Rett syndrome is a X-linked genetic neurological disorder characterized by developmental regression, particularly in relation to expressive language and use of the hands, together with profound mental retardation, that almost exclusively affects females. Oral manifestations of the disease are commonly associated with the clinical conditions such as convulsion activity, difficulties for oral hygiene behavior, walking problems and oral/digitalmanual habits. Bruxism is the most frequently observed oral habit in patients with Rett syndrome. Two cases with the Rett syndrome were reported. Both patients had the typical manifestations like stereotypic hand movement, bruxism and digit-hand sucking. Caries control for the patients was perfomed under general anesthesia.
A retrospective study was conducted to determine the patterns of Temporomandibular Disorder(TMD) among adolescents (12-19 years) over last ten years. 174 patients who had visited the Oral medicine of Pusan National University Dental Hospital in 2000 and 491 patients in 2008 were found on chart review. The results obtained were as follows : 1. The number of adolescent patients was significantly increased in 2008 than in 2000, especially in male. 2. Bruxing, clenching, holding habits and bad sleep hygiene were highly increased more in 2008 than in 2000. 3. Osteoarthritis was significantly increased in 2008 than in 2000 and anterior disc dislocation without reduction was slightly increased. 4. The ratio of male to female adolescent patients with osteoarthritis was remarkably increased. Among the patients who had holding, bruxing, clenching habits, significantly increased osteoarthritis found to be common. This could imply holding, bruxing, clenching habits are prominently associated with increasing risk of developing osteoarthritis.
This study was performed to investigate the dental ages of the patients with temporomandibular disorders(TMD), using a method of age estimation by dental attrition, and to compare the differences of dental ages between control and experimental groups. In addition, they were compared according to oral habit. Clinical evaluations for the tooth attrition were to examine the study models of 65 TMD patients (mean age of 26.6 years, aged 18 to 40) had visited to the Department of Oral Medicine and Orofacial Pain Clinic, Dental Hospital, Dankook University as a experimental group and 22 volunteers (mean age of 25.4 years, aged 23 to 36) without TMD as a control group. Age differences were calculated subtracting real age from estimated age. Age differences between experimental and control groups were compared statistically between groups classified according to oral habit. As a results, dental age was increased significantly in TMD patients compared with normal subjects. Oral parafunctions which increased dental age were bruxism, clenching and heavy chewing in order. Conclusively, it should be consider that there can be much discrepancy between real age and age estimated by dental attrition in patients with TMD. In addition, it is suggested that dentists should try to protect the tooth from attrition by various parafunction on treating the patients with TMD.
Excessive tooth wear causes loss of tooth structure, disharmony of occlusal plane, functional and esthetic problems. Although the decrease of occlusal vertical dimension may be compensated by growth of alveolar bone, if the length of tooth is not enough for the retention of restoration, minimum increase of occlusal vertical dimension is required without discomfort of the patient. In this case, 33-year-old woman drinks more than 1 liter of soft drinks a day and has bruxism in night time, visited in Seoul National University Dental Hospital with chief complaint of generalized tooth wear and related esthetic and functional problems. It was considered as a loss of occlusal vertical dimension based on the accelerated tooth wear caused by erosion and bruxism and facial appearance, phonetic, esthetic, functional evaluations. It was planned to raise occlusal vertical dimension by provisional restoration two times for patient's adaptation, 3 mm and 2 mm each, total 5 mm. Confirming no discomfort and clinical symptom during total 16 weeks after restoration with provisional fixed restoration, it was restored with porcelain fused to gold crown and bridge. Because the patient was young woman, anterior teeth were restored with collarless porcelain fused to gold crown. This case presents that satisfactory esthetic and functional result by full mouth rehabilitation with increase of occlusal vertical dimension.
This study was intended to review comprehensively the articles related to temporomandibular joint disorder patients and to analyze psychoanalytically the case with temporomandibular dysfunction and bruxism as a symptom of hysterical conversion disorder. As a result, the author found that , in the aspect of psychoanalysis, the temporomandibular problem and bruxism was a kind of symptom of emotional conflict being converted to the face. I would like to emphasize that the referral process and the ability to work as a therapeutic team including dentist and psychiatrist can enhance the probability of successful treatment for several types of patients.
초봄에는 엄마 품을 처음으로 떠나 유치원이나 초등학교로 입학한 병아리 친구들을 볼 수 있다. 어른들이 보기에는 인생의 한 통과의례로 볼 수도 있지만 어린이들 입장에서는 심리적으로 큰 스트레스를 받을 수 있으며 건강에도 문제가 생길 수 있다. 단체 생활을 하므로 운동량이 많아지고 다른 친구들과의 접촉이 많아지므로 호흡기 질환에 걸리는 빈도가 잦아질 수 있다. 치과적인 측면에서도 취학기 아동은 치아 관리에 있어 중요한 시기이다. 만 6세부터 이갈이가 시작되어 평생 간직해야 할 영구치가 나기 시작하고, 유치원이나 학교에서 급식을 할 경우에는 스스로 올바르게 이를 닦을 줄 알아야 하며, 친구들과의 놀이가 지나치면 치아 외상을 입는 경우도 많아지는 시기이기 때문이다.

뇌성마비(Cerebral Palsy)는 임신 중이나 출산, 또는 신생아기에 발생된 뇌손상에 의해 발생하는 중추성 자세 및 운동 장애를 말한다. 뇌성마비는 여러 가지 합병증 및 동반증상을 가지게 되는데, 그 증상들에는 지적장애, 언어장애, 간질, 시각장애, 자폐, 이갈이 및 섭식연하장애 등이 있다. 뇌성마비 장애인에서 나타나는 치과적 특징과 고려사항, 그리고 이들의 구강건강관리에 대해 알아보고자 한다.
This study tried to find the prevalence and distribution of temporo-mandibulr disorders(TMD) for workers (employee) in Seoul area to investigate the correlation between TMD and possible etiological factors such as general muscle and joint symptoms, headache, unilateral chewing and parafunction. This study was an epidemiological investigation of answers obtained from 282 persons by questionaire from Aug. 1995 to Nov. 1995. The major findings from the questionaire were as follows. (1) 43.26% of the subjects(282 persons) has TMD. (41.88% of man and 43.26% of women) (2) The more often people have general muscle and joint symptoms and headache, the more susceptible they are to TMD. Their correlation was very significant(p<0.001). (3) The more people have bad oral habits such as bruxism, clenching and biting habit, the more likely they have TMD. The more sensitive people are to stress, the more frequently they have bad oral habits such as bruxism, clenching and biting habit. (4) Unilateral chewing has higher TMD index than bilateral chewing. There is no relationship between ache areas and TMD index.

Patients with TTH(n=58) visited the Department of Oral Medicine, Kyung Hee University Dental Hospital during two months were recruited to this study. 20 patients with OPF(bruxism, clenching, bruxism & clenching) are the experimental group and 38 patients without OPF are the control group. Both groups were asked to answer the questionnaire based on the diagnostic criteria of TTH(IHS). Then it was analyzed statistically. But, there was no difference between two groups(p=0.40). 4. In the experimental group, pain shown bilaterally was presented more frequently than in the control group. But, there was no difference between two groups(p=0.52). 5. In the experimental group, pain was more increased by physical activities than in the control group. But, there was no difference between both groups(p=0.74). The pain of TTH patients with OPF was presented to be non-dull pain frequently and more bilaterally and severely, also increased more by physical activities than the pain of TTH patients without OPF. But, there was no significant difference between two groups. Therefore, it is considered that the pain of TTH is not influenced by OPF.
