Effect of Unmet Healthcare Needs on Quality of Life (미충족 의료 경험이 삶의 질에 미치는 영향)

  • The purpose of this study was to investigate the potential causal relationship between unmet healthcare needs and metrics related to quality of life. To examine the relationships between these variables, the study conducted an analysis by processing additional irradiated raw materials of the Korean Medical Panel. The metrics related to quality of life included six variables: athletic ability, self-care, daily activities, pain/trouble, uneasiness/depression, and subjective health conditions. Linear regression analysis revealed that the unmet healthcare needs had statistically significant negative effects on all six quality of life variables. A review of the magnitude of the linear regression β values for those variables showed that the relative level of influence on the quality of life variables decreased in the following order: pain/trouble, subjective health conditions, uneasiness/depression, daily activities, athletic ability, and self-care. Based on the results, practical applications related to strengthening working-level links between health and welfare is considered an effective policy response to the continued presence of unmet healthcare needs; such applications could contribute to improving the quality of life of those with unmet healthcare needs.

Factors associated with the choice of tertiary hospitals in patients with diabetes or hypertension (당뇨 또는 고혈압 환자의 상급종합병원 이용에 미치는 요인)

  • 목적: 이 연구는 당뇨와 고혈압 환자의 상급종합병원 이용에 미치는 요인을 관찰하였다. 방법: 이 연구는 한국보건사회연구원과 국민건강보험공단이 공동으로 조사하는 한국의료패널 자료를 사용하였다(2008년, 2010년, 2012년). 연구대상은 20세 이상 성인이었으며 각 2008년에 2409명, 2010년에 2424명, 2012년에 2429명을 관찰하였다. 당뇨와 고혈압에 대한 외래방문 건수는 총 64,438건이었으며 당뇨 또는 고혈압 환자의 3차 의료기관 선택에 영향을 주는 요인을 관찰하기 위해 다수준 로지스틱 회귀분석을 활용하였다. 결과: 교육수준이 높은 환자가 낮은 환자에 비해 상급종합병원을 이용할 확률이 각각 2008년에 2.04배, 2010년에 1.83배, 그리고 2012년에 1.65배 더 높았다. 또한 고소득 환자가 저소득 환자에 비해 3차 의료기관을 이용할 확률이 2008년에 1.77배, 2010년 1.91배 그리고 2012년 1.94배 더 높았다. 결론: 정책입안자들은 의료전달 시스템과 관련하여 정책을 실행할 때 이러한 환자의 특징을 고려할 필요가 있다.

Predictors of Cognitive Function Decline of Elderly : Using Living Conditions and Welfare Needs of Older Korean Persons Panel Data (노인의 인지기능 저하 예측요인: 노인실태조사 패널자료를 이용하여)

  • Objectives : The aim of this study was to investigate predictors of cognitive function decline among community dwelling elderly. Methods : Data were analyzed from the Survey of Living Condition of Elderly panel study. Cognitive function was measured with the MMSE-KC at baseline and year 3. The study subjects were 5,464 community dwelling people aged 65 years or older who had no disability at baseline. Logistic regression analysis was used to predict cognitive function decline. Results : From 2008-2011, 4,417(80.8%) elderly people had no cognitive decline, and 1,074(19.2%) showed cognitive function decline measured by the MMSE-KC. After adjusting for demographics and baseline MMSE-KC score, the best predictors for cognitive function decline at 36 months were diabetes mellitus, smoking, low intensity physical activity, relationship with relatives and friends. Conclusions : Health promotion programs that are focused on the elderly are essential in preventing cognitive function decline. Promoting regular physical activity, and social relationships should be included in health promotion for elderly. When treating patients with diabetes, preventing cognitive impairment should be considered through education and counseling.

