Health Promotion Through Healthy People 2010 ("2010년대 건강한 시민" 정책을 통한 미국의 건강증진 방향)

  • 뉴저지주 보건교육/건강 증진정책을 논하기전에 건강증진과 보건 교육사의 뜻을 먼저 기술하기로 한다. 건강증진이란 일상 사회생활과 행동과학의 응용에서 시작하며 교육의 효율적 작전 및 기술, 질병 역학 조사, 개인 및 가족단위 건강 위해 행위 절감, 사회연관 구축망 조성, 그리고 적게는 이웃, 더 나아가 조직체계 및 지역 사회의 네트웍 실시등을 실시한다. 보건교육 및 건강증진 전문가란 ' 전국 보건교육 인증 위원회(NCHEC) ' 에서 채택된 다음 7개 활동 영역에서 개인적, 그룹, 각주단위, 그리고 범 국가적 조직에서 종사하는자로 한다. 개인 및 지역사회 보건 교육 필요성 분석- 계회, 실행, 효율성 평가, 사업 진행 조정, 자문, 컴뮤니케이션 등의 활동범위를 들 수 있다. 공인 보건 교육사(CHES)란 대학 및 대학원에서 보건 교육학 소정의 필수 과목을 이수하고 학.석사 소지자로서 ' 전국 보건 교육 인증 위원회 ' 에서 그 자격을 인정 받고 공인 자격 시험에 합격한자로 한다. 합격자는 자기 성명뒤에 CHES란 칭호를 부치며 매 5년마다 75단위이상 인정된 전문 직업 보수 교육을 받아야 한다. 보건 교육사 고용 분야는 연방, 주, 지방 정부의 보건 교육사(10-15%) 및 건강 증진 전문가로 종사하며; 이들은 지역 사회 조직화, 프로그람 기획, 공공사업 마켓팅, 메디아, 컴뮤니케이션 자질을 갓추어야 하며; 상해 예방, 학교 보건, 지역 사회 영양 실태 향상, 그 외 모든 건강 증진과 질병 예방에 일익을 담당 하여; 의사, 간호사, 약사, 영양사,환경 위생사드의 전문분야종사자들괴 한팀이 되어 지역 사회 보건 사업에 기여한다. 쥬저지 보건 교육사들은 주법령 8조 '||'&'||' 보건행정 표준 시행령 ' 에 따라 포괄적 보건교육/건강증진 프로그램을 개발하여 총체적으로 조절 관장한다. 특희 ' 미국 학술원 의료 연구원 ' 에서 제정한 ' 10대 필수 공중 보건 사업 ' 에 기준을 두고; 1) 개인 및 지역사회 필수 보건 여건 분석 평가, 2) 보건 교육 이론에 따른 사업 계획 설정, 3) 교육 전략과 보건문제 발굴에 따라 일반 대중 대상 보건 교육 실행 (프로그람 기획, 연수 교육, 미디어 캠페인, 공중보건 향상책 옹호), 4) 사업 진행 과정 정리, 그 결과에 대한 영향력과 결과 평가, 5) 프로그램진행, 인사 및 예산관리 참여, 6) 근무향상을 위한 보수교육 프로그램 개발, 7) 보건 의료 업무 종사자 상호 협조성 향상 훈련, 8) 지역 사회자원 밭굴, 9) 적절한 고객 의뢰 체제 시행, 10) 위기 관리 컴뮤니케이션 체제 개발실시, 11) 일반 대중에게 공중 보건 향상 고취, 12) 각종 협력 지원금 신청서 작성 제출, 13) 문화/인종적으로 적절한 시청각 교재 발굴, 15) 질적 및 양적 보건교육/건겅증진책 연구 실시, 16) 비 보험 가담자, 저 보험자, 빈곤자, 이민자 색출 선도, 17) 관활 구역내 상재하는 각 건강증진 프로그램 밝혀 내서 불필요한 중복 회피등이다. 그 외에도 보건 교육사들은 사회 복지 단체인 미국 암 협회, 미국 심장 협회,미국 폐장 협회 등 각종 사회 복지 비영리단체 와 자선 사업 단체들과 긴밀희 협조하거나 그 단체 임직원으로서 건강 증진 사업에 종사한다. 병원 및 의료기관에선 임직원 보수 교육, 환자의 질병 예방및 건강증진 교육, 그리고 의료 사업장내 건장 증진업무에 종사한다. 건강 유지 의료 기관(HMO)에선 예방주사, 정기검진 촉진등을 통한 입원일수 절감, 응급실 사용도 절감등으로 의료비 감축, 삶의질 향상상에 종사한다. 사업장 보건 교육사는 스트레스 관리, 금연 및 흡연 중단선도, 체중 절감, 종업원 건강증진 생활화참여 유치, 컴뮤니케이션 개발, 마켓팅, 질병 예방등에 그 전문 직업적 노하우를 사업체 건강 증진 프로그램 개발에 접목한다. 뉴저지 2010년대 건강 증진책은 5대 목표 설정하여 현재 시행하고 있다. 특이한점은 2001년 9.11사태 이후 연방정부와 주정부의 상당한 예산 지원을 그랜트 지원금 형식으로 받아 연방, 주정부, 지방 정부, 의료 기관등에서 일사 불란하게 생물/화학/방사성 테러에 대비하는데 보건 교육사들은 시민 인지도 향상과 위기관리 컴뮤니케이션 영역에서 활약한다. 총체적인 보건 교육/건강 증진책은 다음 천년간 뉴저지 건강증진 백서와 미연방 정부 건강증진 2010에 준하여 설립한 뉴저지 건강 증진 2010 에 의한다. 그 모델을 보면; 1) 생활 습관 향상으로 위해 행위 절제; 적절한 영양 섭취 와 과체중화 차단 불필요한 투약 절제와 그 관리 흡연 탐익 절감, 금연, 흡연관련 신체/정신적 피해 관리/치료 습관성 약물 중독 조기발견 예방 낙상 예방 폭력, 의도적/비의도적 상해 예방 2) 심장질환, 암, 뇌졸중, 당뇨, 폐염, 인프루엔자등 주사망원인 질병 조기 발견 예방 책 마련; 독감.폐렴 예방 주사 실시 3) 보건 교육 대상과 표적 설정 특히 보건사업 참여 동반자 발굴하여 그 동참과 책임분담 책려; 주. 지방 정부기관, 의료 종사자, 의료 보험 업자, 대학 등 교육 기관, 연구 기관, 교육자, 지방 보건소, 지역 사회 비 영리단체, 종교 단체 및 교역자 등의 참여 촉구., 지역 사회 비 영리단체, 종교 단체 및 교역자 등의 참여 촉구.

