우리나라의 병원 전 응급의료체계는 비교적 짧은 기간 동안에 소방서를 축으로 운영되고 있다. 병원 전 단계에서 응급처치의 적절성을 높이기 위해서는 119구급대에 의한 응급처치에 대하여 체계적으로 조사를 시행함으로써 기초 응급의료에 대한 지속적인 질 관리를 해 나가는 것이 매우 중요하다. 특히 뇌경색은 급성기 치료를 위하여 혈전용해제 치료가 중요하며 증상 발현 후 치료가 시작되기까지의 소요시간이 매우 중요하므로 뇌경색 환자의 발생과 동시에 응급의료체계의 반응시간과 뇌경색을 인지하기 위한 환자평가 및 인지 후 응급처치 정도를 소방 119구급서비스를 통하여 살펴보고자 한다.

As the well-being mindset to keep our minds and bodies free and healthy more than anything else in the society we live in is spreading, the meaning of health care has become a key part of the 4th industrial revolution such as big data, IoT, AI, and block chain. The advancement of the advanced medical information service industry is being promoted by utilizing convergence technology. In digital healthcare, the development of intelligent information technology such as artificial intelligence, big data, and cloud is being promoted as a digital transformation of the traditional medical and healthcare industry. In addition, due to rapid development in the convergence of science and technology environment, various issues such as health, medical care, welfare, etc., have been gradually expanded due to social change. Therefore, in this study, first, the general meaning and current status of digital health care medical information is examined, and then, developmental tasks to activate digital health care medical information are analyzed and reviewed. The purpose of this article is to improve usability to fully pursue our human freedom.

병원 전 단계에서는 일반인 단계, 일차 반응자 단계, 기초응급의료 제공 단계 그리고 전문응급의료 제공단계로 구분되는데 각 단계의 적절한 병원 전 응급의료 서비스의 제공은 환자의 예후에 긍정적인 영향을 미치는 것으로 알려져 있다. 그러나 국내의 일반인 단계 및 일차 반응자 단계의 응급의료 제공은 매우 낮은수준이며, 응급의료체계는 전문 응급의료가 뒤따르지 않는 기초 응급의료만을 제공할 뿐인데도 그 적절성에 대하여 많은 문제점이 지적되어 오고 있다. 우리나라의 병원 전 응급의료체계는 비교적 짧은 기간 동안에 소방서를 축으로 운영되고 있다. 병원 전 단계에서 응급처치의 적절성을 높이기 위해서는 119구급대에 의한 응급처치에 대하여 체계적으로 조사를 수행함으로써 기초 응급의료에 대한 지속적인 질 관리를 해 나가는 것이 매우 중요하다. 이러한 질 관리의 평가를 위해서는 적절한 평가도구가 필요한데 객관성이 높으면서도 비용-효과적인 도구와 지표는 아직 확립되지 못하고 있는 것으로 보인다. 그동안 119 구급대에 의한 응급처치의 적절성에 대하여 몇 차례 보고가 있었지만, 단일 기관 또는 단일 질병을 대상으로 수행된 연구였거나 일개 응급의료기관으로 이송된 환자들만을 대상으로 했던 비교적 소규모의 보고들이었으며, 한 도시를 전체를 대상으로 한 광범위한 조사가 이루어진 적은 아직 없었다. 이에 이 연구에서는 대구 경북 지역의 119구급대원을 대상으로 병원 전 응급처치의 실태와 및 교육현황을 조사하여 응급처치의 질적 향상을 위한 프로그램 개발에 필요한 기초 자료를 제공하고자 한다.

Since Precision Medicine deals with individual health and life, the issues of personal information protection and health and medical information protection are emerging.Accordingly, this paper presents security improvements by domestic and foreign standards, laws, and systems in the cloud and medical field, and proposes a plan to protect healthcare and medicine information protection for safe Precision Medicine.

Please Interests in the medical service are increasing in internet environment as life quality of the people improves because of development in information and medical technology. The medical information in today's modern internet environment can violate privacy of the patients. Many medical institutions in Korea are very passive in the privacy protection of patients in the internet environment. The law, standard scheme and systematic guidance to prevent drain of medical information are not developed. This study examines cases of infringement pattern on information of each patient in the internet environment. This study will also try to find a solution to protect the personal information of patients in the internet environment in the measures of law system, technique and management.

Recently, RFID technology can successfully be used to reduce medical errors. This technology can aid in the accurate matching of patients with their medications and treatments. The enthusiasm for using RFID technology in medical settings has been tempered by privacy concerns. In this paper, we propose a secure and efficient RFID authentication system to not only authenticate patients' authenticity but also protect patients' personal medical informations. The proposed system consists of RFID-based patient authentication protocol and database security protocol. As a result, since the proposed RFID authentication system provides strong security and efficiency, it can be used practically for patient authentication and personal medical information protection on the high technology medical environments such as u-Hospital and u-Healthcare.

