSE-K, GDS, CDR으로 측정하였다. 결 과 MMSE-K 점수는 정상군>MCI>AD 순이었으며 통계적으로 유의한 차이를 나타내었다(p=0.000). CDR과 GDS점수는 정상군이 가장 높았고, AD군이 가장 낮았으며 역시 통계적으로 유의한 차이를 나타내었다 BMI와 MMSE 점수 사이에는 유의한 양의 상관관계가 있었다(r=0.238, p=0.000). BMI와 CDR과는 음의 상관관계를 나타내었으며(r=-0.174, p=0.008), BMI와 GDS 역시, 음의 상관관계를 보였다(r=-0.233, p=0.000). 결 론 BMI와 인지기능과의 연관성이 있으므로, 치매나 경도인지장애 환자를 대상으로 BMI를 측정하는 것은 임상적 표지자로서 의미가 있을 것으로 예상된다. 본 연구는 AD의 진단 및 예방, 그리고 치료적 접근에 도움을 줄 수 있을 것으로 기대하며, 향후 이에 대한 대규모의 장기 추적 연구가 필요할 것으로 생각된다.
