Hesperidin 대사에 의한 대금음자(對金飮子)와 평위산(平胃散)의 처방해석 (Scientific Analysis of Daekumeumja and Pyungwesan by the Metabolism of Hesperidin)

  • To analyze scientifically the polyprescription principle of Daekumeumja(對金飮子), which has been used for alcoholic damage, and Pyungwesan(平胃散), which has been used for indigestion, the transforming rate of hesperidin of these polyprescriptions to hesperetin was investigated. The transforming rate of the former was higher 3 times than that of the latter. The transforming rate of hesperidin of Aurantii nobilis Pericarpium was inhibited by Magnoliae Cortex, but was activated by Glycyrrhizae Radix. The activity of trypsin was inhibited by Glycyrrhizae Radix and Daekumeuja. However, Aurantii nobilis Pericarpium, Atractylodis Rhizoma, Magnoliae Cortex and Pyungwesan did not inhibited it. When human intestinal microflora were cultured with the media containing Daekumeumja, Pyungwesan and herbal medicines consisting of them, Aurantii nobilis Pericarpium and Magnoliae Cortex inhibited the activity of ${\beta}$-glucosidase. These results suggests that the therapeutic effect of Daekumeumja may be better than that of Pyungwesan on alcoholic damage and the therapeutic effect of Pyungwesan may be better than that of Daekumeumja on indigestion, although these prescriptions are consisted of the same herbal medicines.

역류성 후두염과 코골이 및 수면 무호흡 관련 증상과의 관계 (The Relationship between Reflux Laryngitis and Snoring and Sleep Apnea Related Symptom)

  • Background and Objectives : Gastric acid reflux has been suggested to have an association with sleep apnea(SA). This study's aim is to evaluate the relationship between reflux laryngitis and SA through the fact that the treatment of reflux laryngitis may impact the snoring and SA related symptoms in selective individuals. Methods : Population consist of 24 males and 10 female aged 34 to 66 years(mean age 50 years) confirmed by Reflux Finding Score(RFS) of PC Belafsky. Thirty four patients with reflux laryngitis and associated symptoms of SA were treated with proton pump inhibitor(Rabeprazole sodium 10mg/day) for 60 days. The degree of snoring and apnea related symptoms were evaluated using questionnaires, and palatine tonsillar hypertropy(PTH) and RFS were compared preoperatively and postoperatively. Results : After antireflux treatment for 60 days, the snoring(p=0.039), daytime sleepiness(p=0.002), and concentration(p=0.011) were significantly improved(p<0.05) and RFS was significantly decreased(p=0.000), but morning headache(p=0.057) and sleep apnea(p=0.083) were not significantly improved(p>0.05) and PTH was not significantly decreased(p=0.328). Conclusion : Treatment of reflux laryngitis significantly impacted the snoring, daytime sleepiness, and concentration in selective individuals. These results suggest some close relationship between reflux laryngitis and SA, and the treatment of reflux laryngitis may be some effective in those with both disorders.

평위산 전탕액의 보관온도 및 기간에 따른 주요성분 및 미생물 군집 변화 (Changes of Principal Components and Microbial Population in Pyungwi-san Decoction according to the Preservation Temperature and Period)

  • Objectives: To optimize the preservation method of herbal decoction, we investigated the content of principle components of Pyungwi-san, liquiritin, glycyrrhizin, and hesperidin according to preservation temperature and period. We also investigated the changing patterns of pH and microbial population in Pyungwi-san decoction as a model case. Methods: With samples preserved at different temperatures, the content of liquiritin, glycyrrhizin, and hesperidin was determined using HPLC and microbial population was determined as viable counting method up to 8 times every month. Identification of isolated bacteria was performed by 16S rDNA analysis. Results: The content of liquiritin and glycyrrhizin did not change according to the preservation temperature and period, but that of hesperidin was severely decreased at room temperature. The isolate from the decoction was identified as Bacillus licheniformis by 16S rDNA sequence analysis. Microbial population appeared after 3 months' preservation and reached maximum value at 4 months; at all tested temperatures, the pH showed the lowest value (4.4-4.5) simultaneously. Conclusion: From the results, it seems to be that the microbial growth affects the pH of preserved decoction but not the change of liquiritin and glycyrrhizin content.

