Background: Intraoperative and postoperative ocular complications are rare. We retrospectively investigated the abnormal ocular findings in patients who underwent arthroscopic shoulder surgery. Methods: The records were investigated of patients who underwent shoulder arthroscopic surgery at our hospital between January 2018 and March 2019. Ocular complications were confirmed by review of medical records. We performed logistic regression analysis to defined the main factors associated with ocular complications. Results: The overall prevalence of postoperative ocular complications was 8.5% (18/211). A significant association was found between anti-adhesion barrier agent and ocular complications (p=0.020). Conclusions: Anti-adhesion barrier agent used during surgery may be the cause of postoperative ocular complication.
To determine the efficacy of OK-432 as pleural sclerosant, we examined the outcomes in 81 patients (age : 27 to 82 years) with malignant pleural effusion and the outcomes in 64 patients ecieving OK-432 3-10KE(1 Klinische Einheit unit) through a chest tube for malignant pleural effusions. Of 81 patients with malignant pleural effusion, 40 patients had lung cancer. Lung cancer is the most frequent cause of malignant pleural effusion in men and women, in which 57 oyo of it was adenocarcinoma. Eighty seven percent of patient had respiratory symptom. Of the 64 patients with intracavitary injection of OK-432 for malignant pleural effusion, 59 patients had a complete short-term response (no fluid reaccumulation during 1 month after intracavitary injection of OK-432). Five patiens of the non-responders had partial control of effusion, with improvement in respiratory symptoms and these patients underwent thoracentesis. Of the 51 patients who survived longer than 1 month, 48 patients did not have re- accumulation of the fluid during follow up. Fever after intracavitary injection of OK-432 was a majors side effect although but that was easily controlled with non-steroidal anti inflammatory drug therapy, Thus the efficacy of intracavitary OK-4)2 injection for malignant pleural effusion was very helpful.

치아는 인체중에서도 혹한 환경에서 부분으로 높은 하중과 타액과 같은 강 부식성 매체로 그 환경이 상상을 초월한다. 즉 반복적으로 가해지는 하중(응력)과 침식을 유발하는 타액과 음식물 등이다. 따라서 치아가 쉽게 파괴되거나 썩는 현상이 나타나게 된다. 이렇게 사용되다가 치아의 역할을 다하게 되면 인공치아를 사용하게 되는데 그 재료가 바로 타이타늄(Ti)이다. 생체매식재로 사용되는 Ti는 반응성이 높아 산소와 쉽게 결합하여 표면에 TiO, $TiO_2$, 및 $Ti_2O_3$와 같은 산화피막을 표면에 형성함으로써 뛰어난 부식저항성과 생체적합성을 가지며 생체에 독성이 없고 탄성계수가 골과 비슷하여 골과 임플란트 경계면에서 응력분산에 유리한 성질 등 물리적, 기계적 성질이 뛰어나 외과용 임플란트 재료로 가장 좋은 재료이다. 금속 임플란트의 생체적합도는 임플란트 재료 자체보다는 생체 내 산화막이 화학적으로 불안정할 때 부식이 발생하게 되고 그 결과 금속이온이 주위로 유리되어 조직반응을 일으키므로 금속의 표면을 덮고 있는 산화막에 의해 좌우된다. Ti는 생체불활성재료로서 매식재료로 사용할 경우 뼈와 잘 융합되는 골유착을 나타내나 골과 화학적결합은 하지 않고 골형성을 적극적으로 유도하지 못함으로 환자의 치유기간이 길어지게 된다. 이러한 이유로 골조직내 임플란트의 접합을 개선하기위한 연구가 이루어져 골과의 결합을 높이기 위해 골유착을 일으키는 Ti에 골성장을 유도하는 뼈성분인 하이드록시 아파타이트(HA)라는 물질을 플라즈마 코팅법을 사용하던가 아니면 Hanks' solution내에서 침적 후 HA도금을 하는 방법 등으로 처리하고 있다. 그러나 플라즈마 코팅법은 고온에서 처리를 행하고 Hanks' solution내에 침적할 경우 Ti표면에 밀착도가 저하되거나 합금의 상변화 등으로 인하여 표면처리 과정 중에서 내식성이 크게 감소될 수 있다. 이러한 여러 가지 코팅법을 통하여 골 유착을 증진시키기 위한 연구는 계속되고 있지만 임상적으로 사용 후 문제가 단시일에 발생되는 것도 아니고 수년이 지나야 나타나게 된다. 이러한 방법으로 코팅을 하게 되면 골과 잘 유착이 되어 자연차아와 같은 기능을 하게 된다. 따라서 이러한 문제를 최소화하는 방법이 나노구조를 표면에 형성시켜 골유착을 쉽게 함으로써 이를 개선할 수 있을 것으로 생각되어 본 강의에서는 임플란트의 문제와 사용되는 재료에 대하여 고찰하여 자연치아를 대체 할 수 있는지 알아보았다.
