최근 클라우드 컴퓨팅에 대한 관심이 고조됨에 따라, 이를 활용한 데이터베이스 아웃소싱(Outsourcing)에 대한 연구가 활발히 진행되고 있다. 그러나 아웃소싱 된 데이터베이스는 사용자 개인 정보, 증권, 또는 의료 정보 등 민감한 정보를 포함하기 때문에 서비스 제공자로부터 수행한 질의 결과 데이터가 데이터 소유자로부터 생성된 데이터이며, 정확한 질의 결과를 포함하는지 확인하기 위한 질의 결과 무결성 검증 기법의 필요성이 대두되었다. 기존 질의 결과 무결성 검증 기법은 질의 결과에 포함되는 무결성 검증 데이터의 크기가 증가하여 검증 데이터 전송 오버헤드 증가 및 데이터 노출 위험 증가 문제를 지닌다. 따라서, 본 논문에서는 데이터 보호를 지원하는 암호화 데이터 기반 질의 결과 무결성 검증 기법을 제안한다. 제안하는 질의 결과 무결성 검증 기법은 암호화된 데이터 그룹을 주기 함수를 이용하여 재분할하고, 최종 데이터 그룹 id를 힐버트 커브를 통해 변환한다. 따라서, 검증 데이터 오버헤드를 감소시켜 효율적인 질의 처리를 지원하며, 그룹 id 변경을 통해 검증 데이터 유출 위험을 방지한다. 성능평가를 통해, 제안하는 기법이 기존 기법에 비해 질의 처리 시간 측면에서 평균 2배, 검증 데이터 오버헤드 측면에서 최대 20배의 성능을 개선함을 보인다.

This study analyze long debate issues by the analysis of existing studies and the effect of private health insurance in the satisfaction of health service utilization. Then make developmental role of private health insurance. The analysis results of literatures, high-income earners are more subscriptions and the poor people in health status are excluded. Thus, enable private health insurance has the potential to lead the polarization of people. The medical use of private health insurance subscriber is more than non-subscriber and is likely to result in additional expenditure spending of public insurance. The contribution of private health insurance on improvement of the health option is clear. However, is not clear the contribution on health care quality improvement and health service customer satisfaction. The contribution on the national health care system efficiency of private health insurance is not clear. Private health insurance in the satisfaction of health service utilization is on effect. In short, supplementary private health insurance is desirable in our country.

본 논문에서는 효율적인 환자 이송 및 치료를 위한 응급구조사와 의료기관 간의 의사소통 및 질 관리를 위한 EC&M (Efficient Communication and Management) 시스템을 제안한다. 이는 관련 앱을 효율적으로 사용하여 응급차 배치부터 병원 도착 시 환자 수속 작업 등을 최소화하고, 각 작업의 수행시간을 최소화 한다. 또한 이 시스템의 사용자는 소방청, 의료기관, 119 응급 구조사이고, 이 중 응급구조사에 초점을 두었다. 이는 PRE-KTAS를 소방청과 의료기관에 동시전달하여 전달된 PRE-KTAS를 통한 적절한 의료기관을 배치하고 선정된 응급의학 전문의에게 직접의료지도를 요청할 수 있다. 그리고 응급구조사는 의무기록활동지를 작성하여 의료기관에 전달하여 내용전달의 효율성을 높인다. 본 논문에서는 시뮬레이션을 통하여 제안하는 응급구조사와 의료기관 간의 정보 공유 시스템은 기존에 제안하는 앱을 사용하지 않고 발생했던 많은 문제를 해결 할 수 있을 것이므로 기존의 앱보다 우수할 것으로 기대된다.

The Internet of Things (IoT) connects all markets and industries, enabling new business models for a variety of services and service providers. The Internet of Medical Things (IoMT) not only accelerates medical advances, but also enables treatment with a more human approach. In addition, it improves treatment methods and quality of precision medical care through data, enables timely treatment, and improves operational productivity of medical institutions through a simplified workflow. However, since the medical field directly affects human health and life, securing security has become an issue above all else, and is a target for hackers trying to exploit it. Therefore, in this study, IoMT technology and security threats and countermeasures in the medical field are analyzed.

