Korea is facing the problems of old aging society recently. In the future the ploblems will become more serious. Specially health care services for the aged are very poor in the aspect of both systems of services and supplies of facilities. This paper aims to identify the types as well as present statistical conditions of health care facilities for the aged in Korea.

병원은 다양한 의료서비스를 제공하기 위해 의료기관 별 운영 전략에 따라 각기 다른 방식으로 운영된다. 특히 종합의료시설은 중증도가 높은 환자를 대상으로 수술이나 입원 등의 의료서비스를 효율적으로 제공하고자 그에 따른 운영 전략이 상이하다. 이러한 병원의 운영방식에 의해 결정되는 건축 역시, 건립 시기, 유형, 중증도에 따른 병원의 규모 등에 따라 시설 수준의 차이가 나타나며 이는 최근 정부에서 요구되는 제도 및 정책의 수용 여부를 결정짓는 중요한 요인이 되기도 한다. 최근 의료법은 기존 의료시설뿐만 아니라 신규 의료시설에 대한 설치 및 운영기준이 강화 추세에 있고, 서울시 또한 공공의료 확충을 위해 감염관리시설이나 필수의료시설 설치를 위한 용적률 인센티브 정책을 추진 중이다. 병원 운영 환경이 상이함에 따라 종합의료시설 설치 및 운영에 대한 인센티브제도의 적용 기준을 일괄적으로 적용하기에는 어렵다. 그러나 공익을 위해서 종합의료시설 인센티브제도를 지속 가능하게 운영하기 위한 객관적이고 합리적인 기준은 반드시 필요하다. 공공의료 기능 확충을 위한 서울시 종합의료시설 지구단위계획 수립·운영 기준은 공공과 민간이 모두 만족하는 의료환경 구축을 위해 매우 깊은 고민이 필요했다. 본 논설은 서울시 공공필요의료시설 확충 제도의 주요 내용과 공공필요의료시설 설치 및 운영 기준에 관해 소개하고자 한다. 특히 기준의 주요 내용을 정립하게 된 배경과 이유 등을 설명하여 본 제도의 의의를 정확히 전달하고 향후 보완해야 할 부분들을 검토하고자 한다.

최근 전 세계적으로 다양한 유형의 재난이 발생하고 있으며, 재난으로 인한 피해를 최소화하기 위한 재난관리 체계구축의 중요성이 대두되고 있다. 국내에서도 마우나리조트, 세월호 사건 이후 국민안전처 신설, 국가안전관리기본계획 발표 등 재난안전과 관련하여 많은 정책적 개선을 추진하였으며, 재난거점병원을 확충하는 등 시설 확보에도 많은 노력을 기울였다. 그러나 미국, 유럽, 일본 등 선진국과 비교하여 인구 당 응급의료시설이 부족한 실정이며, 재난 시 다양한 기관의 미흡한 협조체계로 효율적 재난관리가 이행되지 못하고 있는 실정이다. 따라서 본 연구에서는 재난상황에 요구되는 응급의료시설의 효율적 구축과 체계적 운영방안을 제시하고자 하며, 이를 위해 선진국 대비 국내 응급의료시설의 현황을 파악하고, 재난현장 응급의료시설 유형 및 상황별 구축 방안을 제시하고자 한다.

이 연구는 노인의료복지서비스의 질적 제고를 위해 노인의료복지시설 소속 종사자들을 대상으로 관리의 변수들 간의 구조적인 관계를 살펴보기 이전에 시설유형에 따른 자기효능감과 직무만족에 대한 요양보호사들의 주관적 인식의 차이를 분석해 보고자 하였다. 실증분석결과 요양보호사들이 근무하는 노인의료복지시설 유형에 따른 인식의 차이는 없었지만 전체적으로 요양보호사들의 자기효능감과 직무만족의 충족이 노인의료서비스 제공에 있어서 중요하다는 것을 강조하였다. 따라서 이 연구는 노인의료복지시설 종사자 관리의 효율화 방안을 위한 이론적 기초를 제공하였다는데 그 의의가 있다.

The average life span of Koreans has been increased recently. The percentage of the old(over 65), which was 5% in 1990, is expected to be increased to 15% in 2020. Therefore, the social problems of old people occur in various aspects nowadays, and will become more serious in the near future. In Germany, the have developed a comprehensive welfare system which enables the aged to be more independent, such as various welfare facilities for the aged that are built in consideration of the physical conditions.