Background: This study was designed to evaluate the efficacy of a mixture of hyaluronic acid and sodium carboxymethyl cellulose ($Guardix-sol^{(R)}$) on experimental pericardial adhesion. Material and Method: Thirty rats were divided into 2 groups of 15 rats each and pericardial mesothelial injury was induced during surgery by abrasion. In the control group, blood and normal saline were administered into pericardium; in the test group, blood and HA-CMC solution were administered. Pericardial adhesions were evaluated at 2 weeks (n=5), 4 weeks (n=5), and 6 weeks (n=5) after surgery. The severity of adhesions was graded by macroscopic examination, and the adhesion tissue thickness was analyzed microscopically with Masson trichrome stain and an image processing program. Result: The test group had significantly lower macroscopic adhesion scores ($2.9{\pm}0.6$ : $3.9{\pm}0.4$, p<0.000) compared with the control group. For microscopic adhesion tissue thickness, the test group had lower scores compared with the control group, but this difference was not statistically significant ($91.73{\pm}49.91$ : $117.67{\pm}46.4$, p=0.106). Conclusion: We conclude that an HA-CMC solution ($Guardix-sol^{(R)}$) reduces the formation of pericardial adhesions in this animal model.
This study was performed in rats to find the minimum dose of melatonin that can effectively prevent the formation of postoperative intraperitoneal adhesions. Forty-two Sprague Dawley male rats were divided into six groups consisting of 7 rats, respectively. After celiotomy, five abrasions of $0.5{\times}1cm$ area were made on the antimesenteric serosal surface of the colon with a scalpel blade. The abdominal cavity was filled with 1 ml of solution containing 1 mg/kg(Mel 1), 3 mg/kg(Mel 3), 10 mg/kg(Mel 10), 30 mg/kg(Mel 30) and 5% ethanol solution(sham) through the catheter, using a sterile syringe before abdominal closure. Control group was given no adjuvant. The locations and values of adhesion were assessed through the second operation on the 14th day after the first operation. The adhesions were located on serosa to mesentery(54 of 210, 25.7%), serosa to serosa(44 of 210, 21%), serosa to omentum (12 of 210, 5.7%) and serosa to parietal peritoneum(0 of 210, 0%). The incidences of adhesion in Control, Sham, Mel 1, Mel 3, Mel 10 and Mel 30 were 68.6%, 91.4%, 57.1%, 60.1%, 17.1% and 20%, respectively. The values of adhesion separation in Mel 10 and Mel 30 group were lower than those in other groups. However, there was no significant(p<0.05) between Mel 10 and Mel 30 group. This study showed that 10 mg/kg of melatonin were effective in reducing the intraperitoneal adhesion.
