  • This study was investigated the effects on functional recovery of eccentric exercise-induced muscle damage by phonophoresis transdermal permeation of piroxicam gel and observed the change of amplitude at muscle action potential. Through eccentric exercise-induced muscle damage, performed healthy men and women take eccentric resistance exercise and measured action, potentials. The subjects were divided into three groups of four men each 24 hour, 48 hour, 72 hour. The results of this were as follows: 1. Change of maximal action potential at maximal voluntary contraction : The phonophoresis group was increase more than control group and gel group. 2. Change of average action potential at maximal voluntary contraction : The gel group was increase more than control group and phonophoresis group. 3. Change of maximal action potential at pain subthreshold voluntary contraction : The phonophoresis group was increase more significantly than control group and gel group. 4. Change of average action potential at pain subthreshold voluntary contraction : The phonophoresis group was increase more significantly than control group and gel group. In conclusion, the change of muscle action potential amplitude by eccentric exercise-induced muscle damage showed that the phonophoresis by pulsed ultrasound of piroxicam gel was improved the recovery of muscle function.

  • This study conducted the following experiment to examine anesthetic effects of 500 kHz ultrasound with lidocaine. Clinical experiment was conducted to compare local anesthetic effects by ultrasound frequencies and drugs with 40 normal adults and it divided subjects into experimental group (I) applying only ultrasound, ointment group (II) applying only lidocaine, phonophoresis group of 1 MHz (III), phonophoresis group of 500 kHz lidocaine (IV) for pain threshold and nerve conduct experiments. Mechanical threshold was measured with von Frey monofilament, thermal pain threshold with halogen lamp and digital thermometer, action potential in median nerve with diagnostic electromyography before and after treatment, and at 30 min., 60 min., 90 min., and 120 min. after treatment. Results of this study can be summarized as follows. Topical anesthesia experiment showed that pain threshold of phonophoresis groups was significantly increased, but there were little differences in ultrasound frequency and drugs among phonophoresis groups. Conduction anesthesia experiment showed that phonophoresis group of 500 kHz using lidocaine had significant difference in pain threshold and sensory nerve conduction compared to ointment group and ultrasound group, but there was no great difference from other phonophresis groups and light nerve block effect was found. It was considered that application of 500 kHz ultrasound using lidocaine will be more effective in deep anesthesia or nerve block than 1 MHz ultrasound. However, researches considering various frequencies, intensities and application hours in low frequency areas including kHz ultrasound are needed to increase deep permeation of drugs.

  • Transdermal permeation enhancer has been used to increased skin absorption. External control of drug release and skin absorption can also be achieved by iontophoresis or phonophoresis. However, because several problems with iontophoresis are that it has a risk to skin damage because of the change of pH and the increase of current density in applying it and that it can be applied only in the form of water solution, This study is to enhance drug permeation via skin following application of ultrasound. For this goal, in gel containing piroxicam, the degree of skin permeation in vitro and anti-inflammatory effect in in vivo were investigated. Permeation study using hairless mouse skin was performed at 37 $^{\circ}C$ using buffer saline as the receptor solution. The amount of piroxicam were quantified using a HPLC system consisting of solvent delivery system. Following adoption of ultrasound 1 MHZ, it showed relatively high permeation rate where it was compared with non treated by ultrasound. The influence of duty cycle having an effect on skin permeation rate was slight higher in the case of using pulsed mode. Skin permeation increase attended by intensity of ultrasound, the permeation of trice was accelerated at 2.0 W/$cm^{2}$ than 1.0 W/$cm^{2}$. The skin permeation of piroxicam was substantially influenced by ultrasound. Anti-inflammatory effects were determined using carrageenan-induced paw swelling method in SD rat. Paw swelling tests showed that pulsed phonophoresis group was more effective than control group and only gel application group. The conclusion of phonophoresis was found to improve significantly the skin permeation in vitro and the anti-inflammatory effect in vivo.

  • This study was investigated the effects of a direct current and ultrasound on transdermal transport of dexamethasone into the rabbits which had contusion in the thigh. Each group was treated under the tallowing conditions. 1. EXP group I : $10\%$ dexamethasone ointment and ultrasound 2. EXP group II : $1\%$aqueous solution of dexamethasone and iontophoresis 3. EXP group III : the application of $10\%$ dexamethasone ointment 4. Control group : No treatment The degree of anti-inflammation was evaluated by the naked eye, the change in girth of thigh, and a light microscope. The results were as follows. 1.8y the naked eye. an inflammation sign was seen in all groups and especially. symptoms of redness. heat swelling were prominent in EXP group I. 2. In comparision in the change of girth of thigh, only EXP group II showed no significant change. Therefore, it meant that there was effective anti-inflammatory reaction in EXP group II. 3. The infiltration of inflammation cells, the degree of swelling, and the degree of crosslinking of connective tissues were evaluated with a light microscope. As a result, EXP group II showed the most effective anti-inflammatory reaction. And, in order of EXP group III, control group, the effect of anti-inflammation reaction was decreased. 4. EXP group I showed more intensive inflammation than control group.

  • This study was carried out to evaluate the body fat breakdown during topical seaweed extract treatment and in combination with the iontophoresis, phonophoresis method to enhance skin absorption of lipolytic compounds. Of 24 female obesity subjects, 7 treated a gel types of seaweed extract only(control group) and 10 treated in combination with the iontophoresis(experimental group I) and 7 treated in combination with the phonophoresis(experimental group II). Topical treatment of each sample was designed for 2 weeks, treated with 2 times a day about $10m{\ell}$ on the abdomen, triceps, thigh and superior ilium, respectively. Experimental group treat with the iontophoresis and phonophoresis during 5 minutes immediately after topical treatment. After one and two weeks of topical treatment, measured body weight, body fat, skinfolds thickness respectively. Then venous blood samples were taken and analyzed serum lipids. The results were as follows; 1) There were decrement of body weight, $\%$ of body fat, superior ilium, triceps and thigh thickness between pre and post treatment in all group. Especially, there were more decrement of body weight, $\%$ of body fat in the iontophoresis treated group. 2) There were decrement in total CHOL, LDL, TRIG and increment in HDL in all group. These phenomena were predominant in the iontophoresis treated group. CHOL decrement in the experimental group were significant after two weeks iontophoresis treatment.
