A study on the standardization for outpatient management and adminstration process of some regional hospitals (일부지역 의료기관의 외래원무관리 표준화에 관한 연구)

  • This study evaluated the standardization of the outpatient management process in several regions of South Korea. The hospital workers in the administration department of the different medical institutions that are registered with the Korean Hospital Association were surveyed. These institutions can be standard hospitals or hospitals greater in size. A summary of the research results are as follows. There was no significant correlation in their registration procedures in relation to the institution's founder, number of sickbeds, number of staff employed in the administration department, and average number of outpatients per day. On the other hand, the prepayment of medical fees occurred more frequently when the number of sickbeds was larger. In addition, there was no large difference in their appointment procedures in relation to those features. Nevertheless, the prepayment of medical fees accounted for 11.8 % of the entire payment in institutions with less than five hundred beds, while fifty percent of the payment was made in advance in larger institutions with five hundred or more beds. From this research, there was only a small difference among the institutions' outpatient management, but a notable difference was observed in their electronic data processing systems and facilities. Therefore, more financial support should be generated for the implementation of a more integrated process. As a follow-up study, to provide patients with quality medical services, it will be necessary to apply the standardized procedure to an actual medical institution and analyze the expected effect.

The Opinions of Dental Related Major Students on Dental Hygienists (치과관련학과 대학생들의 치과위생사에 대한 인식조사)

  • This study investigated perceptions and opinions on medical personnel related to dental hygienists. The subjects of this study were 25 students, including dentistry students, dental hygiene students, dental technology students, and nursing students. The survey items consisted of general information, the pros and cons of medical hygienists and their reasons, the scope of legal work, medical practice, and the professionalism of dental hygienists. Frequency analysis and 𝑥2-tests were used for the analysis method. As a result of the study, the pros and cons of the dental hygienist's medicalization were 56.2% in favor and 40% in opposition. There were significant differences in the opinions of the dental hygienists involved with medical hygiene by 90% and 25% in nursing students. In addition, there was a significant difference in the perception of the legal work of dental hygienists and the rate of recognition of medical activities among the departments. In conclusion, it is expected that understanding and support for dental hygienists will be achieved only if proper knowledge and education about dental hygienists are made available in other departments.

Problems of the Current Referral System of the Terminal Cancer Patients in Korea (말기 암 환자의 완화 의료 연계 시스템의 문제점과 개선 방안)

  • Purpose : The system to refer terminally ill patients to palliative or hospice care which ultimately give them emotional, psychological, and social support hasn't been fully developed and organized yet in Korea. The controversies concerning the current referral system are being analyzed to present the improvements. Methods : The questionnaires were asked to be filled out by family members of the 76 patients by phone interview, who were referred from the Seoul National University Hospital between April, 2001 to March, 2002. They were referred to the 35 palliative and hospice care-giving institutes and hospitals which were given questionnaires by mail. Results : Of the 76 patient's family members, 47 family members accepted to answer the questionnaire. The first thing that influence to family to determine the referral of patient was solicitation of doctors or nurses (44%). And they were influenced by allowance for the other things such as convenience of patients (32%), convenience of caring family members (24%). In the course of determining of referred institutes, responders had considered at first their dwelling area, and then fame of institutes, the place which patent had wanted to spend last hours or which is suitable for patient's funeral service, and their financial condition. Thirty-eight the 47 responders answered that they had experienced difficulties in referral procedure. The worst among difficulties was unwanted discharge, and followings were lack of information about the referred institutes, concern about patient's suffering, resistance of patient and opposition of other family members, etc. Although they expressed dissatisfaction in referral procedure, most of them answered they had been satisfied with hospice care at referred institute after referral. Merits of referral which responders counted were patient's peace, caring family's comfort and reduced cost in order. Of the 35 referred institutes, 24 institutes' staffs responded mail questionnaires and sent to us in return. Except one responder, the rest approved the referral system and thought that referred patients had been satisfied with their hospice care. And they claimed that systemic support of the government is definitely necessary. The most difficult thing which responders experienced in care of referred patients was lack of information about patients. Besides, there were patient's financial problems, lack of understanding about their institutes of patients or family, and inconvenience of terminal cancer patient's pain control. Conclusion : The development and support of the organized referral system is needed to alleviate the troubles which patients, family members, and palliative or hospice institutes and hospitals have to face through the procedure of the referral.