이 연구는 노인의료복지시설 소속 요양보호사들을 대상으로 자기효능감과 직무만족에 대한 요양보호사들의 주관적 인식의 차이를 분석해 보고자 하였다. 분석결과 요양보호사들이 근무하는 노인의료복지시설 유형에 따른 인식의 차이는 없었지만 전체적으로 요양보호사들의 자기효능감과 직무만족의 충족이 노인의료서비스 제공에 있어서 중요하다는 것을 강조하였다. 따라서 노인의료복지시설 소속 요양보호사들이 노인들에게 의료서비스 등을 제공함에 있어서 직무만족과 동시에 지속적으로 높은 수준의 자기효능감을 유지할 수 있도록 하는 인적자원관리전략이 필요함을 시사하였다는데 그 의의가 있다.

This study is to understand the architectural types and characteristics of the medical facilities during ancient period. The study is based on the research of the medical and architectural history. The medical or healthcare facilities are influenced by their social, cultural and conceptual idea, especially how they think about 'disease', 'cure' and the 'medicine'. As the results of the examination of this study can be summarized as followings. Firstly, Ancient medical facilities are classified into four types according to the ideas of 'disease' and 'cure' ; 1) God oriented facility 2) health welfare facility 3) treatment oriented facility 4) practice & educational facility. Secondly, there are three typical types of the spatial characteristics what modern hospitals have ; 1) nursing ward 2) treatment ward 3) hostel or hospice. And they are all assembled around the courtyard in common. Thirdly, their architectures are not 'designed' but 'derived' plan by reasonable other building types, and transformed by medical function. Fourthly, a consideration of the surrounding circumstances is the most important point to make the medical health facilities during ancient period.

Elderly people should have a great load because of life in facility away from their a community. It needs careful concern for Public spaces. The mayor questions of this thesis are to seek a correlation of a Layout patterns of Public-spaces and Resident's behaviors in Public spaces ; and to seek a correlation of a Layout patterns of Public-spaces and the scale of Paid residential care facilities for Elderly people in Korea and Japan. Finally, the thesis suggests the layout patterns of public spaces that is suitable to scale of facilities and Elderly people's behaviors.

이 논문은 노인의료복지시설의 현황과 시설체계에 관한 논문으로 부산시를 대상으로 하였다. 현황 분석과 설문조사을 통해 그에 따른 문제점을 분석하고, 해결방안으로 새로운 유형의 시설체계를 제시하였다. 현재 노인의료복지시설들은 입소대상자들을 다양하게 수용하고 있지 못하며, 각각 독립적으로 운영되고 있어서 한 시설 안에서 전문적인 치료와 요양을 동시에 할 수 없다. 이러한 문제점들을 토대로 본 논문에서는 그 해결방안으로 좀 더 넓은 범위에 걸쳐서 한 장소에서 요양과 전문적인 치료가 가능한 'one-stop service system' 체계를 제언하였다. 설문조사 결과들을 종합하여, 구상한 5가지 유형의 시설체계는 다음과 같다. 1) 무료전문요양시설 + 노인전문병원, 2) 유료전문요양시설 + 노인전문병, 3) 노인전문병원 + 실버타운 주거공간 형식의 입원실, 4) 실비전문요양시설 + 노인전문병원 + 실버타운 주거공간 형식의 입원실, 5) 노인전문병원 + 재가복지서비스. 노인관련시설은 더 이상 특수한 시설이 아니라, 누구든 언젠가는 이용 하게 될 시설이라는 점에 착안할 때, 그에 대한 대책 마련은 시급하다고 생각된다. 앞으로 노인의 질환의 종류 및 정도가 보다 중복, 중대화 될 것으로 보인다. 따라서 보다 다양하고 전문화된 시설이 요구될 것으로 예상된다.

Facing the aging society in Korea, the supply of elderly care services has to be increased. Among these services short-stay care service is one of the alternative service connecting home and communities and this service is at the beginning stage in Korea. This paper aims to identify the current status and characteristics of short-stay care facilities in Korea. On the bases of survey and analysis the basic data for architectural planning and design of short-stay care facilities for elderly have been proposed.