  • Dysphonia is a medical terminology for voice disorders characterized by hoarseness, harshness, weakness, or even loss of voice ; any impairment in ability to produce voice sounds using the vocal organs, larynx, The causes of dysphonia can be classified into two groups, organic and functional. Functional dysphonia includes spasmodic dysphonia, muscle tension dysphonia, mutational dysphonia and conversion dysphonia, etc, The findings of laryngoscopy in these dysphonia are almost normal. Therefore, physicians should diagnosis these diseases from careful history taking and abundant understandings about the phonation pattern, Organic dysphonia is caused by anatomical problems in the larynx, especially on the vocal fold, Some lesions, however, are not easily found because these lesions are too small, or located on the lower lip of vibrating vocal fold. Laryngopharyngeal reflux induced laryngitis, vascular lesions, sulcus vocalis, vocal atropy including presbylaryngis, and mucosal tears are common lesions easily missed in laryngoscopy, Therefore, a high index of suspicion is necessary to avoid missing vocal fold mucosal lesions, and the strobovideolaryngoscopy is indispensable in making the diagnosis,

전탕방법에 따른 전탕액의 추출률 비교연구 - 평위산 전탕액의 Hesperidin HPLC 분석 - (Comparative Study of Extraction Efficiency of Water Decoction with or Without Non-woven Fabric)

  • Objectives : To evaluate the extraction efficiency of water decoction with or without non-woven fabric underlying comparison of the HPLC (high-performance liquid chromatography) profiles of the hesperidin which is the standard of Citri Pericarpium. Methods : Pyungwisan, which is comprised of Atractylodis Rhizoma, Magnoliae Cortex, Citri Pericarpium, Glycyrrhizae Radix, Zingiberis Rhizoma, and Ziziphy Fructus, was extracted in boiling water with non-woven fabric or without non-woven fabric. Then hesperidin content was analyzed by HPLC. Results : The contents of hesperidin in the water extract of Pyungwisan which was decocted without non-woven fabric showed $219.57{\pm}2.51\;mg/g$ and $138.90{\pm}6.32\;mg/g$ in with non-woven fabric decoction. Conclusions : These results suggest that herbal decoction without non-woven fabric might increase extraction efficiency.

평위산가미방으로 치료한 만성 특발성 두드러기 환자의 추적조사 (Follow-up Study on Chronic Idiopathic Urticaria Patients Treated with Pyungweesangamibang)

  • Objective : The purpose of this study is to investigate the recurrence rate of Chronic Idiopathic Urticaria patients treated with Pyungweesangamibang through long-term follow-up survey.Methods : This tracing study was conducted for sixty-seven Chronic Idiopathic Urticaria patients who received and have finished treatment with Pyungweesangamibang at Woobo Korean Medicine Clinic in 2013 and 2014.Result : 81.4% of the patients, who had spent over 1 year since the finish of treatment with Pyungweesangamibang, did not relapse. In case of more than 2 years elapse, 66.7% of the patients did not suffer relapse.Conclusion : The result of tracing recurrence of the Chronic Idiopathic Urticaria patients treated with Pyungweesangamibang describes that the Korean medical treatment (Pyungweesangamibang prescription) shows lower recurrence rate than other general treatment shows.

인후두 위산 역류증의 동물 모형의 개발 (Animal Model of LPRD (Laryngopharyngeal Reflux Disease))

  • Background and Objectives: Complications arising from endotracheal intubation are uncommon but, when they do occur, can be significant. Placement of an endotracheal tube frequently results in trauma to the underlying laryngeal and tracheal tissue, although the trauma is usually reversible. Occasionally, these changes can be of a more permanent nature and result in severe impairment of the airway and/or voice. It is proposed that a common factor-gastroesophageal reflux-might be responsible. This study was performed in order to develop the animal model of LPRD using rats and investigated that LPRD could produce significant damage to larynx especially vocal cords. Materials and Methods : The each four rats were used in the experiment and control study. Each was anesthetized and larynx was exposed and injured in the unilateral aritenoid. Injured site was contact with normal saline(control group) and synthetic gastric juice(experimental group). The larynx was examined after 7days in normal environment. Results : All was survived in the control group and two was survived in the experimental group. In the control group, some inflammation cells was found but in the experimental group, granulation was found. Conclusion : We developed animal model of LPRD using rat and thought LPRD may Play an important role in the development of permanent laryngeal injury.