Development of Evaluation Index for Infection Control and Prevention at Dental Hospital and Its Validity Verification (치과의료기관 감염관리 평가지표 개발 및 타당성 검증)

  • This study is to develop an evaluation index for infection control and to verify its validity by examining each set of weighted data collected from 121 infection control personnel at dental hospitals who agreed to the preliminary survey and advisory. The study was conducted from 14th December, 2010 to 31st January, 2011, and PASW Statistics 18.0 and AMOS 5.0 had been used for the statistical data analysis. As a result of the study, five evaluation factors with 21 sub-indicators have been identified at structural level, eight evaluation factors with 32 sub-indicators at processing level, and one evaluation fact with five sub-indicators at resulting level, total 14 evaluation factors with 58 sub-indicators throughout all levels. The path analysis added on the result that 'standard precautions ($x_1$)', 'infection control support system ($x_2$)', 'internal and external characteristics ($x_3$)' are exogenous variables that affect on other variables, and 'standard infection control ($y_1$)','Organization equipment management handwashing ($y_2$)', 'environmental infection control ($y_3$), 'personal protective equipment ($y_4$)', 'waste and laundry management ($y_5$)' are endogenous variables that are infulenced by others. The standardized metrics are more needed than anything else when examining on infection control. This study attempts to develop proper dental infection control metrics adequately adjusted for domestic circumstances, and therefore to contribute to effective systematic management and decision-making in infection control.

The Effect of Occupational Stress among Occupational Groups Related to Healthcare Accreditation on Turnover Intention (의료기관인증평가 관련 직무 스트레스가 직군간 이직의도에 미치는 영향)

  • Purpose: This study aims to investigate the factors influencing health personnel's occupational stress on turnover intention regarding healthcare accreditation. Methods: A survey was conducted from May 17 to May 31, 2021, among participants to examine occupational stress and turnover intention among health personnel working at a 188-bed specialized hospital in Seoul that is preparing for healthcare accreditation. Results: Occupational stress regarding healthcare accreditation was found to have a positive correlation with turnover intention (r=.698, p <.001), and influenced turnover intention the most, which explains the variance of 55.8% (F=29.015, p <.001). There were significant differences between occupational groups in job stress (F=13.292, p<.001) and turnover intention (F=10.930, p <.001) in the healthcare accreditation. Conclusion: Occupational stress regarding healthcare accreditation is higher in nursing than in other occupations, indicating the need to lower the turnover intention of nurses by preparing a national institutional standard for nursing manpower and also put in place an appropriate compensation system for each hospital seeking accreditation.