컴퓨터 보급의 급속한 발전과 멀티미디어의 등장으로 기존의 텍스트와 이미지, 음성, 오디오, 동영상 등의 정보를 디지털화하고, 컴퓨터를 이용하여 저장, 처리, 전송하게 되면서 의료 분야에서도 상당한 업무의 변화를 요구하게 되었다. 의료 분야에서의 이러한 급격한 개방과 더불어 초고속 정보 통신의 발달은 원격진료라는 또 다른 요구를 대두시키고 있다. 이를 위해서는, 멀티미디어 기술, 대용량의 정보를 저장하는 데이터베이스 기술, 초속의 광 대역 기술 등을 통합하여 종합적인 멀티미디어 의료 정보 시스템을 구축하는 것이 시급하다. 이러한 이유들로 본 논문에서는 병원/의원의 의료진들로 하여금 의료영상이나 자료를 상호 전송하여 환자의 진료 또는 검진결과를 확인하고 전문가의 조언 등을 구하는 원격 진료용 의료영상 시스템의 요구사항을 분석, 설계하고 구현하였다. 본 시스템은 클라이언트/서버 구조로써 영상 획득 및 출력, 의료영상 국제 표준 포맷인 DICOM 포맷으로의 영상 저장, MCA(Multi Channel Analyzer), ROI(Region Of Interest) 등의 영상 분석, 필터링 및 영상 확대/축소/회전 등의 각종 영상 처리의 주요 기능을 갖으며, 사용자가 편리하고 쉽게 사용할 수 있도록 아이콘(icon) 중심의 직관적인 인터페이스를 갖는다.

The development of information technology such as the internet has brought about rapidly changes the old medical technology, e-Healthcare has been to raise social issue. The e-Healthcare which new turning point of paradigm in the medical information develops the medical policy in Korea and the technology, the prospective of reverse engineering in internet environment is incurring problems such as distribution of critical information and invasion and infringement of privacy, etc. In this research, we suggest the Role Based Access Control System, HPIP-e-Healthcare Privacy Information Protection, for solving above problem. The HPIP is composed 4 mechanisms such as Consolidate User Identity, Hospital Authorization, Medical Record Access Control, Patient Diagnosis and we are also prototyping the HPIP for feasible approach in the real computing environment.

국가 보건의료체계(health care system)가 올바르게 기능하기 위해서는 의료체계를 구성하는 각 요소들간의 유기적인 상호 작용이 대단히 중요하다. 정보체계는 이러한 유기적인 상호 작용을 위해 보건의료체계의 각 영역에서 꼭 필요한 시스템이다. 보건의료 서비스를 효율적으로 생산하기 위해서 투입하는 자원의 개발과 배치 체계는 물론 생산된 보건의료 서비스를 소비자가 효과적으로 이용할 수 있도록 하는 서비스 전달체계(delivery system)의 전과정에서 보건의료정보체계는 중요한 인프라가 되고 있다. 정보체계는 궁극적으로 보건의료체계가 지향하는 목적을 달성할 수 있도록 짜여져야 한다. 다시 말해, 의료체계의 각 하부영역에서 의료이용의 접근성(accessibility), 형평성(equity), 서비스의 의과학적 질(scientific technical quality), 포괄성(comprehensiveness) 등 보건의료체계가 추구하는 가치를 정보체계에서도 그대로 담아 낼 수 있어야 한다. 또한 정보체계를 구축하기 위한 요건으로 각 사업 영역별 표준화, 사업 영역간의 상호관계, 공공과 민간의 역할 분담, 정보 관련 시선과 인력, 정보기술 등의 인프라, 각 사업 영역별 정보화에서 담아야 할 정보 내용(contents, DB) 등에 대한 사전 기획이 수립되어야 한다. 그 동안 보건의료분야의 정보화사업은 1996년부터 보건소기관의 행정, 진료, 보건 업무지원을 위한 보건소정보화사업과, 1995년부터 초고속공공응용서비스 사업의 일환으로 추진해 온 전염병, 혈액유통, 장기이식, 외래진료예약, 원격진료, 응급의료, 원격치매진료 시스템 구축사업 등이 있으며, 응급의료, 혈액관리, 장기이식, 전염병정보, 외래진료예약시스템을 통합한 보건의료정보통합서비스시스템(1998년) 구축사업 등이 있었다. 이러한 사업들에 대해 현재 평가되고 있는 문제점들로는 정보화 사업에 대한 전략과 비젼이 부재한 상태에서 지나치게 정보기술 중심으로 추진되다 보니 보건의료 정보의 공유나 활용이 전혀 이루어지지 않거나 매우 제한적이었고, 시범사업의 경우는 초기 사업을 추진하기 위한 주체가 명확하지 않았고, 관련 인력의 훈련과 양성이 안 되었으며, 사업 확대를 위한 법적 기반의 취약, 정보화 사업이 부실한 데 대한 평가 기준과 대안 제시 등이 매우 취약한 것이 문제점으로 나타났다. 특히, 정보 내용과 DB 구축을 위한 표준화 등에 대한 사전 설계 부재는 개발된 정보시스템의 운영을 결정적으로 어렵게 만든 요인이 되고 있다. 전반적으로 기존의 정보화사업은 사업 추진을 위해서 꼭 필요한 비젼 수립, 비젼에 따른 목표 설정, 전략 개발, 관리체계 구축, 운영방안 수립 등 단계적인 정보화 계획이 부재한 것으로 평가되고 있어 보건의료정보체계 구축을 위한 종합 비젼 기획(ISP)이 시급한 실정이다.