Background: Parenteral tetracycline is no longer available for pleural sclerosing agent for pleurodesis in Korea due to the discontinuation of the production. The purposes of this study were to determine whether oral doxycycline (ODC) could be used as an effective sclerosing agent for pleurodesis, and to compare the effectiveness of ODC to other agents, such as homologous blood and talc. Material and Method: Twenty male rats were divided into four groups (A to D). Following agents were given to each group intrapleurally; 10 $m\ell$/kg of 0.9% saline to group A, 10 mg/kg of ODC to group B, 2 $m\ell$/kg of homologous blood to group e, and 70 mg/kg of talc slurry to group D. All animals were sacrificed 28 days after instillation. The pleural spaces were assessed grossly and microscopically and were graded from 0 to 3, and the thicknesses of the pleura were measured. Result: The gross score of group A was 0.0, group B was 1.4$\pm$0.9, group e was 1.0$\pm$0.7, and group D was 2.2$\pm$0.8. Significant adhesion were examined in group B and D grossly (p < 0.05). The pleural thickness of group A was 0.7$\pm$0.2 /10$^2$ mm, group B was 1.2$\pm$0.4 /10$^2$ mm, group C was 1.4$\pm$0.4 /10$^2$ mm, and group D was 3.5$\pm$0.9 /10$^2$ mm. Group D showed pleural thickening significantly (p < 0.05). The microscopic score of group A was 1.0, group B was 1.7$\pm$0.5, group e was $1.5\pm$0.4, and group D was 2.8$\pm$0.4. Group B and D showed significant inflammations and depositions of collagen (p < 0.05). Conclusion: ODC showed significant pleurodesis grossly and microscopically, and homologous blood did not show adhesion. Talc was a significant sclerosing agent for pleurodesis causing extensive inflammation and collagen depisotion.
Background: The intrapleural instillation of tetracycline for pleural sclerosis had been most commonly used in patients with symptomatic malignant pleural effusion or recurrent pneumothorax for a long time. Unfortunately, at a time of expanding use of this agent, the production of injectable tetracycline hydrochloride used for pleurodesis was discontinued by its sole manufacturer in mid-1991 because the manufacturer was unable to meet US Food and Drug Administration purity standards. So we performed a preliminary study of doxycycline, as a alternative pleural sclerosant on rabbit pleura and compared its efficacy with that of tetracycline. Method: Eighteen New Zealand white rabbits weighing 2 to 3kg(mean 2.6kg) were devided into three groups. In each groups, one tetracycline(20 mg/ml/kg) and two doxycycline solutions(7 mg/ml/kg and 20 mg/ml/kg) instillated into the right pleural space through an 18-gauge angiocath with care to prevent pneumothorax. All rabbits were sacrificed after 28 days. Results: 1) In the group of tetracycline 20 mg/ml/kg(six rabbits), five rabbits showed partial pleural symphysis with several fibrous bands, and one rabbit died on 22th day. 2) In the group of doxycycline 7 mg/ml/kg(six rabbits), three rabbits showed partial pleural symphysis and the other three rabbits showed complete pleural symphysis without necrosis of underlying parenchymal lung tissue. 3) In the group of doxycycline 20 mg/ml/kg(six rabbits), two rabbits showed complete pleural symphysis without lung necrosis, another two rabbits showed complete pleural symphysis with lung necrosis, and the other two rabbits died on 4th and 13th day, respectively. Conclusion: We concluded that doxycycline is a highly effective sclerosing agent having stronger pleurodesis effect with that of tetracycline by dose base and its optimal dosage was considered as 7 mg/ml/kg with minimal complications.