노인인구의 증가에 따라 노인성 질환이 증가하고, 대표적 노인성 질환직 하나인 치매도 점점 증가하고 있다. 이제까지 치매가 노화의 한 정상적인 과점으로 여겨져왔고 최근에 와서야 하나의 질병으로 인식하기 시작하였기 때문에, 그간 치매노인의 의료복지적 측면의 관리가 소홀하였고 가족의 어려움이 켰었다. 본 연구는 치매노인 시설을 이용하고 있는 치매노인 가족간호자를 대상으로 치매노인의 일상생활 기능손상정도 및 건강상태, 가족간호자의 간호부담정도, 사회적지지, 치매노인과 가족간호자의 삶의 질 정도, 그리고 그들이 어떤 사희복지적지지 서비스를 이용하려 하는 지를 파악하여, 앞으로의 치매노인과 그 가족간호자의 부함을 경감하고 삶의 질을 개선할 수 있는 대안마련의 기초자료를 제공할 목적으로 수행되었다.

Recently medical service environment, the clinical information exchange which contribute to medical safety, promotion of service quality and patient's convenience, efficiency of medical procedures and medical management is essential medical service model. But, practical exchange of clinical information which variation of information level, absence of standardization system, build of heterogeneous information systems is difficult in each medical institute. In this paper, We analyzed the related technical standardizations and the models of clinical information exchange. So, we designed the clinical information exchange system based on the ideal lazy response model which is aimed at vitalizations the exchange of clinical information under domestic law environment. In case of exchange the clinical information, we separate CDA document flow from metadata flow. As a experimental result we acquired 24% improved performance compared with existed system based on the lazy response model.

In this paper, we suggests a algorithms of recognizing the disease region by extracting particular organ from medical image. This method can extract liver region in spite of input image including many organs and charged format by using multi-threshold of feed-back-structure for segmentation liver region, and suggest the recognition of disease region in extracted liver, using multi-neural network structured by RBF and BP, overcoming the defect of single-neural network. The algorithm in this paper is proficient in adaptation for a multi form change of input medical image. This algorithm can be used at tole-medicine through automatic recognition after recognizing of the disease region by real-tire medical Image.

Objective : The aim of this study was to compare the sociodemographic characteristics, depression, quality of life, and unmet medical need in elders with and without limitation of activity by aging or dementia. Method : The data were used for raw data of seventh national health and nutrition examination survey (2016). We compared sociodemographic characteristics, depression, quality of life, and unmet medical needs between elders with limitation of activity by aging or dementia (n = 32) and elders without limitation of activity (n = 1201). Result : Elders with limitation of activity were significantly older than elders without limitation of activity (p<.001), income was lower (p<.05) and married status had more bereavement (p<.001). The elders with limitation of activity had higher depression (p<.001) and lower quality of life (p<.001) than elders without limitation of activity. Unmet medical need was greater than elders without limitation of activity (p<.001). Conclusion : Elders with limitation of activity showed that they did not receive enough medical services than elders without limitation of activity. These results suggest that the welfare system for the health care of population with aging or dementia needs to be improved.

진료 및 간호서비스는 다른 제조업과는 달리 무형의 서비스에 대한 간접비의 비율이 두드러지는 특성을 지니며, 이러한 특성으로 인해 의료기관의 전략적 계획수립, 수익성 분석, 경제성 분석, 의료서비스 제공의 질 관리 등의 의사결정에 적극 활용되어야 하는 원가 분석에 어려움을 겪어왔다. 전문 간호 분야인 가정간호는 1회 방문 시에 다양한 활동들이 모여서 가정간호서비스가 수행되므로 가정간호의 원가분석에는 각 행위들이 모인 활동에 대한 분석에 근거한 정확한 원가 정보가 요구되고 있다. 활동기준원가의 기본 요소는 자원, 활동, 자원 동인, 활동 동인, 원가 대상이며 이에 따라 가정간호사업에서 활동기준원가의 각 요소를 정의하고 전통적 원가 계산이 아닌 활동을 중심으로 가정간호사업의 업무 알고리즘을 분석하여 활동기준원가관리시스템을 설계한다. 가정간호사업의 활동기준원가관리시스템은 가정간호의 효율적 운영과 의료기관의 경영 개선과 항께 실질적 원가계산을 통해 정부가 건강 보험 수가를 결정하고, 보험 수가의 적절성을 평가하는데 중요한 기초 자료를 제공하게 될 것으로 기대한다.
