  • Artificial intelligence (AI) technology is actively being applied for the interpretation of medical imaging, such as chest X-rays. AI-based software medical devices, which automatically detect various types of abnormal findings in chest X-ray images to assist physicians in their interpretation, are actively being commercialized and clinically implemented in Korea. Several important issues need to be considered for AI-based detection assistant tools to be applied in clinical practice: the evaluation of performance and efficacy prior to implementation; the determination of the target application, range, and method of delivering results; and monitoring after implementation and legal liability issues. Appropriate decision making regarding these devices based on the situation in each institution is necessary. Radiologists must be engaged as medical assessment experts using the software for these devices as well as in medical image interpretation to ensure the safe and efficient implementation and operation of AI-based detection assistant tools.

  • Today modern people have been searching on managing themselves in easy ways without side-effects to lead a healthy and quality life, accordingly concerns about complementary and alternative medicine have been increasing. The scientific term' complementary and alternative medicine' has been originally used in the US and has become the general term for natural treatment of medical methods, which can be suggested as an alternative solution to modern medical problems. There should be a study based on users' subjectivity to reinvigorate complementary and alternative medicine more than it does now. The purpose of this study is to offer basic materials when assessing practical applications on complementary and alternative medicine using the Q method which is subjective pattern analysis, help understand features of complementary and alternative therapy according to patterns, and find out preferences for patterns of complementary and alternative therapy. As a result patterns of complementary and alternative medicine were classified into three types. Firstly, type I prefers to biological/herbal treatment. Secondly, type II prefers to mixed treatments. Thirdly, type III prefers to manual healing.

  • Coronavirus disease (COVID-19) has threatened public health as a global pandemic. Chest CT and radiography are crucial in managing COVID-19 in addition to reverse transcription-polymerase chain reaction, which is the gold standard for COVID-19 diagnosis. This is a review of the current status of the use of chest CT and radiography in COVID-19 diagnosis and management and anㄷ introduction of early representative studies on the application of artificial intelligence to chest CT and radiography. The authors also share their experiences to provide insights into the future value of artificial intelligence.

  • A Siemens HD-270 MLC$^{TM}$, a virtual-micro MLC, allows to the application of a smooth field edge method due to the finite leaf-width of MLC. This technique was implemented into a Pinnacle planning system in order to evaluate the dose distributions during the planning stage. The necessary dosimetric aspects, such as undulation and effective penumbra, were investigated with variations in the resolutions of a virtual-micro MLC and field edge angle. The positional accuracy of the couch movement was also assessed for clinical implementation. The overhead time for planning and treatment was confirmed as negligible.e.

  • 통증감소를 위주로 하는 기존의 슬 골관절염 치료는 슬관절의 퇴행을 가속화시킬 수 있기 때문에 한의학적 각종 침구요법 및 물리요법, 약물요법이나 의학적 주사요법 등을 '슬관절에 대한 부하 힘을 감소시킨 상태'에서 시행하는 진료형식을 제안한다. 그리고 이러한 슬관절질환 치료의 추나요법을 채택하여 임상적용이 가능하도록 이를 기계화 도구화하고, 진단과 치료를 동시에 시행할 수 있는 방식이 되도록 연구할 필요가 있다.

  • Radiosurgery treatment in the brain requires detailed information on three-dimensional dose distribution. A three-dimensional treatment planning is a prerequisite for treatment plan optimization. It must cover 3-D methods for representing the patient, the dose distributions, and beam settings. Three-dimensional dose models for non-coplanar moving arcs were developed using measured single beam data and efficient 3-D dose algorithms for circular fields. The implementation of three dimensional dose algorithms with stereotactic radiosurgery and the application of the algorithms to several cases are discussed.

  • Hepatitis B virus (HBV)는 전 세계적으로 약 3억 5천만 명이 만성적으로 보균하고 있으며, 한국에서도 보건의학적으로 가장 중요하여 높은 간암의 발병율과도 밀접한 관련이 있다. 최근에 와서 HBV가 유도하는 임상적 위험질환의 병태생리에 대해서 면역학적 지식의 발전을 바탕으로 이해가 깊어졌는데, 바이러스 자체보다는 인체의 다양한 면역반응이 질병의 경과에 더욱 중요하다. 이러한 사실로 한의학적 치료법의 과학적 연구는 만성 HBV 보균자들에게 효과적인 치료방법으로 적용될 것으로 기대될 수 있는데, 우선 여기서는 전형적으로 HBV와 관련해서 의학적 문제를 모두 가지고 있는 한 가족을 통해서 HBV에 대한 폭넓은 이해에 도움을 얻고자한다.

  • Psychosomatic medicine is also known as consultation-liaison psychiatry. Pediatric consultation-liaison psychiatry is composed of all consultation, liaison, diagnostic, therapeutic support and research activities carried out by psychiatrists and other mental professionals in pediatric ward to provide mental health services to physically ill pediatric patients. As the differences in the basic concepts of disease models between psychiatry (psychosocial model) and pediatrics(biomedical model) exist, active communication between the child psychiatrist and pediatric medical staffs is required. Although the general guidelines are similar, there are specific considerations for consultation in children and adolescents. Much work is still needed to identify empirically supported treatments which are effective for managing a board range of psychosocial difficulties in children and adolescents.

A Criticism Regarding the Medical Rationale of Nucleoplasty Studies for Internal Disc Disruption (수핵성형술(Nucleoplasty)을 이용한 디스크 내장증(Internal Disc Disruption)의 치료 연구들의 의학적 논거에 대한 비판)

  • Nucleoplasty is a type of percutaneous disc decompression that has been developed to treat herniated intervertebral discs. Currently, in some clinics, researchers have also applied this procedure to patients with internal disc disruption, apart from the originally intended usage on herniated intervertebral discs. The purpose of this study is to evaluate the feasibility of this extended use based on medical logic. To achieve this, the author analyzed case studies on performing nucleoplasty on patients with internal disc disruption. The main points of the analysis are, first, the validity of the treatment evidence presented by the researchers and, second, the relevance of the patient selection criteria. As a result, it is judged that the therapeutic rationale of existing papers applying nucleoplasty to the treatment of internal disc disruption is unclear or inconsistent with general medical logic, and in the process of patient screening, discs that may be deemed inappropriate for percutaneous decompression are included. Therefore, the author believes that existing studies applying nucleoplasty to the treatment of internal disc disruption have the nature of somewhat adventurous experiments that are unnecessary or can cause potential side effects. In order to uphold patients' rights and improve the completeness of the study in the research process on this topic, the author thinks that it is essential to establish clearer therapeutic evidence than the current level of understanding and to have an elaborate patient screening process based on it.
