Secondary Adrenal Insufficiency Associated with Megestrol Acetate in a Patient with Lung Cancer (Megestrol Acetate와 관련된 이차성 부신기능저하증의 폐암 1예)

  • Loss of appetite is an important factor in the quality of life for advanced cancer patients. Megestrol acetate is used to stimulate appetite, but it can cause suppression of the pituitary adrenal axis due to the affinity of the glucocorticoid receptor. Adrenal insufficiency is a life threatening disorder if left, untreated, but the initial clinical symptoms of the patients are vague. Awareness of the glucocorticoid-like activity of megestrol acetate and its side effects are important for the diagnosis of adrenal insufficiency. We present a case of secondary adrenal insufficiency associated with megestrol acetate in a patient with lung cancer.

Silica Enrichment in Mantle Xenoliths Trapped in Basalt, Jeju Island: Modal Metasomatic Evidences (제주도 맨틀포획암내의 실리카 부화작용: 모달 교대작용의 증거)

  • Mantle-derived xenoliths, corresponding to spinel harzburgite and lherzolite in alkali basalts from Jeju Island, are metasomatized to various extents. They contain distinctive secondary orthopyroxene, forming corona or poikilitic textures. It clearly indicate that this secondary orthopyroxene has been produced at the expense of olivine along the grain boundaries and margins, suggesting silica-enriched metasomatic melt infiltrated through grain boundaries. Based on the geotectonic characteristics of Jeju Island and textural characteristics and major elements composition of mantle xenoliths, it is suggested that the silica-enriched melt/fluid could have derived from the ancient subducted slab, possibly in the mantle wedge, implying that the high $SiO_2$ activity in the lithospheric upper mantle beneath Jeju Island at that time.

  • This review focuses on the use of $^{18}F-FDG$ PET/CT to evaluate second primary cancers. The emergence of a second primary cancer is an important prognostic factor in cancer patients. The early detection of a second primary cancer and the appropriate treatment are essential for reducing the morbidity and mortality associated with these tumors. Integrated $^{18}F-FDG$ PET/CT, which can provide both the metabolic and anatomic information of a cancer, has been shown to have a better accuracy in oncology than either CT or conventional PET. The whole body coverage and high sensitivity of $^{18}F-FDG$ PET/CT along with its ability to provide both metabolic and anatomic information of a cancer make it suitable for evaluating a second primary cancer in oncology. Whole body $^{18}F-FDG$ PET/CT is useful for screening second primary cancers with a high sensitivity and good positive predictive value. In order to rule out the presence of a second primary cancer or an unexpected metastasis, further diagnostic work-up is essential when abnormal findings indicative of a second primary cancer are found on the PET/CT images. PET/CT is better in detecting a second primary tumor than conventional PET.

Stomach Cancer Secondary to Hematologic Diseases (혈액질환에 속발하는 이차성 위암)

  • Purpose: Patients with hematologic diseases such as chronic myeloid leukemia (CML) or chronic lymphoid leukemia (CLL) are known to have an increased chance of acquiring a secondary neoplasm. Stomach cancer is one of the most common malignant diseases in Korea, and we investigated whether the incidence of secondary stomach cancer in patients with a hematologic disease increases, in order to determine if a more intensive screening program for detecting secondary gastric cancer was required. We also investigated the safety of performing a gastrectomy in hematologic disease patients. Materials and Methods: From 1992 to 2006, the medical records of 8376 patients diagnosed with one of the six common hematologic diseases were reviewed. Results: Nine secondary stomach cancers were found among the 8376 patients during the 15-year observation period. No surgical-related complications occurred, and there was no recurrence of stomach cancer if detected early. Conclusion: It seems that a more intensive screening program for detecting secondary gastric cancer in hematologic disease patients is not required, and surgery is not risky in these patients.