Development of Evaluation Indicators for the Accreditation of Small-Sized Dental Hospitals within Local Communities (지역사회 중·소규모 치과의료기관 인증 평가지표 개발)

  • Objectives : The aim of this research was to develop evaluation indicators that ensure safe services and enhance the quality of small-sized dental hospitals within local communities to prepare for the dental hospital accreditation system. Methods : Materials and data from a total of 460 respondents were analyzed after quality improvement indicators were formed by surveying experts and studying documents in Korea and abroad, and by conducting a survey from August 21, 2013 to September 30, 2013 for validation purposes. Results : Based on the results of factor analysis. all measured items appeared to be statistically significant with a factor loading score of at least 0.5, and the correlation between the factors all appeared to be high as well. The goodness-of-fit for the model also appeared to be within the recommended acceptance level. Thus, the proposed evaluation indicators for dental hospital accreditation developed in this study consisted of a total of seven major category factors and a total of 91 detailed items. Conclusions : Furthermore, the results suggest that the evaluation indicators can be used to improve the quality and to ensure the safe services of dental hospitals in local communities.

Working Conditions that Impact the Workload of Cytotechnologists: A Study Calculating the Actual Man Power Required (세포병리사의 업무량에 따른 적정인력 산정을 위한 업무실태 조사 연구)

  • Cytotechnologists evaluate and analyze disorders of cells that constitute the human body, and are involved in the primary assessment of diverse diseases, including cancer. However, the employment conditions and workload of cytotechnologists are poorly understood. This study was undertaken to provide basic data for establishing the criteria for quality control certification factors based on the scope of effective task performance of cytotechnologists, and to provide results of their workload analysis according to the type of medical institution. The study was conducted by enrolling certified cytotechnologists working at various nationwide medical institutions. Our analysis revealed that 178 personnel (72.7%) were involved in primary screening of samples. On an average, the daily number of primary screening of samples performed per cytotechnologist (76 respondents) was determined to be 75.4 chapters (16.8 chapters/hours) at the university hospital level, 72.4 chapters (18.6 chapters/hours) at the general hospital level, and 231 chapters (32.6 chapters/hours) at professional trust institutions. Our results indicate the necessity to establish a consultant with the Korean Cell Pathology Association, to enable finding solutions to solve existing issues by establishing accurate standard guidelines for assessing cell screening.

Reversible Data Hiding and Message Authentication for Medical Images (의료영상을 위한 복원 가능한 정보 은닉 및 메시지 인증)

  • Nowadays, most hospitals have been used to create MRI or CT and managed them. Doctors depend on fast access to images such as magnetic resonance imaging (MRIs), computerized tomography (CT) scans, and X-rays for accurate diagnoses. Those image data are related privacy of a patient. Therefore, it should be protected from hackers and managed perfectly. In this paper, we propose a data hiding method into MRI or CT related a condition and intervention of a patient, and it is suggested that how to authenticate patient information from an image. In this way, we create hash code using HMAC with patient information, and hash code and patient information is hided into an image. After then, doctor will check authentication using HMAC. In addition, we use a reversible data hiding DE(Difference Expansion) algorithm to hide patient information. This technique is possible to reconstruct the original image with stego image. Therefore, doctor can easily be possible to check condition of a patient. As a consequence of an experiment with MRI image, data hiding, extraction and reconstruct is shown compact performance.