후두 및 기관협착증의 유발인자로서의 위산 및 펩신의 역할에 대한 실험적 연구

  • 후두 및 기관지 협착증은 대개가 기도확보를 목적으로 후두 및 기관내에 삽관을 시행하였을 때 삽관튜브에 부착된 cuff내에 압력이 높게 되면 점막에 혈류장애가 초래되고 궤양등의 점막손상이 뒤따르며 여기에 2차감염이 초래되어 육아종과 반흔이 생기면서 결국은 협착이 초래되는 것으로 알려져 있다. 이와 같이 손상된 후두및 기관의 점막에 산도가 강하고 소화효소가 함유된 위액이 역류되어 접촉될 경우 협착이 더욱 조장되리라고 가정할 수 있다. 따라서 위액이 과연 후두 및 기관지 협착증에 관여 한 것인지를 실험적으로 규명해 보고자 하였다. 실험동물은 가토를 대상으로 하여 Urethan마취를 시행하고, 삽관한 다음 합성위액 pH 1.5, 3, 5 짜리 용액을 기관 내에 1시간정도 관류시킨 다음 후두 및 기관을 적출하여 점막상태의 변화를 현미경으로 관찰하였다. 그 결과 산도가 강하고 펩신이 첨가된 합성위액으로 관류한 군에서는 성모의 파괴점막의 궤양, 부종과 염증소견 등이 심하게 관찰되었다. 그러나 산도가 약할수록, 그리고 펩신이 첨가되지 않은 합성위액으로 관류한 군에서는 섬모 및 점막의 손상이나 염증 및 부종등의 소견이 경미하게 관찰되었다. 이러한 결과로 미루어 강한 산도의 위액이 후두 및 기관으로 역류될 경우 협착증의 발생에 깊이 관여하리라고 추측되었다.

평위산(平胃散)과 보화환(保和丸)으로 호전된 연하장애 환자 2례 (Two Patients with Dysphagia after Stroke who were Treated with Pyungwesan and Bowhawhan)

  • Dysphagia is an important complication for the rehabilitative treatment and life quality of stroke patients. This report is about two patients with dysphagia after stroke who were treated with Pyungwesan and Bowhawhan. The severity of the patient's dysphagia was evaluated with stage of dysphagia according to severity. After treatment, dysphagia were remarkably improved(from stage 5 to 2, 5 to 1). This result shows that Pyungwesan and Bowhawhan could be effective against dysphagia after stroke.

향사평위산의 항염증 효과 및 이상지질혈증 개선 작용에 대한 연구 (A Study on the Anti-Inflammatory Effect and Improvement of Dyslipidemia on Hyangsapyeongwi-san)

  • Objectives: This study aims to investigate the anti-inflammatory effect and improvement of dyslipidemia on Hyangsapyeongwi-san. Methods: In this study, HUVEC cells were cultured and treated with Hyangsapyeongwi-san to measure intracellular KLF2, eNOS, MCP-1, ICAM-1, and VCAM-1 gene expression levels related to anti-inflammation. The weight of experimental animals administered with Hyangsapyeongwi-san was measured, blood samples were biochemically analyzed, and liver tissues were reviewed to research histological changes. Results: Gene expression levels in the cells treated with Hyangsapyeongwi-san generally showed a meaningful anti-inflammatory effect. The body weight of the experimental animals decreased, and total cholesterol, triglyceride, and LDL-cholesterol in the blood generally declined while HDL-cholesterol tended to increase. Fat accumulation between hepatocytes was also reduced after the administration of Hyangsapyeongwi-san. Conclusions: This study confirmed that Hyangsapyeongwi-san has the effect of suppressing vascular inflammatory responses through the regulation of genes involved in the vascular inflammatory process and improving dyslipidemia through the reduction of blood lipids and weight loss.