An Empirical Study on Emergency Medical Care Transportation Policy (응급의료 이송정책에 관한 실증적 연구)

  • This research made a survey to 119 EMT laying stress on general contents connected with job in a frame of mutually organic cooperation system between the processes, composing Emergency Medical Care Transportation Policy in Korea, as a step before hospital, of happening emergency patients, 119 first-aid service of the spot, transportation of patients, construction of communication network etc.. As a result of analysis to that, it is found that there must be systematic devices which makes EMT not to be caught on medical dispute, a modernization of emergency equipments, professional first-aid agents, a proper personnel arrangement. Consequently, it suggests policy plan focusing on structural and functional aspect to improve an Emergency Medical Care Transportation system into a realistic one.

Factors Affecting the Consciousness of Biomedical Ethics of Nursing Students (간호대학생의 생명의료윤리의식 영향 요인)

  • The aim of this study was to identify the factors influencing the consciousness of biomedical ethics in nursing students. The study was conducted on 516 nursing students in C Province and from September $15^{th}$ to October $5^{th}$ 2013. The data was analyzed using a t-test, one-way ANOVA, Pearson's correlation and multiple regression. The significant positive correlation with a critical thinking disposition and professional self-concept. Significant factors influencing the consciousness of biomedical ethics were the critical thinking disposition, experience of biomedical ethic education, academic grade, professional self-concept, and family members' involvement in medical related areas. Considering this result, the development of educational programs of biomedical ethics for nursing students should reflect affecting factors identified.

A Comparative Study on the Satisfaction of Korean and Canadian Primary Care Based on the Survey of Overseas Korean in Canada (캐나다 재외국민 설문조사에 기초한 한국과 캐나다 1차 의료기관 만족도 비교 연구)

  • This study compares and analyzes satisfaction with Korean and Canadian primary medical care based on a survey of Koreans living in Canada. Based on data collected from Koreans who live in Canada, a paired sample t-test, analysis of variance (ANOVA), and GLM were used. The main conclusions of this study are as follows. First, overall satisfaction with Korean primary care was high. Second, the results of the analysis were stable, regardless of gender, age, education, and residential area. Third, overall satisfaction with Korean medical care was higher than for medical care in Canada. Fourth, satisfaction was high in terms of treatment skill and technology, and promptness in appointments, but was low in terms of medical expense. Fifth, there was a high level of confidence in physician skills and practices, but there was no significant difference in terms of sufficient counseling and explanations. Finally, the survey found that the tendency to increase medical treatment volume in order to increase a physician's own income was large. As a result of detailed analysis, it was concluded that Korea's primary care is more competitive than in Canada, but the primary medical doctors' patient interview and explanation obligations, and incentives to control income and medical expenses, need to be improved.

Change in patient safety, quality of medical care, organization culture and hospital management performance after healthcare accreditation system for convalescent hospital (요양병원 의료기관인증제 시행 후 환자안전, 의료의 질, 조직문화 및 병원경영성과의 변화)