Effect of Temporary Employees and Health Characteristics Female Non-Regular Female Workers (비정규직 여성근로자의 고용특성 및 건강에 미치는 인자: 산재보험패널조사)

  • Objectives : Using several waves of the Panel Study of Worker's Compensation Insurance(PSWCI), this study analyzed the effect of female workers on the health and lifestyles of korean workers in the sample year of 2014 (The first survey was completed in August-October 2013;, the second survey was completed in August-October 2014.) Methods : The research subjects were 24,672 women among workers who responded to the 2014 PSWCI panel report. Statistical analyses were performed with SAS version 9.4 (SAS Institute Inc., Cary, NC, USA). Results : According to the results of the analysis, women showed differences in female temporary workers, working conditions, classification of occupation, working period, and health conditions based on work-time conditions. The results show that female workers with disabilities have a direct positive effect on life satisfaction as we anticipated in the hypothesis. Conclusions : Female workers should focus on factors identified in this study when developing employment interventions to improve a health promoting lifestyle for their work environment.

  • Objectives : Using several waves of the Panel Study of Worker's Compensation Insurance(PSWCI), this study analyzed the effect of work hours on health and lifestyle of Korean workers in a sample from the 2013~2014 year (The first survey was completed in August-October 2013; the second survey was completed in August-October 2014). Methods : The research subjects were 1,312 men from among employees who responded to a 2013 PSWCI panel report. Statistical analyses were done with SAS version 9.3 (SAS Institute Inc., Cary, NC, USA). Results : According to the results of the analysis, the male subjects had differences in economic activity and health conditions based on the length of working hours. There was a significant relationship among working hours and life satisfaction, and the correlation coefficients were statistically significant. The results show that workplace conditions has a direct positive effect on life satisfaction as we anticipated in the hypothesis. Conclusions : Management should focus on the factors identified in this study when developing employment interventions programs to improve the the promotion of healthy lifestyles for adults over the age of 40.

  • Objectives: The aim of this study was to investigate cognitive function decline by socio-demographic and health-related characteristics (health behaviors and health status) using 5th Korean Longitudinal Study of Aging panel data. Methods: The subjects were 4,440 community-dwelling people aged over 57 years. The data were analyzed with descriptive statistics, frequency analysis, χ2-test, and binary logistic regression analysis using SPSS ver. 25.0. Results: The findings revealed that socio-demographic characteristics (gender, age, area of residence, educational level, marital status, number of children, number of grand-children) and health-related characteristics (smoking, drinking, regular exercise, weight category by body mass index, hypertension and diabetes mellitus) were factors that influenced cognitive function decline (p<.05). Conclusions: Cognitive function decline was closely related to health behaviors and disease types. Future studies must examine related constructs to accurately determine these relationships among various populations. The present study could be used as a tool for the development and implementation of health promotion and prevention strategies.

The Study on the annual average direct cost incidence per cancer patient (암환자 1인당 연 평균 직접비용 발생에 대한 연구)

  • Among the participants of the 2012 Korea Medical Panel survey, 308 people who have used emergency, hospitalization and outpatient services for cancer have been selected. The average annual direct cost per cancer patient was analyzed by adding up the patient's medical expenses, industrial copayments, and non-salary costs. The average annual direct direct cost of cancer spent by cancer patients is about 129,093,792 per male, 158,100,612 won for men and 110,482,075 for women.For those with health insurance, the total direct cost per person from cancer was 183,095,125 won and the beneficiaries were 46,241,705 won. By household income, the average annual direct direct costs per person were 112,459,971 won per patient in the household income quartile, 137,910,890 won for patients in the second quartile, 149,556,570 won in the third quartile and 112,730,461 won, quartile 5, respectively.Was 142,926,331 won.

