  • In this paper, the current research for the biomechanics of artificial knee joints including experiments and stress analysis is surveyed and Introduced. The knee joint is the most large and the motion is very complicated, so the design of artificial joint is difficult and most research Is being done abroad. Up to date, most products are foreign products and Imported here and the gap between here and advanced countries of the technical and capability for the design and manufacturing is too deep to follow. So, the contents of papers in this area including the most excellent results are introduced. And the preliminary research on the contact stress analysis of the joints is present.

  • Although rare, the treatment options for a medial collateral avulsion fracture during total knee arthroplasty (TKA) range from conservative management using a cast and orthosis to internal fixation using metal screws. Bioabsorbable suture anchors have been used to replace metal fixators with distinct advantages, such as biocompatibility, radiolucency, and unnecessary second removal surgery, but complications, such as osteolysis, have been reported. This paper reports a potential risk of an extensive osteolysis-related suture anchor fixation of a medial collateral ligament (MCL) avulsion fracture during TKA in conjunction with a literature review.

  • 일반적으로 진행된 골관절염 등 골연골 병변에 대해서는 골연골을 치유시키는 술식에서부터 인공 관절 치환술까지 치료방법이 다양하게 보고되고 있다. 특히 견관절 인공 관절 치환술을 비롯한 여러 수술 방법에 대한 적응증 및 금기증을 잘 숙지하고, 각각의 고유 질병에 따른 병리 소견과 견관절의 정상적 해부학 및 생역학을 이해하며, 적절한 기구 선택과 정확한 술기로 수술을 시행할 때 제한된 치료 목표이지만 보다 긍정적인 치료 결과를 예측할 수 있다.

$^{99m}Tc$-HMPAO-labelled Leucocyte Scintigraphy in the Diagnosis of Infection after Total Knee Replacement Arthroplasty (인공슬관절 전치환술 환자에서 $^{99m}Tc$-HMPAO-백혈구 스캔을 이용한 인공관절 감염의 진단)

  • Purpose: This study was performed to evaluate the usefulness of $^{99m}Tc$-HMPAO-labelled leucocyte scintigraphy for diagnosing prosthetic infection after total knee replacement arthroplasty without the aid of following bone marrow scintigraphy Materials and Methods: The study subjects were 25 prostheses of 17 patients (one man and 16 women, mean age. 65 years) who had total knee replacement arthroplasty. After injection of $^{99m}Tc$-HMPAO-labelled leucocyte, the whole body planar and knee SPECT images were obtained in all patients. The subjects were classified into three groups according to clinical suspicion of prosthetic infection. Group A (n=11) with high suspicion of infection; Group B (n=6) with equivocal suspicion of infection, and Group C (n=8) with asymptomatic contralateral prostheses. Final diagnosis of infection was based on surgical, histological and bacteriological data and clinical follow-up. Results: Infection was confirmed in 13 prostheses (11 in Group A and 2 in Group B). All prostheses in Group A were true positive. There were two true positives, one false positive and three true negatives in Group B, and six true negatives and two false positives in Group C. Overall sensitivity, specificity, and accuracy for diagnosis of the infected knee prosthesis were 100%, 75% and 88%, respectively Conclusion: $^{99m}Tc$-HMPAO-labelled leucocyte scintigraphy is a sensitive method for the diagnosis of infected knee prosthesis. However, false positive uptakes even in asymptomatic prosthesis suggest that bone marrow scintigraphy may be needed to achieve improved specificity.

Surgical Simulation Environment for Replacement of Artificial Knee Joint (CT 영상을 이용한 무릎관절 모의 치환 시술 환경)

  • This paper presents a methodology for constructing a surgical simulation environment for the replacement of artificial knee join using CT image data. We provide a user interface of preoperative planning system for performing complex 3-D spatial manipulation and reasoning tasks. Simple manipulation of joystick and mouse has been proved to be both intuitive and accurate for the fitness and the wear expect of joint. The proposed methodology are useful for future virtual medical system where all the components of visualization, automated model generation, and surgical simulation are integrated.