  • This study was conducted to investigate the effects of implementation of the healthcare accreditation system on patient safety, quality of medical care, organization culture and management performance of convalescent hospitals. Ten hospitals were selected at random from among 36 convalescent hospitals in Daegu that had obtained accreditation until September 2015. To accomplish this, hospital staff were surveyed from 15 February to 25 March, 2016 using a questionnaire with 253 objects. We investigated how the healthcare accreditation system has influenced patient safety, service quality, organization culture and performance of hospital management. The change in the treatment support system showed the highest score of 3.93 among quality change and medical service items, while that of personnel source culture showed the highest score of 3.78 points among organization culture items, and that of internal process view, study and growth view showed the highest score of 3.71 among management performance items. Multiple regression analysis revealed that factors positively changing hospital management performance were nurses, employees and managers, basic value systems, organization culture, personnel source culture, open system culture, hierarchical culture, and production oriented culture. The quality of patient safety and medical care was positively influenced by changes in the quality of convalescent hospitals after implementation of the healthcare accreditation system. However, among the four types of organizational culture, the financial perspective was relatively low compared to other perspectives.

The Impact of Satisfaction with Major Curriculum on Acquisition of Certification and Employment for College students with Major in Healthcare (대학생들의 전공 교육과정의 만족도가 자격증 취득 및 취업에 미치는 영향)

  • 목적: 보건의료 관리 분야의 진로는 급변하는 사회적 요구와 시대적 흐름에 발맞추어 다양한 형태로 변화하여 왔다. 본 연구에서는 학습 수요자 측면으로 교과과정에 대한 만족도와 보건의료 계열 취업에 영향을 미치는 요인을 분석하여 보건의료 계열 학과 학생들의 취업률을 향상시킬 수 있는 방안을 도출하고자 한다. 방법: 경기도 성남시 소재 보건의료 계열 학과 163명을 대상으로 2013년 5월 6일부터 5월 9일까지 설문조사를 거쳐 분석을 실시하였다. 결과: 성별에 따른 교육과정의 만족도는 통계적으로 유의하지 않았으나, 고학년으로 올라갈수록 교육과정에 대한 만족도가 낮은 것으로 분석되었다. 성별과 학년에 따라 자격증 취득 계획 여부는 통계적으로 유의한 차이를 보였다. 즉, 성별이 여학생이고 학년이 낮을수록 전공관련 자격증 취득 계획이 높았다. 전공관련 자격증 취득에 영향을 미치는 요인으로는 '성별', '학년(학번)', '보건의료계열 분야로의 취업 여부'가 유의하게 영향을 미치는 것으로 나타났다. 보건의료계열 취업 여부에 영향을 미치는 요인으로는 '교육과정 만족도', '전공관련 자격증 취득 계획'이 통계학적으로 유의하게 영향을 미치는 것으로 분석되었다. 결론: 보건의료관련 학과 학생들의 동일계열 취업률을 향상시키기 위해서는 전공관련 자격증 취득에 직접 간접적으로 영향을 미치는 학생들의 요구를 파악하여 교육과정에 반영하는 것이 중요할 것이다.

Choosing Patterns for Dental Clinic among Community People (지역사회주민의 치과의료기관선택에 관한 연구)

  • This study was conducted to find out the factors affecting community people choice of dental clinic so that the results could be served as basic information to formulate marketing strategies for dental clinic. 238 people in Ulsan metropolitan city were surveyed through a self administered questionnaire. 1. All the Cronbach's ${\alpha}$ values related with dental clinic's internal factor, external factor and access were more than 0.6. 2. The most important factor of dental clinic choice was dentist's faithfulness. 3. On the occasion of choosing dental clinic, internal factor was more important than external factor. 4. The most important factor of dental clinic choice was dental clinic's access on more than 46 years old.

An Empirical Study for the Management Performance of Primary Medical Centers (1차 의료기관의 경영성과에 관한 실증적 연구)

  • This study investigates the management performance of primary medical centers on the basis of power process perspective to suggest the direction for their competitive advantages. To do so, this study conducts the questionnaire survey to middle and top managers of primary medical centers in Seoul, Incheon, and Kyoungki Province and analyzes the data by using cluster analysis and variance analysis of SPSS statistical package. According to the results, the types of primary medical centers are determined by the interaction of power determinants variables, and the type of primary medical centers which has the strongest interaction of power determinants variables shows the highest management performance compared to other types.