This study was performed to determine the minimum dose of Poloxamer/Sodium alginate (PX/SA) mixture on preventing intraperitoneal adhesions to evaluate organ toxicity. Twenty five healthy adult mongrel dogs (weighing 4.68${\pm}$1.67 kg) were divided into five experimental groups composed of five dogs respectively; negative control group (NC, non-treated), positive control group (PC, 2% carboxymethyl chitosan solution treated), and experiment 1 group (E1, 0.25 ml PX/SA mixture of abraded area), experiment 2 group (E2, 0.5 ml PX/SA mixture of abraded area), experiment 3 group (E3, 1.0 ml PX/SA mixture of abraded area). Venous blood specimens were collected from all experimental animals for hematologic and biochemical analysis: WBC, fibrinogen, AST, ALT, ALP, BUN and creatinine. The anti-adhesion effect was evaluated using a serosa abrasion model. The denuded ileum was coated with PX/SA mixture, carboxymethyl chitosan solution or neither. The tensile strength of the adhesion site was evaluated with a tensiometer. For histopathological examination, tissue samples of the liver and kidney were collected from all dogs. According to the results, the frequency and tensile strength values for adhesion separation in PX/SA group were significantly lower than those in negative control group (p < 0.05). In E2 group, the tensile strength was significantly decreased in consideration of PX/SA dose. The values of AST, ALT, ALP, BUN and creatinine of the control and the experimental groups showed no statistical differences. No obvious microscopic differences were noted among tissue sections obtained from all groups. The results suggest that PX/SA mixture may be effective on reducing peritoneal adhesion formation in dog and that 0.5 ml PX/SA mixture of abraded area is most effective dose. Moreover, PX/SA mixture was considered not to have toxicity for the liver and the kidney.
Purpose: To evaluate the efficacy of ultrasound guided injection of prolotherapy and steroid mixture injection in patients with adhesive capsulitis. Materials and Methods: 53 patients with adhesive capsulitis were included in the study and in all the patients a mixture of steroid and prolotherapy agent was injected into the coracohumeral ligament under the sonographic guidance. The patients were evaluated using the VAS and ROM of the shoulder before the injection and at 8 weeks and at 1 year after the injection. Results: Forward flexion was 93.4 degrees before the injection and was 142, 153 degrees at 8weeks and 1 year after injection. Abduction was 79.2 degrees before the injection and was 125.4, 152.6 degrees at 8 weeks and 1 year after the injection. The VAS score was 6.7 before the injection and was 3.5, 3.7 at 8 weeks and 1 year after the injection. Conclusion: The ultrasound guided injection of prolotherapy and steroid mixture into the coracohumeral ligament in patients with adhesive capsulitis is allowing both tissue distension and inflammatory process controlling procedure. It is effective in improving the range of shoulder motion significantly and is also effective in decreasing the pain.
This study was conducted to determine the effect of repeated ultrasound-guided transvaginal retrieval of oocytes recovery, estrous cycle and ovarian adhesion in Korean native, Hanwoo heifers. Heifers were at unknown stages of the estrous cycle at the start of experiments in which all follicles $\geq$6mm in diameter were ablated. The results obtained in this study were as follows; Follicle developing number and oocytes collected number were no effected to repeated OPU to nine session, 4 e.a range oocytes collected to repeated OPU session. Oocytes were observated follicles were 8.7$\pm$4.2 e.a, collected oocytes were 4.1$\pm$3.4 e.a to two times collected per week and observated follicles was 10.2$\pm$6.1 e.a, collected oocytes were 4.3$\pm$2.9 e.a to one times collected per week, but no difference significantly(P<0.05). Ovaries adhesive percentage to repeated OPU was eight ovaries adhisived(20%) of forty ovaries, three ovaries(7.5%) to 1~3 times oocytes collected, four ovaries(l0%) to 4~6 times, one ovaries(2.5%) to 7~9 times oocytes collected session. To repeated OPU effection, ovaries adhisive heifers were long estrous cycle(>25 day) to 7 heads(87.5%) of 8 head, non-adhesive ovaries heifers were 5 heads(41.7%) were long estrous cycle to repeated OPU 12 heads. Although, now unknown about the dynamics of follicles wave and about functional changes to repeated OPU ovaries, more question about ovaries adhesive cause remain.