A Survey on the Public Awareness of the Bioinformation Utilization (바이오정보 활용에 대한 시민 인식 조사 연구)

  • In Korea, currently, only the growth possibility of bioinformation industry has received the public attention, and social discussion on bioinformation management is meager. Based on this critical perspective, this study investigates the current status of public awareness of the bioinformation which can be a basis for the social discussion for bioinformation management. The most significant characteristics identified in the survey is that many citizens, while expressing their agreement on the use of biometric technology by purpose and taking an open stance on national fingerprint information collection, have considerable concern for the identity authentication system using the current biometric technology. In particular, the survey pointed out that it is hard to trust the institutions that collect the bioinformation. Also, the public expressed the particular concern for the health data including the body information and showed overwhelming consent for the necessity of more stringent regulation compared with other personal information. In regards to the medical information gathered from medical institutions, nearly 80% of the survey respondents did not agree on providing the information to commercial enterprises, and even 60% of them disagreed on using it for academic and statistical research. This result well demonstrates that thorough management of the bioinformation is required for its utilization.

Gross Alpha Analysis of Nasal Smear Samples and Internal Dose Assessment Procedure in Radiation Emergency (방사선비상시 비강스메어 시료의 전알파 분석 및 내부피폭선량평가 절차)

  • The gross alpha analysis of nasal smear samples for the radiation emergency and the additional follow-up steps were established. Cotton swab sticks using in local hospitals for nasal smear in Korea were used for the verification. The measurement results of standard samples spiked with certified reference source were well agreed within ${\pm}20%$ compared with reference values. The clearance ratio of smear samples conducted with wet smear condition showed higher value. To eliminate the quenching effect of liquid scintillation samples, dry of smear samples should be followed up before counting samples. Based on the measurement results, medical decision levels and internal dose assessment were established for the victims in the beginning of radiation emergency.

Study on Safety Management Activity of Blood Test Room of Nuclear Medicine Department (핵의학과 혈액검사실의 안전 관리 활동에 대한 고찰)

  • Purpose: The object evaluation method about medical institutes of these days increases credibility of consumers about medical services by conducting a certification system about medical institutes. In addition, as nuclear medicine test rooms and diagnosis test medicine room adopt many kinds of international certification systems, the matters regarding safety management of test rooms are being regarded as important. Since the blood test rooms of nuclear medicine are exposed to many harmful factors such as infection from clinical specimen and radioactive isotope reagent, there is a need to pay lots of attention to the safety management of staff and patients. Therefore, this study discusses safety management activities of staff and patients, which are conducted in the blood test rooms of the nuclear medicine department in Asan Medical Center. Materials and Methods: In the blood test rooms of the nuclear medicine department in Asan Medical Center, the matters regarding comprehensive safety management by the person in charge of safety management are offered and all staff members of the test rooms apply them into work. Safety management education is regularly conducted according to established regulations, and infection is prevented through implementation of wearing personal protectors and hand sanitation during test work. In addition, technical safety guides and accident guides for interruption of electric power are provided against emergencies. Through infection management guides, infection prevention and preparation methods for infection are learned and radioactive isotope management, safety management about reagent use and safety guides about harmful chemical substances are being applied to work. Results: The blood test rooms of the nuclear medicine department apply safety management regulations to work. Under the situation where hand sanitation should be conducted, hands are washed to prevent infection between staff and patients, and for preventing infection from clinical specimen, personal protectors are worn. The reagent, which is classified as harmful substance, is separately stored to be easily recognized, radioactive wastes and general medical wastes are also safely managed. Through these lots of safety management activities, safety management awareness of staff members is enhanced, and patients are protected from many dangers. Conclusion: Staff members of the blood test rooms of the nuclear medicine department should be fully aware of safety management regulations and apply them to work. When better safety management suggestions are made, they need to be examined and applied for increasing quality of safety management for staff and patients.

Progress and Future Tasks of Korean Health Promoting Schools (한국형 건강증진학교 추진 경과와 향후 과제)

  • Purpose: The purpose of this review is to introduce the progress of health promoting schools in Korea and future tasks. Method: From 2009 to 2018, we reviewed the literature on health promotion schools in Korea conducted by the Ministry of Education Results: Over the past decade, Korean health promoting schools have operated from a minimum of 14 schools to a maximum of 98 schools each year. As a result of the effectiveness evaluation, it was found that the students had a positive influence not only on the health behavior but also on the whole school life. Under the organic cooperation of the Ministry of Education, the Office of Education, the Korean Educational Development Institute and the schools, consulting, provision of educational opportunities for teachers, promotion of good practices, and public relations support systems for generalization are well established. Conclusion: Promotion and support system for Korean health promoting schools are already well established. Nevertheless, there are limitations on the universalization of health promoting schools, so various attempts such as introduction of certification system should be continued to overcome them.