Changes in Public Health Perceptios after the Outbreak of Coronavirus Disease-19 among the Gangwon Province Residents Focusing on the Results of the Gangwon Province Residents' Panel Survey 2019-2020 (코로나바이러스감염증-19 전후 강원도민의 공공의료 인식 비교 : 2019-2020 강원도민 보건의료패널조사 결과를 중심으로)

  • Objectives: The purpose of this study is to investigate changes in public health-related perceptions of residents of Gangwon province after the outbreak of Coronavirus disease-19(COVID-19) of the public. Methods: We performed paired T-test analysis to measure the change in public health-related perceptions before and after COVID-19. We also utilized generalized estimating equations to identify demographic factors correlated with public health-related perceptions. Results: The public perceived public health as 'All citizens can use medical care and protect/promote health.' The concept was the most popular, from 94.3% in 2019 to 95.5% in 2020. In addition, after COVID-19, residents of Gangwon province's satisfaction with medical services increased, but the overall level was not high. Among the eight essential healthcare needs after COVID-19, cardiovascular disease and injury services have emerged as preferred services. However, by sociodemographic factors, distinctive responses were detected. Conclusions: Through COVID-19, Gangwon residents' awareness of the public's health rights has increased. Those living in vulnerable areas or with unmet medical care, supported strengthening public health care. In addition, although medical satisfaction has increased, it is not satisfied, so listening to the voices of the population group with low satisfaction is essential. Lastly, since the necessity of essential health care may change due to specific events, the local government needs to plan health projects reflecting the needs of residents. Therefore, when designing the public health care strategy in Gangwon province, the local government should consider not only political factors but also environmental factors, demographic and conceptual factors.

The Effects of Medical Service Utilizations on Life Satisfaction among the Elderly: Focusing on the Moderating Effects of the Presence of Chronic Illnesses (노년기 의료서비스 이용이 삶의 만족도에 미치는 영향: 만성질환 유무의 조절효과를 중심으로)

  • The rapidly aging society entails the increases of medical service utilizations among the elderly. Medical services utilized by the elderly influence their life satisfaction. However, little is known about the effects of medical service utilizations on life satisfaction. Much less is known about whether the effects of medical service utilizations on life satisfaction tend to vary by the presence of chronic health conditions. Including 3,944 individuals aged 65 and over who participated in the 3rd wave of Korean Welfare Panel Study, the current study aims to examine (1) the relationships between medical service utilizations and life satisfaction and (2) whether the effects of medical service utilizations on life satisfaction vary by the presence of chronic health conditions. Data were processed through structural equation modeling(SEM) and multi-group SEM. Results indicate that (1) levels of both outpatient and inpatient service utilizations are related to life satisfaction, whereas levels of health monitoring service utilizations are not and (2) the effects of medical service utilizations varied by the chronic health condition status of the elderly. These results indicate that, in order to enhance levels of life satisfaction, medical services should be strategically utilized by the elderly depending on their chronic health condition status. Based on the findings, we discussed implications for practice and policy, suggesting future research directions based on the limitations of the current study.

Association between Changes in Multiple Chronic Conditions and Health Expenditures among Elderly in South Korea: Korean Longitudinal Study of Aging 2014-2018 (고령화연구패널조사 2014-2018년 데이터를 이용한 한국 노인의 복합만성질환 변화와 본인부담 총 의료비의 연관성)

  • Background: Aging societies face social problems of increased medical expenses for older adults due to increased geriatric diseases. This study aims to analyze the relationship between the state change of multiple chronic conditions (MCC) and out-of-pocket medical expenses in the elderly aged 60 or older. Methods: The 2014-2018 Korean Longitudinal Study of Aging data were used for 2,202 elderly people. Four status change groups were established according to the change in the number of chronic diseases. The association between the change of MCC and the out-of-pocket medical cost was analyzed using the generalized estimating equation model analysis. Results: The average out-of-pocket total medical costs were 1,384,900 won for participants with MCC and 542,700 won for those without MCC, which was a statistically significant difference (p<0.0001). Compared to the reference group (simple chronic disease, SCD→SCD), the change in multiple chronic conditions significantly increased the total out-of-pocket medical expenses in MCC→MCC and SCD→MCC groups (MCC→MCC: 𝛽=0.8260, p<0.0001; SCD→MCC: 𝛽=0.6607, p<0.0001). Conclusion: In this study, it was confirmed that the prevalence of MCC increased with age, and the out-of-pocket medical cost increased in the case of MCC. Continuity of treatment can be achieved for patients with MCC, and the system and management of treatment for MCC are required to receive appropriate treatment.