Second primary cancer in reconstructed neopharynx: a case report (재건된 새 인두의 이차암에 대한 증례 보고서)

  • Background. Ever since the first report of deltopectoral flap in pharyngo-esophageal reconstruction in 1965, various methods of flap reconstruction have been introduced, allowing surgical eradication of tumors that were once thought to be inoperable. Despite these advancement, many literatures emphasize distant metastasis and second primary malignancies as the most important factors that contribute to the low 5-year survival rate of the patients. Specific consensus about defining second primary cancer is still debatable, due to small number of reports regarding second primary tumors arising in flaps used for reconstruction of defects in the head and neck region. Case. We report a case of a 72-year-old male patient who, under the diagnosis of hypopharyngeal cancer, underwent total laryngectomy with partial pharyngectomy, extended right radical neck dissection with extended left lateral neck dissection, right hemithyroidectomy and radial forearm free flap reconstruction on June 16, 2003. After 37 cycles of radiation therapy, the patient exhibited no sign of recurrence. The patient revisited our department on June 14, 2016 with chief complaint of dysphagia that started two months before the visit. Radiologic studies and histology revealed squamous cell carcinoma in neopharynx, one that had been reconstructed with forearm free flap. Conclusion. Until now, only a handful of reports regarding patients with second primary cancer in reconstructed flaps have been described. Despite its rarity, diagnostic criteria for second primary cancer should always kept in consideration for patients with recurred tumor.

  • 제주도는 대한민국의 남단대한해협에 위치(동경 120$^{\circ}$ 16'-126$^{\circ}$ 57' : 북위 33$^{\circ}$ 12'-33$^{\circ}$ 34$^{\circ}$)하는 화산도이다. 제주도화산은 전기 선신세에서 제사기에 걸친, 주로 알카리 현무암의 용암류로 되며 화쇄암을 동반한 외에 많은 암재구로 구성되고 있다. 또한 이섬은 세계에서도 유수의 용암동굴지대로써 저명한 존재로 되고 있다. 그러나 이제까지 이들이 동굴에 형성되고 있는 이차생성동굴광물에 대한 연구는 적고, 황금물, 협재굴, 구린굴, 만장굴 및 옥산굴에서 방해석, 석고, 타라나키석등 3종의 광물의 산출이 알려져 있을 뿐이다.(중략)

Risk of Death and Occurrence of Secondary Disease of Cancer and Cardiovascular Disease Patient by Income Level in Korea (암, 심뇌혈관 질환자의 소득수준에 따른 사망 및 이차 질환 발생 위험)

  • In this study, we analyzed the effect of the income level of cancer, stroke, and myocardial infarction on mortality by using National Health Insurance Service(NHIS) Cohort 2.0 DB. Patients who newly developed the disease in 2007 were observed till 2015. The analysis used the Cox probability proportional risk model and the competing risk model. The income level used information at the time of the onset of the disease in 2007, categorized into low / mid / high. The results showed that there were differences in the risks of death and secondary disease in patients with cancer, stroke, or myocardial infarction according to the income level. In addition to the need for a social safety net to lower the incidence of early deaths in low-income families, it seems necessary to continue to strengthen universal protection for serious diseases similar to the current policy.

  • This study was conducted to identify the level of quality of life and factors influencing on quality of life in Korean women with breast cancer and cervical cancer. This study was secondary analysis using The seventh Korea National Health and Nutrition Examination Survey (KNHANES). In the result of this study, women with the lower age, higher household income, and higher self-rated health status, had the higher level of quality of life. Following the result, intervention strategies are needed to enhance self-rated health and to improve quality of life in women with lower age and lower household income.

Intramedullary Spinal Cord Metastasis : A Report of Two Cases and a Review of the Literature (척수내 전이암 - 2례 보고 및 문헌고찰 -)

  • Intramedullary spinal cord metastases (ISCM) account for only $3.4\%$ of symptomatic metastases to the spinal cord. The survival of patients with ISCM is characteristically short, often no longer than 2 months, due to a rapid neurologic deterioration and the presence of widespread metastases, Including metastases to the brain. We report two cases of ISCM arising from primary sphenoid sinus carcinoma and primary lung cancer along with a review of the literature. The case of ISCM from the primary sphenoid sinus is the third case of secondary syringomyelia due to ISCM In the world literature, and ISCM from the primary lung cancer is the first case reported in Korea. One case showed a slow progression of symptoms and a longer survival (26 months after the radiotherapy to the spine), and the other showed a rapid deterioration of symptoms with a shorter survival. More effective palliation can be achieved if the disease is diagnosed at an early stage when the neurologic deficits are still reversible.
